  • Bimaxillary Protrusion can be treated effectively in growing patients and in adults with conventional orthodontic therapy. However, In the adult patient, combined surgical and orthodontic treatment modalities may offer distinct advantages over such conventional therapy. In those cases complicate by vertical jaw dysplasia, sagittal dysplasia, or transverse skeletal discrepancy in addition to bimaxillary protrusion, the possibilities of obtaining successful results through orthodontic treatment alone greatly diminish. Surgical retraction of both maxillary and mandibular anterior segments with subapical osteotomies and ostectomies in the extraction site may be a good treatment alternative. Treatment time and possible adverse effects of lengthy orthodontic therapy may be reduced and optimum esthetic improvement may be facilitated. On the following cases, patient who had bimaxillary protrusion with Angle class III malocclusion was treated with combined orthodontic - surgical therapy by anterior subapical segmental osteotomies.

  • The purpose of this study is to evaluate the effects of facemask therapy in patients with Class III malocclusion with two types of bonded expanders covering different numbers of anchored teeth and to compare the anchorage value of two types of bonded expander. Eighteen subjects with Class III malocclusion in early mixed dentition were included in this study, and subjects were divided into two groups based on the number of teeth covered by bonded expander: group 1 (splinting four teeth on each side, 9 subjects) and group 2 (splinting three teeth on each side, 9 subjects). Lateral cephalograms were obtained and assessed before (T1) and after (T2) the treatment. The facemask therapy showed skeletal effects including anterior movement of maxilla and backward rotation of mandible in both groups, with no significant differences between groups. Mesial movement of maxillary molars which indicates anchorage loss of the bonded expander was found in both groups, but significantly larger mesial movement was found in group 2 than in group 1. In conclusion, the value of anchorage was different according to the number of teeth covered by bonded expander as an intraoral anchorage of facemask, but there were no significant differences in skeletal effects.

  • Tweed-Merrifield directional force technology is a very useful concept, especially for the treatment of Glass II malocclusion. It has contributed to treating a favorable counter-clockwise skeletal change and balanced face, while head gear force using high pull J-hook (HPJH) in an appropriate direction is also essential to influence such results. Clinicians have encountered some problems concerning patients' compliance; however skeletal anchorage has been used widely of late because it does not necessitate patients' compliance, yet produces absolute anchorage. In this case, a good facial balance was obtained by Tweed-Merrifield directional force technology using HPJH together with skeletal anchorage, which provided anchorage control in the maxillary posterior area, torque control in the maxillary anterior area, and mandibular response. This indicates 4hat skeletal anchorage can be used to reinforce sagittal and vortical anchorage in the maxillary posterior area during the retraction of anterior teeth. The author used HPJH for torque control, Intrusion, and the bodily movement of maxillary anterior teeth during on masse movement. However, it is thought that such a result nay also be achieved by substituting mini- or microscrews for HPJH. Consequently, Tweed-Merrifield directional force technology using skeletal anchorage for the treatment of Class II malocclusion not only maximiaes the result of treatment but can also minimize patients' compliance.
