Sporadic reports of hypernatremic dehydration in breastfed newborn infants have appeared in medical literature for at least 3 decades. We report the first case of hypernatremic dehydration resulting from inadequate breast-feeding in Korea. A 14-day old baby, born to a mentally retarded mother, was transferred to our hospital with a body weight loss of 460 g since birth(17%) and a serum sodium(Na) level of 179 mEq/L, after initial hydration at another hospital. On admission, a cardiac murmur was heard and an enlarged liver was palpated. Cardiac ultrasonogram revealed ventricular septal defect and ostium secundum atrial septal defect. During hydration, a seizure-like attack developed. Serum Na decreased to 135 mEq/L on the 5th day of admission. Brain ultrasonography and brain magnetic resonance image revealed no remarkable abnormalities. Electroencephalography was normal. She suffered from prerenal azotemia, hyperglycemia and disseminated intravascular coagulation at admission but was treated successfully. Heart failure was also controlled with dobutamine, diuretics and digoxin.
Purpose : This study investigated the clinical significance of AN in children and adolescents with obesity induced metabolic complications. Methods : Forty-nine patients who had obesity induced metabolic complications were participated in this cross-sectional study. Obesity induced metabolic complications are as follows: hypertension, dyslipidemia, impaired fasting glucose (IFG), impaired glucose tolerance (IGT), nonalcoholic steatohepatitis (NASH), homeostasis model assessment of insulin resistance (HOMA-IR)>3.16. Clinical characteristics, such as, age, percentage-weight-for-height (PWH), pubertal status, blood pressure (BP), fasting plasma insulin level, fasting and post-oral glucose tolerance test 2-hour glucose levels, liver function test, lipid profile, HOMA-IR were compared according to the presence of AN. Results : Sixty-five percent of patients had AN, 57.1% NASH, 57.1% dyslipidemia, 55.1% hypertension, 46.9% IFG, 24.5% HOMA-IR>3.16 and 16.2% IGT. The patients who were moderately to severely obese with AN had higher incidence of IGT and HOMA-IR>3.16. The patients with AN had significantly higher diastolic BP ($79.4{\pm}6.9$ vs $75.4{\pm}5.6mmHg$), fasting levels of plasma insulin ($10.6{\pm}6.0$ vs $6.2{\pm}5.4{\mu}IU/mL$), HOMA-IR index ($2.6{\pm}1.4$ vs $1.4{\pm}1.3$) and PWH ($42.4{\pm}13.0$ vs $34.3{\pm}1.8%$). The increasing tendency for the presence of AN was significantly related to the cumulative number of obesity induced metabolic complications. Binary logistic regression analysis revealed that the presence of AN was significantly associated with fasting plasma insulin level, PWH and IFG. Conclusion : AN could be useful as a clinical surrogate of obesity induced metabolic complications.
The objective of this study was to investigate whether betulinic acid can inhibit the activities of carbohydrate-digesting enzymes and reduce postprandial hyperglycemia in mice with streptozotocin-induced diabetes. Our results revealed that betulinic acid has potent inhibitory effects on α-glucosidase and α-amylase activities. The half-maximal inhibitory concentrations (IC50) of betulinic acid were 12.83±6.81 and 18.32±3.24 μM for α-glucosidase and α-amylase, respectively. This result indicates lower IC50 values and higher inhibitory activities than those of acarbose, an oral hypoglycemic drug. The increase in postprandial blood glucose levels was significantly suppressed in the betulinic acid group than in the control group of diabetic and normal mice. Postprandial blood glucose levels were 23.22±1.1, 24.38±1.31, and 21.05±1.36 μM in the betulinic acid group compared to 24.64±1.7, 27.22±1.58, and 26.36±1.40 μM in the control group of diabetic mice at 30, 60 and 120 min, respectively. The area under the curve also significantly decreased with the administration of betulinic acid in diabetic mice, however, it did not decrease more than that after acarbose administration. Our results showed that betulinic acid may be a potent inhibitor of carbohydrate-digesting enzymes and ameliorate postprandial hyperglycemia in diabetic mice.

The hepatic glycogen storage disease type 0 (GSD type 0) is an autosomal recessive disorder caused by a deficiency of hepatic glycogen synthase encoded by the glycogen synthase 2 (GYS2) gene, leading to abnormal synthesis glycogen. The clinical findings of GSD type 0 are hyperketotic hypoglycemia at fasting state and accompanying postprandial hyperglycemia and hyperlactatemia. GSD type 0 has only been reported in a very small number so far, and the diagnosis is likely to be missed because symptoms are mild, severe hypoglycemia is rare or asymptomatic, or symptoms gradually disappear with age. Essential management strategies include feeding high-protein meals to stimulate gluconeogenesis, frequent meals to prevent hypoglycemia during the day and feeding complex carbohydrates such as uncooked cornstarch to slowly release glucose during nignt. GSD type 0 has a good prognosis, with appropriate treatment, normal growth can be achieved and no complications occur. Significant hypoglycemia occurs less common in adulthood, but ongoing dietary management may be necessary.

The present study was undertaken to investigate the hypoglycemic effect of Hamcho yogurt fermented by lactic acid bacteria containing Hamcho (Salicornia herbacea) extract powder in streptozotocin(STZ)-induced diabetic rats for 3 week period. The Hamcho yogurt given to the STZ-diabetic rats had significant effect in lowering the serum glucose concentrations and food intakes compared to the STZ-diabetic rats. The Hamcho yogurt administration increased the tolerance to oral glucose loading in the STZ-diabetic rats. The Hamcho yogurt and yogurt given to the STZ-diabetic rats were significantly decreased the serum total cholesterol concentration and the atherogenic index. Based on the beneficial effect against the blood glucose and cholesterol levels, we suggest the Hamcho yogurt can be used as functional food for the hypoglycemic and hypocholesterolemic effects.
This study was conducted to identify the factors affecting sarcopenia in middle-aged and older women in Korea. This is a cross sectional study by using 5th Korea National Health and Nutrition Examination Survey data. The data were analyzed by using the SPSS 26.0 program. The prevalence of sarcopenia was 10.8%. There were significant differences according to ASM, age, education, current job, marital status, perceived health status, energy intake, protein intake, BMI, body fat, alcohol drinking, menopause, central obesity, impaired fasting glucose and chronic disease. Woman with central obesity and osteoarthritis increased risk of sarcopenia each 4.15 times(p<.001), 3.06 times(p=.041) and energy intake decreased risk of sarcopenia 0.99 times(p=.043). In order to prevent and manage sarcopenia, strategies for managing central obesity and osteoarthritis in mddile-aged and older woman and adequate food intake are needed.

This study was conducted to examine the effects of $\beta$3-adrenergic receptor polymorphism on the blood glucose level and obesity in 530 volunteers, who attended a weight loss program in a local obesity clinic. The age differences in total subjects and the distribution of male and female were 26.55$\pm$0.31 yr, 9.1% (n=48), 90.9% (n=492). The genotype distribution of $\beta$3-AR gene polymorphism were WW type 75%, WR type 22% and RR type 3%. Among many parameters, fasting blood glucose was significantly higher in WR+RR type (p=0.001) compared with WW type. When the subjects were divided into two groups by 6.105 mmol/L of the fasting blood glucose level, the frequency of hyperglycemia was 23.3% in WW type subjects, while there was a increase to 35.6% in WR+RR type subjects (p=0.011, $\chi$$^2$-analysis). When hyperglycemia group was compared with normoglycemia group, obesity index (p=0.044), %body fat (p=0.046) and TG (p=0.000) were significantly higher, and HDL (p=0.006) was significantly lower in the hyperglycemia. When all of the above factors were included in stepwise logistic regression analysis to find risk factors of hyperglycemia, the results were that the odds ratio for hyperglycemia were 2.015 (p=0.011) for WR+RR type of $\beta$3-AR gene, 2.165 (p=0.000) for TG and 0.419 (p=0.059) for HDL cholesterol. There was a significantly positive correlation between the blood glucose vs BMI, WHR, body fat in the WW type (r=0.099, 0.119, 0.082) However, in the WR and RR type there were no significance between the blood glucose vs BMI, WHR, body fat. These data suggest that the WR+RR genotype of $\beta$3-AR has a very strong association with increased blood glucose level and might be a significant risk factor for hyperglycemia among Korean subjects.
The purpose of the study was to investigate the effectiveness of self-paced walking exercises in elderly women with hypertension, hyperglycemia, and hypercholesterolemia. Thirty-eight elderly women with hypertension, hyperglycemia, and hypercholesterolemia (16 hypertension, 11 hyperglycemia, 11 hypercholesterolemia), aged between 65 and 80, were invited to participate in this study. Each subject participated in a self-paced walking exercise five times a week for twelve weeks from 26 June to 16 September 2006. The changes between pre- and post-exercise were analyzed using the analysis of a paired t-test with the SPSS version 12.0 package program. There were significant decreases in systolic blood pressure (p<.01) and diastolic blood pressure (p<.05), blood glucose(p<.05), and cholesterol in the blood (p<.01). These results show that self-paced walking exercises may be helpful in treating elderly women with hypertension, hyperglycemia, and hypercholesterolemia.
Purpose: The purpose of this study was to examine the effect of a blood glucose control protocol for medical intensive care unit (ICU) patients. Methods: The subjects were recruited from medical ICU adult patients whose blood glucose levels exceeded 200 mg/dL in two consecutive tests. The experimental group (n=62) received the modified Yale (MY) insulin protocol, whereas the control group (n=64) was treated with the conventional insulin therapy methods. Results: In the experimental group, the mean blood glucose levels (p<.001) and the time to reach the target range of glucose (p<.001) decreased significantly while the incidence rates of a target range of glucose of 100-140 mg/dL (p<.001) increased significantly as compared to the control group. However, no statistically significant differences were found in the incidence of hypoglycemia(p=.644), or the number of glucose tests (p=.236) between the groups. The length of stays in the ICU (p=.001), ventilator care days (p=.038), and the Sequential Organ Failure Assessment (SOFA) score (p=.029) in the experimental group were significantly lower than those of the control group. Conclusion: Application of the protocol was effective in improving the state of blood glucose control in medical ICU patients. Therefore, this protocol is expected to be used as a part of nursing intervention in critical care nursing.
