Purpose: The purpose of this study was to confirm whether the Swiss Ball exercise program is effective to improve lower extremity function and activity of daily living of elderly women with mild cognitive impairment (MCI). Methods: This study was a double blind control study. Subjects participated 34 female elderly women who were assigned to exercise group (n=17) and control group (n=17). The Swiss Ball exercise program was performed 2 times a week during the 12 weeks. Pre-and post-exercise various measurements were made: lower extremity function (OLS, TUG, STS). Results: In exercise group, There showed statically significance improve between pre-and post-exercise in OLS (7.29${\pm}$1.10 and 7.64${\pm}$1.32), TUG (10.47${\pm}$2.03 and 10.05${\pm}$2.04), STS (7.71${\pm}$1.04 and 7.94${\pm}$.82)(p<.05). ADL also showed statically significance improve between pre-and post-exercise (13.76${\pm}$3.54 and 12.82${\pm}$3.39, p<.05). There showed statically significance difference between two groups (p<.05). Conclusion: The Swiss Ball exercise program improved lower extremity function and ADL of elderly women with MCI. Further studies are required to examine the significance of the assessment of motor function of lower extremities.
Purpose: This study aimed to investigate objectively measured physical activity (PA) in institutionalized older adults with mild cognitive impairment (MCI) and to elucidate the influence of autonomic nervous function, salivary cortisol, and PA on cognitive functions based on neurovisceral integration model. Methods: Overall cognitive function was evaluated using the mini-mental state examination (MMSE) and executive function was evaluated using semantic verbal fluency test and clock drawing test. Actigraph for PA, HRV and sAA for autonomous function, and the geriatric depression scale for depression were used. Saliva specimens were collected in the morning for sAA and cortisol. Results: Ninety-eight older adults from four regional geriatric hospitals participated in the study. They took 4,499 steps per day on average. They spent 753.93 minutes and 23.12 minutes on average in sedentary and moderate-to-vigorous activity, respectively. In the multiple regression analysis, lower salivary cortisol level (β = - .33, p = .041) and greater step counts (β = .37, p = .029) significantly improved MMSE score. Greater step count (β = .27, p = .016) also exerted a significant influence on verbal fluency, and greater sAA (β= .35, p = .026) was significantly associated with a better clock drawing test result. Conclusion: Salivary cortisol, sAA and physical activity were significantly associated with cognitive functions. To prevent older adults from developing dementia, strategies are needed to increase their overall PA amount by decreasing sedentary time and to decrease salivary cortisol for cognitive function, and to maintain their sympathetic nervous activity for executive function.
The present study investigated the effects of AD on stereoscopic vision. Sixty participants (20 AD patients, 20 mild cognitive impairment patients, 20 healthy elderly controls) participated in the study. Two cubes, one on the left and the other on the right of the center of the monitor, appeared at varying distances from the observer with their relative distances controlled in two disparity conditions (absolute vs relative disparity) combined with two disparity directions (crossed vs uncrossed disparity). Participants identified the object that appeared closer to them. Results demonstrated comparable performance with all three groups performing accurately, suggesting that the effect of AD on stereopsis is negligible. Discussion focused on brain pathology affected by AD involving high level visual processing.
Objectives: We compared the characteristics of the pain threshold and pain experience between demented group and non-demented group. Methods: This study was part of Gangwon projects for early detection of dementia in 2010. We recruited 8302 local resident ages over 65 years old. Of theses, 1259 people who scored low MMSE were selected and 365 of them completed CERAD-K(Consortium to Establish a Registry for Alzheimer's disease). Finally, 90 in non-demented group and 57 in demented group(mild to moderate Alzheimer's disease) were analyzed. Pain threshold was experimentally measured by pressure algometer and we investigated the pain experience, by Brief pain inventory (BPI), a self-report test. Results: In the demographic characteristics, there are more female, higher ages, lower education in the demented group. There was no significant difference between the two groups in the pain threshold. On the BPI results, 'shoulder pain', 'the number of pain' and 'interference of working' were significantly more prevalent in non-demented group. However, there are no significant differences between the groups in the 'pain severity', 'prevalence of pain' and 'pain treatment'. Conclusions: Demented group report less pain experience but, still perceived pain. It support previous studies that patient with dementia have increased pain tolerance but preserved pain threshold. Thus, active pain assessment and treatment for patients with dementia is needed.

Objectives: The purpose of this study was to identify the effectiveness of Korean medicine treatment in mild cognitive impairment (MCI) among a group of community dwelling elderly. Methods: Two-hundred and twenty-nine elderly living in a community and diagnosed with MCI were recruited. Participants were evaluated with various instruments such as the Korean version of Mini-Mental State Examination for Dementia Screening (MMSE-DS) and the Korean version of Montreal Cognitive Assessment (MoCA-K). Korean medicine treatment consisted of herbal medicine, acupuncture, and pharmacoacupuncture. The change in cognitive ability was assessed by using the MMSE-DS and the MoCA-K. Data were analyzed by SPSS/WIN 22.0 using the paired t-test, and the ANOVA. Results: The MMSE-DS and the MoCA-K score generally increased after six months of Korean medicine treatment and the differences in both instruments were statistically significant. Additionally, some consecutive participants maintained long-term cognitive improvement. When analyzed specifically by herbal medicine group based on syndrome differentiation and pharmacoacupuncture group, most showed improvement in the MMSE-DS and the MoCA-K but not all data were statistically significant. The satisfaction score was mostly high and most participants were willing to re-participate in the program. Conclusions: Korean medicine treatment may contribute to the improvement and prevention of cognitive decline in the elderly. However, further systematic research based on large scale sample data and standardized protocols is needed to uplift the welfare and mental health of the elderly.

Subjective cognitive-linguistic complaints in older adults contribute to the diagnostic and prognostic investigation of MCI or dementia. However, the utility of subjective test for predicting cognitive-linguistic decline is controversial. Few domestic studies have included the correlation between subjective and objective tests systematically. The current study analyzed 26 studies published since 2000, and the effect sizes of their correlation coefficients between two tests were computed. The results of qualitative analysis indicated that the number of subjects ranged from 26 to 657. Subjective tests included the self-report 75.4% and the informant-report 24.6%. In objective tests, memory comprised the largest proportion, followed by global cognition, and language, etc. As a result of meta-analysis, self-report test had the predictive value for dementia, and informant-report test contributed to discriminate among 3 groups. In the elderly group, self-report test was correlated with reasoning, and informant-report test with memory and language. In MCIs, self-report test predicted several abilities including language, and informant-report test signaled the future decline of domains like global cognition. Two types of subjective tests in dementia also represented memory, language, and global cognition accurately. This study provides evidence-based information to support relationships between subjective and objective tests for cognitive-linguistic ability in 3 groups.
Impaired naming difficulty is common in normal elderly as well as in patients with neurological impairment. The 60-item Korean version-Boston Naming Test(K-BNT) is one of the most commonly used test for measuring confrontational naming ability. However, age-related cognitive decline may make the elderly difficult concentrating during the 60-item test, therefore, item reduction of the K-BNT would improve test validity and reliability. Thus, the purpose of this study was to develop a short form of the K-BNT based on Item Response Theory(IRT). Considering item-fit index, sex factor, and item difficulty through Rasch analysis, the 15-item K-BNT(i.e., K-BNT-15) was developed. Via administration of the K-BNT-15, we observed age-related decline in naming ability and significantly different performance between the normal elderly and patients with mild cognitive impairment. This study demonstrates the utility of IRT for developing a short-form language evaluation tool. The K-BNT-15 can be effective as a language screening tool to differentiate between normal aging and pathological diseases.

PURPOSE: This study examined the effects of coordinative locomotor training on the physical factors for falls in the elderly with mild cognitive impairment. METHODS: This study examined thirty subjects diagnosed with mild cognitive impairment by the radiologic findings, history, and physical examination. The subjects were assigned to a control group (n = 15) or experimental group (n = 15, coordinative locomotor training). The experimental group underwent coordinative locomotor training for four weeks, with training sessions two times per week. The control group was given a fall-prevention education for 60 minutes without coordinative locomotor training. To evaluate the physical factors for falls, the lower extremity strength and the Korean version of the Fullerton advanced balance scale and biorescue were measured for balance. These tests were conducted before and after training. RESULTS: Significant differences were observed between the two groups after the four weeks of coordinative locomotor training for the elderly with mild cognitive impairment the experimental group had a greater degree of improvement in the physical factors for falls. CONCLUSION: These findings suggest that coordinative locomotor training may have a functional effect on fall-prevention and the mobility of the elderly with mild cognitive impairment. In addition, it is expected to provide systematic and effective data that can be used as a fall prevention program for the elderly with mild cognitive impairment in each institution.
The purpose of the study was to improve the efficiency of Breath & Forced Vital Capacity(FVC) of Stroke Patients Using Record Playing Focused on Breath Training. The Training of Forced Vital Capacity(FVC) is focused on trunk muscles reinforce for Breathing. So the study tried to play Recorder for reinforcing FVC. The play included the two parts. One was for solo play, the other was play in concert base on solo play using Korea country song, Arirang. The Recorder play used abdominal breath, Pursed-lip & Paced methods. The course of play included Long Tone, Staccato, Crescendo, Decrescendo and throughout the course we tried to improve the efficiency & Quantity of breathe. Experiment performed at Social welfare institute for the old. They played & checked FVC, $FEV_1$, $FEV_1/FVC$ before & after breathe 19 times for 6 weeks using Spirovit SP-1, Schiller. After experiment we performed statical process(p<0.05) using statical Package software. The findings were as follows: Analysis for Experiment showed the numerical Value of increasing Of FVC, $FEV_1$, $FEV_1/FVC$. Increasing Percentage of Measured/Predicted FVC, $FEV_1$ (or calculated based on theory) ration was 25%, 13% and Quantity of FVC, $FEV_1$ 35%, 40%. Also $FEV_1/FVC$ Ratio(showed efficiency of breathe) raised 13%. And experiment data was confirmed by statical process. The experiment data was valuable under p<0.05. In Conclusion we think the results of experiment showed the efficiency of Music therapy for Breathe(FVC, $FEV_1$, $FEV_1/FVC$) reinforce.
