종합병원 간호관리자의 간호관리역량 요구도 분석 (Need analysis for managerial competencies of nurse managers in general hospitals)

  • Purpose: This study aimed to identify the needs for the managerial competencies of nurse managers in general hospitals. Methods: A cross-sectional design was used. The participants were 203 nurse managers, 141 head nurses or unit managers, and 62 directors or team managers from 17 general hospitals. Data were collected by a self-administered questionnaire with 42 nursing management competencies and 181 behavioral indicators. Participants rated the importance and performance of each indicator. Data were analyzed using a paired t-test, independent t-test, importance-performance analysis, Borich's needs assessment model, and locus for focus model. Results: In all 42 competencies, importance was rated significantly higher than performance. The head nurses or unit managers had high demands for "human resource development," "performance management," and "information management," while the directors or team managers had high demands for "change management." Competencies in high demand in both groups were "development of nursing standards" and "self-management." Conclusion: Based on the results, it is necessary to select priorities and prepare educational content when developing educational programs to strengthen the nursing management competencies of general hospitals' nurse managers.

종합병원 간호사의 섬망 간호수행 영향요인 (Factors Affecting Nursing Performance of Delirium for General Hospital Clinical Nurses)

  • Purpose: This study was conducted to identify factors affecting nursing performance of delirium among general hospital clinical nurses. Methods: This was a cross-sectional study and conducted from July 20 to July 30 2015, with a sample consisting of 167 registered nurses in three general hospitals. Data were analyzed with SPSS 21.0. Results: Nursing performance of delirium had significant positive correlations with knowledge about delirium (r=.25, p=.001), self-confidence in caring for patients with delirium (r=.45, p<.001) and nursing work environment (r=.29, p<.001). In this study, factors affecting nursing performance of delirium were self-confidence in caring for patients with delirium (${\beta}=.33$, p<.001), nursing work environment (${\beta}=.26$, p<.001), experience in nursing education about delirium(${\beta}=.18$, p=.007) and clinical experience (${\beta}=.18$, p=.007). The factors explained 32% of nursing performance of delirium. Conclusion: This study's results can be used as basic data to develope educational programs related to delirium care through continuing education and to improve work environments in developing strategies to enhance nursing performance for patients with delirium.

간호학의 기본 지식 구조 (Fundamental Structure of Knowledge in Nursing)

  • 오늘날은 과학문명의 발달로 인하여 기존지식의 수명이 점차 짧아져 가고있는 것이 특징이다. 지식의 증가는 단순히 지식의 양을 증가시키는 역할뿐 아니라 많은 기존지식을 불충분하고 쓸모 없는 것으로 바꿔버리게 한다. 그러므로 학교에서는 학생들에게 어떤 특정지식의 축척보다는 그 학문에 내재해 있는 기본적인 지식의 구조를 학습하게 하여 여러 가지 개념을 관련시키는 논리적 방법을 학습하게 하고 합리적인 탐구방법을 구사할 수 있도록 하여 변화하는 미지의 세계에 대처해 나갈 수 있도록 하는 것이 중요하다. 본 연구는 간호학의 기본 지식구조를 확인하는데 그 목적이 있다. 본 연구를 하게 된 동기는 간호업무의 근거로 활용도리 지식체계는 교육과정의 조직원리로 작용될 유형이나 구조를 지니고 있으며 이런 유형이나 구조를 중심으로 간호교육과정을 구성하는 것이 간호교육에 필수적이라는 문제에서 비롯되었다. 연구방법은 1982년 9월부터 1983년 5월에 걸쳐 간호학 문헌을 체계적으로 분석하여 간호학의 개념적 지식구조와 구문적 지식구조를 확인하였다. 그 결과 얻어진 결론은 다음과 같다. 1. 간호학의 개념적 구조: 모든 학문에는 탐구의 대상인 특수현상을 설명하고 서술하는데 활용되는 일련의 실질적, 개념적 구조를 가지고 있다. 그러나 그 학문의 중요한 부분 또는 중심을 포함하고 있는 개념들이 그 분야 또는 학문의 개략이라고 할 수 있는데 연구결과 간호학에서 가장 높은 순위의 대표적 특질을 지닌 개념은 인간, 건강, 환자/대상자, 간호, 행동으로 분석되었다. 2. 간호학의 구문적 지식구조 : 지식구조의 두 번째 요소인 학문의 구문(syntax)은 간호학에서의 특징적인 탐구방법과 관련되나 개념적 구조와 마찬가지로 탐구방법은 학문에 따라 다르며 그 분야의 주요양상을 나타낸다. 연구결과 간호학에서의 특징적인 탐구방법은 공동적으로 간호과정(nursing process)임이 나타났으며 그 요인으로는 사정, 진단, 계획, 수행, 평가의 다섯 단계로 분석되었다.

내부마케팅과 동기부여, 간호조직유효성간의 경로모형구축 (A Path Analysis among the Internal Marketing Activities, Motivation, and Nursing Organizational Effectiveness)

  • Purpose: The aim of this study was to analysis path model of the research variables. Methods: The subjects of this study were 647 nurses who were working in the 8 general hospitals located in Seoul and Incheon area. The data were collected by self-reporting questionnaires. The data were analyzed using descriptive statistics and path analysis. Results: In the modified path model, overall fitness indexes were $X^2$= 223.27, goodness of fit index=0.90, root mean square residual=0.039, root mean square error of approximation=0.12, non-normed fit index=0.96, and normed fit index=0.90. From the model, among research variables that influence organizational effectiveness motivator, job satisfaction and organizational identification affected directly work performance. In internal marketing factors, paid-leave, communication and reward affected directly motivator. Motivator and hygiene factors affected directly job satisfaction, organizational commitment and organizational identification. Conclusion: With these findings, paid-leave, communication, reward, motivator, job satisfaction and organizational identification were direct or indirect predictors of the work performance. Therefore nursing managers ought to develop internal marketing strategies and motivation enhancing system for nurses based on this path model in order to improve the nursing organizational effectiveness.

재가간호서비스 제공자의 업무 수행 현황과 장애요인 (Current Status of Home Visit Programs: Activities and Barriers of Home Care Nursing Services)

  • Purpose: The purpose of this study was to examine the current status of home care nursing services provided by community health nurses and to identify barriers to the services. Methods: A cross-sectional survey was conducted with three types of community health care nurses. Participants were 257 nurses, 46 of whom were hospital based home care nurses, 176 were community based visiting nurses, and 35 were long term care insurance based visiting nurses. A structured questionnaire on 7 domains of home care nursing services with a 4-point Likert scale was used to measure activities and barriers to care. Data were analyzed using SPSS WIN 21.0 program. Results: Hospital based home care nurses showed a high level of service performance activity in the domain of clinical laboratory tests, medications and injections, therapeutic nursing, and education. Community based visiting nurses had a high level of service performance in the reference domain. Long term care insurance based visiting nurses showed a high level of performance in the service domains of fundamental nursing and counseling. Conclusion: The results show that although health care service provided by the three types of community health nurse overlapped, the focus of the service is differentiated. Therefore, these results suggest that existing home care services will need to be utilized efficiently in the development of a new nursing care service for patients living in the community after hospital discharge.

보건소장의 변혁적 리더십과 보건간호사의 사회적 자본이 임파워먼트에 미치는 영향 (Influence of transformational leadership of managers perceived by public health nurses and their social capital on empowerment)

  • 본 연구는 보건간호사의 임파워먼트 정도를 파악하고 보건간호사가 인식한 관리자의 변혁적 리더십과 보건간호사의 사회적 자본이 임파워먼트에 미치는 영향 요인을 검증하였다. 서술적 조사 연구로써 대구광역시 8개 보건소와 경상북도 23개 보건소 및 1개 보건의료원에 근무하는 정규 간호직 공무원인 보건간호사 전체 360명을 대상으로 2016년 2월 29일부터 4월 8일까지 설문지를 이용하여 자료 수집하였고, 수집된 자료는 IBM SPSS 21.0 프로그램을 이용하여 빈도, 백분율, 평균과 표준편차, 독립 t-test, ANOVA, Scheffe test, Pearson correlation coefficient 및 위계적 다중선형회귀분석으로 분석하였다. 연구 결과, 보건간호사의 임파워먼트는 보건간호사가 인식한 보건소장의 변혁적 리더십과 보건간호사의 사회적 자본이 증가할수록 보건간호사의 임파워먼트가 증가하는 것으로 나타났다. 이에 보건소장이 변혁적 리더십을 발휘한다면 보건소 업무의 중추적인 역할을 감당하는 보건간호사의 임파워먼트는 높아지고 업무수행에 발휘될 것이고. 또한 보건간호사의 사회적 자본을 증가시켜 준다면 보건 간호사의 임파워먼트는 증가시킬 수 있음을 확인하였다.

욕창, 낙상예방 및 통증간호의 간호과정 적용 평가도구 개발 (Development of Performance Measures Based on the Nursing Process for Prevention and Management of Pressure Ulcers, Falls and Pain)

  • Purpose: This study was conducted to construct evidence based clinical guidelines and to develop nursing process based performance measures for prevention and management of pressure ulcers, falls and pain. Method: Clinical guidelines were drafted through a comprehensive review of relevant literature, national guidelines and hospital protocols. The proposed guidelines were reviewed by a panel of experts and 90 hospital nurses, and refined on the basis of their suggestions. Nursing process based performance measures were developed based on the clinical guidelines and content validity was examined by surveys from 90 hospital nurses. Results: All items, except timetable for position change and pressure ulcer nursing record, in the guidelines for prevention and management of pressure ulcer were appropriate. Most items, except fall risk assessment tools, were appropriate for the guidelines of fall prevention. All other items, except the purpose of pain management, were appropriate for the guidelines of pain management. Performance measures developed in this study were acceptable as a tool to evaluate quality of nursing care. Conclusion: Nursing process based performance measures provide important indicators to monitor whether necessary nursing care is implemented and can be used as the primary resources to improve quality of nursing services.

경인지역 보건소 방문간호사의 의사결정유형에 따른 셀프리더십과 의사소통 능력 (Relations among the Decision Making Style, Self Leadership and Communication Competence of Visiting Nurses)

  • 현장에서 직접 대상자의 가정을 방문하여 요구사정에서부터 중재, 평가에 이르기까지 간호제공의 전 과정을 수행해야 하는 방문간호사의 업무수행 능력을 제고의 관점에서 의사결정 유형과 셀프리더십 그리고 의사소통 능력간의 관련성을 분석하였다. 방문간호사들의 의사결정 유형과 의사소통 능력과 셀프리더십은 모두 상관관계가 있었으며 서로 유기적인 관계가 있음을 확인하였다. 방문간호사의 의사결정 유형은 합리적 유형이 가장 많은 것으로 나타났으며, 이와 관련하여 셀프리더십 정도와 의사소통 능력 측면에서도 합리적 의사결정 유형에서 가장 높은 점수를 나타냈다. 합리적 의사결정 유형은 의사결정 유형 중 가장 바람직한 것으로 논의되어 온 바 방문간호사가 높은 문제해결 능력을 가지고 있음을 실증적으로 보여준 사례로 해석할 수 있다.

간호사의 팔로워십에 따른 직무만족, 조직몰입 및 간호업무수행 (Effects of the Followership on Organizational Commitment, Job Satisfaction, Nursing Task Performance)

  • Purpose: This study was designed to find out the fact that the followership types of nurses would effect on job satisfaction, organizational commitment and nursing task performance. Methods: The sample consisted of 219 nurses working in 2 medical institutions. SPSS program was used for descriptive statistics, ANOVA, chi-square test, Duncan test, Pearson Correlation, and Multiple Regression. Results: In the followership type, effective follower was the largest as 47.0% and passive follower was 26.9%. Nurses who had a effective followership style had the highest level of organizational commitment, job satisfaction and those who had a passive followership style had the lowest level of organizational commitment, job satisfaction, and nursing performance. The followership was significantly related to the organizational commitment, job satisfaction and nursing task performance. The active involvement had a positive effect on the organizational commitment, job satisfaction and nursing task performance. The critical thinking had a positive effect on the job satisfaction. Conclusion: These results demonstrate that the most effective followership is effective follower. And organizational managers must take into account followership types when they develop the strategies for organizational effectiveness.

혈액투석실 간호사의 근무환경이 간호업무수행에 미치는 영향: 직무만족 및 임파워먼트의 효과를 중심으로 (Effect of Work Environment on Nursing Performance of Nurses in Hemodialysis Units: Focusing on the Effects of Job Satisfaction and Empowerment)

  • Purpose: This study was performed to investigate the effect of work environment on nursing performance and the effect of job satisfaction and empowerment on nurses performance in hemodialysis units. Methods: Participants in this study were 206 nurses from 22 hemodialysis units in local clinics, general and university hospitals in two metropolitan areas. The work environment, nursing performance, job satisfaction and empowerment of the participants were measured using four self-report questionnaires. Data were analyzed using SPSS/WIN 18.0. Results: Nursing performance correlated significantly with work environment, job satisfaction, and empowerment in the participants. Predictability of work environment for nursing performance was 28%. Job satisfaction and empowerment showed complete mediating effects, but not moderating effects in the relationship of work environment and nursing performance in the participants. Conclusion: Findings indicate that work environment is an important variable affecting nursing performance in nurses in hemodialysis units and that job satisfaction and empowerment are mediating variables in the relationship of work environment and nursing performance in nurses in hemodialysis units. Improvements in the work environment are needed to induce the high job satisfaction and empowerment that can lead to improvement of nursing performance.
