Mutational Analysis of Mitochondria DNA in Children with IgA Nephropathy (소아 IgA 신병증 환자에서 미토콘드리아 DNA 돌연변이 분석)

  • Purpose: The association of mitochondrial DNA (mtDNA) mutations, deletions and copy number with progressive changes in patients with some glomerular disease and end-stage renal disease have been reported. In this study, we performed mtDNA mutation analysis in children with IgA nephropathy to investigate its role in progressive clinical course. Methods: Seven children with IgA nephropathy were involved in this study. MtDNA isolated from platelet was amplified by PCR and sequenced entirely. Results: The mean age at renal biopsy was $11.5{\pm}2.2$ year and the mean age at latest evaluation was $17.9{\pm}3.2$ year. The mean follow-up period were $7.8{\pm}3.1$ years. Patients was divided into 2 groups according to the amount of proteinuria at presenting manifestation. Group 2 patients were nephrotic syndrome. Renal function reveals within normal range in all patients. In group 2 patients, the mean serum albumin level was significantly lower than those of group 1 ($3.7{\pm}0.6g/dL$ vs. $4.7{\pm}0.2g/dL$, P=0.0241) and the mean total cholesterol level was significantly higher than those of group 1 ($222.7{\pm}35.7mg/dL$ vs. $148.3{\pm}29.1mg/dL$, P=0.0283). In Group 2 patients, total amount of protein of 24 hour collected urine also significantly higher than those of group 1 ($1,466.0{\pm}742.5mg$ vs. $122.5{\pm}48.1mg$, P=0.0135). Pr/Cr ratio in random urine sample was also higher in group 2 than those of group 1 but the statistical significance was not noted ($1.8{\pm}1.6$ vs. $0.2{\pm}0.2$, P=0.0961). Deletion of mtDNA nt 8272-8281 were observed in two patients, one patient in each groups, respectively. This is noncoding lesion. No patients demonstrated the mtDNA mutations. Conclusions: We have identified a deletion of mtDNA nt 8272-8281 in two children with IgA nephropathy. Further studies are needed to clarify the role of mitochondrial function in the progressive change of IgA nephropathy.

The Change of Peripheral Eosinophil Count after Bronchial Provocation with Inhaled Histamine in Bronchial Asthmatics (기관지 천식 환자에서 히스타민 기관지유발검사후 말초혈액 호산구수의 변화)

  • Background: Recently, bronchial provocation of the airway of atopic asthmatic subjects with inhaled allergen has been shown to produce an initial peripheral blood eosinopenia followed by an eosinophilia occurring approximately 12 to 18 hrs after the challenge. However there are few studies about the change of peripheral eosinophil count (PEC) after bronchial provocation with nonspecific stimuli such as histamine or methacholine. Interestingly our preliminary study demonstrated a notable change of PEC during bronhial provocation with inhaled histamine in some asthmatic subjects. This study was designed to reevaluate our preliminary data and to further investigate the change of PEC during as well as after bronchial provocation with inhaled histamine in bronchial asthma tics. Methods: Sixteen asthmatic subjects participated in this study. Bronchial provocation with inhaled histamine was done between 9 AM and 12 MD. Blood samplings for PEC were done with 5 minutes intervals during the procedure, and repeated at 1 hour, 2 hours, 4 hours, 8 hours, 24 hours, and 48 hours after the procedure. Results: The results were as follows; 1) The patients were divided into two groups characterized by each pattern in the change of PEC during the procedure. A group (11 of sixten, group I) showed an increasing pattern of PEC and another group (5 of sixteen, group II) showed a decreasing pattern of PEC during the procedure. 2) Group I demonstrated a tendency to maintain continuously higher level of PEC than the baseline value until 48 hours after the procedure. 3) Group II demonstrated a tendency to maintain continuously lower level of PEC than the baseline value until 48 hours after the procedure. 4) There were no significant differences in their clinical parameters including baseline eosinophil count, baseline $FEV_1$, $PC_{20}$ of histamine, and serum IgE level between group I and group II. Conclusion: Our results suggest that the change of PEC produced by inhaled histamine in asthmatic subjects is much different from that produced by inhaled allergen, and that each patient may have their individual characteristics in the change of PEC in response to bronchial provocation with inhaled histamine. Alternatively these findings suggest that eosinophils may be partially involved in the early asthmatic reaction.

Clinicopathological Analysis of Glomerulonephritis in Children (소아 사구체신염의 임상 및 병리학적 분석)

  • Purpose: To evaluate the prevalence and clinical manifestations of various glomerulonephritis (GN) in children, a clinicopathological anlysis of 310 biopsied cases were performed. Method: We conducted retrospective study with review of histopathologic findings and clinical manifestations of the 310 cases diagnosed as glomerulonephritis by percutaneous renal biopsy which were done between January 1986 and December 1996 at department of pediatrics, Pusan Paik hospital. Results: 1) Male to female ratio was 1.54:1 and the range of age was from 13 months to 15 years 10 months. 2) Among these, 217 (70.0%) patients were belong to primary GN and 93 (30.0%) patients were belong to secondary GN. As a whole, the most common pathologic diagnosis was minimal change lesion (MC, 32.6%), which was followed by IgA nephropathy (IgAN, 15.8%), $Henoch-Sch\"{o}nlein$ purpura nephritis (HSPN, 13.5%), Poststreptococcal glomerulonephritis (PSAGN, 8.1%). 3) Clinical manifestations of patients were asymptomatic urinary abnormality (43.2%), nephrotic syndrome (41.0%), acute glomerulonephritis (14.2%), chronic glomerulonephritis (1.0%), rapidly progressive glomerulonephritis (0.6%). 4) In primary GN, the most common pathologic diagnosis was MC (46.5%), IgAN (22.6%), thin glomerular basement membrane (GBM) disease (7.8%), membranoproliferative glomerulonephritis (MPGN, 5.5%), mesangial proliferative glomerulonephritis (MesPGN,4.6%), focal segmental glomerulosclerosis (FSGS, 4.6%), membranous nephropathy (MN, 0.9%), sclerosing glomerulonephritis (SCGN, 0.9%), crescentic glomerulonephritis (CreGN, 0.5%) and non-specific glomerulonephritis (NonspGN, 6.0%). 5) Major causes of secondary GN were HSPN (45.2%), PSAGN (26.9%), hepatitis B associated glomerulonephritis (HBGN, 17.2%), lupus nephritis (LN, 6.5%), Alport syndrome (2.2%), hemolytic uremic syndrome (1.0%), fibrillary glomerulonephritis (1.0%) in descending order. Conclusions: There are some differences of the results of clinicopathological stuidies of glomerulonephritis in children because of its different indications of renal biopsy, pathologic classification of renal disease and methods of analysis among investigators. In order to establish more reliable data of incidence and classification of childhood glomerulonephritis in Korea, multicenter cooperative study were necessary.

Active and Passive Behaviours of the Guard Cells for Stomatal Opening and Closing in Heteromeres arbutifolia and Ferocactus acanthodes (Heteromeres arbutifolia 와 Ferocactus acanthodes의 기공개폐를 위한 공변세포의 능.수동적 행동)

  • Stomatal resistances of the leaves in Heteromeres arbutifolia and of the stems in Ferocactus acanthodes were studied to estimate active and passive behaviors of the guard cells on a theoretical basis. Active and passive stomatal responses to light and water deficit were observed. When the change rate of existent water due to variation of osmotic potential in the guard cells and the loss rate of transpirational water from the guard cells are $\Delta$wi-$\Delta$wt and leaded to active behaviors for opening and closing stomata. However, when stems of F. acanthodes with stomata closecd under the solar irradiation were covered with black cloth and then taken off, behaviors of the guard cells occurred in the condition of $\Delta$wi<$\Delta$wt and were passive. Under the conditiion of $\Delta$wi<$\Delta$wt due to cutout from stems, passive behaviors of the guard cells in H. arbutifolia and F. acanthodes always occurred in spite of the solar irradiation and darkness, respectively. The transpirational resistance coefficients of the guard cells in stems of F. acanthodes (0.380) and Opuntia bigelovii (0.135) wer emuch higher than in leaves of H. arbutifolia (0.034). Moreover, stomatal opening in stems of F. acanthodes during the daytime could be induced by watering. Those results are interpreted as that since the guard cells in desert Crassulacean acid metabolism (CAM) plants always exist in the state of stomatal opening, nocturnal stomatal opening and daytime stomatal closing are exhibited by passive behaviors of the guard cells in the alternant conditioins of $\Delta$wi>$\Delta$wt and $\Delta$wi<$\Delta$wt, respectively.

Thoracic Surgeon's Role in Differential Diagnosis of the Interstitial Lung Disease (간질성 폐질환의 확진을 위한 흉부외과의 역할)

  • Background: Pathologic confirmation is needed to diagnose various forms of interstitial lung diseases. We wanted to find out how much the thoracic surgical lung biopsies will be needed for definite diagnosis of interstitial lung diseases. Material and Method: 17 patients underwent surgical lung biopsy in the department of thoracic and cardiovascular surgery, Gyeongsang National University Hospital from June 1995 to November 2002. Chart review and telephone questionnaire were done for retrospective study. Result: Mean age was $49{\pm}22$ years. Age ranged from 1 to 70 years. Dyspnea was the most common complaint. They were referred for definite differential diagnosis from pediatrics and internal medicine. Biopsy methods were thoracotomy in 11 cases, and thoracoscopy in 6 cases. Pathologic confirmation was possible in 11 cases (65%). According to the pathologic reports, treatment plans were changed in 13 cases (76%). Conclusion: Surgical lung biopsy was effective method in differentiating diagnosis of the interstitial lung disease, There was no mortality during operation. It is important that undiagnosed fibrous lung disease should be recommanded the lung biopsy for planning patient's treatment.

Screening of Medicinal Plants with Antifungal Activity on Major Seedborne Disease (주요종자전염병 억제를 위한 항균성 약용식물탐색)

  • Antifungal activity on major seedborne diasease of crops was screened by the treatment of the extracts from 50 medicinal plants in vitro and in vivo. The extracts of garlic and taxus, Rheum undulatum, Achiranthes japonica, Glycyrrhiza uralensis, Oenothera lamar kiana treated with the blotting filter paper and water agar methods inhibited the growth of Pyricularia oryzae, Alternaria sesamicola, Colletotrichum gloeosporioides, and Alternaria brassicicola among the tested plants. Antifungal activities on infected seeds by soaking methods were shown even at the dilution of the extracts by 10 times. The activity was the highest in soaking seeds at $25^{\circ}C$ for 24 hours. The effect of plant extract on seed germination was not significant as compared with untreated seed. However, early growth of seedling was increased by the treatment of extracts. The extract of taxus slightly inhibited the seed germination of radish and chinese cabbage but those of Achirunthes japonica, Glycyrrhiza uralensis and Oenthfera lamarkiana showed severe damage on the seed germination and early growth of seedling.

Black Rot of Broccoli Caused by Xanthomonas campestris pv. campestris (Xanthomonas campestris pv. campestris에 의한 브로콜리의 검은썩음병)

  • A new bacterial disease of broccoli (Brassica oleracea var. italica) was observed on field-grown plants in Pyungchang during 2003 and 2004. Seedling infections first appeared as a blackening along the margins of the cotyledon. Cotyledon shriveled and dropped off. Infected seedlings were stunted and yellowed and eventually died. The disease was easily recognized by the presence of yellow, V-shaped, or U-shaped areas extending inward from margin of the leaf. As the disease progressed, the yellow lesions turned brown and the tissues died. Isolations made from diseased leaves on yeast extract dextrose calcium carbonate agar yielded nearly pure cultures of a yellow-pigmented bacterium typical of a xanthomonad. Two bacterial strains were purified and used for further tests. Pathogenicity of strains was confirmed on 3-week-old crucifer (cabbage, Chinese cabbage, kale, radish and broccoli) plants cut by scissors with bacterial suspensions containing $10^8 cfu/ml$ of phosphate buffered saline. The Biolog and fatty acid analyses and 16S rDNA sequencing of two strains (SL4797 and SL4800) from broccoli black rot showed that they could be identified as X. campestris pv. campestris because of their high similarity to the tester strain (X. campestris pv. campestris NCPPB528) with a match probability of 100%. This is the first report of black rot of broccoli in Korea.

Application of Bicuspidalized Cryopreserved Allograft Valves for the RVOT Reconstruction in Complex Cardiac Anomalies (복잡 심장기형 환자에서 우심실 유출로 재건술시 이첨판화 냉동보전 동종이식편의 적용에 관한 연구)

  • In recent years, the use of allograft conduits in repair of congenital cardiac disease is widely accepted. However, the supply of homograft. is currently limiting their increased clinical application, especially small cryopreserved homografts for use in neonates and inf'ants. We used a technique to surgically reduce the size of the more readily available large-diameter allografts, making them suitable for right ventricular outflow tract reconstruction in small infants and children. From December 1994 to March 1996, a total of 11 patients ranging in age from 10 months to 6 years (mean age, 27.3 months) and ranging in weight from 5.6 to 18.5 kg (mean 11.5 kg) underwent reconstruction of the right ventricular outflow tract using this surgical technique (pulmo ary atresia with ventricular septal defect, 9 cases ; tetralogy of Falloff, 2 cases). The diameter after downsizing ranged from 14 to 19 mm with a mean of 16.8 mm. There was one operative death due to rupture of the infected homograft. Evaluation of these patients between 2 and 15 months (mean 6.9 months) after homograft implantation reveals excellent clinical and echocardiographic results. There were no significant homograft insufficiency and RVOT obstructions. Although a longer follow-up is certainly required to evaluate the long term fate of the surgically modified bicuspid homografts, we believe that this technique may represent a valuable therapeutic alternative, at least in the short term, to the use of synthetic grafts when an appropriately sized homograft is not available.

Breeding of Self-compatible Pear "Wonkyo Na-jasoojung 2" (배 자가결실성 "원교 나-자수정 2호")

  • Pear has a gametophytic self-incompatibility (SI) system and its SI reaction is controlled by a single multi-allelic S-locus. 'Wonkyo Na-jasoojung 2' was selected from a cross between 'Wonwhang', early season major pear cultivar with high fruit quality and self-incompatible, and 92-18-79 (${S_4}^{sm}{S_4}^{sm}$) obtained from self cross of 'Osa-nijisseiki' (${S_2S_4}^{sm}$) (SM, stylar-part mutant), self-compatible bud mutant that originated from self-incompatible 'Nijisseiki' ($S_2S_4$) made in 2001 at the National Institute of Horticultural and Herbal Science, Rural Development Administration in Korea. '92-18-79' was selected as a self-compatible source through field investigation. It bloomed 1 day earlier than 'Osa-Nijisseiki' and similar to 'Wonwhang' in 2008. It is medium in tree vigor and spreading in tree habit. 'Wonkyo Na-jasoojung 2' is classified as highly susceptible to pear scab (Venturia nashicola) similar to 'Osa-Nijisseiki' and as resistant to black spot (Alternaria kikuchiana) similar to 'Wonwhang'. It had 65.7% fruiting rate by self pollination. The average optimum harvest time of 'Wonkyo Na-jasoojung 2' was 148 days after full bloom and it matured 2 days earlier than 'Osa-Nijisseiki' and 11 days later than 'Wonwhang'. The fruit is roundish oblate in shape and yellowish brown in skin color. Average fruit weight was 445 g and soluble solids content was 13.3 $^{\circ}Brix$. The flesh had abundant juice and negligible grit.
