A Study on Oral Health Knowledge and Behavior among the Elderly (노인들의 구강보건지식과 구강보건행태에 관한 연구)

  • The subjects in this study were 281 elderly people in some regions. After they were respectively interviewed, the following findings were given: The group of elderly people who brushed their teeth three times or more and who did toothbrushing in an arbitrary way were more concerned about oral health, and the gap between them and the others (p<.05). The group who paid a regular visit to dental clinics had more interest in oral health than the other (p<.01). The group who brushed their teeth in an arbitrary way had more remaining teeth, and the gap between them and the others(p<.05). The group who did toothbrushing for about two minutes had more remaining teeth, and more edentulous jaws were found in the group who brushed their teeth once (p<.01). The group who did toothbrushing three times or more scored higher in oral health knowledge, and the gap between them and the others(p<.01). And the group who paid a regular visit to dental clinics scored higher in oral health knowledge, and the gap between them and the other (p<.01). In the future, the development of a wide variety of oral health programs and prolonged oral health education are both required. Diverse oral health programs and education are expected to stir up elderly people's interest in oral health, to improve their oral health knowledge and to give a positive change to their oral health behavior.

Distribution of the Korean Dental Hygiene Research Agenda Suggested by the Korean Dental Hygienists Association and Affiliated Groups (대한치과위생사협회 및 산하단체에서 제안한 한국 치위생학 연구의제 분포)

  • The objective of this study was to realize research demands by gathering the research agenda from the Korean Dental Hygienists Association and affiliated groups through a Delphi survey. The study subjects were the heads of eight groups. The Delphi survey was conducted by e-mail after a researcher explained the objective of the study to the heads. The research agenda was classified according to the Korean dental hygiene academic classification and the National Dental Hygiene Research Agenda classification. To compare the number of research agendas according to classification among the groups, we conducted a chi-squared test by using IBM SPSS 20.0. In total, 316 agendas were submitted. According to the Korean dental hygiene academic classification, there was a significant difference among the groups, and the agendas associated with clinical dental hygiene (36.7%) were the best. According to the National Dental Hygiene Research Agenda classification, there was no significant difference and the agendas associated with health promotion and disease prevention (27.8%) were the best. In conclusion, the Korean Dental Hygienists Association and affiliated groups demanded similar research agendas, corresponding to the purpose of each establishment. However, the groups did not seem to show unique characteristics according to the purpose of each establishment. In other words, the groups should corroborate such characteristics and suggest a corresponding agenda to establish specific areas of research in dental hygiene. Also, dental hygiene researchers should mark the academic classification of the studies they want to submit.

BOLD Responses to Acupuncture on Each Side of ST36 (족삼리 좌우측 자침에 대한 BOLD 반응)

  • Objectives : There has been some controversy about the modulatory effects on brain function during acupuncture on each side of the same acupoint. This study was designed to investigate and compare the blood oxygen level-dependent(BOLD) responses of acupuncture on each side of ST36. Methods : Fourteen healthy subjects were recruited for imaging and received acupuncture or placebo stimulations either on the left or on the right acupoint of ST36 in each scan. For the voxel-wise statistical analysis, one sample T-test and the within-subject analysis of variance(ANOVA) test were performed using SPM8 software. Results : This study showed that acupuncture on each side of ST36 showed different BOLD signal patterns. Higher BOLD responses after acupuncture stimulations at the left ST36 compared to the right were observed mainly in the parahippocampal gyrus(BA 28), dorsolateral prefrontal cortex(DLPFC, BA 44), thalamus, culmen and claustrum. We investigated the different neural responses between rest and activation periods of placebo and acupuncture stimulations on each side of ST36. Acupuncture at the right ST36 elicited activation mainly in the insula, supplementary motor area(SMA) and anterior cingulate cortex(ACC), while acupuncture at the left ST36 elicited activation mainly in the insula, primary somatosensory cortex(SI, BA 2) and DLPFC(BA 44). Conclusions : To our knowledge, this is the first reported functional MRI study directly comparing when needling at the right and at the left side of ST36. This study's preliminary results proved to be evidence of acupuncture's different effects when performed on opposite sides of an acupoint.

Automatic Detection of Stage 1 Sleep (자동 분석을 이용한 1단계 수면탐지)

  • Stage 1 sleep provides important information regarding interpretation of nocturnal polysomnography, particularly sleep onset. It is a short transition period from wakeful consciousness to sleep. Lack of prominent sleep events characterizing stage 1 sleep is a major obstacle in automatic sleep stage scoring. In this study, we attempted to utilize simultaneous EEC and EOG processing and analyses to detect stage 1 sleep automatically. Relative powers of the alpha waves and the theta waves were calculated from spectral estimation. Either the relative power of alpha waves less than 50% or the relative power of theta waves more than 23% was regarded as stage 1 sleep. SEM (slow eye movement) was defined as the duration of both eye movement ranging from 1.5 to 4 seconds and regarded also as stage 1 sleep. If one of these three criteria was met, the epoch was regarded as stage 1 sleep. Results f ere compared to the manual rating results done by two polysomnography experts. Total of 169 epochs was analyzed. Agreement rate for stage 1 sleep between automatic detection and manual scoring was 79.3% and Cohen's Kappa was 0.586 (p<0.01). A significant portion (32%) of automatically detected stage 1 sleep included SEM. Generally, digitally-scored sleep s1aging shows the accuracy up to 70%. Considering potential difficulties in stage 1 sleep scoring, the accuracy of 79.3% in this study seems to be robust enough. Simultaneous analysis of EOG provides differential value to the present study from previous oneswhich mainly depended on EEG analysis. The issue of close relationship between SEM and stage 1 sleep raised by Kinnariet at. remains to be a valid one in this study.

Preparation and Characteristics of Bread by Medicinal Herb Composites with Immunostimulating Activity (면역활성을 가진 생약복합물을 이용한 빵의 제조 및 특성)

  • In this study, the breads with medicinal herbs (MH) composites showing immunostimulating activity were prepared and their characteristics were examined. Fourteen kinds of medicinal herbs were extracted with hot water and divided into 3 groups (MH-1, MH-2, MH-3) based on their contents. All groups showed immunostimulating activity in terms of macrophage phagocytosis, nitrite production, cytostatic activity and cytokine production. In the preparation of breads containing MH extracts of various contents (0, 30, 50, 70, and 100%), there was no significant difference among dough pHs of all groups after first fermentation, but loaf volume was significantly (p<0.05) increased in 70% added group while decreased in 30%, 50%, and 100% added groups compared to the control. The "a" and "b" values of bread crumb increased with the content of MH extracts while "L" value decreased, but these values of bread crust were similar to the control group. Most improvements in hardness, adhesiveness, gumminess and chewiness of bread were noticed by the addition of 70% MH extracts, but those of springiness, cohesiveness and resilience were mostly by the 50% addition ones. Through the sensory evaluation, it was revealed that mouth feeling, taste and overall preference decreased at breads containing 70% and 100% extracts, although appearance and crumb texture were not significantly (p<0.05) different among all groups.

Effect of 'Breakfast Club Program' on Dietary Behaviors and School Life in High School Students Residing in Seoul Metropolitan Areas (서울지역 고등학생 대상 '아침밥 클럽'의 식생활 및 학교생활 개선 효과)

  • The adolescent period, during which physical, spiritual, and physiological growth and development occurs, is important for the development of healthy dietary habits. Particularly, eating a regular breakfast improves eating habits and school life in adolescents but few studies have investigated the effects of eating breakfast on these outcomes. Therefore, this research was performed to investigate the effects of eating a regular breakfast provided by the 'Breakfast Club Program' on eating behaviors and school life. A total of 315 high school students were recruited for the program from 10 high schools in nine districts among 25 districts in Seoul during August to December of 2013. The average age was $17.8{\pm}0.8$ years and 38.1% of students were females. Based on 315 subjects, 38.3%, 35.7%, and 17.1% increased their intakes of breakfast (P=0.001), fruit (P=0.033), and milk (P<0.001) up to 5~7 times per week, respectively. The intake frequency of fruit (P<0.001) and eating breakfast (P<0.001) increased noticeably after program participation. Based on 64 students performing dietary record, intakes of cereals (P<0.001), mushrooms (P=0.027), and fruits (P=0.002) increased while intakes of eggs (P=0.001) and fat and oils (P=0.019) decreased after the program. Consumption of energy (P=0.004), plant protein (P=0.012), carbohydrates (P<0.001), dietary fiber (P=0.026), iron from plant sources (P=0.009), potassium (P=0.043), zinc (P=0.013), vitamin $B_1$ (P=0.043), and vitamin C (P<0.001) increased whereas intake of cholesterol (P=0.002) decreased. Regarding school life, the score for 'comfortable and cheerful starting of a day' (P=0.001) and 'healthy starting of a day' (P<0.001) increased significantly. This study indicates that a regular 'Breakfast Club Program' may establish healthy eating habits and school life in teenagers.

Assessment of Foodservice and Cooking Program for Children Attending Community Child Centers in Korea (지역아동센터를 이용하는 아동을 위한 급식 및 요리활동 프로그램의 적용효과 평가)

  • This study aimed to evaluate the effectiveness of a foodservice and cooking program for children attending community child centers in Korea. The foodservice and cooking program included a reference menu, nutrition and hygiene class for foodservice managers, as well as cooking classes for children during the winter or summer vacation in 2015. In order to evaluate the program, a survey was conducted before and after the program. A total of 1,120 children who participated in the program completed questionnaires regarding eating behaviors, food intake, and their opinions and levels of satisfaction with the program. The scores of children's eating behaviors increased significantly from 59.71 to 63.62 out of 85 points (p<0.001). A significant increase was detected in children's food intake. Vegetable intake, which received the lowest scores, increased significantly from 3.63 to 3.81 out of 5 points (p<0.001). The score for opinions and satisfaction concerning the program was 28.34 out of 35 points. These results suggest that the foodservice and cooking program could be helpful in promoting healthy food habits in children attending community child centers.

대구지역 대기환경용량평가에 관한 연구

  • 우리나라는 산업화와 도시화의 급속한 발전으로 인한 대기오염물질배출시설, 자동차 통행량, 에너지 사용량의 증가 등으로 대기오염물질배출원의 수와 규모가 증대되어 광역도시를 중심으로 대기질이 악화되고 있다. 특히 우리나라의 수도권대기질은 선진국의 주요 도시에 비해 대기오염 상태가 좋지 않은 것으로 평가됨에 따라 정부는 대기질을 OECD선진국 수준으로 개선한다는 목표를 세우고 있으나, 지역의 대기상태 및 그 동안의 대기질 개선을 위해 이행된 정책의 효율성 측면에서 볼 때 사후적인 규제위주의 농도규제 방식으로는 급증하는 대기오염배출시설의 배출량총량 관리가 어렵고, 지자체별로 개별적인 분산관리로는 광역적으로 이동되는 대기오염물질의 관리가 불가능하다. 또한 대기오염과 상관성이 큰 에너지정책, 산업정책, 도시계획 등 관련 정책과의 통합적 접근이 어렵기 때문에 사전에 이를 예방하는 총량관리가 요구되어 진다. 총량규제란 특정지역의 기상, 지형조건 등을 이용하여 대기환경용량을 산출하고 이를 기초로 지역별 배출허용총량을 할당하여 궁극적으로는 오염원별로 대상오염물질의 삭감량을 정하는 제도로 선진국에서는 대기환경용량을 바탕으로 1970년대부터 사업장을 중심으로 배출농도 규제와 함께 총량규제를 병행 실시하고 있으며, 최근에는 자동차에도 실시하고 있다. 우리나라에서도 대기환경보전법 제9조에서 환경기준을 초과하여 사람의 건강이나 재산, 동식물의 생육에 중대한 위해를 가져올 우려가 있다고 인정되는 경우에는 동 지역 또는 특별대책지역 중 사업장이 밀집되어 있는 구역에 대하여 배출되는 오염물질을 총량으로 규제할 수 있도록 규정하고 있다. 또한, 환경부는 2003년도에 서울, 인천, 경기도내 19개시 지역을 대상으로 대기오염물질의 배출총량을 관리하는 대기오염총량제 실시를 포함한 '수도권대기질개선에관한특별법'을 제정하였고, 현재는 사업장에게 연도별 배출허용총량을 할당하고, 할당량 이내로 오염물질을 배출하도록 관리하는 사업장 대기오염물질총량관리 제도로 시행 중에 있다. 그러나 수도권대기질개선특별대책을 수립하면서 총량관리의 본격 이행 및 배출권 거래제도 도입에 대한 특별법안이 제정되고 부분적으로 시행되고 있으나, 우리나라에 총량관리를 본격 이행하는데 있어서의 필요한 준비여건은 아직 초기 단계이고, 관련 연구의 수행실적 또한 수도권에 제한되어 적은 편이다. 따라서 현재는 총량관리가 수도권에 국한하여 실시되고 있으나, 점차 타 도시까지 광역적으로 확대될 것으로 예상되는 바 이에 필요한 제반 사항들에 대한 조사 분석을 통하여 정책방향을 설정하는 더 많은 연구가 필요할 것이다. 이러한 배경에서 본 연구에서는 대구지역을 대상으로 대기오염농도 및 대기오염물질 배출량에 대한 현황조사를 실시하고, 이를 토대로 고농도가 자주 발생되거나 그러할 가능성이 높은 $NO_X$을 대상으로 대기오염기여도를 평가하고 대기확산모델을 통한 대기환경용량을 산정하였다. 대기오염농도 현황을 살펴본 결과, 대구지역의 대기오염은 $NO_2$, $SO_2$, CO는 전형적인 1차오염물질의 변화경향을 보였으며, $PM_{10}$는 봄철에 황사의 영향을 크게 받는 것으로 나타나 실제 대구지역에서 배출되는 양을 추정하기 힘든 것으로 판단된다. 또한 $NO_2$는 공업, 상업지역에서 $SO_2$$PM_{10}$는 공업지역, CO는 상업지역, $O_3$은 교외지역에서 높은 농도를 나타내는 것으로 파악되었다. 대구지역의 대기오염물질 배출량 현황은 CO가 47%, NOX가 43%로 전체 배출량의 90%를 차지하였고, 2005년 이후 $NO_X$는 감소하고 $SO_X$가 증가하는 추세이다. 또한 배출원대분류 중도로 및 비도로이동오염원에서 발생되는 선 오염원이 75%로 대구지역에서 가장 크게 기여하는 것으로 나타났다. ISCST3 대기확산모델을 이용하여 대기환경용량을 산정하기위하여, 먼저 대구지역의 대기환경용량평가는 가시적인 위해성이 높고 개선정책이 용이한 $NO_X$을 대상물질로 선정하였고, 배출량과 오염농도간의 상관도가 0.659로 높은 것으로 판단되어, 배출량을 삭감하였을 때 대기오염농도의 개선이 명확히 나타나는 것을 알 수 있었다. 다음으로, 단위격자 당 한계배출율을 알아내는 작업을 실시하여, 대구지역을 동일하게 장기환경기준 80%수준인 22.4ppb를 만족시키기 위한 한계배출율은 2.23g/s가 필요한 것으로 파악되었고, 산출한 한계배출율을 이용하여 장기환경기준치 80%수준 달성을 목표로 하는 경우의 대기환경용량을 산정하고 실제 배출량과 비교 분석하였다. 그 결과, 대구지역 전체의 환경용량은 약 3만 톤으로 실제 배출량 2만2천 톤에 약 8천 톤 이상의 여유가 있는 것으로 파악되었다. 그러나 구역별로 상이한 차이를 보였으며, 이에 따른 구역별 개선정책이 필요할 것으로 사료된다. 대기환경용량을 파악한 후 단위격자 당 한계배출율을 초과하는 대상 지역을 추출하여 삭감한 결과 초과배출량의 80%를 삭감해야 대구지역 전체에서 50ppb이하 농도가 되는 것을 알 수 있었는데, 실제로 초과배출량의 80%를 삭감하는 것은 어려움이 있다고 판단되어, 대구지역을 동일한 %율로 삭감한 결과 30% 삭감했을 때 50ppb수준을 달성하였고, 50%삭감했을때 2007년 환경기준인 30ppb수준을 달성하였다. 또한 배출원대분류 중 기여율이 높은 도로와 비도로오염원을 50%삭감한 결과 도로이동오염원의 삭감만으로도 상당한 고 배출지역의 농도저감에 효과가 있는 것으로 파악되고, 비도로오염원을 포함하여 삭감하였을 때는 대구지역 전체에서 50ppb이하로 내려가는 것을 볼 수 있었다. 따라서 향후 총량규제의 실시에 맞추어 대구지역의 실제적인 환경용량의 정확한 파악과 고배출지역에 대한 삭감방법에 관한 더 많은 연구가 필요하다고 사료된다. 이 연구 결과는 앞으로 시행될 지역총량규제에 대한 기초적인 방법론으로 활용될 수 있을 것이다.

Proposal for Amendment of the Basic Environmental Policy Act ('BEPA') Article 31 (환경정책기본법 제31조 무과실책임규정의 개정방안)

  • The Basic Environmental Policy Act (BEPA) (Law No. 4257 effective 1. August 1990) sets forth the basic policies and administrative framework for environmental preservation, leaving more detailed regulations, and emission controls to separate laws targeting air, water, and solid waste, etc. The BEPA Article 31 adopts an unprecedented strict liability standard for damages as an absolute liability. The BEPA Article 31 provides for liability as follows. If a company is alleged to have caused damage through pollution of the environment, it will be liable for damages unless it can show that the pollution did not cause damages, or that it did not actually cause pollution. If the company did cause pollution, and if the pollution is the cause for the damages in question, the company will be liable irrespective of whether it was negligent or otherwise at fault. If there are two or more companies involved in the pollution, but it is unclear which company caused the damages, all of the companies will be jointly and severally liable for the damages. In this paper, the author attempts to uncover the problems of BEPA Article 31 and then seeks desirable amendments by comparing it to the German Environmental Liability Act. First, it will be necessary to provide definitions of 'companies etc.'. Second, it will be necessary to enumerate the kinds of company facilities. Third, it will be necessary to provide exclusionary clauses on material damages. Fourth, it will be necessary to show 'presumption of cause and effect'. Fifth, it will be necessary to provide a clause on 'right to information'. Sixth, it will be necessary to provide a clause for force majeure. Seventh, it will be necessary to take measures to secure abundant liability for damages which can be caused by the owner of the facility, the potential polluter. Finally, it is appropriate that Korea now legislate an Environmental Liability Act akin to the German Environmental Liability Act.

Clinical Characteristics of Patients with Taste Disorders (미각 장애 환자의 임상적 특성에 관한 연구)

  • There is tremendous variability in the ways patients present with taste problems. Because of complex and multifactorial etiological background, it is not simple to evaluate patients with taste disorders. Accurate assessment of patients' status by prudent, thorough history taking and symptom analysis is the most essential for exact diagnosis of taste disorders. The aim of this study was to investigate the clinical characteristics of patients with taste problems as a primary complaint. Consecutive series of 50 patients (12 males and 38 females, mean age $53.6\;{\pm}\;14.7$ years) were included for the present study. All subjects were requested to complete a comprehensive questionnaire. Clinical evaluation procedures included oral examination, interview, questionnaire analysis, panoramic radiography, blood test and measurement of salivary flow rate. The obtained results were as follows: 1. Among the patients, 36 patients (72%) complained of oral mucosal pain or burning sensation. Of these patients, 18 patients (36%) were diagnosed as burning mouth syndrome. 2. Nineteen patients (38%) complained of subjective oral dryness. The flow rate of unstimulated whole saliva was less than 0.1 mL/min in 14 patients (28%) and 17 (34%) had a stimulated whole salivary flow rate of less than 0.5 mL/min. 3. Among the types of taste disorders, hypogeusia, the most frequently reported, was found in 25 patients (50%), dysgeusia in 18 patients (36%), phantogeusia in 15 patients (30%), hypergeusia in 10 patients (20%), and ageusia in 5 patients (10%). Nineteen patients (38%) reported more than one type of taste disorder and the most frequent combination was dysgeusia + hypogeusia (n=6, 12%). 4. Based on data from the medical and dental histories and examinations, the patients were assigned to 12 probable causal categories. Taste disorders due to oral mucosal diseases and idiopathic taste disorder were the most frequent (n=9; 18%, each), followed by psychogenic taste disorder (n=8; 16%), drug-induced taste disorder (n=7; 14%), and taste disorder due to dry mouth (n=6; 12%). These 5 categories of taste disorder accounted for 78% of all cases in this study.
