A Review of Self-Monitoring of Blood Pressure for Self-Management of Hypertension (고혈압 환자의 혈압자가측정에 관한 문헌고찰)

  • 연구목적: 고혈압환자의 효과적인 자가관리 간호중재를 개발하는데 필요한 기초자료로 혈압자가측정(self-monitoring of blood pressure)에 대한 특성, 장점, 범위 및 제한점을 파악한다. 연구방법: 1996년부터 2005년까지 PubMed에 수록된 고혈압환자의 자가관리 방법중 혈압자가측정방법이 포함된 외국의 연구를 중심으로 체계적인 고찰을 실시하였다. 전체 69개 자가관리방법을 적용한 논문 중에서 혈압자가측정이나 가정에서의 혈압측정(home monitoring)에 대한 45개 논문을 선정하여 분석하였다. 연구결과 전반적으로 혈압자가측정 방법은 고혈압환자의 혈압조절을 향상시키는 효과가 유의한 것으로 나타났다. 환자가 혈압을 자가측정한 방법에서 평균 혈압, 수축기 혈압, 맥압이 유의하게 감소한 것으로 보고되었다. 장기적 프로그램에서 혈압조절율이 향상되거나, 이완기혈압의 감소효과도 보고되었다. 혈압자가측정의 기준치는 연구마다 일치하지는 않았지만 135/85mmHg 수준이 정상혈압의 상한수준으로 제시되었다. 일부 연구에서는 가정 내 혈압측정이 24시간혈압측정을 대체하는 방법으로 추천되었다. 보다 정확한 혈압자가측정을 위해서 신뢰성 있는 혈압측정기구의 사용과 정확한 측정방법이 환자들에게 교육될 필요가 있다. 결론: 혈압자가측정은 고혈압환자의 혈압조절에 유용한 방법으로 고찰되었다. 또한 고혈압의 진단, 혈압치료와 고혈압 약물복용 행위를 향상시키는 교육적 효과도 긍정적으로 나타났다. 따라서 고혈압환자를 위한 효과적인 자기조절프로그램에 혈압자가측정법을 포함하는 것이 추천되며, 추후 한국에서 시도된 관련 연구에 관한 문헌고찰의 비교연구가 필요하다.

The effect of telephone counseling hypertension self-management program for hypertensive patients with disabilities using the welfare center for the disabled (장애인 복지관을 이용하는 장애인 고혈압 환자 대상 전화상담 고혈압 자가관리 프로그램의 효과)

  • The purpose of this study was to identify the effectiveness of the 8-week telephone counseling hypertension self-management program provided through welfare center nurses and nursing student volunteers for hypertensive patients with physical disabilities and brain lesions who use the welfare centers for the disabled. A quasi-experimental with non-equivalent control group pretest-posttest design was implemented. From December 2019 to May 2020, data from 28 controls and 29 experimental groups who completed pre and post-investigation were analyzed using the SPSS program. As a result of the main study, after the intervention, the knowledge of hypertension, seriousness and sensitivity among the health beliefs, and quality of life EQ-VAS indicated significant difference between the experimental group and the control group. Therefore, efforts to improve self-care behaviors and quality of life by applying various self-care programs to hypertensive patients with the disabled which require long-term health care will be needed continuously.

Effects of an Experiences Sharing Self-management Program for Hypertension and Diabetes Patients in Primary Public Health Center (보건진료소 중심 고혈압, 당뇨병 환자의 경험공유 자가관리 프로그램 효과)

  • This study examined the effects of an Experience Sharing Self-management Program (ESSP) on physiological indexes, knowledge, and self-management implementation among hypertension and diabetes patients in a primary public health center in G city. A quasi-experimental research was conducted with a non-equivalent control group pretest-posttest design. The experimental group (n=30) participated in a 12-week ESSP, and the control group (n=25) received only general services of a primary public health center between May 1 and October 31 in 2018. Data were analyzed with x2 test (Fisher's exact test), Mann-Whitney test, and Ranked ANCOVA using SPSS/WIN 22.0 program. The results showed significant differences in systolic blood pressure (F=14.56, p<.001), diastolic blood pressure (F=7.05, p=.012), knowledge (F=14.30, p=.001), self-management implementation (F=29.73, p<.001) of hypertension patients, knowledge (F=7.92, p=.010), and self-management implementation (F=4.54, p=.044) of diabetes patients between the two groups. Therefore, the ESSP was effective for reducing the blood pressure of hypertension patients and improving the knowledge and self-management implementation among hypertension and diabetes patients in a primary public health center.

  • 고혈압과 같은 만성질환 환자들은 약물치료 외에도 운동, 식습관 조절등과 생활습관 개선을 위해 꾸준한 자가관리가 필요하다. 본 논문은 최근 대중적으로 보급되고 있는 스마트폰을 통해 이러한 고혈압 환자들의 자가관리를 돕기 위한 프로그램을 개발하고자 한다.

Effects of Self Care Program on Hypertensive Control in Hypertensive Patient (고혈압환자에게 적용한 자가관리프로그램 중재 효과)

  • This study was conducted to estimate the effects of self-care program on knowledge and symptoms related hypertension self-care and physiological index in essential hypertensive patients aged between 35-74 year. The subjects for the experiment group and the control group of this study were 70 men and women selected through random sampling from adults at Sangju Red Cross Hospital in Gyeongsanbuk-do, and the experiment was carried out during the period from the 15th of September to the 30th of April in 2002. This study measured systolic and diastolic blood pressure (SBP, DBP, the mean value of the two measures) and total cholesterol (TC) and surveyed the subjects' diet and life style in relation to hypertension using a self-report questionnaire. In order to study the significance of the effects of self-care program, the author carried out t-test, paired t-test, ANCOVA, chi-square analysis and effectiveness index (EI) analysis. Results of the study are as follows: The experiment group got higher mean scores than the control group in the degree of low sodium intake and the degree of high calcium and high potassium intake, and the difference was statistically significant(P<0.05). The effectiveness index of the self-care program in smoking was 0.797 at the 1st posttest and 0.601 at the 2nd posttest, and in physical activities 0.600 at the 1st posttest and 0.849 at the 2nd posttest. The rate of regular antihypertensive drugs intake of the experimental group was higher than that of the control group. and the effectiveness index of the self-care program was 0.715. The mean score of the systolic blood pressure of the experimental group was lower than that of the control group, and the difference was statistically significant(P<0.05). In conclusion, these findings support usefulness of self-care programs in reducing systolic blood pressure and in promoting self-care related to diet and life style for treating and preventing hypertension.

  • This study described hypertensive patients characteristics associated with a health incentive program, and analyzed the effects of the health incentive program on blood pressure control. Based on the Incheon Chronic Disease Management System (iCDMS), a community partnership program for hypertensive patients, a cross-sectional design was used. There were statistically significant differences between the control group (n=17,278) and the experimental group (n=5,020) in gender, age, points earned, and number of hospital visits. Furthermore, being male, being over 60 years of age, and points earned were statistically significant factors associated with blood pressure control. To increase the effectiveness of chronic diseases management systems, various health incentive programs will be developed and introduced.

  • Purpose: This study was to examine the effects of the hypertension self-help program on knowledge, self-efficacy, self-management compliance, and physiological parameters for workers with hypertension. Methods: The subjects of study were 54 patients with hypertension, divided into 28 of experimental group and 26 of control group, working in a general work place located in K city from June to September, 2009. Experiment treatment was a 12-week self-care program given for two hours, once a week. Results: After conducting a self-help program, the experimental group showed significant increase in knowledge level, self-efficacy, self-management compliance, and physiological parameters including blood pressure, BMI, total cholesterol level, neutral fat, and LDL-cholesterol of blood lipids compared to the control group. Conclusion: This program is greatly recommended for workshops.

Evaluation of Hypertension Prevention and Management Programs Conducted by Community Health Nurse Practitioners in Rural Areas of Korea (농촌지역 보건진료소의 고혈압 예방 및 관리사업 평가)

  • Objectives: This study aims to evaluate effectiveness of hypertension prevention and management programs (HPMPs) in rural Korea. Methods: A questionnaire survey was conducted with 700 community health practitioners working at primary health care posts (PHCPs). The questionnaire had four domains, which were hypertension management, hypertension prevention, the PHCP environment, and evaluation system, each with different maximum weighted values (WVs). Weighted values of 100 indicate the best effectiveness for the HPMP in all four domains. Results: The average WVs and percent effectiveness of all four domains combined was 61.65 (62%); the hypertension management domain was 28.81 (72%); the hypertension prevention domain was 23.44 (67%); the PHCP environment was4.29(43%); and the evaluation system was5.10(34%). Conclusion: The HPMPs were generally effective, and hypertension management was the most effective. The environment of PHCPs and the evaluation system of HPMPs should be improved to increase the effectiveness of the HPMPs.

A Study on Hypertension Management of Community Health Practitioner Posts (보건진료소 고혈압 관리사업의 실태)

  • Objectives: The purpose of this study is to provide data for the improvement of hypertension management of community health practitioner posts through the study on hypertension management in community health practitioner posts. Methods: A questionnaire was mailed to 700 community health practitioners and 205 of them responded during the period from March 13, 2003 to May 13. The survey results were analyzed using SPSS program, version 11. Results: The results are as follows; 1. There are two major activities in a hypertension prevention project for community: health education and early detection. About 57% of community health care practitioners perform a health education for community people four times a year. The 64.5% of them used the materials for health education provided from a community health center and 22.1% of them performed a post-evaluation. The main method of early detection of hypertension was measurement of blood pressure of person to visit, which was 96.1%. Other methods included home visiting(89.3%), a referral from community hospitals and other resources(49.1%), health promotion events(39.5%), and a review of medical records(35.7%). 2. For the registration and management of patients with hypertension, about 36% of community health centers used a special form and more than 50% of them have registered patients who were managed by other health care institutions in the community. A computerized program was used for the management of patients with hypertension in 68.5% of them. More than 60% of them responded that it was used for report, treatment, and follow-up of patients with hypertension.

The Effects Walking Exercise Program on Blood Pressure as a Related Indicator for Aged Hypertension Patients in Rural Areas (농촌지역 고혈압 노인에서 걷기 운동 프로그램이 혈압관련지표에 미치는 효과)

  • Purpose: This purpose of this study was to examine the effect of a walking exercise program on blood pressure and related factors in older patients with hypertension living in rural areas. Method: The participants were 233 patients with hypertension, 60years or age or over, who were registered at one of 36 public health centers in North Chung Cheong Province and agreed to participate in this study. The program was implemented for 12 weeks from Dec. 2005to Feb. 2006. The data were collected before and right after the program and paired t-test was used to analyze the data. Analysis was done with the SPSS program. Result: Systolic pressure (t=7.460, p=0.000) and diastolic pressure (t=5.309, p=0.000) decreased significantly. Total cholesterol l (t=-9.991, p=0.000), LDL cholesterol and HDL cholesterol (t=-3.180, p=0.000) all increased significantly. There was no change in weight or obesity level. Blood sugar (t=2.218, p=0.028) decreased significantly. Knowledge of hypertension increased significantly as did the self-care behavior of these patients with hypertension. Conclusion: The walking exercise program was found to be effective to control blood pressure in older patients with hypertension in rural areas and to help HDL cholesterol, knowledge of hypertension, and self-care behavior of hypertension increase greatly, while blood sugar level decreased.
