Change of the Pelvic Tilt Angle on Bobath Approach in Hemiplegic Patients (편마비 환자에 있어서 Bobath Approach가 골반경사각도 변화에 미치는 영향)

  • The purpose of this study was to compare a pelvic tilt angle between sound side and affected side in hemiplegic patients and the changing affected pelvic posterior tilt angle was measured at intervals of 3, 6, 9 weeks after Bobath approach. The subject for the study were 10 hemiplegic patients(mean age of 54.1 years)without orthopedic disability on pelvic bone. The data were analyzed by t-test, one-way ANOVA. The results of this study were as follows. There was a significant difference in the pelvic tilt angle between sound side and affected side in hemiplegic patients. There was a significant difference in affected pelvic posterior tilt angle between pre-treatment and post-treatment(9 weeks).

  • Purposes: The purposes of this study were to investigate the relationship between the standing position of the subject and the normal standing position(NSP) and the straight standing position(SSP) and to investigate the possibility of different body shape test results depending on the status of the image inspection apparatus. Methods: The images of the NSP and SSP were compared with each other by body line BLS system. Results: At the time of examination, the position of the camera was captured at a position 2.3 m vertically from the posterior position 45 cm behind the subject. This is a privacy protection method for covering the breast of the subject. Results: The physiological characteristics of the anatomical position of the body align image test are the living body. NSP and SSP tests showed different shapes of the pelvis AS(antero-supero) and pelvis rotation in the transverse plane. Shoulder and arm displacement was observed in the trunk extension image capture. Conclusions: In the body alignment test, the pelvis position test images of NSP and SSP are evaluated differently for pelvis rotation, AS, and PS. At the extension position of the trunk, a test of the maximal extension range showed that the left and right shortening of the shoulder anterior muscles could be observed. Inducing and testing the trunk extension is also useful.

  • The purpose of this study was to survey the elderly patients through Basic information, Problems Wheelchair, Wheelchair Satisfaction, Usage and Requirements. The meaning of purpose is identify the problems and difficulties of wheelchair use in geriatric patients and an alternative for the correct use of wheelchairs elderly patients. The study investigated through questionnaires. That used questionnaires to modified to Gu Hyeon-Mo questionnaire (2005) and Quebec User Evaluation of Satisfaction with assistive Technology 2.0. 120 of 150 questionnaires were collected and one of them is excluded. Analysis results suggest the following. It was often feel joint pain results from physical damage. In addition when you use a wheelchair it was often feel discomfort in lower back and pelvis. And it feels uncomfortable when moving the tilt. Wheelchair satisfaction of the participants responded to the most 'Normal'. Results of the wheelchair operation ability is as follows. In the case of manual wheelchairs was feeling the difficulty to cross the threshold, Wheel lying, Stairs usage. Also In the case of an electric wheelchair was feeling the difficulty to use Tilt and Decline, Separate from the motor, and Replacement battery. In conclusion, To reduce of back pain and hip joints of elderly patients need to modify design and appearance of the wheelchair. In a later study will find to reduce discomfort and to improve satisfaction of the elderly who use wheelchairs. It will have to periodically examine the usage and satisfaction with them.

Effect of Dynamic Tubing Gait Training for Life-Care on Balance of Stroke Patients (라이프케어 증진을 위한 동적탄력튜빙 보행훈련이 뇌졸중 환자의 균형에 미치는 영향)

  • The present study investigated the effects of dynamic tubing gait (DTG II) program on the balancing ability for the promotion of life care of patients with chronic stroke. In the study, 25 sessions of DTG II program (30 minutes per session, 5 sessions per week, for a total of 5 weeks) were applied to 10 patients with chronic stroke. To determine the effects of DTG II program for improving balance, surface electromyography(external oblique, erector spinae, iliopsoas, gluteus maximus), symmetry index test on three pelvic axes, and dynamic gait index test were performed before and after the intervention. The results showed statistically significant differences between preand post-intervention measurements of the gluteus maximus muscle at early and mid-stance phases(p<.05). The pelvic symmetry index differed significantly between pre- and post-intervention measurements of diagonal and rotational movement(p<.05). Comparison of dynamic gait index also showed statistically significant differences between pre- and post-intervention measurements(p<.05). Based on these findings, it was determined that the DTG II program was able to improve the balancing ability of patients with chronic stroke by activating their trunk muscles and improving the symmetry of diagonal pelvic movement and rotation. Therefore, DTG II program is recommended as an interventional method to improve life-care through improving the balancing ability of patients with chronic stroke.

  • This study was carried out tn invstigate and compare biomechanical characteristics during free speed gait in hemiplegic patients after stroke who took therapeutic exercise by analyzing kinematic and kinetic data in the sagital plane and electromyographic data. Six patients($41\~69$ years old) and age-matched six volunteers in good health(51-61 years old) wire studied. The patients were sorted into two groups, depending on their self-speed of walking : fast speed group(3 patients) and slow speed group(3 patients). The results were as fellows. : 1. In spatio-temparal parameters, affected and unaffected side of fast group showed symetry but blew group showed asymetry of single limb support, opposite foot contact and stance phase (p<0.05). Compared with normal group, patient group showed slower velocity, shoter stride length and longer double limb support (p<0.05). 2. In the pelvic anterior tilt, patient group showed lower valued than normal group. It. In the ground reaction force-vertical force, fast group showed similar double peak gragh compared with normal group, butvslow group showed lower values without double peak (p<0.05).

The study on difference of gait asymmetry ratio according to static pelvic inclination level in hemiplegic patient (편마비환자의 정적 골반경사각에 따른 보행 비대칭율의 차이에 대한 연구)

  • Purpose: This study was performed to determine the difference or temporal-spatial gait asymmetry ratio according to static pelvic inclination level in hemiplegic patients. Methods: The subjects were 25 hemiplegic patients who was experienced stroke on MCA territory. Gait parameters and static pelvic inclination were recorded by $GAITRite^{(R)}$ system and radiologic PACS. The subjects were divided into three group according to static pelvic inclination. In the group I, the subjects have static pelvic inclination below $58^{\circ}$. The group II has $58^{\circ}{\sim}62^{\circ}$ or static pelvic inclination and the group m has over $62^{\circ}$ or static pelvic inclination. The data or three groups were analysed with ANOVA. Results: In comparison or single support time asymmetry ratio among 3 groups, the score or group II was significantly higher than the other groups(p<0.05). But the swing time asymmetry ratio was not significant(p>0.05). Conclusion: Asymmetry ratio of single support time was statistically significant by static pelvic inclination level. But asymmetry ratio or group II was the highest among three groups. It means that the patients or normal range of pelvic inclination was showed the most asymmetry or gait. And swing time asymmetry ratio was not significant among three groups. Even if the patient has normal ranged static pelvic inclination, it doesn't suggest that the patient has low gait asymmetry.

The Effect of Pelvic Tilt Exercise with Changing the Body Position on Foot Contact Pattern in Hemiplegic Patients (자세에 따른 골반경사운동이 편마비 환자의 발 접촉양상에 미치는 효과)

  • Purpose : The purpose of this study was to investigate the effect of pelvic tilt exercise with changing the body position on foot contact pattern in the hemiplegic patients. Methods : Thirty seven hemiplegic patients were randomly divided 3 groups; control group (CG), sitting exercise group (SIEG) and standing exercise group (STEG). F-mat system and F-scan system were used for the measurement of foot contact pattern of hemiplegic side in walking. Data were analyzed statistically using paired t-test and one-way ANOVA. Results : The results were as follows : 1) Contact area of CG and SIEG were not significant difference in walking. Contact area of STEG was significant increased in walking. 2) Anteroposterior distance of COP of SIEG and STEG were significant increased in walking. Conclusion : These results suggest that pelvic tilt exercise in sitting and standing position are effective in the improvement of Anteroposterior distance of COP and gait stability are increased in only standing position.

  • Objectives : The purpose of this study was to investigate the Correlation between Pelvic angle and the Tibiofemoral angle in the Genu varum. Method : Age of All subjects were 20-37 years. 33(male 10, female 23) subjects were assessed by whole spine x-ray and orthogram. The collected data consisted of items on general characteristics, Pelvic Incidence(PI), Pelvic Tilt(PT), Sacral Slope(SS) and Tibiofemoral Angle(TFA) of the subjects. Results : The PI, PT and TFA had a relationship significantly. However, there was no relationship between SS and TFA. Conclusion : The results suggest that TFA is related to PI, PT. Internal rotation due to genu varum of femoral at hip influence that PI and PT increase.

  • Objectives : To evaluate the effect of foot orthotic in patient with chronic pain and pelvic obliquity in standing. Methods : Four cases of functional spinal scoliosis, were investigated for the changes in the calcaneal stance position angle, pelvic height, pelvic angle, Cobb's angle and walking pattern. Standing full spine X-ray for measuring the pelvic height, pelvic angle, Cobb's angle were checked before and after application of foot orthosis. The foot orthosis was composed of polyprophylen and chamude cover. Results : 1. There was no change in resting calcaneal stance position. 2. Difference of pelvic height and pelvic angle was reduced after application of a foot orthosis. 3. Cobb's angle in 2 cases was reduced after application of a foot orthosis. 4. Walking balance was improved. 5. Visual analogue scale was decreased. Conclusions : The study showed that foot orthosis seemed to be effective for chronic pain, spinal curve, pelvic obliquity and walking balance.

The Influence of Pelvic Tilt Exercise Using Visual Feedback upon the Gait Characteristics of Patients with Hemiplegia (시각적 되먹임을 이용한 골반경사 운동이 편마비 환자의 보행특성에 미치는 영향)

  • The purpose of this study was to investigate the influence of anterior, posterior, and lateral pelvic tilt exercise upon the gait characteristics of patients with hemiplegia including their gait velocity, cadence, stride length, step length of the non affected side, step length of the affected side, foot angle of the non affected side, foot angle of the affected side, base of support, and so on. The subject of this study was 24 men and women patients with hemiplegia. The patients, the subject of this study, were classified into 12 patients of treatment group applying pelvic tilt exercise using visual feedback and 12 patients of control group applying general pelvic tilt exercise, and then analyzed their gait before and after exercise. Temporal distance gait analysis (Boening, 1977) was used to analyze their gait, and ink foot-print was applied as on of measurement methods. To find out meaningful difference between control group and treatment group, this study carried out independent sample t-test for each item by utilizing SPSS/Win 10.0, compared changes in control group's and treatment group's gait characteristics before and after exercise as percentage, and applied paired t-test to conduct before-after test in same group. Statistical significance level was ${\alpha}$ < 0.05. The results of this study were as follows. As a result of independent sample t-test for control group's and treatment group's gait characteristics after exercise, it was not statistically significant so there was no meaningful difference between two groups. However, it was statistically significant in the change rate(%) of gait characteristics, and treatment group's patients with hemiplegia had been highly changed in gait characteristics in comparison with control group. From the above-mentioned results, could find that pelvic tilt exercise using visual feedback for patients with hemiplegia was effective to improve their gait ability and it could increase the ability in comparison with general pelvic tilt exercise. In the future, studies on the effect of pelvic tilt exercise using visual feedback shall be continued based on more quantitative methods.
