This study investigates the current situation of medical technologists for blood collection, which is considered the most important step for diagnosis. The survey enrolled 650 medical technologists working in hospitals and medical check centers in Busan, Ulsan, and Gyeongnam. We found that each medical technologist performed blood collection for about 100 patients. There was more than one blood collection failure per day, with more than one case of pain and filing of civil complaints per year. Hence, there was a high work burden on the medical technologists. Cases where a medical technologist was stabbed with a used needle occurred more than once a year, and about 15% of them received infection control and treatment because of stab wounds. Additionally, more than half of the participants suffered from musculoskeletal disorders and mental stress due to blood collection work. Unlike administering intravenous and intramuscular injections using the same needle, no fee is charged for blood collection. Based on the results of this study, it will be possible to improve the safety and rights of medical technologists by calculating the actual fee for blood collection work and assigning a relative value score.
Unstandardized medical data collection and management are still being conducted manually, and studies are being conducted to classify CT data using deep learning to solve this problem. However, most studies are developing models based only on the axial plane, which is a basic CT slice. Because CT images depict only human structures unlike general images, reconstructing CT scans alone can provide richer physical features. This study seeks to find ways to achieve higher performance through various methods of converting CT scan to 2D as well as axial planes. The training used 1042 CT scans from five body parts and collected 179 test sets and 448 with external datasets for model evaluation. To develop a deep learning model, we used InceptionResNetV2 pre-trained with ImageNet as a backbone and re-trained the entire layer of the model. As a result of the experiment, the reconstruction data model achieved 99.33% in body part classification, 1.12% higher than the axial model, and the axial model was higher only in brain and neck in contrast classification. In conclusion, it was possible to achieve more accurate performance when learning with data that shows better anatomical features than when trained with axial slice alone.
Objectives: This study performed a comparative evaluation of the effects of oil pulling on the periodontal health of smokers. Methods: The experimental (15 subjects) and control (15 subjects) were provided coconut oil and distilled water, respectively. We evaluated the pocket depth (≥4 mm), bleeding on exploration, and Patient Hygiene Performance (PHP) index in both groups following the interventions. Clinical parameters were assessed at baseline, after 4 weeks, and after 8 weeks. Dry mouth and oral health-related quality of life were evaluated at baseline and after 8 weeks. Results: Bleeding on exploring in the control group decreased from 26.17 to 18.33 and from 26.07 to 12.53 in the experimental group (p=0.030), with significant differences in measurement time (p<0.001), and the interaction between group and measurement time (p=0.002). The PHP index in the control group decreased from 24.50 to 16.17 and from 24.00 to 9.83 in the experimental group (p=0.027), with significant differences in measurement time (p<0.001), and the interaction between group and measurement time (p=0.001). Furthermore, the experimental group showed a significant decrease in dry mouth (p<0.001) and a significant increase in oral health-related quality of life (p=0.025). Conclusions: The coconut oil intervention positively affected the periodontal health of smokers.
Korea government has launched long-term care insurance from 2008. However, one of the most important issues, whether or not providing cash benefit, is still unresolved. In this paper, in order to provide policy guidelines for the long-term care insurance, I attempt to estimate the Willingness-To-Accept (WTA) of the cash subsidy for informal care by using Double Bounded Dichotomous Choice method, a branch of Contingent Valuation Method (CVM). In doing so, I also estimated the determinants of the preference for cash benefit. Data were obtained from face-to-face survey interviews with 300 informal care-givers at three major general hospitals in Seoul, Korea. The questionnaire was constructed with two scenarios (mild/severe symptom). The results from logistic regression analyses and the estimation of WTA indicate that informal care-givers are willing to accept the cash benefit as low as 628 thousands won for mild fragile elderly and 1,072 thousands won for severe fragile elderly. The strength of this paper is that I estimated the WTA of the cash benefit by reflecting the changes in preferences of informal care-givers. The analytic results from the this paper suggest that the cash benefit in long-term care insurance is indispensible in achieving the goal of the long-term care system.
The main purpose of this study is to provide the sustainability and continuous development of Long-term care Insurance in projecting changes of the Long-term care Insurance beneficiary population and Cost. We conducted a transformed cohort-component projection method that are employed for the beneficiary population projection and applied the previous experiences in Japan and German. A transformed cohort-component method means that we also projected the increasing beneficiary of long-term care insurance for using the data of geriatric disease in NHIC and estimated the cost of insurance's financial resources. First of all, beneficiary increase and strategy of extending to level 4 are categorized 2 and the expense account projection are categorized 2. If it is thought experience of Japan and German, The Level 4 extend of insuree is projected 2012 or 2013. With the results of this study, we proposed that extended level 4 insuree include the 40%~90% of geriatric disease in elderly people. The number of beneficiaries in 2011 is expected to reach to about 342,896 and in 2015 is 415,905 on scenario 1. Scenario 2(40%of geriatric disease in elderly people), the number of beneficiaries in 2011 is 342,896 and in 2015 is 483,453. Scenario 3(90%of geriatric disease in elderly people), the number of beneficiaries in 2012 is 545,068 and in 2015 is 565,565. The cost of beneficiaries insurance benefit of scenario 1 are projected from 3,000billion in 2012 to 3,500billion won in 2015. Scenario 2 are projected from 3,100billion in 2012 to 4,000billion won 2015. Finally, The cost of Level 4 extending are need minimum 300billion to maximum 1,400billion won.
The purpose of this study is to identify how to efficiently integrate long-term care facilities into geriatric hospitals. We conducted a survey on the current operations of facilities and medical services of 2009 of 192 long-term facilities and 168 geriatric hospitals in Korea between October and November. Technical statistics and chi-square test were conducted on the collected data using the SPSS 13.0/Win program. There was a difference between the two facility types in terms of the co-payment levels of the food services. Both types selected the budget deficit as their major management problem. Ease of access and the surrounding environment were critical factors used to select the location of both types of facilities. Facility users benefited from the discounted co-payments of both facility types. However, facility users wanted more frequent visits and support from their family members during their stay at the facilities. It was discovered that users in the long-term care facilities stayed longer, that is until they died, compared to their counterparts in geriatric hospitals. The two types of facilities provided their services totally separately to users. Users of the two types of facilities are poorly supported and cared for by their families. This study suggests that setting reasonable service fees, paying caretakers, introducing an integrated facility, strengthening facility assessment standards, introducing the family doctor system, and introducing the handling of long-term care insurance by geriatric hospitals would allow the integration between long- term care facilities and geriatric hospitals to be beneficial.
The purpose of this study was to develop the classification of home-based long-term care activities and to test its validity. In this study, the taxonomy of long-term care activities was structured according to the service domain and process. Two expert groups participated in making a draft of the taxonomy that was composed of 7 service domains, 22 care needs, 22 service objectives, and 114 activities. Reliability and validity of the taxonomy was tested in a sample of 152 elderly subjects who used the home-based long-term care services. Based on the factor analysis of 114 activities, 21 factors were extracted from 114 activities. Internal consistency of the factors was high. Content validity was confirmed by the CVI. Long-term care insurance grade was used to assess the criterion validity. Among 21 care needs, 12 cares needs were significantly different from their grade. The classification of home-based long-term care activities demonstrated reliability and validity. In conclusion, the use of this classification is recommended while communicating with the elderly subjects, service providers, and the 3rd party payers.
This study set out to investigate the effects of individual motivation on turnover intention among social workers and examine their turnover intentions in details by focusing on the mediation effects of multiple commitment. To be specific, it aimed to propose and test a prediction model for social workers' turnover intentions based on the Self-determination Theory and Theory of Planned Behavior. For those purposes, a mail survey was taken among social workers working for use facilities, residential facilities, public health centers, social welfare foundations and associations, and all kinds of centers and institutions in 15 cities and provinces across the nation. Total 1,918 questionnaires were distributed, and 1,671 ones were returned, and 979 whose respondents expressed a turnover intention were used in final analysis. The analysis results indicate that psychological motivation of social workers had direct impacts on their turnover intention. However, their role stress had no direct impacts on their turnover intention, which suggests that the impulsive routes model for turnover intention is supported only in psychological motivation and job characteristics. Secondly, their psychological and job motivation turned out to have indirect impacts on turnover intention through the multiple commitment, which suggests that the reflective routes model for turnover intention is supported in all career, job, and organizational commitment. Career commitment had the most significant impacts on turnover intention, being followed by job commitment and organizational commitment in the order, which suggests that the social welfare academy should increase their interest in career commitment. This study reviewed the present state and differences of rehabilitation programs of the gambling addiction by comparing with other psychiatric disorder(including psychotic disability and alcohol addiction). This study also intended to suggest necessities, meanings and inherent fields of the rehabilitation in gambling addiction. First of all, the government and a few gambling industries run clinic centers for gamblers and their families, but have been lacked rehabilitation services for social comeback and adaptation or devaluated rehabilitation services than therapies. Gambling addict didn't have impairments of the cognitive function and their daily abilities was better than any other psychiatric disorders. But Damage of social role or function of gambling addiction was severe. And it is caused by nonadaptive nature of gambling behavior, personality/emotional change through gambling addiction process, and previous personality problem etc. There are many severe failure of social role and its attendant bankrupcy, family's problems and social poverty in gambling addiction, Therefore, important fields in the rehabilitation of gambling addiction should be services for basic social comeback support service, credit recovery support, monetary management, support of rehabilitation of family and vocational rehabilitation. Finally, the significance and critical points of the current study has been discussed as well.

After participating in the Second Boer War in 1899, the Australian Army participated in world wars such as World War I, World War II, the Korean War, and the Vietnam War. To overcome the decline in the social recognition of soldiers and the continuous shortage of troops caused by the protracted war, the status and role of female personnel were expanded. The use of female manpower, which started as medical support during the Second Boer War, expanded to combat support missions such as communications, maintenance, driving, secret document management, and radar surveillance during World War II. After the Vietnam War, the Chiefs of Staff of the Australian Army, Navy and Air Force established a committee to expand women's participation in the military, improved service conditions for female personnel and supplemented regulations so that they were treated as soldiers, not women, and reached a turning point in expanding the role of female personnel in the Australian military. As a result, all combat positions, including special forces, were opened to women in 2014, and a plan was established to increase the proportion of women in each service by 2030.
