The prosthetic approach and principle for an collapsed VDO : A clinical case of Class II div.2 patient (저위교합환자의 보철적 접근법과 이론 : Class II div.2 교합환자 증례)

  • The prosthodontic treatment of Class II division 2 malocclusions is challenging. Ideally, these malocclusions should be identified at an early age and corrected with orthodontic treatment; otherwise, the individual develops a habitual position characterized by deep overbite and significant retruded position of mandibular condyle at the TMjoint fossa. This article describes a clinical protocol for the occlusal rehabilitation of patients with Class II div.2 malocclusions. Within this protocol, an occlusal splint was used to locate the most suitable maxillary-mandibular relationship for function and range of motion. The splint increased the vertical dimension and reduced pain on TMjoints. After transfer this relationship to an articulator for fabrication of provisional restorations, the CR position and centric prematurity contact between maxilla and mandible was used to determine the tentative vertical dimension of occlusion(VDO). The amount of elevation of VDO was decided on the articulated model. The provisional restorations were accurately transfered to a patient's mouth in clinical procedures using tattoo points. The final restoration was delivered after some trial periods with provisional restorations. The theory behind this protocol and its associated clinical procedures is presented along with a discussion.

Intraarticular Osteochondroma of the Phalanx of the Hand - A Case Report - (수지골에 발생한 관절내 골연골종 - 증례 보고 -)

  • The osteochondroma is the most common benign bone tumor, which occupies 40% of the benign bone tumor. About 80% of lesions arise in the long bone of the extremities, particularly in the knee and the upper extremity. But the occurrence of an intraarticular osteochondroma is rare, especially in the interphalangeal joint. We report the case of a intraarticular osteochondroma which occurred at the interphalangeal joint of the hand in a 12-year-old male patient. The plain X-ray demonstrates an exostosis arising from intraarticular dorsal aspect of the proximal phalanx of the right middle finger. The excised mass was round, measuring $8{\times}3$mm in size and 1 mm in thickness with definite cartilage capsule.

  • Malignant peripheral nerve sheath tumors(MPNSTs) are uncommon sarcomas that mostly arise in the soft tissue. They can develop from the pre-existing neurofibromas or schwannomas, or denovo from the peripheral nerves, or they can occur following the radiation therapy. We report a case of MPNST that developed in the sciatic nerve of the patient with neurofibromatosis type-1(NF-1). The patient was a 39-year-old man with the history of NF-1, who's main symptom was a rapidly enlarging painful mass in his posterior thigh. The well demarcated tumor, $6.5{\times}5{\times}4.5$cm in size, was composed of closely packed spindle cells. Since the patients with NF-1 have a high risk for developing a recurred MPNST, the importance of the clinical follow up is emphasized.

  • Female and male nematodes isolated from an eye of a 36-year old Korean man in August 1987 were identified as Thelagia callipaeda. This is the 17th record of human thelaBiasis in Korea. The internal structures of both female and male were described.

Radiologic Evaluation for Differentiating Benign from Malignant Solitary Pulmonary Nodule (고립성 폐결절에서 양, 악성 감별을 위한 화상적 고찰)

  • We are now detecting an increasing number of solitary pulmonary nodules (SPNs) that are difficult to diagnosis. The purpose of this study was to evaluate the useful radiologic findings for differentiating benign from malignant pulmonary nodules. Material and Method: The high-resolution CT (HRCT) findings of SPNs smaller than 3 cm in largest diameter were evaluated in 134 patients with malignant and benign nodules in regard to internal structures, margin characteristics, and surrounding parenchymal responses. Result: The nodules with the area of ground-glass attenuation (GGA) greater then 50% were noted in adenocarcinoma, inflammatory lesions and some of metastatic tumors, and the lesions greater than 90% were noted only in adenocarcinoma. The area of GGA in non-adenocarcinoma, benign tumors and tuberculomas were less than 50%, and mainly less than 10%. The findings of air bronchogram, spiculation, lobulation, vascular involvement, and pleural indentation were some noted at every types of malignant tumors, but especially high over than 30% in adenocarcinomas. Conclusion: Most peripheral lung adenocarcinomas form a characteristic radiologic findings especially in HRCT. Evaluation of these findings would be helpful in differentiating between lung cancer, especially adenocarcinoma, and other lesions.

Fifteen Human Cases of Fibricola seoulensis Infection in Korea (Fibricola seoulensis의 인체감염 15례)

  • A mass fecal examination was performed in Seoul, Korea from December 1983 to March 1984 and 15 egg positive cases of Fibricola seoulensis were discovered. They were treated with praziquantel($Distocide{\circledR}$) 20mg/kg single dose and magnesium salt for purgation. The adult worms of F. seoulensis were collected from 6 of them. The number of the flukes ranged 1-78 by the case. The cases had no clinical manifestation at all. They were all Korean males in twenties, and had the histories of eating the roast or raw snakes or frogs. Fibricoliasis is expected to be prevalent among the snake-eaters in Korea. Further cases will be detected by sedulous differentiation of thinshelled immature operculated eggs of 0.08~0.102mmn length. Praziquantel seems to be an excellent chemotherapeutic for fibricoliasis.

  • Adamantinoma is a rare malignant tumor and approximately 85% of cases occur in the tibia. In 1985, Moon and Mori reviewed the world literature and identified 195 cases. A 59-year-old woman was presented with nonspecific pain on left leg after trauma. Radiographic finding showed well-circumscribed soap bubble appearance with areas of sclerosis in diaphyseal region of the tibia. Magnetic resonance imaging and biopsy was performed for diagnosis. Histologically, the lesion was compatible with adamantinoma. Wide resection with bone graft was performed. There was no recurrence at 3 years follow-up. We report a case of adimantinoma that arose in diaphyseal region of the tibia with review of literature.

  • A giant cell granuloma and a cementifying fibroma occurring in the same lesion of right mandibular body in a 10 year old boy is presented with a 12 month follow up without recurrence or any other complications after operation. The relatively small lesion of cementifying fibroma was well delineated from the larger lesion of central giant cell granuloma, and as their origins are different each other(odontogenic or connective tissue origin), we have considered that both lesions had developed independently. Clinically, the evidencesa of aggressiveness of giant cell granuloma were also found, that is, large size, earlier age of 10, root resorption of lower right 1st annd 2nd molars and cortical perforation. With curettage and electric cauterization, we have treated both lesions satisfactorily.

  • Vitamin D-resistant rickets(VDRR) is hereditary disease manifesting marked hypophosphatemia caused by renal tubular loss of phosphate into urine and an associated decrease in the calcium and phosphorous ion product. VDRR is identified by clinical symptoms, such as in the limbs, gait disturbance, dwarfism, familial occurrence, bowlegs, and knock-knees, as well as by laboratory findings. Dental findings are enlarged chambers and extension of the pulp horns into the cusp tips in both the primary and the permanent dentition in contrast to the vitamin D-deficient rickets. The major oral manifestations are multiple spontaneous abscesses in a caries-free dentition. We reported the clinical features and treatment of a 6-year-old boy with vitamin D-resistant rickets referred to the department of pediatric dentistry in Kyung Hee University for multiple spontaneous periapical abscesses and gingival fistula without severe dental caries.

  • This case describes a case of desmoplastic fibroma of the mandible. A 9-year-old boy was seen with a history of progressive swelling and expansion of the left mandible for one month period. Desmoplastic fibroma was diagnosed on histopathologic examination This report reviews the diagnostic criteria, differential diagnosis and surgical treatment of choice in brief of this uncommon primary bone tumor of the oral and maxillofacial region.
