우리나라 성인의 직업 수준에 따른 구강건강불평등 현황 (Oral Health and Occupational Status among Korean Adults)

  • 본 연구는 한국 성인의 직업 수준에 따른 구강건강상태 및 행태의 현황을 확인하기 위하여 제4기 국민건강영양조사 자료를 이용하여 현재 경제활동을 하고 있다고 응답한 19~64세 성인 중 구강검사, 건강설문조사를 모두 완료한 7,676명을 최종 연구대상으로 삼아, 직업계층, 종사상 지위 및 직업분류에 따른 구강건강수준 및 행태의 현황을 분석하여 다음과 같은 결론을 얻었다. 남녀 모두에서 직업계층에 따른 구강건강상태 및 행태의 유의미한 차이를 확인하였고, 특히 비육체직에 비해 육체직 종사자가 치아우식증 유병 위험도가 남자는 1.19배, 여자는 1.67배 높았고, 하루 3회 미만 칫솔질 실천 위험도가 남자는 1.83배, 여자는 2.39배 높았다(p<0.05). 종사상 지위에 따라 구강건강상태 및 행태에서 정규직 종사자에 비해 일용직 종사자의 불건강 위험도가 가장 높았고, 직업분류에 따른 치아우식 유병, 하루 3회 미만 칫솔질 실천, 구강검진 미검진, 저작불편호소 위험도는 남자 및 여자 모두에서 관리자, 전문가 및 관련 종사자에 비해 농림어업 숙련종사자의 위험도가 가장 높았다(p<0.05). 근로자의 높은 구강질병 유병수준은 근로손실 및 경제손실을 초래할 수 있고, 이는 개인의 경제적 부담을 가중시킬 뿐아니라, 노년기의 구강건강문제를 심화시킬 수 있다. 직업관련 환경은 개인의 소득, 교육수준과 같은 사회경제적 위치와는 달리, 정부의 직접적인 규제 및 보호가 가능하며, 이로 인한 복지제도 및 환경의 변화는 개인의 삶에 직접적인 영향을 미칠 수 있다. 따라서 추후 노동시장의 변화와 다양한 고용 조건에 따라 구강건강문제를 지속적으로 모니터링하고, 직업군 또는 산업군 내 취약 계층을 발굴하며, 계층 간 구강건강의 차이를 매개하는 관련 요인을 확인하여 미시적, 거시적 관점에서의 사업장 구강건강증진을 위한 전략 개발이 필요할 것으로 생각된다.

보건관리자 공동채용 모형 개발 (I) -산업간호사 의견을 중심으로- (Developing the Joint Employment System of Occupational Nurse Practitioners for the Minor Enterprises in Korea (part 1))

  • The occupational health services for the minor enterprises are poor comparing with that of large enterprises in Korea. In 1997, Korean government announced officially the law on joint employment system of occupational health practitioners to solve the problems. This study is to develop the model on the joint employment system of occupational nurse practitioners for minor enterprises in Korea based on the opinions of occupational health nurses (OH Ns). The data were collected by questionnares from October to November in 1997. The number of subjects was 210 occupational health nurses who had participated the mandatory nursing education program provided by the Korean Occupational Health Nurses Association(KOHNA). The response rate was 47.1%. The contents of questionnaires were general characteristics, occupational & job characteristics of OHNs, and the views on the joint employment system. The SAS-PC program was used for the statistical analysis. The results were as followed, (1) The proper number of industies was 3 and the appropriate number of workers was 448 for one joint employed occupational nurse practitioner (J EONP). (2) 44.7% of the OHNs wanted that the Ministry of Labor monitored the way and content of contraction b/w JEONP and minor emterprises, 82.6% of the OHNs wanted that KOHNA managed the employment informations. (3) The OHNs of occupational health agencies showed more positive attitude than the OHNs of private industries on the joint employment system. (4) 88.3% of the OHNs wanted the education training for JEONP. Especially in occupational hygiene and safety control. (5) The OHNs expected the role expansion of Occupational Health Nursing by JEONP system. At the same time they worried the adverse effects. For the successful execution of this system, it is necessary the cooperation among the Ministry of Labor, KOHNA, the minor enterprises, and the OHNs.

HTML5-기반 서울시 공공체육시설 통합 안내 사이트 구현 (Implementation of HTML5-based Website providing Public Sports Facilities in Seoul)

  • 건강에 대한 사람들의 관심이 높은 가운데 각종 체육 프로그램에 참여하고 있는 시민들도 늘어나고 있다. 하지만 서울시에 존재하는 공공 체육시설 정보를 모두 통합한 웹서비스가 없어 공공 체육시설 이용에 불편함이 많은 실정이다. 따라서 본 논문에서는 서울시 공공 체육시설 정보를 누구나 손쉽게 찾을 수 있는 통합 정보 시스템 Be Healthy Seoul을 구현하였다. 서울시가 제공하는 공공 체육시설 API를 사용하여 서울 전 지역의 공공 체육시설 정보를 통합했다. 또 HTML5를 기반으로 구현함으로써 사진 및 동영상 등의 다양한 기능을 지원하며, pc와 모바일 모두에서 지원이 가능한 반응형 웹페이지를 제공한다. 또한 관리자 페이지를 구현하여 서울시 공공 체육시설을 지역별/종목별/테마별로 분류하여 검색 가능하도록 하였다.

화학제품 제조업 보건관리자의 직무 스트레스 (Job Stress of Occupational Health Managers in Chemicals Manufacturing Factories)

  • Purpose: The purpose of this study aimed to investigate job stress levels of occupational health managers and whether job stress levels were affected by various factors such as size and types of company and work-related characteristics of occupational health managers. Methods: The study was conducted from May to September 2011 in the chemicals manufacturing factories in Korea and total subjects were 59 occupational health managers. We measured job stress levels of occupational health managers using Korean occupational stress scale (KOSS) questionnaires and the information of company characteristics was obtained using a self-reported questionnaire. Results: The characteristics of company such as type, size and industries and job characteristics of occupational health managers were significant factors in job stress. Job demand, job control, job insecurity, organizational system and occupational environment scores were significantly associated with type, size and industries of company. In multiple regression analyses, we found that job demand was significantly associated with occupational position and type of company, and job insecurity was significantly associated with working hour. Also, we found that lack of reward was significantly associated with education level, speciality, duration of work and hour and type of company. Conclusion: These results indicate that job stress of occupational health managers is significantly associated with work-related characteristics and company's characteristics.

IPA (Importance-Performance Analysis)를 활용한 제조업 보건관리자의 자격별 직무분석 (Job Analysis with IPA (Importance-Performance Analysis) based on the Qualification of Occupational Health Managers Working in Manufacturing Work-sites)

  • Purpose: To provide the data of health manager education program in order to improve the quality of work-sites health management with qualification based job analysis of health managers (Occupational Health Nursing, Industrial Hygienist, Environmental Engineer). Methods: A descriptive research on 132 health managers using IPA and SPSS/WIN. Results: The overall average of importance of health management job was 8.0 (10 being the maximum score). Nurses had significantly higher score in the level of importance per areas and health management jobs. The overall average of performance of health management job was 6.7 (10 being the maximum score). Nurses had higher score in the area of health management. IPA matrix distributions per health management job area showed the correlations in qualification backgrounds and all of 3 main areas. Conclusion: There was difference in the level of importance and performance on health management jobs based on the qualification backgrounds of health managers. To improve the health of workers, an integrated health management must be provided. And to provide this, it is necessary to offer the additional education to health managers with an institutional complementary plan.

보건관리자의 역할수행이 유병근로자의 출근으로 인한 업무손실에 미치는 영향 (Effect of Workplace Health Manager's Role Performance on Presenteeism in the Workers)

  • Purpose: This study was conducted to determine the effect of the performance of Workplace Health Manager at the workplace on presenteeism in the workers. Methods: Three months before this study, between June 2010 and September 2010, a self-reporting survey of 316 employees in 136 workplaces in each of each hired a Workplace Health Manager was performed at their workplace with explanations of the purpose and methods of this study after their consent was obtained. Results: The average performance score of the Workplace Health Manager as graded by the employees was 3.8 out of 5 points. The duties of the Workplace Health Manager which received higher grades were posting of the Material Safety Data Sheets (MSDS), health education, and future management with respect to the results and procedure of health care-related work. According to the employees' survey, the score for presenteeism was evaluated as 14.3 out of 30 points. Based on the presenteeism analysis results, when (1) the employee was a male, (2) the workplace was established and managed by Industry Safety and Health Committee, (3) the employees were aware of the role of Workplace Health Managers, and (4) Workplace Health Manager fulfilled his/her role actively and successfully, presenteeism was observed to a lesser extent. Conclusion: According to the results of the study, presenteeism was observed to a lesser extent when the Workplace Health Manager actively performed his/her role.

보건관리자와 임상간호사의 직무특성, 지식공유 및 조직문화가 조직몰입에 미치는 영향 (Relationships among Job Characteristics, Knowledge Sharing, Organizational Culture, and Organizational Commitment in Occupational Health Nurses and Clinical Nurses)

  • Purpose: This study aimed to investigate the relationships among job characteristics, knowledge sharing, organizational culture, and organizational commitment in occupational health nurses (OHN) and clinical nurses (CN) to identify factors that might influence organizational commitment. It aimed to contribute to the improvement of knowledge management in health care organizations, including industrial workplaces and hospitals. Methods: A convenience sample of 188 nurses, specifically 100 OHN and 88 CN, was selected. Data were analyzed with descriptive statistics, t-tests, correlation coefficients, and multiple regression analysis. Results: OHNs had a significantly higher education level and current work experience; higher proportion of participants with contract employment status, day work shift, less turnover experience, good lifestyle; and higher scores on organizational culture ($3.3{\pm}0.61$) and organizational commitment ($3.1{\pm}0.60$). Additionally, the correlations among organizational commitment, job characteristics, knowledge sharing, and organizational culture ware higher (r=.38~.57) among OHNs as compared to those among CNs. Further, the organizational commitment of OHNs was significantly influenced by knowledge sharing, organizational culture, working hours, and salary ($R^2=.32$), while that of CNs was influenced by organizational culture and life event stress ($R^2=.11$). Conclusion: There is a need to develop measures to enable nurses to contribute to the improvement of knowledge management in health care organizations as major human resources, by including interventions for the major factors leading to between-group differences in organizational performance.

  • 최근 발효식품이 건강식으로 대두되면서 발효식품에 대한 사람들의 관심이 증가되고 있다. 발효식품의 맛, 건강 기능성 및 저장성 등에 영향을 주는 요소들은 발효 재료, 온도, 습도 pH 등이 있으며, 숙성도를 나타내는 지표는 여러 가지가 있으며 그 중에서 pH 변화에 대한 모델 식은 아직 정립이 되지 않았다. 또한 식품 관리자나 소비자 입장에서는 발효 식품에 대한 숙성도 및 품질 상태를 알 수 있다면 발효 식품의 신뢰성이 높아지고 소비성도 높아질 것이다. 그러므로 본 논문에서는 발효 식품의 pH를 측정하여 회귀방정식을 이용하여 모델식을 구하여 발효 식품의 숙성도를 구하는데 플랫폼을 제공한다. 제안된 모델 식의 유용성을 확인하기 위해서 대표적인 발효식품 중 하나인 김치와 막걸리를 사용한 실험을 실시하였다.

정신장애인의 체험홈 이용경험에 관한 연구 (The Lived Experiences of Supported Housing among People with Psychiatric Disabilities)

  • 이 연구의 목적은 정신장애인들의 체험홈 이용 경험을 깊이 있게 연구하고자 현상학적 접근을 활용하였다. 총 9명의 정신장애인들이 연구참여자로 모집되었으며, 2016년 9월부터 2018년 3월에 걸쳐 심층인터뷰를 통해 자료를 수집하였다. 모든 자료는 녹음하였으며, 자료는 Colaizzi방법으로 분석하었다. 분석결과 6개의 주제모음, 17개의 하위주제가 도출되었으며, 다음과 같다: 1) 생활터전의 혼란스러움, 2) 체험홈에서 느끼는 여유로움, 3) 나에 대한 인식이 변화함, 4) 주도적인 행동을 선택함, 5) 스스로 살아가는 힘을 키움, 그리고 6) 진정한 삶에 통합함으로 나타났다. 연구결과는 정신장애인을 위한 체험홈 프로그램 확대 및 프로그램 질 향상의 필요성을 제기한다. 이와 더불어, 정신장애인에 대한 정신건강서비스 개입과 회복을 위하여 전문지식을 갖춘 사례관리자 확대가 정신보건영역에서 필요할 것으로 사료된다.

지역사회 거주 장애인의 건강관리 향상을 위한 지원 요구 분석: 장애인지원주택 운영 관리자의 관점에서 (Support Needs for Health Promotion of Community-Dwelling People with Disabilities: Perspectives of Operators Managing Disability Supportive Housing)

  • Purpose: Recent studies have focused on policies aimed at supporting the independence of individuals with disabilities in communities. As part of this initiative, supportive housing, integrated care, and residential spaces offer tailored services based on individual needs and autonomy. The attitudes and knowledge of the administrators supporting supportive housing residents regarding health management can influence the well-being of individuals with disabilities. Therefore, this study aimed to explore the challenges faced by supporting housing workers in enhancing the self-management skills of individuals with disabilities. Methods: In this qualitative study, focus group interviews were conducted in August 2023 with nine administrators working to support housing in Seoul. Qualitative content analysis was used to analyze the interview data. Results: The needs and challenges in enhancing the self-management skills of individuals with disabilities were as follows: (1) the complexity of health management challenges, (2) bidirectional strategies for strengthening health management capabilities, and (3) support for systematic health management. Additionally, eight subthemes were derived. Conclusion: By investigating the difficulties experienced and identifying the necessary support requirements for supportive housing workers, this study seeks to uncover insights and identifies areas for improvement and strategies for health management. This study acknowledges the educational and institutional support necessary to improve the health and quality of life of individuals with disabilities residing in supportive housing.
