RELATIONSHIP BETWEEN DEPRESSION/ANXIETY AND ATTENTION DEFICIT HYPERACTIVITY DISORDER (주의력결핍 과잉운동장애와 우울, 불안증상과의 상호관계)

  • This study was performed to assess depression/anxiety of attention deficit hyperactivity disorder(ADHD) in children and adolescents and to use them as basic material for subdivied ADHD on phenomenological aspects. 51 hospitalized ADHD children and adolescents were assessed using the Korean form of the Kovacs' Children's Depression Inventory(CDI), Korean Form of the State-Trait Anxiety Inventory for Children(STAIC). Their data were compared to normal control of 50 mentally healthy children and adolescents in relation with the dermographic characteristic. The mean scores of CDI and STAIC-5(State) of ADHD group were statistically higher than those of the control group(p<0.01 or p<0.05). The mean scores of STAIC-T(Trait) of ADHD group were higher than those of the control group. These results suggest that the authors suggest that ADHD can be subdivide into pure ADHD, depressive ADHD and anxious ADHD by the comorbidity of the depression/anxiety.

TEMPERAMENTAL CHARACTERISTICS OF ATTENTION DEFICIT HYPERACTIVITY DISORDER (주의력결핍 ${\cdot}$ 과잉운동장애아의 기질적 특성)

  • The authors applied Toddler Temperament Scale(TTS) to ADHD patient group, other disease group and normal control group to elucidate temperamental characteristics of ADHD patients in their toddler stage. 1) ADHD group showed significant difference from the control group in 'activity', 'rhythmicity', 'approach', 'adaptability', 'intensity', 'persistence' and 'threshold' categories. 2) ADHD group and other disease showed significant difference from the control group in 'adaptability' and 'persistence' categories. 3) ADHD group was significantly different from other disease group or control group in 'activity' and 'approach' categories. 4) In total scores of TTS, ADHD group and other diease group showed significant difference from control group. 5) Classification ratio of TTS for ADHD by discriminant analysis was 84.3%. Above results suggest that the ADHD patients may reveal characteristic traits in their toddler stage Further research should be focused on the development and refinement of assessment tools for the early detection and prevention for ADHD.

A STUDY OF THE CONTENT VALIDITY IN HYPERKINETIC CONDUCT DISORDER (과잉운동 품행장애의 내용 타당도에 관한 연구)

  • Conduct disorder is a heterogenous disorder of various etiology affecting 4-10% of school-age children. There is suggestive evidence that the comorbid group of conduct disorder and attention deficit hyperactivity disorder is different from these two separated diseases groups. There findings suggest that this comorbid group may present a meaningful subgroup. This study is conducted to examine the content validity of hyperkinetic conduct disorder that accepted first in International Classification of Disease 10th edition. The results are summarized as follows : 1) Using Conners Parenting Rating Scale, Parent Rating Scale Form for DSM-III-R, the mean scores of attention deficit hyperactivity disorder in conduct disorder were significantly higher compared with those of normal controls. 2) 72.4% of conduct disorder and 41.5% of normal control groups showed simultaneous attention deficit hyperactivity disorder. The diffenence between conduct disorder and normal control groups was significant. There were many previous informations that children with attention deficit hyperactivity disorder had conduct disorder as comorbid disorder and these result show that children with conduct disorder also significantly had attention attention deficit hyperactivity disorder reversibly. Then these show that hyperkinetic conduct disorder-diagnosis used when both the overall criteria for hyperkinetic disorders and the overall criteria for conduct disorders are met-have satisfactory content validity. Biological, familial or long term studies are needed to further validate this diagnostic category.

A COMPARATIVE STUDY UPON EVENT-RELATED POTENTIALS OF THE PATIENTS WITH ADHD AND NORMAL CHILDREN USING FOURIER TRANSFORMATION AND WAVELET ANALYSIS (푸리에 변환과 웨이브렛 분석을 통한 주의력결핍 ${\cdot}$ 과잉운동장애 아동과 정상 아동의 사건관련전위 비교 연구)

  • Using Fourier transformation and wavelet analysis, we compared the auditory event-related potentials of the patients with attention deficit-hyperactivity disorders(abbr. ADHD, 13 boys) and normal control children(8 boys). Amplitudes of the event-related potentials which were calculated via Fourier transformation were compared between the groups and between conditions(non-target versus target) in each group. To the non-target stimuli, the patients with ADHD showed significantly greater amplitudes across almost all of the electrode sites and frequencies. To the target stimuli, the incidents which ADHD patients showed much higher amplitudes than normal controls significantly decreased, while those of the reverse results increased significantly. These results were consistent with the comparison results about negative difference wave(abbr. Nd wave) using Fourier transformation. In summary, it was proved that non-target stimulus which should be ignored elicited more robust electrical response from the patients with ADHD than normal children, but the target stimulus which reguired active processing did much less electrical activity in the patients. For the patients, they showed much inhibited electrical response to the target stimuli in some electrodes and frequency ranges. Normal children were more strongly stimulated by the target stimuli in almost all electrodes and frequency ranges than the patients, but less in prefrontal leads and frontal leads. Wavelet analysis results proved that early responses(0-300msec) to the nontarget stimuli of the patients were significantly greater than the normal controls in prefrontal, anterior frontal, some parts of temporal, and occipital lobes and that late response(300-370msec) were significantly lesser than normal children in parietal and central electrodes. Target stimuli elicited significantly higher electrical activity in both group than non-target stimuli did. Prefrontal and frontal lobes showed stronger responses in the patients than normal children irrespective of stimulus condition, but parietal and temporal lobes did higher activities in normal children than the patients only to the target stimuli. In conclusion, the patients with ADHD showed much greater responses to the stimuli which should be ignored, but failed to activated the necessary processes to the target stimuli. Also, we found that the frequency-dimension analysis and wavelet analysis were useful for the signal processing such as event related potentials.

  • Objective : This study was conducted to find out the electrophysiological abnormalities of the patients with attention-deficit/hyperactivity disorder(abbr. ADHD) through identifying the profiles of the event-related potentials. Methods : Through measuring event-related potentials of 20 patients with ADHD and 16 normal control children using Oddball paradigm, we evaluated the latencies and amplitudes of each wave form. Results : In the group older than 9 years old, the latencies that reflect selective attention and discriminative capacity of the patients with ADHD were significantly longer than those of normal controls, which suggests that patients with ADHD be more distractible to non-significant stimuli and that they be in lark of sustained attention and discriminative ability. Conclusions : We concluded that the patients with ADHD are more distractible to trivial stimuli than normal children but they lack discriminative, sustained ability of attention.

FAMILY ENVIRONMENT OF ATTENTION DEFICIT HYPERACTIVITY DISORDER (주의력결핍 ${\cdot}$ 과잉운동장애의 가정환경에 대한 연구)

  • Objective : The authors investigated family environemtn of ADHD referred to child psychiatric clinics. Method : Seventy two patients were grouped into pure ADHD and comorbid ADHD. Patient's lither and mother conducted the Korean Form of the Family Environment Scale(K-FES). And then, scores were compared with that of 77 normal controls. Results : Among subscales of K-FES, expressiveness, achievement orientation and organization were scored significantly lower in ADHD mothers' reporting than ADHD fathers' And only organization subscale was scored significantly lower in ADHD fathers' reporting than controls' In post hoc comparison, subscales of expressiveness, moral-religious emphasis and organization were significantly lower in mothers' reporting of pure ADHD and comorbid ADHD than of controls, and only organization subscale was significantly lower in fathers' reporting of comorbid ADHD than of controls. Conclusion : Parents of ADHD patients agreed that their families were not well organized, and each perceived somewhat differently on the family environment. Further study is needed to eludicate whether specific family environment causes ADHD or is caused by behavior of ADITD patients.

  • The purpose of this study is to investigate the effect of psychomotor by the emotional behavior disorder children(ADHD) on the improvement of Self-esteem, Depression or Hyperactivity. From the results, there was no significant difference of the main effect between groups about the improvement of Self-esteem, Depression but that of Hyperactivity. About Self-esteem, Depression, Hyperactivity, there was significant difference of the main effect between prior and post and significant difference of the interaction effects between groups and periods. In conclusion, we can know psychomotor program have a positive effect on the improvement of Self-esteem, Depression or Hyperactivity for emotional behavior disorder children(ADHD). Considering that psychomotor, regarding convergence, may have a positive effect on the ADHD children's emotional development, not only motion development, we suggest the emotional behavior disorder children(ADHD) may have to continue to participate in the Psychomotor Activities.

  • Characteristics of attention deficit of attention-deficit hyperactivity disorder(ADHD) were investigated by administering six computerized attention tests of Vienna Test System and four neuropsychological tests to children aged 6-12, with ADHD(n=21) and age-matched normal control children(n=22). The findings indicated that ADHD children show lower level of vigilance, more decline of performance in vigilance task on time, and impaired preparedness to response. They also have selective attention deficit on monitoring tasks, but did not have sustained attention deficit compared with normal control children. On the tasks wich overload their information processing capacity, ADHD children show more impulsive response pattern than normal control children. The performance of ADHD was worse than control on the neuropsychological tests sensitive to frontal lobe dysfunction. The presense 'These' attention deficits supports the theory that the defect of ADHD is due to the dysfunction of more than one brain region, including brain stem reticular formation and frontal lobe.

  • This study assessed psychiatrically referred 5-to 13-year-old children who presented inattention or hyperactivity as chief complaints. Demographic characteristics, primary diagnosis, and comorbid psychiatric conditions of them were identified, and they were assessed using questionnaires and neuropsychological tests. Primary diagnoses included ADHD, anxiety disorder, mental retardation, depression, oppositional defiant disorder, developmental language disorder and others. functional enuresis, conduct disorder, and developmental language disorder were among the secondarily diagnosed disorders. In patients diagnosed as ADHD, overall comorbidity rate was 55.3%. The disorders that frequently co-occured with ADHD were specific developmental disorder, conduct disorder, oppositional defiant disorder, anxiety disorder and other. ADHD groups with or without comorbidity differed in performance IQ and CPT scores. ADHD group differed from externalizing disorders group in the information subscore of IQ, MFFT, and CPT scores, and differed in teachers rating scales, the uncommunication factor of CBCL, and CPT card error compared with internalizing disorders group. The authors concluded that inattentive or hyperactive children should be assessed using various instruments to differentiate other disorders and to identify possible presence of comorbid conditions.

  • Objective ' The purpose of this study was to investigate the behavioral characteristics of children with allergic disease Method : In a population of 80 children who were registered at a pediatric allergic clinic in Kangnam St Mary's hospital, parents completed a standardized child behavior checklist (CBCL) to evaluate their children Clinical factors associated with allergy were examined in relation to the rated scores of CBCL Results : Allergic children were rated significantly more aggressive and delinquent, and less withdrawned on the CBCL rating when compared to the age- and sex-matched normal control children. Within the allergic population, children who had family history of allergic disease had higher scores of CBCL in hyperactivity and aggressive behavior than the children without family history. Conclusion : The results were discussed with the respect to familial traits of allergy and temperament as risk factors for the emergence of behavior problems in childhood allergy
