Comparisons of the Nursing Workforce with Japan, and the U.S. (한국, 미국, 일본의 간호인력 양성 및 활동 현황 비교)

  • This study aims to derive implications on current problems in the nursing manpower management in Korea through the comparison with the nursing workforce and employmentrent in the United States and Japan. There are various issues in nursing policy, such as nursing shortage, quality of the nursing service, and the increased cost of management due to the high turnover rate of nursing staffs and it is urgent to seek various policy measures to resolve this. Although nursing shortage is a commom problem in the world, the U.S. and Japan were higher rate of employment than Korea in nursing staffs, which implicates the importance of the legislation of mandatory minimum staffing ratios, the establishment of policies such as the fees and charge policy and the nursing work environments. For quality nursing care and patient safety through the stable workforce of qualified nursing staffs, administrative mechanisms that support adequate nurse staffing and promote positive work conditions are needed, for which the improvement of legal system is required.

A Study on Knowledge, Attitude, toward Patient Safety and Patient Safety Care Activities among Long-Term Care Hospitals' Nursing Assistants (요양병원 간호보조인력의 환자안전에 대한 지식, 태도 및 환자 안전활동)

  • This study investigated knowledge and attitudes of long-term care hospital nursing assistants such as nursing aides and direct care workers toward patient safety and their safety care activities. This was a descriptive study that attempted to enhance the competence of nursing assistants' patient safety management, and the data were used to develop a training program for nursing assistants. A total of 230 nursing assistants working at six hospitals whose wards were over 200 participated and were assessed with a rating 1 or 2 in 2013 in B city. Data were collected using questionnaires from March 2 to March 26 in 2017. The collected data were analyzed by the SPSS/WIN 20.0 program using number, percentage, mean, standard deviation, t-test, ANOVA, $Scheff{\acute{e}}$ test, correlation coefficient, and multiple regression. In conclusion, the findings show that patient safety care activities were better performed by nursing aides than direct care workers. Higher education and a higher number of patient safety training programs and patient safety care activities were frequent. In addition, nursing assistants' knowledge of patient safety affected attitudes toward patient safety, and attitudes affected patient safety care activities. Therefore, an actual regular training program for patient safety should be executed over five times a year to improve patient safety perception. Furthermore, a hospital system including adequate working time needs to be improved. Based on the results of the study, patient safety in long-term care hospitals should be improved, and this study could be utilized as basic data to develop a training course for improvement of knowledge and attitudes of nursing assistants toward patient safety.

Institutionalization of a Patient-Sitter Program in Acute Care Hospitals (보호자 없는 병원 제도화 방안)

  • In an effort to resolve the burden of patients hiring patient sitters, this study sought to review the Ministry-of-Health-and-Welfare-initiated pilot program of running hospitals without patient-sitter to identify its background, operation method, performance results, and limitations. Based on the review, the study derived the necessity of introducing a hospital system without patient-sitter as well as its operation and systemization methods. The ministry-initiated pilot programs were conducted twice: in 2007, and in 2010. A review of the 2007 pilot program revealed that the patients and families' satisfaction score with nursing services was 9.1 points (on a 10-point scale), their intention to reuse the service was 97.8%, and their intention to recommend the service was 98.0%, all high scores. Appropriate nursing manpower, derived from the 2007 pilot project, indicated 2.3 patients per nurse and 4.0 patients per nurse aid. The 2010 pilot project results indicated that the patients and families' satisfaction was high at 8.0-9.1 points (on a 10-point scale), and that the intention to reuse and recommend the service was also high. Compared with the 2007 pilot project, however, the types of medical institutions and the nurse to patient ratios were diverse, offering limitations. In conclusion, to systemize hospitals without patient-sitter, it is necessary to develop policies designed to establish criteria for the appropriate nurse to patient ratio and skill-mix, to standardize the work, to prepare finances for securing nursing staff, to evaluate the nursing demands, and to monitor the quality management.

  • 본 연구는 와상환자에게 자주 발생하는 낙상, 욕창, 불면증을 영상기술을 통해 인식하여 섬망의 조기진단 모형을 구현한다. 실시간 모니터링을 통해 섬망 잠재환자를 선별하고 집중적인 관리와 치료로 이어질 수 있도록 간호인력을 보조하는 데 주된 목적을 두고 있다. 과활동형 섬망은 파생위험 중 하나인 낙상과, 저활동형 섬망은 원인 요소인 욕창과 묶어 자세인식을 통해 판정한다. 또한 주로 밤에 악화되는 섬망의 특성을 고려해 눈 깜빡임을 통한 불면증 검사를 추가로 반영하였다. 낙상과 욕창을 섬망과 묶어 융복합적인 위험예측 시스템을 구축함과 동시에, 기존의 섬망 사정도구들이 지니는 시공간적 제약을 개선함으로써 간호인력의 부담을 덜어줄 것으로 기대된다.

The Current Status of Professional Medical Support Staffs in General Hospitals less than 500 Beds -Focusing on Nursing Staffs- (500병상 미만 종합병원에서의 전문지원인력 운영실태 -간호인력을 중심으로-)

  • Purpose: This study aimed to investigate the current statistics of professional medical support staffs(PMSSs) working in general hospitals with less than 500 beds. Methods: This study was conducted on 35 general hospitals with less than 500 beds from September 11th to October 27th, 2017. Results: Four hundred fifty one PMSSs were currently providing medical support. The number of clinical nurse experts was the highest among the roles, followed by Physician Assistants(PA) and Advanced Practice Nurses. The mean job satisfaction score was 3.07 out of 5. In the case of PA group, most of the delegated prescriptions were performed, however the delegated roles were not much documented in written format. The paucity of documentation requires a development of a committee for PMSSs, including a development of selection criteria and a scope of practice in each institution. Conclusion: The results of this study suggested the composition of a committee for PMSSs in the medical institutions and renaming the specified titles of PMSSs.

A study on delegation level of nursing activities, nursing professionalism and turnover intention for long-term care hospital nurses (요양병원 간호사의 간호행위 위임정도, 간호전문직관 및 이직의도와의 관계)

  • This study was aimed to prepare the evidences for establishing policies on nursing manpower by identifying the relationship among delegation level of nursing activities, nursing professionalism and turnover intention in long-term care hospital nurses. The study was a descriptive survey research that used a structured questionnaire. 146 nurses agreed to take part in the research. They worked in six long-term care hospitals located in A and B province. Data were collected from July to August 2016 and analyzed through SPSS 20.0 program, using descriptive statistics, t-test, ANOVA and pearson's correlation coefficients. As a results, first, as for the delegation of nursing activities by long-term care hospital nurses, the levels of delegation of direct nursing activities were 34.63±14.12, and the level of delegation of indirect nursing activities were 1.71±1.75. Second, the mean score of nursing professionalism and the turnover intention of long-term care hospital nurses were 3.62±0.77 and 2.71±0.97, respectively. Third, the delegation of direct nursing activities by long-term care hospital nurses significantly differed depending on the number of nurses in the ward and the number of caregivers in the ward. The delegation of indirect nursing activities significantly differed depending on the number of nurses in the ward, the presence of conflicts and stress related to delegation. Forth, there was no significant correlation of the delegation of nursing activities by long-term care hospital nurses with nursing professionalism and with turnover intention. Most of nursing activities, were delegated to nurse aids, since there were no clear guidelines or legal procedures on nursing activities in long-term care hospital nurses. Therefore it is required to find how to clearly establish positive nursing professionalism and to develop a proper nursing manpower arrangement and the guidelines for the delegation of nursing activities in a long-term care hospital in terms of policy.

Relationships among personal and organizational communication skills, occupational stress, and patient safety activities of nursing workforce working in the integrated nursing care service ward (간호·간병통합서비스병동 간호사와 간호보조인력의 개인 및 조직 의사소통능력, 직무스트레스와 환자안전활동의 관계)

  • Purposes: This study aimed to identify the relationships among personal and organizational communication skills, occupational stress, and patient safety activity levels of two nursing workforce groups (nurses and nursing assistants) who provide integrated nursing care services. Methodology: The study design is a cross-sectional study. Seventy-one nurses and forty-three nursing assistants working in the integrated nursing care service wards participated in this study. The data were collected using a self-reported questionnaire from June to July 2021. The relationships among the variables were analyzed using the Pearson correlation coefficient. Findings: Nurses' personal communication skills (r=.294, p=.013), organizational communication skills (r=.408, p<.001), and occupational stress (r=.243, p=.041) were associated with their patient safety activities. However, nursing assistants' personal communication skills, organizational communication, and occupational stress were not correlated with their patient safety activities. Practical Implication: Patient safety activities of nurses were related to their communication skills and occupational stress, but nurse assistants were not. Therefore, nurses should encourage nursing assistants to responsibly engage in patient safety activities and supervise their works appropriately to achieve high-quality care.

An Algorithm for Detecting Residual Quantity of Ringer's Solution for Automatic Replacement (링거 자동 교체를 위한 잔량 검출 알고리즘)

  • Recently, ere are many researches to improve the quality of e medical service such as Point of care (POC). To improve the quality of the medical service, not only good medical device but also more man power is required. Especially, the number of nurses are very few in Korea that is almost the lowest rank compared to OECD countries. If the simple repetition works of the nurse could be removed, it is possible to use the skillful nurse for other works and provide better quality services. There are many simple repetition works which the nurses have to do, such as replacing the ringer's solution. To replace the ringer's solution automatically, it is necessary to detect residual quantity of the ringer's solution. In this paper, image processing is used to detect the residual quantity of ringer's solution, and modified self quotient image (SQI) algorithm is used to strong background lights. After modified SQI algorithm, the simple histogram accumulation is done to find the residual quantity of the ringer's solution. The implemented algorithm could be use to replace the ringer's solution automatically or alarm to the nurses to replace the solution.

A Convergence Investigation on Nursing Task Performance, Appropriate Performer, and Job Satisfaction of Nurses with Shift Work in General Ward (교대근무를 하는 일반병동 간호사의 간호업무 수행실태, 수행주체와 직무만족에 대한 융복합적 조사연구)

  • This study is a secondary data analysis to investigate the nursing tasks performance, appropriate performer, and job satisfaction in the general wards with sift work. We found that nurses performed direct (45%) and indirect nursing (55%). The workload per shift was 37.2% in the day, 35.6% in the evening, and 27.2% at night. The tasks performed after handover were 'direct nursing (34.5%)' and 'documentation and notification (25.8%)'. Nurses responded that there were some tasks that could be delegated to nursing assistants or had ambiguous boundaries with other medical personnel. There was a significant correlation between compliance to the job description of the night shift and job satisfaction (rs=.43, p=.01). These results imply that it is necessary to establish strategies that will enhance work efficiency based on job analysis by shift work, reduce handover time using EMR system, stmart devices, and clarify appropriate performers.

  • Purpose: This study was to investigate the nationwide operational status of the professional medical support staffs (PMSS) who practice the expanded roles in the hospital setting. Methods: The data were obtained through survey from 36 hospitals with over 500 beds from 25th May to 12th July 2016. Data from 1,666 PMSS were analyzed. Results: Since the job titles varied, we classified them into 5 groups according to their roles; advanced practice nurse, clinical nurse expert, PA (physician assistant), coordinator, and others. There were differences in the operation status of PMSSs depending on the region, nurse staffing grade and number of hospital beds. Qualification criteria varied from hospital to hospital, and almost half of the hospitals didn't have any qualification standards for them. There were differences in age, educational level, clinical careers, rewards, and job satisfaction in 5 groups. Especially PA group had low salary, poorer working conditions, more difficulties in performing their work, and lower job satisfaction than other groups. Most PMSS (99.5%) were using a delegated prescription authority, however only 68.3% had job description and 19.9% had documented delegated role. Conclusion: Adequate training curriculum, documented delegated roles, and the protocols for legal protection and efficient medical services are needed.
