지역보건의료 정보화 사업의 현황과 발전방향

  • 보건소 정보화 사업은 지역단위 공공보건의료의 최일선 기관인 보건소에 정보시스템을 설치하여 내부운영을 효율화하고 생산성을 증대시킴으로써 양질의 보건의료 서비스를 제공하고, 동시에 지역보건의료의 효과적인 정보체계 구축과 정보의 원활한 교류를 통하여 국가보건정책수립을 위한 기본정보를 제공하며, 이를 통해 공공보건의료의 환경변화를 이끌어 갈 기반을 확충하려는 목적으로 추진되었다. 보건소 전산화 사업을 근간으로 하는 지역보건의료분야 정보화 사업은 보건 의료부문의 정보화를 위한 국민복지망 기본계획에 의거하여 94년 12월부터 2기로 나누어 8차 년도에 걸쳐('94.12 $\sim$ '01. 10) 연차사업으로 시행 중에 있다. 보건소 정보화 프로그램 개발을 목적으로 시행된 제1기 정보화 사업('94.12 $\sim$ '98.7)을 통해 개발된 보건소 정보시스템이 현재 242개 보건소 중 108개 보건소에서 사용중이며, 이를 통해 보건소 업무의 효율화와 함께 지역보건의료분야 전산화 구축의 기초단계가 확립되었다. 제2기 사업은('98.10 $\sim$ '01.10) 보건소 중심의 지역보건의료망 구축과, 보건소 정보시스템의 전국 확산, 관련기관 정보망과의 연계를 통한 정보교류 확대와 정보활용도 제고 등의 목표로 시행중이다. 2기 1차 사업을 통하여 보건소-보건지소 진료소간 지역보건의료망의 기본 하부골격의 토대가 구축되었고, 2기 2차 사업에서는 보건의료원의 전산화 프로그램이 개발되었다. 또한 현재 진행중인 2기 3차 사업에서는 지역보건정책 수립을 지원하는 진료현황, 보건사업현황 등의 자료를 통합D/B로 구축하여 시범적으로 운영함으로써 정보망의 활용도를 높이고, 광역단위의 지역보건정보 의사결정시스템(EIS) 개발을 추진하고 있다. 한편, 최근 급속한 정보기술의 발달과 보건의료 환경의 변화로 인하여 보건정보시스템의 변화 필요성이 증대되고 있다. 이를 위해서 중앙정부와 광역자치단체, 보건소를 연결하는 전국 단위 정보네트워크 구축에 있어 신기술 적용방안 연구를 통하여 보건소 정보화 사업의 발전 방향(ISP)을 모색 중에 있으며, 시군구 행정정보망과 연계를 통해 생애주기에 따른 주민 평생건강관리를 위한 정보시스템 구축을 도모하고 있다.

An Efficiency Evaluation of Gyeongnam Public Health Center by Data Envelopment Analysis (경남지역 보건소의 효율성 평가)

  • In this research, we analyzed the efficiency of 20 public health centers of Gyeongnam Province during 2007-2009, so weakness of input and output factor were found. We used the CCR, BCC model of Data Envelopment Analysis as a method of evaluation, made a choice human resource as the input variable, made a selection the performance of public health care center, ward as the output variable. The results of this study show that the efficiency of 20 public health centers have got better because Government and Gyeongnam have provided administrative, financial support. It is expected that this research can give good information for effective management of public health centers.

Antecedents of Job Satisfaction and Perceived Job Performance in Public Organizations (공공기관 조직 구성원의 직무만족과 인지된 직무성과의 선행요인에 관한 실증 연구)

  • As the level of people's demand for service quality provided by public institutions increases, interest in improving organizational effectiveness of internal members is increasing. In this context, this study aims to identify the predisposing factors affecting the job satisfaction and perceived job performance of public institution members. In this study, public service motivation, work autonomy, and person-system fit were presented as antecedents to verify the organizational effectiveness, and the results were tested by regression analysis. As a result, all three antecedents have significant effects on job satisfaction and perceived job performance. Public service motivation, work autonomy has a relatively higher impact on job satisfaction. On the other hand, person-system fit has a relatively higher influence on perceived job performance. In particular, the results of this study confirmed that PSM theory is consistent with the main trends and is significant in organizations created through artificial integration. This result suggests the need to maintain the effectiveness of the public organization in the future due to the change of integration of public organizations.

Organizational Personality Type and Citizenship Behaviors Perceived by Public Health Center Workers in Chonnam Province, Korea (전남지역 보건소 직원이 인지하는 조직성격유형과 조직시민행동)

  • Objectives: The purpose of this study was to assess organizational citizenship behaviors (OCBs) and organizational personality types (OPT) perceived by public health center workers and to analyze differences in perceptions according to organizational characteristics. Methods: The subjects of this study were workers at 12 of the 22 public health centers in Chonnam Province, South Korea. To measure organizational characteristics, we employed the OPT Index and OCBs Index. Data were collected for one month, from 15 September to 14 October 2009. Data for 422 workers were used in the final analysis. Descriptive statistics and t?tests were performed as appropriate, utilizing SPSS/WIN 14. Results: Among 16 types of OPT, ESTJ, ISTJ and ESTP were the most prevalent for the 12 public health centers. The average weight of OCBs was 4.77, and courtesy and civic virtue held the first and the second ranks among OCBs, respectively. Statistically significant differences between types of OPT were detected, in that T type was gender (p=.007) while J type was age (p=.010), education (p=.043) and career (p=.000). Statistically significant differences between OCBs were found in age, education and career. Conclusion: Public health centers should have programs to innovate OPT and to improve OCB. The development of such programs should consider characteristics of public health center workers such as age, gender, education and career.

The Lived Experience of Occupational Therapists in Public Health Centers Regarding Community Rehabilitation and Health Services: Phenomenological Study (보건소 작업치료사의 지역사회 재활 및 보건서비스 제공에 대한 업무 경험에 관한 현상학적 연구)

  • Objective : The purpose of this study was to examine the roles and practices of occupational therapists in public health centers based on their working experience of occupational therapists who are working in these facilities. Methods : We used Giorgi's descriptive phenomenological research method. Six occupational therapists were interviewed, and their recorded data were analyzed using Claizzi's thematic analysis. Results : As a result, 4 categories, 16 themes, and 24 elements were elicited. The major roles of occupational therapists in public health centers were the evaluation of clients, home-visit occupational therapy, management of dementia programs and community connection projects. Interventions that are needed to create a healthy community include case and program management, compensatory approaches, and healthy lifestyle interventions. The difficulties experienced by occupational therapists include their limited number job insecurity, performance-oriented administrative policy, and the lack of communication among professionals. Occupational therapists require education in health management and practice in public health centers to build their capacities. Conclusion : The results of this study could be used for guidelines for occupational therapists who work in community healthcare centers, as well as basic training for competent occupational therapists in the community.

A Convergence Study on the Organizational Diagnosis of Public Health Center using Six-Box Model (Six-Box model을 이용한 보건소 조직진단에 관한 융합연구)

  • This study was conducted to identify the organizational commitment to health centers in the city of G from September 1, 2018 to September 29, 2018, and empowerment, which is the output of the organization, and to examine organizational diagnosis using the Six-Box Model. In the organizational diagnosis of the health center using the Six-Box Model, the support area was 3.62 points, and the attitude toward change was 3.62 points, which was higher than other areas. In the organizational diagnosis according to gender, the scores of women were higher in males than in males. In the organizational diagnosis according to the type of jobs, the purpose, relationships, rewards, and area scores of nursing jobs were higher than those of other types of jobs. In the future, the public health center is a public institution that provides health administration and medical services to residents of the community, and it is necessary to improve the capacity of the organization through continuous health center organizational diagnosis.

  • In accordance with the analysis results on the questionnaires about the awareness of employees according to positions and educational backgrounds in a public health organization, this thesis suggests the basic data on the purpose of the efficient operation and improvement of working environment in a public health organization. Two answers of the questions that "there is a possibility for promotion in the organization" and "the unit section in organization which I work for has a well-designed structure of organization" showed respectively high points in the questionnaire. On the other hand, three questions like there is a resonable way to bind the organization up, completed mission and rewards are connected, and my salary is fit for the current mission showed high points among the questions about the awareness on the organization according to the positions.

The Medical Doctors' Opinion of Public Health Center on the Development and Supply of Medical Doctor for Public Health Sector (공공보건분야 의사 인력 양성과 개발에 대한 보건소 근무 의사들의 인식)

  • Objectives: This study, as for activating measures for capable medical physicians to penetrate into the public health sector, is to provide a basic informations which are used for the enlargement of human resources of physicians in the public health sector, by investigating the perception of physicians, who are now working in the public health center, on the training and development of physicians in the public health sector. Methods: The subjects of this study were 126 individuals. The data was analyzed by frequency analysis using SPSS ver. 17.0K. Results: According to the investigation of 'how to support physicians in public health sector', the necessity of almost questions is considered to be important. Especially, regarding to investigation on 'obstacles of physicians' entrance to public health sector', 'relatively low salary' and 'lack of promotion chances' were thought to be considerable. The most significant education programs to work for public health sector is to improve the ability of health administration planning and service performance. Conclusions: The important methods to reinforce and easily obtain the human resources of physicians in public health sector are not only to improve the penetration of physicians to public health sector, but also to enhance the ability of present physicians, even though sufficient recruitment of physicians is essential.
