Development and Evaluation of a Community Staged Education Program for the Cardiocerebrovascular Disease High-risk Patients (심뇌혈관질환 고위험군을 위한 지역사회 단계별 교육프로그램 개발 및 효과 평가)

  • Objective: This study was conducted to evaluate an education program for cardiocerebrovascular high-risk patients. Methods: This program was developed according to Tyler's model for curriculum development. To evaluate the effects of this program, we measured clinical outcome change (weight, waist circumference, systolic blood pressure, diastolic blood pressure) and behavior change stages (checking blood pressure, blood sugar levels, doing physical activity, consistent maintenance of food intake, eating low amounts of salt, abstention from tobacco and alcohol) before and 4 weeks after participation in the education program. The group of subjects consisted of High-risk group patients who attended basic program(32 patients), and staged program(37 patients) during KHyDDI meetings from Oct. 2009 to May 2010. Results: The staged educational program was developed three aspects(disease, nutrition and exercise)and three stages(basic, in-depth and individual education). In the staged education program, the evaluations were made by measuring clinical outcome and stage of behavior before and after education. Significant differences were found in waist circumference, systolic blood pressure, diastolic blood pressure, consistent maintenance of food intake(p<0.05), and eating low salt(p<0.001)and their self efficacy. Conclusion: In the practice-oriented staged education program, significant differences were found in the clinical outcomes and stage of behavior before and after education. Possible limitations of the study include the small number of participating subjects and the short follow-up management period, but the results indicate that continued application of this program could contribute to the prevention of cardiocerebrovascular diseases for the elderly patients with long periods of chronic diseases.

  • Purpose: This study was to develop and prove the effects of aself management compliance promotion program for primary hypertension patients who reside in rural communities. Method: The content of the self management compliance promotion program developed by this study was as follows: A leader trains patients as a group or individually, in walking, education and green tea therapy from the first to twelfth week. From the thirteenth to twenty fourth week, the patients should perform walking and green tea therapy by themselves. One hundred twenty subjects volunteered to participate in the study, who were among those registered as hypertension patients in the 14 community health clinics located in Chungcheongbuk-do. Result: Systolic blood pressure, total cholesterol, low-density lipoprotein cholesterol, triglyceride, step width, and degree of obesity decreased significantly. High-density lipoprotein cholesterol, step length, knowledge of hypertension, and self management compliance significantly increased, Conclusion: A self management compliance promotion program for primary hypertensive patients enhances biophysical index and knowledge on hypertension, thus ultimately suggesting a nursing intervention for promoting self management compliance.

Effects of Professional Oral Care for Long-term Patients in Nursing Facilities on the Streptococcus mutans Population in the Intraoral Region (전문가구강관리가 요양병원 장기입원환자들의 구강 내 Streptococcus mutans 수에 미치는 영향)

  • In this study, professional oral care was provided and 3 times a day twice a week by a dental hygienist for 3 months from July 24 to October 25, 2011, to 43 long-term patients in a single nursing facility located at the city of D, and the number of mutans strepococci colonies within the saliva and the level of salivation were then analyzed after 3 samplings with an interval of 1 month to compare the intraoral conditions among the subjects and examine the effects of professional oral care on the Streptococcus mutans population in the intraoral region of long-term patients in a nursing facility. The level of salivation was elevated from 5.8 ml to 6.4 ml after one month and to 7.5mL after two months. The population of bacteria decreased from $6{\times}10^8CFU/ml$ to $3{\times}105CFU/ml$. In terms of the change in the bacterial population in accordance with systemic diseases, the patients with hypertension and diabetes showed a meaningful decrease in the population. As more dental charge and prosthesis are present, the number of bacteria decreased significantly. On account of the close relationship between a professional oral care and the population of Streptococcus mutans at the intraoral region of long-term patients in nursing facilities, various dental health programs should be researched and developed to consider the characteristics of long-term patients in nursing facilities and manage them continuously but effectively.

  • Purpose: The purpose of this study was to identify and profile distinct subgroups of patients with hypertension based on knowledge, attitudes, and behavior regarding their medication and health lifestyle. Method: A descriptive-exploratory research design was employed. Two hundred and twenty-three patients with hypertension using W Public Health Center were randomly recruited on the basis of being over 30 of age. Upon the receipt of their written consents, direct interview with a structured questionnaire were conducted by a public health nurse. Descriptive statistics and $X^2-test$ were utilized. Results: Three subgroups were identified. Group I members had a low score on all three factors including knowledge, attitudes and behavior related to medication and health lifestyle. Group II members had a low score on one or two of the factors. Group III members had a high score on all three factors. The three subgroups were significantly associated with education level, economic status, non-smoking and non-drinking. Conclusions: Further research should be conducted to validate these findings and test tailored nursing intervention for patient compliance.

  • This study focuses on analyzing no-shows associated with patients' clinical characteristics described by diagnoses in their medical data. A dataset of 7,055 patient-records from a Veterans hospital in the United States was used to test if there is difference on no-shows along with each patient's diagnosed diseases and the number of diagnoses. Patients with mental diseases such as drug dependence abuse and major depression, and chronic diseases such as hypertension are more likely to no-show. In comparisons with the number of diagnoses, the no-show decreases as the number of diagnoses increases up to four and doesn't change significantly afterwards. We provide managerial insights on clinical operations problems from statistical analysis. We believe that our results can be used to develop appropriate solutions on no-shows in clinics.

Convergence Factors Related to Glycemic Control in Workers with Diabetes Mellitus : using the Korean National Health and Nutrition Examination Survey, 2009-2013 (직장인 당뇨병 환자의 혈당조절 정도와 융합적인 영향요인 분석 : 2009-2013 국민건강영양조사를 바탕으로)

  • The purpose of this study was to identify convergence factors related to glycemic control in workers with diabetes mellitus. A secondary data analysis was conducted using the data of "The Korea National Health and Nutrition Examination Survey, 2009-2013." The survey included 44,085 adults aged over 19 under 65 years with diabetes mellitus and 764 samples with worker were used for this study. Data analysis was conducted using SPSS 18.0 program and descriptive statistics, Pearson's correlations, and multiple regression analyses were performed. The general characteristics variables shown statistically significant difference between the good and the poor glycemic control group was gender. The Health behavior-related and disease-related variables shown statistically significant difference between the good and the poor glycemic control group were diabetes mellitus duration, diabetes mellitus treatment, hypertension diagnosis, smoking, moderate physical activity, walking exercise practice. Factors related to glycemic control were gender, diabetes mellitus treatment, walking exercise practice. These findings suggest that researchers need more active treatment and walking exercise as important factors affecting glycemic control of Korean worker with diabetes mellitus and intervention focusing on the issues needs to be developed in workplace environment.

Literature Review of Empowerment Intervention Studies for Patients with Chronic Physical Health Conditions in Korea (만성적인 신체질환을 가진 환자에게 적용된 임파워먼트 중재 연구 고찰 - 국내를 중심으로)

  • This study was a systematic review examining the effect of empowerment intervention studies among patients with chronic physical health conditions in Korea. The PICOT-SD (Participants, Intervention, Comparison, Outcomes, Timing of outcome measurement, Setting, Study design) strategy was applied. A total of 19 studies published between January 2000 and April 2020 were selected. Study participants were rheumatoid arthritis patients, hemodialysis patients, stroke patients, diabetes mellitus patients, chronic pulmonary disease patients, hypertensive patients, cancer patients, chronic kidney disease patients, and kidney transplantation patients, etc. A total of 17 studies were found to have presented a conceptual framework. Empowerment skills were participation, dialogue, experience, interaction, support, and problem solving, etc. The most frequently used dependent variables were self-care (n=13), empowerment (n=10), self-efficacy (n=7), and depression (n=5). Self-care, empowerment, and self-efficacy showed significant improvement among patients in at least 60% of the included studies. Based upon this review, there is evidence that empowerment intervention studies are an effective method to improve the health of patients with chronic physical health conditions. This study further suggests the development of an empowerment intervention program to verify the effects of the various variables identified in this study.

A Hospital-based Case-control Study on the Risk Factors of Cerebrovascular Disease (뇌혈관질환의 위험요인에 대한 환자 - 대조군 연구)

  • A hospital-based, matched case-control study was carried out to evaluate the relation ship of various suspected risk factors including snoring and serum level of cholesterol to cerebrovascular disease in Korea. A total of 127 incident cases of cerebrovascular disease(74 cases of cerebral infarction and 53 cases of intracerebral hemorrhage) admitted to. the department of neurology in a university hospital from December, 1993 to March, 1995 were compared with 127 matched controls admitted to same hospital in same period. A multivariate analysis suggested that ECG abnormality(left ventricular hypertrophy and atrial fibrillation), family history of cerebrovascular disease, fundoscopic abnormality, previous history of transient ischemic attack and hypercholesterolemia were risk factors of cerebrovascular disease, ECG abnormality, fundoscopic abnormality, smoking and hypercholesterolemia were also suggested as risk factors of cerebral infarction.

  • The obesity is a medical condition in which excess body fat has accumulated to the extent. It may have an adverse effect on health, leading to reduced life expectancy and increased health problems such as lifestyle disease, hypertension, and diabetes. Furthermore, it may cause the mental illness such as depression, bipolar disorder, anorexia, hyperphagia, and so on by shrinking psychologically. So, it is regarded as the serious worldwide social problem. Recently, although there are many general obesity care methods with various approaches, these can't be the radical obesity care because of the side effects such as surgery aftereffect, drug addiction, and so on. For these reasons, a methodical approach with a consideration of individual obesity pattern is strongly demanded. In this study, complex obesity care and management system which is overcome the weakness of non-surgical obesity treatment is proposed. And the individually adapted obesity treatment system with the far-infrared therapy module, oxygen therapy module, color therapy module, and microwave therapy module was developed. The complex obesity treatment system which is proposed in this study, could improve the efficiency of treatment by suggesting the various treatment protocols. And proposed system could be the foundation of the high quality obesity treatment system.
