A Study on Structural Relationship Among Perceived Interactivity and User Related Variables in Health Information Websites (건강정보 웹사이트에서 지각된 상호작용성과 이용자 변수간의 구조적 관계 연구)

  • This study analyzed the effect of perceived interactivity in health information websites on users. First, this study analyzed the effect of users' individual characteristics on the perceived interactivity. As a result, hypochondriasis had a negative relationship with perceived interactivity, and health literacy had a positive relationship with it. On the other hand, although the perceived interactivity had a positive effect on users' attitudes toward health information websites and expected outcomes, it did not have an effect on continuous usage intention. However, their attitudes and perceived outcomes had a positive influence on continuous usage intention and at the same time mediated the perceived interactivity. Eventually, the perceived interactivity had an effect on continuous usage intention of health information websites by the medium of other positively related variables.

A Survey on the Consumer Attitude Toward Health Food in Korea (I) -Consumer Perception on Health and Food Habit- (건강식품에 대한 소비자 인식 연구 (I) -건강과 식습관에 관한 소비자 의식구조-)

  • The consumer perception on health and food habit, the experience of health food use and the discrimination between health food and drug of Korean consumer were surveyed by using a questionnaire containing 15 items in order to obtain the basic data for the assessment of the benefit and risk of health foods in Korea. A total of 1,000 people over 20 years of age living in Seoul and the vicinities were interviewed and asked to fill out the questionnaire during the period from the October 1995 to the February 1996. Among the 882 answers collected, 23 was incomplete data, and 859 answers were used for the statistical analysis by using SAS program. The perception of Korean consumer on health and food habit indicated that food habit was considered the most important factor for the maintenance of health, as appeared in 39.8% of the subjects, among which 93.9 % believed that food habit could cause disease, and 97.1% believed that disease could be cured by changing food habit. The most worried disease was cancer (30.6%), degenerative diseases (14.1%), diseases by accident (12.6%) and obesity (10.0%). The disease which likely to be caused by food habit was diabetes (35.6%), obesity (22.4%), high blood pressure (12.8%), constipation (12.7%) and cancer (7.9%). The disease which was believed to be cured by changing food habit was diabetes (40.1%), obesity (25.9%), constipation (16.5%), high blood pressure (7.4%) and cancer (3.3%). It appeared that the people had a perception that food habit was highly related with diabetes and obesity, but less with cancer which was mostly worried.

A Study on Psychoticism in College Freshmen (정신증적척도에 의한 대학신입생의 정신건강 평가)

  • The authors studied psychotic ism, using Derogatis's SCL-90, in the subjects of 3,499 male and 1,335 female college freshmen of Yeungnam University. The authors collected the reports of Self-Rating Psychoticism Scale during the periods from January to February, 1986, and applied ANOVA and t-test on psychoticism scores in order to compare them between various psychosocial factors, and sexes. The results are as follows : There was significant difference in the mean averages of total psychoticism socres between male and female students: male students scored 4.62+4.01, female students scored 5.03+3.89(P<0.01). Thirteen male students(0.4%) showed high psychoticism scores of 20 or higher, while seven female students(0.5%) showed the same scores. The male students who have maturated in city and the female students whose educational fees were paid by self, showed higher level of psychoticisrn scores(P<0.05). The male and female students who believed protestantism showed higher level of psychoticism scores(respectively P<0.05, P<0.01). There was a strong tendency toward higher psychoticisrn scores in the students who were dissatisfies with their home atmosphere, college, department and familiarity of parents and those who had pessimistic views of self in the past, present or future in both groups(P<0.001).

A Study of Dietary Intake and Vitamin/Mineral Supplement Usage among Adolescents (청소년의 식이섭취와 비타민.무기질 보충제 복용에 관한 연구)

  • The relationship between dietary intake and vitamin/mineral supplement usage was examined in 706 adolescent girls who were high school students. 43.8% of subjects used vitamin/mineral supplements during one year. The higher the family income and parents education level, the higher percentage of vitamin/mineral supplement usage was. But there was no significant difference between grades and scores. And vitamin/mineral supplement usage was higher int he thin and obese groups than the average weight groups. Nutrition knowledge and food habit did not affect vitamin/mineral supplement usage. Calorie intakes of vitamin/mineral supplement users and nonusers were similar. However, independent of the supplements, the diets of supplement nonusers contained significantly more dietary protein, vitamin A, vitamin $B_2$, niacin, vitamin C, and calcium than the diets of the users. A considerable portion of both the users and nonusers had dietary intakes of less than 2/3 of the Recommended Dietary Allowances for vitamin $B_1$, vitamin $B_2$, niacin, calcium, and iron. Vitamin/mineral supplement nonusers generally consumed a more vitamin, mineral from diet. Reasons for taking supplements were to take energy, advice and illness.

The Influence of MMPI Characteristics on the Outcome of Cognitive Behavioral Therapy for Panic Disorder (공황장애 환자의 다면적 인성검사 (MMPI) 특성이 인지행동치료 결과에 미치는 영향)

  • Objective : This study was designed to find the characteristics of MMPI that could influence the outcome of cognitive behavioral therapy(CBT) for panic disorder. Methods : 34 patients who met DSM-IV criteria for panic disorder with or without agoraphobia had completed 11 weekly sessions of cognitive behavioral therapy. All the patients were assessed with MMPI before the initiation of treatment. Five self-report measures including Beck Depression Inventory(BDI), Spielberger State-Trait Anxiety Inventory(STAI), Agoraphobic Cognition Questionnarie(ACQ), Body Sensation Questionnaire(BSQ), and Daily Anxiety Selfrating (0 - 8 scales) were also assessed as a pre- and post-treatment assessment. After the completion of treatment, patients were classified by the High End-State(HES) functioning group and the Low End-State(LES) functioning group for the data analysis. Results : 1) The LES group showed significantly higher scores in Hypochondriasis Scale(HS), Depression Scale(D), Hysterical Scale(Hy), Obsessive Scale(Pt), Schizophrenia Scale(Sc) and Validity Scale(F) of MMPI than the HES group. However, these differences gave impressions that the LES group had more severe symptoms rather than that they could be the factors influencing the outcome of CBT. 2) Though, the severity of symptoms of the LES group in the 5 measures of pre-assessment was basically higher than that of the HES group. The fact that both group showed the similar improvement between pre- and post-assessment supported the above interpretation. Conclusion : In regarding the above results, MMPI was not a proper tool that could provide the factors influencing the outcome of CBT. In the future study, the authors need to use a different tool that can find the personality characteristics more directly.

Effect of Duloxetine in Functional Gastrointestinal Disorder : In the Perspective of 'Brain-Gut Axis' (기능성 위장관 장애에서 Duloxetine의 효과 : '뇌-장관 축' 모델을 중심으로)

  • The pathophysiology of functional gastrointestinal disorder(FGID) is not completely understood, but the importance of the 'Brain-Gut Axis(BGA)' model in FGID is being increasingly recognized. The BGA model is a bidirectional, hard-wired and homeostatic relationship between the central nervous system(CNS) and the enteric nervous system(ENS) via neural, neurohormonal and neuroimmunological pathways. In addition, the BGA model would provide a rationale for the use of psychotropics on FGID. The authors experienced two cases in which duloxetine, a serotonin-norepinephrine reuptake inhibitor, was effective in relieving FGID symptoms as well as psychiatric symptoms such as depression and hypochondriacal anxiety. Therefore we discuss the vignettes from the perspective of BGA theory. Duloxetine showed efficacy in these two patients by reducing visceral hypersensivity (bottom-up regulation) and by relieving depression and anxiety(top-down regulation).

Result of Multiphasic Military Personality Inventory Among Grave's Disease in 19-Year-Old Men (19세 남성에서 그레이브스병의 다면적 군인성검사 결과)

  • Objectives : The purpose of this study was to quantitatively assess the degree of Graves' Disease affecting mental health through the results of multiphasic military personality inventory (MPI) in 19-year-old men. Methods : We collected and analyzed the results of the MPI for 592 healthy subjects and 148 Graves' disease patients among the 19-year-old men who visited the Military Manpower Administration from February 2008 to January 2010. Results : Among demographic characteristics, both Graves' disease and control group are 19-year-old men. Educational level, socioeconomic status, parental relationship, and BMI were not statistically different between the two groups (χ2=0.089, p=0.766; χ2=5.090, p=0.278; χ2=5.988, p=0.112 ; t=-0.635, p=0.526). In the MPI, among validity scales, the score of the faking-good subscale was significantly lower in the Graves' group (t=3.507, p<0.001), but in the faking-bad and infrequency subscales scores were not significantly different between two groups (t=-1.700, p=0.090; t=-0.519, p=0.604). Among the neurosis scale, anxiety, depression, somatization, and personality disorder subscale scores were all significantly higher in Graves' disease group (t=-3.323, p<0.001; t=-4.210, p<0.001; t=-6.202, p<0.001; t=-2.872, p<0.01). Among the psychopath scale, each score of schizophrenia and paranoia subscales were not statistically different between the two groups (t=-0.158, p=0.874; t=-0.846, p=0.398). Conclusions : According to MPI result, we can confirm that Graves' disease patients in 19-year-old-men may appear features such as anxiety, depression, somatization and personality tendency (introversion or impulsivity) than the control group.

Pharmacotherapy for Patients Complaining With Somatic Symptoms (신체증상을 호소하는 환자의 약물치료)

  • Patients complaining with somatic symptoms are very common in clinical practice, and are often consulted to medical department. But it is difficult to treat well. The treatment of somatic symptom disorder is multi-modal as none of the methods on their own provide a satisfactory outcome. The treatment of somatic symptoms disorders is complicated by lack of boundary, conceptual clarity, and overemphasis on psychosocial causation and effectiveness of psychological treatments. In clinical practice all classes of psychotropics are used to treat somatic symptoms disorder. Drugs such as tricyclic antidepressants, serotonin reuptake inibitors(SSRI), serotonin and noradrenalin reuptake inhibitors (SNRI), atypical antipsychotics are studied. The evidence indicates that these drugs are effective in somatic symptom disorders. All classes of antidepressants seem to be effective against somatic symptom disorders. SSRIs are more effective against hypochondriasis and body dysmorphic disorder, and SNRIs appear to be more effective than other antidepressants when pain is predominant. The author suggest that psychiatrists should know how to treat patients complaining with somatic symptoms by using not only psychotherapeutic approach but also pharmacological treatment. It will be helpful to reduce suffering and increase quality of life of these patients.

Differences of Illness Behavior in Depressive Patients According to the Presence of Somatization (우울증 환자에서 신체화 증상에 따른 질병행동의 차이)

  • Objectives : Illness behavior is defined as the persistence of an adaptive/maladaptive mode of perceiving, evaluating and responding to health status and symptoms according to the status. In a cognitive aspect, somatizing symptoms are regarded as being originated from distortions, including magnification and/or amplification, of perceiving, evaluating and responding to symptoms such as bodily sensations. That is somatization may be explained by maladaptive illness behavior. In this study, we tried to investigate differences of illness behavior in depressive patient according to the presence of somatization. Methods : We divided 45 patients who were diagnosed as depression with ICD-10 diagnostic criteria into two groups(somatizing and non-somatizing group) according to the somatization subscale of Korean Depression Scale and compared two groups in the differences of illness behavior using the Illness Behavior Questionnaire. Results : Somatizing group showed significantly higher scores than non-somatizing group on the disease affirmation subscale($6.79{\pm}2.08$ vs. 4.76, p=0.003) and the denial subscale($3.25{\pm}1.22$ vs. $2.10{\pm}1.41$, p=0.006). There were no significant differences between two groups on the general hypochondriasis subscale and the affective unstability subscale. In a logistic regression analysis, somatizing group also showed higher odds ratio (OR) scores on the disease affirmation subscale(OR=1.418, p=0.089) and the denial subscale(OR=1.880, p=0.083). Conclusion : The disease affirmation and denial may be a discriminative mechanism of somatization in depressive patients. These subscales of Illness Behavior Questionnare could be useful markers, and psychiatric illnesses with somatizing and depressive symptoms may be differentially diagnosed and be predicted through these subscales.

Clinical Characteristics in Patients for Mental Disability Assessment Following Traumatic Brain Injury -Comparison between Patients with and without Abnormal Brain Imaging- (두부외상 후 장애평가 환자들의 임상적 특징 -뇌영상검사 이상유무에 따른 비교-)

  • Objectives : This study was undertaken to identify the clinical and psychological characteristics in patients of mental disability assessment following traumatic brain injury who had the finding with or without abnormal brain imaging study. Methods : A consecutive series of 59 patients were assessed in hospital from January 1994 to December 1998. Patients were divided into 2 groups based on normal or abnormal brain imaging and the two groups were compared in demographic characteristics, psychiatric symptoms, type of head injury, clinical psychological findings. There were 27 patients with abnormal findings and 32 with normal findings in brain imaging study. Results : Abnormal finding group in brain imaging study had significantly higher incidence of psychosis, decreased memory, decreased appetite, increased nihilistic idea, and intracranial hemorrhage. Also, abnormal finding group showed significantly lower level of performance on the block design subtest of K-WAIS and had significantly lower scores on F, hypochodriasis, depression, hysteria, psychopathic deviate, psychasthenia and schizophrenia subscale of the MMPI. Conclusion : The findings suggest that the patients undergone brain surgery due to intracranial hemorrage at that time of brain injury may have higher frequency of abnormal findings in brain imaging study, complain more cognitive and affective symptoms, and have lower the abstract concept formation and perceptual organization abilities.
