Factors Influencing Job Satisfaction Among Nurses in Integrated Nursing Care Service Wards (간호·간병통합서비스 병동 간호사의 직무만족에 미치는 영향요인)

  • Objectives: This descriptive survey study aimed to identify factors that affect job satisfaction among nurses working in integrated nursing care service wards and to explore measures to increase job satisfaction among these nurses. Methods: Data were collected between July 20 and August 3, 2018 from nurses working in integrated nursing care service wards in Busan and Changwon city, in Korea. Data were analyzed with SPSS/WIN 23.0 software(IBM corp., Armonk, NY, USA). Results: Factors affecting job satisfaction among nurses working in integrated nursing care service wards were identified as follows: professional self-concept (${\beta}=.32$, p=.003), which is a subdomain of nursing professionalism, and nursing foundations for quality care (${\beta}=.30$, p=.001), which is a subdomain of nursing work environment. The model had an explanatory power of 54.7%. Conclusions: Nursing professionalism should be strongly established among nurses, as nurses are key players in a health system that allows stable and expanded operations of integrated nursing care. Intervention measures that create a better work environment for nurses should also be implemented. Furthermore, it is necessary to develop systematic strategies and multifaceted efforts to increase nurses' job satisfaction in integrated nursing care service wards.

Level of Complete Knowledge on Five Moments of Hand Hygiene among Nurses Working at Integrated Nursing Care Service Wards (간호간병통합서비스 병동 간호사의 손위생 시점에 대한 완전지식 수준)

  • Purpose: This study aimed to identify the level of complete knowledge about hand hygiene indications among nurses working at integrated nursing care service wards. Methods: A total of 127 nurses in eight integrated nursing care service wards completed structured sheets while observing a video based on six scenarios developed by the research team. Complete knowledge level was calculated as the percentage (%) of participants who responded correctly to all questions among participants. Complete knowledge levels according to the scenarios were calculated and compared according to general characteristics using the chi-squared test or Wilcoxon rank-sum test. Results: The complete knowledge level for each scenario ranged from 7.9% (scenario 6) to 42.5% (scenarios 4 and 5), and no one had complete knowledge of all scenarios. Only 3.1% of participants demonstrated complete knowledge in more than four scenarios, and 26.0% had complete knowledge of four or more hand hygiene moments. Complete knowledge level per scenario did not differ depending on work experience at hospitals and study wards, or prior hand hygiene training in the last year. Conclusion: As the complete knowledge level regarding hand hygiene moment is very low, it is suggested that regular hand hygiene training should be provided to nurses using video media that reflect real nursing tasks. Thus, they can acquire complete knowledge of when hand hygiene is needed or not during complex nursing work situations.

Factors Affecting Turnover Intention of Nurses in Comprehensive Nursing Care Service Wards: Focusing on Occupational Stress, Emotional Labor, and Burnout (간호·간병통합서비스 병동 간호사의 이직의도 영향요인: 직무 스트레스, 감정노동 및 소진을 중심으로)

  • Purpose: The purpose of this study was to explore the factors affecting turnover intention of nurse who work in comprehensive nursing care service wards. Methods: The study design was a cross-sectional study. We recruited participants in 5 general hospitals located in Gyeonggi. Data were collected using structured questionnaires. Data of 150 nurses were included in the final analysis. Multiple regression analysis was used to identify the influencing factors on turnover intention. Results: Work in the thoracic surgery department (β=.158, p=.045), Emotional labor (β=.282, p=.004), occupational stress (β=.222, p=.004), and burnout (β=.249, p=.003) were identified as factors influencing turnover intention. These factors explained 39.1% of the variance of turnover intention. Conclusion: The findings suggest that it is important to reduce emotional labor and occupational stress to reduce turnover intention for nurse in comprehensive nursing care service wards.

Relationships among personal and organizational communication skills, occupational stress, and patient safety activities of nursing workforce working in the integrated nursing care service ward (간호·간병통합서비스병동 간호사와 간호보조인력의 개인 및 조직 의사소통능력, 직무스트레스와 환자안전활동의 관계)

  • Purposes: This study aimed to identify the relationships among personal and organizational communication skills, occupational stress, and patient safety activity levels of two nursing workforce groups (nurses and nursing assistants) who provide integrated nursing care services. Methodology: The study design is a cross-sectional study. Seventy-one nurses and forty-three nursing assistants working in the integrated nursing care service wards participated in this study. The data were collected using a self-reported questionnaire from June to July 2021. The relationships among the variables were analyzed using the Pearson correlation coefficient. Findings: Nurses' personal communication skills (r=.294, p=.013), organizational communication skills (r=.408, p<.001), and occupational stress (r=.243, p=.041) were associated with their patient safety activities. However, nursing assistants' personal communication skills, organizational communication, and occupational stress were not correlated with their patient safety activities. Practical Implication: Patient safety activities of nurses were related to their communication skills and occupational stress, but nurse assistants were not. Therefore, nurses should encourage nursing assistants to responsibly engage in patient safety activities and supervise their works appropriately to achieve high-quality care.

Factors Influencing Professional Quality of Life in Intensive Care Unit Nurses of University Hospitals (간호·간병통합서비스병동 간호사의 직무스트레스가 소진에 미치는 영향: 감성지능의 조절 및 매개효과 )

  • Purpose : This study aims to identify job stress, emotional intelligence, and exhaustion of the nurses in comprehensive nursing service units and then verify the moderating and mediating effects of emotional intelligence on the relationship between job stress and exhaustion. Methods : Participants are 118 nurses working in two general hospitals. The collected data are analyzed using the SPSS WIN 27.0 and AMOS 18.0 programs by assessing frequency and percentage, mean and standard deviation, independent t-test and one-way ANOVA, Scheffé test, Pearson correlation coefficient, and hierarchical multiple regression analysis and bootstrapping. Results : Exhaustion has a statistically significant positive correlation with job stress (r=.40, p <.001) and a statistically significant negative correlation with emotional intelligence (r=-.26, p =.004). A partial mediating effect of emotional intelligence is found between job stress and exhaustion, however, there is no moderating effect. Conclusion : It is expected that to reduce the exhaustion of nurses in comprehensive nursing service units, it is necessary to assess job stress and emotional intelligence, and strengthen emotional intelligence along with job stress intervention.

  • 최근 저출산 문제로 인한 노동인구의 감소로 노동력이 감소하며 인건비가 증대되고, 의료 기술의 발달과 고령화로 인해 병실의 수용 인원이 증가했다. 특히 중환자, 거동이 불편한 환자, 노인 환자와 같은 간병인의 도움과 의료 종사자의 지속적인 진료의 필요성이 증대되고 있다. 환자의 상태를 일정 시간마다 확인해야 하는 불편함, 간병인 고용의 필요로 기본적인 병원비와 인건비가 증가되고, 의료업계 종사자들의 부담도 늘어나는 추세이다. 따라서 이러한 문제를 해결하기 위해 중앙 관제실에서 환자의 상태를 실시간으로 확인하고 자동으로 병실을 관리하는 시스템을 도입하고자 한다. 정보를 실시간으로 전달함으로써 의료진의 부담을 줄이고 위기 상황에 즉각적으로 대응하며, 자동 병실 환경관리와 환자 편의를 위한 의료 환경을 지원하는 시스템을 제안한다.

Is the amount of the medical care utilization affected by the cash benefits for patients in the geriatric hospital? (요양병원 간병비 지급이 건강보험 진료이용량에 미치는 영향)

  • Korean Government had performed three pilot programs to introduce the long term care insurance system. Persons aged 65 or older who are dependent on others for daily living could use long term care services in the pilot program. The long-term care insurance covered nursing home services, home care services and cash benefits. The cash benefits are included that for elderly at home and for patients in geriatric hospital. This study investigated whether there had been any change in the medical care utilization according to cash benefits for geriatric hospitalization. This study used National Health Insurance claims and Long term Care Insurance claims 2003 through 2006. Data were composed of subjects who undertook both insurance coverage. The subjects was divided into two groups. Case group included participants with the cash benefits of geriatric hospitalization. Control group included persons without the cash benefits selected by random sampling according to the distribution of case group. This study showed that the amount of medical care utilization of the case group is more significantly increased than the control group after adjusted their health condition and functional condition. This result will be helpful for making decisions on whether the cash benefit of geriatric hospitalization can be introduced into long term care insurance system.

A Study on the Association between Healthcare Utilization and the Burden of Families Caring for the Elderly in the Last 6 Months of Life (노인의 사망 전 6개월간 의료이용수준과 간병가족의 가족부담과의 관련성)

  • Objective : To investigate the relationship between medical expenses and the burden of families caring for the elderly in the last 6 months of life, and to evaluate the factors relating to the burden of family caregivers. Methods : The families of 301 persons older than 65 years, who died between 1 July and 31 December 2001, and were registered in Resident-based- Health Insurance Programs in Seoul, were interviewed. The medical expenses and length of stay among the elderly were collected from Korean Health Insurance Corporations. Results : 31 percents of the elderly had no medical expenses in the last 6 months of life. On average, the objective burden (4.92) was higher than the subjective burden (3.35). Families caring for male elderly had a higher burden. With increasing age af death, the objective burden was significantly increased. The burden on a family seemed to be influenced more by the family income than the property of the elderly. With increasing total health care costs, the objective burden on the family caregivers was significantly increased, but with increasing medical expenses, the subjective burden was significantly decreased. Conclusion : An association between healthcare utilization and burden on families was observed. The reason for the decreasing subjective burden when medical expenses were decreased was unclear. Further research will be needed.

Development of Functional Uniform for Caregivers (간병인을 위한 기능적 유니폼 개발 -노인의료복지시설 간병인을 대상으로-)

  • The purpose of this study was to develop a caregiver's uniform that is appropriate for nursing and to improve movement adaptability as well as having a pleasing appearance. Based on the results of preliminary study, I investigated the inconvenience and problems with the existing uniforms and made a sample uniform taking into consideration the design, the material, and the pattern. We carried out a sensory assessment and quantitative assessment in order to compare the sample uniform with existing uniforms. Following are the results of this research. The top is a short-sleeved blouse with a round neckline having no collar and with armpit gussets. The armpit gussets are semicircles and the inverted pleats in the back are an improvement designed for activity. The pants are long reaching to the ankles with elastic in the waist so that the waist measurement may be adjusted. The length of the pants can be adjusted with strings and the pants have a funnel-shaped pocket for adjustment of length. We used a mix of polyurethane and polyester for the outer cloth of the uniform in order to improve activity function and used $Coolever^{TM}$, the sanitary textile that absorbs moisture well and dries in a short time, for gussets in the armpits, which is the heavy sweat area. The appearance assessment and movement adaptability assessment were performed with 5 subjects and 9 experts. For the results of the assessment, the sample uniform was appraised to be better than the existing uniforms in every item of an appearance comparison assessment except for the neckline assessment by the experts. As for the assessment of movement appropriateness and wearing assessment, the same subjects were used for analysis. As the results of the assessment, the sample uniform was rated higher than the existing uniforms in movement adaptability and pleasing appearance.

Evaluation of Liver Scintigraphy in Patients with Focal Liver Disease (S.O.L): An Analysis of Histologically Confirmed 401 Cases (국소성(局所性) 간병변(肝病變)에서 간(肝)스캔의 진단적(診斷的) 가치(價値)에 대한 분석(分析))

  • The accuracy of liver scanning was evaluated in 124 cases with primary or metastatic liver tumors, including a few benign localized lesions and in 277 cases without such lesions. All findings were histologically verified by operation or autopsy. 1) Of the 124 cases with space occupying lesions(SOLs) in the liver, 92 cases were detected by liver scan(sensitivity 74.2%). And of the 277 cases without such lesions, 266 cases were evaluated as no SOLs(specificity 96.0%). The overall accuracy was 89.3%. 2) The authors evaluated the liver scan sensitivity in each of different diseases with SOLs. The sensitivity was 88.9% in primary liver carcinoma, 82.4% in primary liver carcinoma with cirrhosis, 88.7% in liver abscess, and 100% in hemangioma. The sensitivity was low in metastatic tumor(65.8%). 3) The sensitivity of the SOLs in the right lobe was 53.3% and left lobe 27.7%. In the interlobar area, detectability was 41.7%. 4) The authors compared the sensitivity of the liver scan with the size of the SOLs. The smaller the size of the SOLs, the lower the detectability. In the pachy lesions, the sensitivity was 46.6%.
