간호대학생의 보건의료법에 대한 교육의 필요성 (The Need for Education on Health and Medical Law for Nursing Students)

  • 본 연구는 4주기 간호인증평가의 범주인 법과 윤리에 속한 프로그램 학습성과를 알아보고 그에 따른 관련 범주에 속한 보건의료법규에 대한 방향성을 알아보고자 하였다. 4주기에서는 4가지의 하위역량을 제시하였고 그에 따른 "학습성과는 법과 윤리에 따른 간호를 수행한다" 로 구성되었으며, 이는 대학마다 간호학과의 목적에 맞게 교육과정의 프로그램 학습성과 달성을 위한 설정을 계획할 것을 권장하고 있다. 특히 보건의료법규는 간호사 국가시험의 중요 과목으로서 최근 간호사 국가시험에서 과락자수가 1순위-2순위로 많았다. 그러나 이러한 교과목이 암기위주의 국가시험으로만으로 인식이 된다면 간호교육인증평가원의 프로그램 학습성과를 달성할 수 없다. 특히 간호사로서의 수준높은 간호를 요구와 업무가 확대됨에 따라 관련된 책임과 의무가 높아지고 있어 보건의료법규에 대한 학습성과 달성을 위한 중요성의 인식하여 학습성과를 연계하기 위한 노력이 필요하다고 본다.

의료계열 대학생들의 생명 윤리 의식 (Bioethical Perception between Nursing Students and Medical Students)

  • Purpose: The purpose of this study was to provide basic data which is necessary for educating nursing and medical students for establishing a desirable sense of ethics values Method: In this descriptive research, a total of 140 nursing students and 124 medical students were selected. The tools used for data analysis were descriptive statistics, t-test, one-way ANOVA, and Scheffe test by using SPSS/WIN 12.0 version. Result: When the differences in the consciousness of biomedical ethics of nursing and medical students were compared, statistically significant differences were found in the category of brain death (t=-3.26, p=.001). Statistically significant differences were found in the general characteristics of religion (F=3.05, p=.018), and participation in religious activity (F=3.34, p=.006). Statistically significant differences were found in the characteristics related to the subjects' ethical values such as satisfaction level of a nurse/doctor (F=7.39, p=.000) occupation, the experience of A Patient's Bill of Rights (F=2.97, p=.020), the intention to attend biomedical ethics (F=5.61, p=.000), and the possibility of increasing ethical problems according to the development in the field of medical science (F=7.92, p=.000). Conclusion: An effort should be made to overcome the differences in the perception of biomedical ethics between nursing students and medical students. Moreover, when investigating the course of an integrated education for the establishment of a desirable sense of biomedical ethics, an alternative plan that could promote ethical values must be considered so that the significant variables can promote bioethical perception.

보건의약관계법규 교육이 간호대학 4학년 학생의 윤리적 가치관, 사회적 책무성 및 정치효능감에 미치는 효과 (Effect of Health and Medical Law Education on Ethical Values, Social Responsibility and Political Efficacy in Senior Year Nursing Student)

  • 본 연구는 보건의약관계법규 교과목 수업 전·후 윤리적 가치관, 사회적 책무성 및 정치효능감에 미치는 효과를 확인하기 위해 시도되었다. 연구대상자는 일개 대학교 간호대학 4학년 학생 59명을 대상으로 한 단일군 사전·사후 실험설계로 2022년 9월부터 12월까지 자료수집을 하였다. 자료분석은 SPSS 27.0 프로그램을 이용하여 빈도분석, 평균과 표준편차, paired t-test로 분석하였다. 연구결과 보건의약관계법규 수업 후 학생의 윤리적 가치관(t=-6.728, p<.001), 사회적 책무성(t=-5.842, p<.001) 및 정치효능감(t=-6.378, p<.001)은 유의한 차이를 보였다. 보건의약관계법규 수업 시 학생들이 능동적으로 참여할 수 있는 프로그램 및 교수방법의 개발 등이 필요할 것으로 보인다.

Nursing Students' Ethical Attitudes toward DNR: The Impact of Educational Experience

  • 본 연구는 전북소재 일개 간호대학생 209명을 대상으로 DNR에 대한 윤리적 태도를 파악하고자 2022년 9월에서 11월까지 자료수집하고, 수집된 자료는 SPSS WIN 25.0 프로그램을 이용하여 기술 통계와 χ2-test로 분석하였다. 간호대학생들의 DNR에 대한 윤리적 태도를 살펴본 결과, 환자의 의사존중, 정확한 정보제공, DNR 지침준수에 대체로 동의하였으며, DNR 결정 후에라도 보호자의 요청에 따른 치료 제공 및 DNR 환자에 대한 무균 원칙 유지 등에 동의하는 경향이 있었다. 가족이 원할 경우 인공호흡기 작동 중단, 살 가망이 없는 환자라도 모든 방법을 동원한 생명 연장, 주치의의 DNR 결정 등에는 중립적 태도를 나타냈다. DNR이 선언된 후 의료진의 관심 감소는 반대의견이 많았다. 19문항 중 '말기 환자에게 모든 치료를 사용하는 것'(p=.028)과 '동료가 DNR 환자를 무균절차없이 치료하는 것을 목격하면 즉시 조언한다' 문항(p=.014)에서 DNR 관련 교육경험에 따른 유의한 차이가 있었다. DNR 상황과 절차, 관련 기준에 대한 더 정확한 근거에 대한 교육이 제공되어야 하며, 높은 수준의 상황적 윤리적 역량을 유지하기 위한 체계적인 교육이 필요하다.

간호대학생의 생명의료윤리인식과 연명치료중단에 대한 태도 (Awareness of Nursing Students' Biomedical Ethics and Attitudes toward Withdrawal of Life-Sustaining Treatment)

  • Purpose: This study was done to analyze nursing students' attitudes to life-sustaining treatment by measuring their awareness of biomedical ethics and resulting attitude toward withdrawal of life-sustaining treatment. Methods: This study was a descriptive research to assess the level of nursing students' awareness of biomedical ethics, attitude toward withdrawal of life-sustaining treatment, and correlations between these variables. After the nursing students signed a consent form they were assessed. Data collection was done from September 1 to October 25, 2016, and analyzed using SPSS 23.0 WIM Program. Results: There was a negative correlation and significant difference between nursing students' awareness of biomedical ethics and attitude toward withdrawal of life-sustaining treatment. There was a significant correlation among attitude toward withdrawal of life-sustaining treatment and medical ethics, mortality ethics. Conclusion: The findings in the study indicate that it is necessary to provide nursing students with easy access to continuous education to help them establish an acceptable view of withdrawal of life-sustaining treatment.

간호학생의 윤리적 가치관 (The Ethical Values of Nursing Students)

  • The purpose of this study was to identify whether ethical values of nursing students are deontological or utilitarinian. The subjects of this study were consisted of 163 nursing students. The data was collected from September 3 to 30, 1997. Sub-scale of Nursing Ethical Values(Lee, 1990) were used to measure human life area, nurse-patient relationship area, nurse-nursing task relationship area, nurse-colleague relationship area. Data were analyzed by SPSS for Windows program. The results of this study were summarized as follows: 1. The students took the position of utilitarian slightly in human life area. 2. The students took deontological position slightly in nurse-patient relationships area. 3. The students took deontological position slightly in nurse-nursing task relationships area. 4. The students took deontological position greatly in nurse-colleague relationships area. 5. There were significant relationship between nurse-colleague relationship area and both human life area and nurse-patient relationship area(p< .01). 6. The ethical values of human life area related to religion(p< .01) and attitude of nursing(p< .05). The ethical values of nurse-colleague relationships area related to attitude of nursing(p< .01).

일개 종합병원 임상 간호사의 경력별, 근무부서별 간호윤리관 비교 (A Comparison of Clinical Nurses' Nursing Ethical Values according to Career and Characteristics of Nursing Unit in General Hospital.)

  • Purpose: This study was conducted to explore and compare the level and type of nursing ethical values according to career and nursing units of clinical nurses, Methods: The subjects were 944 nurses working in General Hospital in Seoul and the data were collected from March 8 to August, 2007 using the nursing ethical values questionnaire, The collected data were analyzed using SPSS WIN 10.0 program. Results: The results were as follows; The level of clinical nurses' ethical values were significantly different according to career and nursing units (p<.01) and it was found that most clinical nurses had deontological ethics in the areas of professional nursing domain ($2.77{\pm}.31$), cooperative relations domain ($2.90{\pm}.26$) and customer relation domain ($3.03{\pm}.32$). but that they had utilitarian ethics in human life domain ($2.53{\pm}.32$). Conclusion: Different approaches are needed to develop to establish clinical nurses' ethical values based on career, characteristics of nursing unit and various clinical situations.

간호에서의 보살핌에 대한 철학적 탐구 -리쾨르(Ricoeur)의 이야기 윤리에 기초하여- (A Philosophical Inquiry into Caring in Nursing - Based on Ricoeur's Narrative Ethics -)

  • Purpose: This paper was aimed to inquire into Ricoeur's self -hermeneutics and narrative ethics, and apply it to personal identity constituting caring and care ethics in the practice of nursing. Its purpose is to provide a philosophical foundation for caring in nursing. Method: According to Ricoeur's narrative identity, ontological caring was interpreted as personal identity constituting caring. His ethics were described as care ethics, which contributed to preserving and promoting the personal dignity of the client, as self in search for the good life in the nursing practice. Results: Narrative understanding of the client pointed to the ontological role of care in the constitution of personal identity. From an ethical aspect of the narrative, respect for personal identity and personal dignity of the client was crucial to an ethical caring attitude, promoting self-esteem in the nursing practice. Conclusion: This paper suggested that Ricoeur's ethics could provide a philosophical basis for understanding ontological and ethical caring in nursing. This contributed to protection of the client from the threat of personal identity, as well as respecting their personal dignity.

요양병원 간호사의 윤리적민감성과 개인-환경적합성이 인간중심돌봄에 미치는 영향 (Influence of Ethical Sensitivity and Person-environment Fit on Person-centered Care of Nurses in Long-term Care Hospitals)

  • Purpose: The study examined the effects of ethical sensitivity and person-environment fit on person-centered care of nurses in long-term care hospitals. Methods: The participants were 111 nurses. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis with the SPSS program. Results: The mean of person-centered care was 2.95±0.62 out of 5. There were significant differences in person-centered care in terms of age, shift pattern, total clinical career, and educational experience of person-centered care. Person-centered care and ethical sensitivity, person-centered care and person-environment fit showed a positive correlation. Factors affecting the person-centered care were the age (20-29), shift pattern (three shift), person-environment fit. The explanatory power was 42%. Conclusion: These results can be used to increase the person-environment fit and adopt a differented approach based on age and shift pattern in order to enhance person-centered care. Therefore, it is necessary to develop and apply an person-centered care program for nurses in long-term care hospitals.

심폐소생술금지(Do-Not-Resuscitate)에 대한 환자보호자의 윤리적 인식 및 태도 (Ethical Awareness and Attitudes of Patients' Families towards DNR(Do-Not-Resuscitate))

  • Purpose: The purpose of this study was to investigate the ethical awareness and attitude of patients' families towards Do-Not-Resuscitate(DNR), and thus provide basic information required to develop Korean appropriate DNR instructions and practice informed consent for DNR. Methods: During April 2010, 219 patient family members visiting the hospital were surveyed using a questionnaire. Results: Most of the participants preferred DNR to meaningless treatment for incurable patients. They recognized the necessity of explaining DNR to the patient with a terminal disease. They also requested DNR orders for themselves if they were in the same medical condition. In making a DNR decision, the patient's family agreed and preferred that it reflect the opinion of the patient and the doctor in charge. They also agreed that treatment should be given with the best efforts even if a DNR decision had been made for the patient. Conclusion: To make a decision on DNR for a patient who is terminally ill or for whom survival is not possible, a practice of informed consent and guidelines for executing the DNR reflecting the patient's opinion are required.
