배경 :경동맥 내막 절제술의 목적은 뇌졸중 예방에 있다. 경동맥 내막 절제술시 경동맥 혈류를 차단하였을 때 뇌허혈 상태를 초래하는지 가 가장 중요한 문제이다. 경동맥 혈류 역류압은 뇌내 측부혈류 상태를 반영하므로 경동맥 혈류 차단시 역류압과 뇌파검사 소견에 딸라 shunt 삽입여부 기준을 알아보려고 하였다 대상 및 방법 : 1996년 2월부터 1999년 3월까지 경동맥 내막 절제술을 시행받은 16명을 대상으로 하였다 남자가 14명있고 여자가 2명이었으며 평균연령은 66.35$\pm$6.53이었다 수술부위 경동맥 협착은 평균 73.8$\pm$12.33%였고 반대측 경동맥 협착은 평균 60.99$\pm$23.03%였다. 수술중 모든 환자에서 뇌파감시를 하였으며 경동맥 혈류압을 측정하여 40 mmHg 이하이거나 수술반대측 경동맥 완전폐색이 있는 경우 shunt를 삽입하였다 결과 : 술후 1례에서 사망이 있었는데 이 환자는 전, 중 뇌내동맥 영역에 큰 뇌경색이 있으며 동측에 심한 경동맥 협착이 있고 의식은 기면 상태여서 바로 응급수술을 하였다 수술시경동맥 혈류 역류압은 35mmHg 여서 shunt를 사용하였다 술후 1일째 의식이 혼수상태로 나빠져 뇌 단층촬영한 결과 뇌경색 부위에 출혈이 발생하여 사망하였다. 수술 직후 모든 환자에서 뇌허혈에 따른 합병증 및 사망은 없었고 1례에서 수술후 1일째 수술부위 반대편에 적은 뇌경색이 발생하였다 평균 21.5$\pm$11.85개우러의 외래 추적 검사에서 뇌졸중 재발이 없었다. 결론 : 뇌졸중이환후 경동맥 내막 절제술은 최소 4-6주 이상 안정화 시킨 다음 수술하는 것이 좋다고 생각된다 경동맥 내막 절제술은 뇌졸중 예방에 효과적인 치료방법이며 경동맥 혈류역류압이 40mmHg 이하일 경우 shunt를 설치하여 수술하는 것이 안전하다고 사료된다.

(주)경동나비엔은 1978년 3월에 설립된 우리나라의 대표적인 보일러 생산 업체로 1979년 송탄공장에서 국내 최초 콤팩트형 사각 기름보일러 '코로나 KDB-202'를 출시하면서 업계의 주목을 받기 시작했다. 또한 최근에는 북경, 상해, 미국 등으로 진출하며 명실 공히 글로벌 기업으로 비상하고 있다. 이 같은 화려한 회사소개를 차치하고서라도 (주)경동나비엔을 모르는 사람이 없을 것이다. (주)경동나비엔은 옛 사명인 '경동보일러'로 1990년대 초 중반 '아버님 댁에 보일러 놔드려야겠어요'라는 광고를 통해 대대적으로 브랜드를 알렸기 때문이다. 이후 2006년에는 경동나비엔으로 사명을 변경하고 온수기 난방, 냉방 공조, 홈네트워크 각방제어시스템으로 사업 영역을 확장하고, 콘덴싱 기술력으로 해외 1억 달러의 수출액을 달성하는 등 국내 보일러 업계의 선두를 유지하고 있다. 이러한 (주)경동나비엔은 사업장의 안전관리에 있어서도 남다른 두각을 드러내고 있다. 대규모 기계공장을 운영하고 있는 만큼 이에 대한 안전관리 기술과 노하우도 남다른 것이다. 이번호에서는 (주)경동나비엔의 안전보건을 책임지고 있는 최종숙 주임을 만나 구체적인 업무내용과 안전에 대한 소견을 들어본다.

The carotid intima-media thickness (IMT) is an early structural marker of the atherosclerotic process and is the only non-invasive test that is currently recommended by the American Heart Association for evaluation of the risk. However, use of this parameter has a limitation because it assumes uniform thickness throughout the blood vessel, whereas atherosclerosis is a focal phenomenon that is confined to intima. In fact, plaque can be found along the atherosclerotic blood vessels even though its value is unknown. The aim of this study is to analyze the carotid plaque and IMT in the stroke patients. We investigated the patients with ischemic stroke, who were admitted to the department of neurology at the Stroke Special Hospital from January to March 2008. After the carotid IMT and plaque were assessed by B-mode ultrasonography, IMT and carotid plaque to risk variables (age, sex) were analyzed. The distribution of CCA IMT was significantly different in terms of age (p = 0.004). Likewise, the distribution of carotid plaque was also significantly different in terms of age (p = 0.006). Carotid plaque was 69 and 92% in normal and abnormal CCA IMTs respectively. The results showed that the CCA IMT was closely associated with carotid plaque.

B-모드 초음파를 이용한 경동맥 내중막 두께(IMT intima-media thickness) 측정은 관상동맥 질환이나 허혈성 뇌질환 위험도의 독립적인 인자로 매우 중요하다. 본 연구에서는 NTSC(national television system committee) 방식으로 경동씩 초음파 동영상신호를 획득하여 프레임으로 나누어 각각의 IMT를 측정하였다. 각각의 측정값을 시간축으로 재구성하여 시간도메인에서 IMT 의 시변동성을 분석하였다. IMT 시변동성 분석을 통하여 상대적으로 부정맥피험자가 정상피험자에 비해 IMT 평균값이 작아 위험도가 낮다고 평가 될 수 있지만 시변동성 분석으로 IMT 평균값이 작아도 혈관의 기능적 특징이 부족하여 위험도가 높다는 것을 알 수 있었다. 경동맥의 형태적 특징과 기능적 특징을 반영하는 이 분석방법은 경동맥 내중막 두께측정의 신뢰성을 향상시키며 관련질환의 위험도와 중증도를 세분화 할 수 있는 새로운 방법이 될 수 있을 것이다.

This study aims to investigate the correlation between the occurrence of colon polyps and carotid IMT by age. This study checked the correlation between the occurrence of colon polyps and carotid IMT, grouping patients who had a colonoscopy and carotid ultrasonography simultaneously by age based on cross-tabulation. As a result of the analysis, by age, the older the patients with metabolic syndrome, the higher the correlation between the occurrence of colon polyps and carotid IMT became. Also, when carotid IMT was more than 1.1mm, the incidence of polyps was high. In conclusion, there was a high correlation of the occurrence rate of colon polyps with carotid ultrasonography and colonoscopy, and the older the patient and the thicker the carotid IMT, the higher the correlation became.

보일러회사들의 사업다각화 기세가 놀라울 정도다. 특히 난방사업과 냉방사업을 아우르는 다각화는 유난히 계절을 타는 보일러 회사들에게는 최적의 조합으로 꼽히고 있다. 귀뚜라미보일러와 경동보일러가 그렇다. 이들 회사들은 동절기를 겨냥한 난방 시즌이 끝나도 하절기를 겨냥한 냉방 시즌이 기다리고 있으니 일년 365일을 항상 같은 템포로 유지할 수 있다. 가을철 추수가 끝나면 기나 긴 농한기에 접어 드는 농촌처럼 한 구석이 허전했던 보일러 회사의 대리점들도 이제는 손발에 땀이 나게 뛰기만 하면 된다. 하지만 귀뚜라미와 경동의 사업 내용은 전혀 다르다. 귀뚜라미는 에어컨의 제조원이고 경동은 판매원이다. 귀뚜라미가 센추리 아산공장을 인수하고 지난달에는 범양공조까지 흡수하면서 에어컨 메이커의 메이저를 꿈꾸고 있는 반면 경동은 세계 최대 에어컨 회사인 캐리어와 손잡고 마케팅 컴퍼니의 메이저 자리를 노리고 있다. 진입방식은 다르지만 냉난방 전문 기업으로 변신을 꾀하고 있는 귀뚜라미와 경동보일러의 에어컨 사업을 들여다 본다.

경동맥 파열은 악성 종양으로 인한 두경부 수술의 1-5%에서 발생하고 80%의 사망률과 생존자의 50%에서 신경학적 후유증을 나타낸다. 저자들은 수술전ㆍ후 방사선치료를 받은 환자 2례에서 경동맥파열을 경험하였다. 제 1례는 51세의 남자로서 후두암($T_4$NoMx)으로 후두전적출술과 6120 rad의 수술후 방사선 치료 후 좌측 악하 부위에 악성 종양의 경동맥 침윤으로 시험적 수술후 6일만에 경동맥 파열로 사망하였다. 제 2례는 51세의 남자로서 하인두암($T_3$$N_2$Mx)으로 7200 rad의 수술전 방사선치료후 악성 종양의 재발로 후두전적출술과 일측의 근치적 경부곽청술후 누공을 형성하여 수술후 14일만에 경동맥 파열로 사망하였다.

In image-based CFD modeling of carotid bifurcation hemodynamics, it is often not possible (or at least not convenient) to impose measured velocity profiles at the common carotid artery inlet. Instead, fully-developed velocity profiles are usually imposed based on measured flow rates. However, some studies reported a pronounced influence of inflow boundary conditions that were based on actual velocity profiles measured by magnetic resonance imaging which showing the unusual presence of a high velocity band in the middle of the vessel during early diastole inconsistent with a Dean-type velocity profile. We demonstrated that those velocity profiles were induced by the presence of modest secondary curvature of the inlet and set about to test whether such more "realistic" velocity profiles might indeed have a more pronounced influence on the carotid bifurcation hemodynamics. We found that inlet boundary condition with axisymmetric fully-developed velocity profile(Womersley flow) is reasonable as long as sufficient CCA inlet length of realistic geometry is applied.

Currently. the north american symptomatic carotid endarterectomy trial, european carotid surgery trial, and common carotid method are used to measure the carotid stenosis for determining candidate for carotid endarterectomy using the projection angiography from different modalities such as digital subtraction angiography. rotational angiography, computed tomography angiography and magnetic resonance angiography. A new computerized carotid stenosis measuring system was developed using MR angiography axial image to overcome the drawbacks of conventional carotid stenosis measuring methods, to reduce the variability of inter-observer and intra-observer. The gray-level thresholding is one of the most popular and efficient method for image segmentation. We segmented the carotid artery and lumen from three-dimensional time-of-flight MRA axial image using gray-level thresholding technique. Using the measured intima-media thickness value of common carotid artery for each cases, we separated carotid artery wall from the segmented carotid artery region. After that, the regions of segmented carotid without artery wall were divided into region of blood flow and plaque. The calculation of carotid stenosis degree was performed as the following; carotid stenosis grading is(area measure of plaque/area measure of blood flow region and plaque) * 100%.
