INTERRATER RELIABILITY OF CARIES DIAGNOSIS BY LASER FLUORESCENCE (레이저형광 우식진단법의 검사자간 신뢰도에 관한 연구)

  • The purpose of study was to measure the interrater reliability of the caries diagnostic instrument, Diagnodent(KaVo, Germany), using a new laser fluorescence technique. Occlusal surfaces and smooth surfaces of one hundred extracted human premolars and molars were examined by four dentists. Pearson correlation coeffcients were 0703 to 0.870 for occlusal surfaces and 0.764 to 0.932 for smooth surfaces (P<0.01, all). Reliability coefficients which were calculated by intraclass correlation coefficients were 0.9450 for occlusal surfaces and 0.9605 for smooth surfaces. Therefore, the interrater reliability of caries diagnosis by Diagnodent is very high, and the rater training would be necessary more for occlusal surfaces than for smooth surfaces.

Study of Validity and Interrater Reliability of Korean Version of the Peabody Developmental Motor Scale 2 (한글판 Peabody Developmental Motor Scale 2의 타당도와 검사자간 신뢰도 연구)

  • Objective : This study aims to verify the content validity and inter-rater reliability of the Korean version of the Peabody Developmental Motor Scale 2 (PDMS-2) and to identify the concurrent validity by comparing it with the Korean version of the Bruininks-Oseretsky Test of Motor Proficiency-2 (BOT-2). Methods : PDMS-2 was translated by the researcher and an eighth-year clinical occupational therapist. The content consistency of the Korean version of the PDMS-2 was verified by three professors with experience using it. After the verification of the content consistency of the PDMS-2 by the five clinical occupational therapists and the additional revision, the Korean version of the PDMS-2 was completed. The researcher and another occupational therapist evaluated the Korean version of PDMS-2 in 50 children and measured the inter-rater reliability. Concurrent validity was measured by comparing the results of the Korean version of PDMS-2 and Korean version of BOT-2. Results : The content consistency test showed overall agreement of mean 3.45, and the content understanding test showed a high level of understanding of mean 3.69. The inter-rater reliability and concurrent validity of the Korean version of the PDMS-2 showed a statistically significant correlation. Conclusion : The Korean version of the PDMS-2 showed high content understanding, reliability, and validity. It can assist clinicians and researchers who work in fields related to child treatment or development.

Inter-rater reliability on the Functional Reach Test (기능적 뻗기 검사의 측정자 간 신뢰도)

  • 본 연구는 기능적 뻗기 검사의 측정자 간 신뢰도를 알아보기 위하여 실시하였다. 본 연구에 참가한 대상자는 총 46명(남 20명, 여 26명)이었고 기능적 뻗기 검사는 두 곳의 실험실에서 이루어졌고 측정자는 총 6명이었다. 같은 장소의 실험실에서 이루어진 검사의 측정자간 신뢰도는 높게 나타났으나 장소가 다른 곳에서 이루어진 검사의 측정자 간 신뢰도는 아주 낮게 나타났다. 따라서 기능적 뻗기 검사의 신뢰도를 확보하기 위하여서는 측정자들의 명확한 기준의 정리가 필요할 것으로 보인다.

A Study of Test-Retest Reliability and Interrater Reliability of the Sensory Processing Scale for Children (SPS-C) (아동감각처리척도(Sensory Processing Scale for Children; SPS-C)의 검사-재검사 신뢰도와 검사자간 신뢰도 연구)

  • Objective : This study examined the test-retest reliability and interrater reliability of the Sensory Processing Scale for Children (SPS-C). Method : Senventy primary caregivers of children with sensory processing difficulties and 3 years old participated in the study. The subjects were recruited through child development centers, welfare centers, and acquaintances located in Seoul, Gyeonggi-do, Busan, and Gyeongsang-do. The test-retest reliability verification targeted 20 main caregivers of children with difficulty in sensory processing. Re-evaluation was performed within 7 to 14 days after the initial evaluation, and Pearson's correlation coefficient was used to confirm the relevance between the two time points, and the Intraclass correlation coefficient was used to confirm the degree of agreement. The interrater reliability verification was conducted with 18 primary caregivers and 18 subsidiary caregivers of children with sensory processing difficulties. Each caregiver evaluated the same child, and the Intraclass correlation coefficient was used to confirm the agreement between the two sets of caregivers. Results : The test-retest reliability was Pearson's correlation coefficient r=.914 and intraclass correlation coefficient ICC=.939, indicating a high level of relevance and agreement. The interrater reliability was an Intraclass correlation coefficient ICC=.727, which showed a moderate level of agreement, but the tactile area (ICC=.455) and proprioceptive area (ICC=.439) were not statistically significant and showed a low degree of agreement. Conclusion : Through this study, it was confirmed that the children's Sensory Processing Scale for Children (SPS-C) is a stable evaluation tool with test-retest reliability and interrater reliability verified, and it will be able to provide help in standardization studies for future clinical use.

Inter-rater Reliability and Training Effect of the Differential Diagnosis of Speech and Language Disorder for Stroke Patients (뇌졸중 환자의 말, 언어장애 선별에 대한 검사자간 신뢰도 및 훈련효과)

  • Distinguishing aphasia in stroke patients and observing the subtle linguistic characteristics associated with it primarily requires the use of instruments that provide reliable assessment results. Additionally, examiners should be fully aware of how to use those instruments. This study examined 46 stroke patients for aphasia and assessed the reliability of their diagnoses according to examiners whose medical fields were different from each other. Furthermore, a comparison was made between the reliability before training and that after training. To this end, 46 stroke patients were tested for aphasia and in terms of their speech disorder degree by 3 groups, each of which consisted of 12 professionals (3 SLP, 3 neurologist, and 3 nurse). In the result, a rating of 'acceptable' was given for speech intelligibility tasks and the voice quality of /ah-/ prolongation, and other sub-tests were marked as 'good-excellent' by the experts with different areas of medical expertise. For the tasks marked as 'acceptable', the subjects were video-trained for 3 weeks and the differences were compared before and after their training. Consequently, the differences in the examiners' ratings in the speech intelligibility tasks showed a significant decrease and the accuracy of their voice quality ratings showed a significant increase. In the result of research on the correlation between the accuracy of the sub-test ratings and the amount of clinic experience, speech therapists developed more accuracy in rating a picture description task and a speech intelligibility task as their experience accumulated. Meanwhile, doctors and nurses showed more accurate ratings in picture description tasks with greater clinical experience. The results of this study suggest that assessing the neurologic-communicative disorders of stroke patients requires ongoing training and experience, especially for speech disorders. It was also found that the rating reliability in this case could be improved by training.

The Reliability and Validity of Clock Drawing Test as a Screening Tool for Cognitive Impairment in Clients after Cerebrovascular Accident (뇌졸중 클라이언트의 인지 손상 선별 도구로서 CDT의 신뢰도 및 타당도)

  • The purpose of this study was to analyze the reliability and validity of CDT as a screening tool for cognitive impairment for stroke clients living in local community. Two evaluator assessed 51 clients' cognitive function using CDT and K-MMSE from October 2010 to August 2011 year. The researcher analyzed test-retest reliability, interrater reliability, construct validity, construct validity. The test-retest and interrater reliability was higher than .54(p<.01). The construct validity and concurrent validity was statistically significant(p<.01). So, The CDT using productive method and scoring system of Freedman et al., may be applied to screen cognitive impairment of clients with stroke.

Reliability and Validity of the Balancia using Wii Balance Board for Assessment of Balance with Stroke Patients (뇌졸중 환자들의 균형평가를 위한 Balancia 프로그램의 신뢰도와 타당도)

  • Balance is the foundation for all active daily livings. Evaluation of balance has been conducted to measure postural sway of center of pressure(COP). The purpose of this study is to introduce the Balancia and to examine the validity and inter-, intra-reliability for measure of balance with stroke patients. COP was measured by Wii balance board. The subjects Participated in this study were 39 stroke patients. Intra- and inter-reliability was examined using intraclass correlation coefficient(ICC) with 95% confidence interval. In the result of intra- and inter-reliability, all COP parameters of postural sway showed high to very high reliability with ICC=.793-.939 and ICC=.791-.955. Validity was determined by correlation with Accusway force plate. There were high validity with ICC=.851-.955 between tools. According to these results, the Balancia showed high validity and reliability. It is a useful clinical tool for evaluating balance in stroke patients.

  • 두개하악장애는 가장 흔히 발병하는 질병중의 하나이다. 이때 저작계에 흔히 호소하는 주소가 근육의 통증이다. 통증의 정도를 측정하는 방법으로 촉진이 가장 널리 사용되고 있으나 정량화하기 어렵다는 단점이 있다. 따라서 통증 측정기 등과 같이 다양한 시도가 진행되고 있다. 근육이나 관절의 병적 소견은 해당조직의 혈액순환장애나 염증반응과 종종 관련이 있다. 이러한 상태는 신체의 표면과 밀접한 관계가 있어 피부온도에 영향을 미치며, 따라서 thermography로 측정할 수 있다. 피부온도의 측정은 질병의 활성 상태를 평가하는 데 유익하며, 또한 치료결과를 평가하는 데에도 효과적이다. 객관적 피부온도 측정장치가 있다면 턱관절이나 저작근의 이상상태를 평가하는데 매우 도움이 될 것이다. 따라서 본 연구의 목적은 경제적이면 양측성으로 특정 부위의 피부온도를 측정할 수 있는 기기를 사용하여 검사자내 및 검사자간 신뢰도를 측정하고 좌우 온도차이를 확인함으로써 향후 질환 및 치료 겨오가에 대한 임상적 평가시 객관적 측정방법의 하나가 될 수 있는 지를 알고자 하는 데 있다. 구강내 염증이나 이상이 없으며 두개하악장애가 없는 치과대학생 15명을 대상으로 조사하였다. 평균연령은 24.9세, 범위는 24-30세이었다. 사용한 기기는 미국산 YSI Precision 4000이다. 18-2$0^{\circ}C$의 일정한 실내온도에서 두 검사자가 각각의 검사를 알지 못하는 상태에서 별도로 정해진 부위에 검사를 시행하고, 약 1주일후 이전 검사에 대한 기억이 없어진 상태에서 두 검사자중 한 검사자가 다시 동일한 검사를 시행하였다. 측정치들을 이용하여 상관관계를 계산하고 이에 대한 유의성을 검정하였다. 1. 검사자내 상관관계(r)는 좌 .798(p=<.0001), 우 .757(p=<.0001)이었다. 2. 두검사자간 상관관계(r)는 좌 .958(p=<.0001), 우 .951(p=<.0001)이었다. 3. 좌우 측정치간의 차이는 유의한 차이가 없었다. 4. 근육부위별 측정에서는 inferior masseter muscle의 검사자간에서만 유의한 상관관계가 없었을 뿐 anterior masseter muscle, anterior temporalis 모두 검사자내 및 간에 유의한 상관관계를 나타내었다. 이상의 결과로 보아 정상인에 있어 근육내 발통점의 피부온도는 검사자간에는 특정부위에 따라 다소 차이가 있을 수 있으나 일반적으로 높은 재현성을 보여줌으로서 향후 교근 및 측두근의 임상연구 평가에 피부온도조사는 도움이 되리라 사료된다.

Intra- and Inter-rater Reliability of BioRescue (BioRescue의 측정자내와 측정자간 신뢰도)

  • BioRescue ($RMIng{\acute{e}}nierie$, Marseille, France) is able to assess dynamic and static balance when a test subject stands or sits over the platform. Although previous studies have examined the reliability of other balance measurement methods, little research has been conducted on the reliability of equipment such as BioRescue. This study examined the intra-rater and inter-rater reliability of BioRescue during balance assessment. Thirty-four healthy adults participated as subjects and two physical therapists participated as examiners to determine intra- and inter-rater reliability. BioRescue was used to conduct measurements during footprint (area and pressure), Romberg, and limit of stability tests. The footprint area and limit of stability tests yielded intra- and inter-rater reliability ICC values above 0.60 in all directions. However, the footprint pressure and Romberg tests yielded values below 0.60 in all directions. BioRescue showed good inter-rater and intra-rater reliability in measurements during footprint area and limit of stability tests. However, because the values of limits of stability show the difference between experienced and unskilled, skill is required for the accuracy of the equipment.

The Reliability and Validity of Useful Field of View Test (UFOV(Useful Field of View test) 검사의 신뢰도 및 타당도 검증)

  • The aim of the study is to examine the reliability and validity of UFOV, which is a visual driving evaluation tool that has been proven to be reliable and valid in western countries, for the purpose of adapting the tool in a systematic manner to the South Korean population. Two evaluator assessed 23 healthy and 19 stroke patients with UFOV, Trail Making Test A & B(TMT A & B) and Motor Free Visual Perception Test(MVPT) from 7 October 2014 to 25 November, 2014. The researcher analyzed inter-rater reliability, correlation between raters of UFOV with Intraclass correlation coefficient, test-retest reliablility, UFOV with spearman correlation coefficient, concurrent validity, UFOV, TMT A & B and MVPT with spearman correlation coefficient, and discriminative validity, comparison mean scores of UFOV between groups, healthy and stroke with Mann-Whitney U test. UFOV score of participants with stroke had lower compared to the healthy control group. The inter-rater reliability(p<.001), test-retest reliability(p<.01) and concurrent validity(p<.01) was statistically significant. Also discriminant validity was statistically significant(p<.001). Based on this study, Use of UFOV for drivers at risk is essential to prevent future traffic accidents and support driving rehabilitation.
