Self-care Status of the Aged Diabetic Patients with Noncompliance (일개 종합병원에서의 치료 미순응 노인 당뇨병 환자의 자가관리 실태)

  • Objective: The purpose of this study was to describe and understand self-care status of the aged diabetic patients with noncompliance after hospital discharge. Methods: A qualitative research method was used for the study design. The participants were 15 diabetic patients aged 65 or older who had been admitted more than two times for hyperglycemia in the past 1 year in a general hospital in Daegu. Data were collected from November 1 2015 to March 1 2016 through in-depth personal interview. Results: Patients' knowledge level on causes and symptoms of diabetes was low. Most participants rarely and irregularly checked blood sugar at home. They were under a lot of stress from their family. Due to old age and illness they did not have sufficient physical activity and they have had rarely regular meals. They considered the self-care education program unnecessary and there was limitations of accessibility for education. Conclusions: The aged diabetic patients who had the repeat admission did not perform self-care activities properly and had problems especially in blood glucose check, support from their family, and health education after returning home. More personalized and community-based self-management education programs will be necessary.

Relationship between Stress and Eating Habits of Adults in Ulsan (울산지역 성인 남녀의 스트레스와 식습관)

  • This study was done to investigate the effect of stress on appetite and eating habits, and other health-related behaviors. The subjects of this study consisted of 188 males and 224 females in Ulsan area. The results were as follows: When stressed, 56% (n = 231) of the subjects experienced a change in appetite and of these, 32% (n = 132) experienced an increased appetite. Stress-induced eating may be one factor contributing to the development of obesity. There was a gender-specific response to stress in which women are more likely to use food to deal with stress, whereas men are more likely to use alcohol consumption or smoking. It was found that types of stressors were individual (52.9%), social (50.7%), family relations (34.5%), work demands (34.2%) and physical environment (32.3%). Stress-induced symptoms of the subjects were anxiety (38.3%), headache (36.7%) and neck or shoulder aches (36.2%), and females experienced those symptoms more than males. Those older than 50 years had a higher eating habit score and lower stress score compared with younger subjects. There were significant differences between sex, age, occupation, family type, BMI, exercise, sleeping hours and eating habits or stress level. This study may be helpful in advancing findings in this area to better provide health professionals with appropriate counseling tools to improve the health of all individuals.

A Study on Nutritional Intake Status and Health-related Behaviors of the Elderly People in Gyeongsan Area (경산시 노인의 영양섭취상태 및 건강관련인자에 관한 연구)

  • The purpose of this study was to investigate nutrients intake and health-related behaviors in elderly people residing in Gyeongsan-si, Gyeongbuk who have no problem in daily living. Information on general characteristics of the elderly, health-related behaviors and dietary habits were obtained by interview based on questionnare. Dietary nutrients intake data were obtained through the 24 recall method. The subject group of this study was composed of 113 males and 112 females, the average age being $73.1\pm6.06$ years old. In health related factors, $76.9\%$ of subjects exercised regularly. The rates of alcohol drinking and smoking showed to be $38.2\%\;and\;22.2\%$, respectively. There were many elderly with neuralgia, hypertension, and gastrointestinal disease, especially female were worse. Average heights of the subjects were lower than the standard established in the Korean Recommended Dietary Allowances, and average weights were similar to the standards. The mean BMI and WHR were 24.8 (male 23.7, female 25.7) and 0.92 (male 0.92, female 0.89), respectively. Most of the subjects had a regular meal pattern comsuming three meals a day, and many elderly, especially more than $79.5\%$ of female, prepared the meals for themselves. Mean daily energy intakes and RDA percentage of energy intakes of the male and female subjects were estimated as 1426.9kcal $(79.3\%)$ and 1381.3 kcal $(86.3\%)$, respectively. Mean daily intakes of nutrients were estimatied as 48.1g for protein, 411.3mg for calcium, 8.05mg for iron, 541.8 R.E. for vitamin A, 0.84mg for vitamin $B_1$, and 0.79mg for vitamin $B_2$. Most nutrients except protein, clacium, iron and vitamin $B_2$ were consumed over $75\%$ of the RDA. Female elderly showed significant lower intakes (p<0.05) for most of the nutrients except calcium, phosphorus and vitamin ethan the elderly male.

The Relation of Maternal Stress with Nutrients Intake and Pregnancy Outcome in Pregnant Women (임신부의 스트레스와 영양상태 및 임신결과와의 관련성)

  • Maternal stress was one of the common symptoms that pregnant women could have experienced during pregnant period. The purpose of this study was to investigate the relation of maternal stress with maternal nutrients intake and pregnancy outcome. Subjects were 248 pregnant women and were recruited at two hospitals in Seoul area. Individual stress levels were divided by the stress scores (total 41 scores), as low stressed group (< 12) and high stressed group (${\geqq}12$). The social characteristics, nutrient intake, anthropometric measurements and pregnancy outcome were compared between low stressed group (LSG) and high stressed group (HSG) to recognize risk factor of maternal stress. We found that subjects experience stress by various factors which were concern about newborn (40.4%), concern about health (28.8%), economic difficulties (13.2%), depress (10.1%), family relationship (2.9%), concern of house work (2.5%), human relationship (2%). In HSG, unemployed rate (p < 0.05) and pre-pregnancy BMI (p < 0.05) were higher than in LSG. Family size in HSG was larger than that in LSG (p < 0.01). Doing regular exercise with the light activity level was significantly higher in LSG (p < 0.05). The nutrient intake in LSG was slightly higher than that in HSG, but not statistically significant. Pregnancy outcome was not significantly affected by the maternal stress. In conclusion, the risk of maternal stress may be related with a life style during pregnancy. Therefore, life style for maternal stress control, such as weight control and regular exercise is recommended to prevent maternal stress.

대구ㆍ경북지역 대장직장암환자의 식행동 특성

  • 본 연구는 대구ㆍ경북지역 대장직장암 환자의 일반 생활 상황과 식행동 특성을 분석 평가하여 지역민의 대장직장암 예방을 위한 영양교육에 활용할 수 있는 식생활 관련 기초 자료를 얻고자 수행되었다. 최근 경북대학교 병원에서 대장직장암 진단을 받고 입원한 수술 전후 환자 123명 (남 79명, 여 44명)을 환자군 대상으로 하였고 대장질환이 없는 정형외과 환자 등 182명(남 117명, 여 65명)을 비교군 대상으로 하였다. 대상자의 일반 특성, 식습관, 식행동 특성, 식품섭취 빈도 등의 내용을 포함한 설문지를 이용하여 개인 면담을 통하여 조사하였다. 현재 BMI와 과거 가장 높았던 체중의 BMI는 남녀 모두 환자군에서 유의하게 높게 나타났다. 환자군의 교육 수준은 비교군에 비해 낮았고 주 거주지역 분포는 농촌이 가장 많았으며 가계 소득은 환자군에서 비교군에 비하여 높은 것으로 나타났다. 주요 일상활동 강도는 비교군에 비해 환자군에서 유의적으로 가벼운 것으로 나타났다. 발병 전 일상생활에서 느끼는 스트레스의 정도가 환자군에서 비교군에 비해 유의적으로 높았던 것으로 나타났다. 자신의 성격 유형을 비관적인 것으로 생각하는 비율과 가족의 암 병력이 환자군에서 유의적으로 높게 나타났다 식사의 규칙성에서도 환자군에서 끼니를 거르는 비율이 유의적으로 높았다. 환자군은 비교군에 비해 육식을 유의적으로 높게 선호하였으며 외식시 고기류를 선호하는 비율이 높았다. 평소 물 섭취량은 환자군에서 비교군에 비해 적었다. 환자군이 비교군에 비해 짠맛에 대한 선호도가 유의적으로 높았고 단 맛과 매운 맛은 비교군에 비해 싫어하는 경향을 보였으며 기름진 맛을 선호하는 경향이 환자군에서 유의하게 높았다. 환자군에서 굽기, 튀기기를 좋아하는 경향이 유의적으로 높았다. 환자군에서 음주 빈도, 음주량 그리고 커피 섭취량이 높게 나타났으며, 1일 흡연량이 많았고, 흡연 시작 연령은 빨랐으며, 금연 연령은 늦어 흡연 기간이 유의적으로 긴 것으로 나타났다. 환자군의 영양지식 정도와 영양태도는 비교군에 비해 유의적으로 낮았고 환자군의 영양지식 수준이 높을수록 좋은 영양태도를 가지는 것으로 나타났다. 과거 10년간 식품 섭취 빈도 조사 결과 환자군은 등푸른 생선류와 콩류 및 된장, 두부를 비교군에 비해 유의적으로 더 많이 섭취하였고 해조류, 생 녹황색채소류, 익힌 녹황색 및 담색채소류와 녹차를 더 적게 섭취한 것으로 나타났다. 본 연구의 결과는 일상생활의 스트레스와 가족의 암 병력 및 비관적 성격 유형이 대장직장암 발병의 위험인자가 될 수 있음을 제시하며 육식 선호, 적은 양의 물 섭취, 짜고 기름진 맛에 대한 높은 기호도, 음주, 흡연, 커피의 섭취 그리고 나쁜 식습관이 대장직장암 발생과 관련된 식생활 위험인자가 될 수 있음을 제시한다. 해조류, 녹황색채소, 담색채소, 녹차가 대장직장암 발생의 보호인자의 가능성도 제시되었다. 따라서 향후 지역적 식문화 특성을 고려한 보다 광범위하고 체계적인 조사 연구를 통해 이 지역의 대장직장암 발생의 위험인자와 보호인자를 재확인할 필요가 있다고 보며 본 연구의 결과는 지역민의 대장직장암 예방을 위한 영양교육 자료로서 활용될 수 있다고 본다.

A Review of Person-centered Care in Nursing Homes (노인요양시설의 인간 중심 케어 (Person-centered care)에 대한 연구)

  • The purpose of this study is to review person-centered care models and the scientific evidence for the impact of person-centered care in U.S. nursing homes, and to analyze the applicability of person-centered care for Korean long-term care system. The most commonly recognized person-centered care models were Eden Alternative, Green House, and Wellspring model. The core components of the three models were autonomy, close relationships, homelike environment, and staff empowerment. Thirty eight empirical studies including 10 model testing studies and 28 studies to examine the impact of person-centered care interventions indicated generally positive outcomes in terms of residents, family, and staff/organization level. The authors analyzed the current situation in the Korean long-term care system with respect to the specific core constructs of person-centered care, and proposed applicable models and practical suggestions in consideration of Korean context. Lastly, future considerations regarding policy and research were discussed.

Nutritional Status and Knowledge of the Elderly over 65 Years in Young-Nam Area (영남 일부지역 65세 이상 노인의 섭취상태와 영양지식에 관한 연구)

  • This study investigated the nutritional status and the knowledge of the 204 elderly in Young-nam area. Their weight and height were measured and their dietary intake and nutritional knowledge evaluated. The sutjects were 74.7$\pm$5.9 years old in average. The BMIs were in the moderate range. Weights and BMI decreased significantly with age. The dietary assessment data showed that each energy intake of the males and the females was 90.9% and 97.0% compared with the Korean RDA, respectively. The dietary intake of vitamin A, calcium and iron was lower than that of the Korean RDA. The fiber intake of the subjects was 7.4g~7.8g. The MAR of vitamin $_{B2}$ was the lowest, 0.66 and that of phosphorus was the highest, 0.99. According to the nutritional knowledge level, the intake of protein, vitamin A, vitamin $B_1$and iron in the excellent group was significantly higher than that in the poor group. The correlation analysis revealed that the score of nutritional knowledge was positively associated with BMI, MAR and protein intake, while negatively with age.e.

Analysis of health behavior, mental health, and nutritional status among Korean adolescents before and after COVID-19 outbreak: based on the 2019-2020 Korea National Health and Nutrition Examination Survey (COVID-19 전·후 한국 청소년의 건강행태, 정신건강 및 영양상태 분석: 국민건강영양조사 2019-2020년 자료를 활용하여)

  • Purpose: This study aimed to investigate the changes in health behavior, mental health, and nutritional status of Korean adolescents before and after the coronavirus disease 2019 (COVID-19) pandemic outbreak. Methods: A total of 800 adolescents (12~18 years old) who participated in the 2019-2020 Korea National Health and Nutrition Examination Survey (KNHANES) were included as study subjects and divided into four groups (204 middle school boys, 172 middle school girls, 219 high school boys, and 205 high school girls). The 2019 and 2020 KNHANES data were classified into data collected before and after the COVID-19 outbreak, respectively. Results: After the COVID-19 pandemic outbreak, middle school boys showed an increased tendency toward becoming overweight and obese, with significantly increased levels of diastolic blood pressure and insulin. While there was no major change in the subjective health status among adolescents, the high school boys showed a significantly decreased physical activity after COVID-19 outbreak. Moreover, the proportion of middle school students feeling a little stressed significantly increased after the COVID-19 outbreak. The rate of skipping breakfast significantly increased in middle school girls, but the rate of having lunch with companions significantly increased among all adolescents after the COVID-19 outbreak. However, the intake of milk, vegetables, fruits, seaweeds, and pulses significantly decreased, although the intake of sugars, beverages, and seasonings significantly decreased as well, during this period. These changes may lead to an increased proportion of adolescents with insufficient intake of nutrients, including potassium, vitamin C, and riboflavin. Conclusion: These results highlight the impact of COVID-19 on comprehensive changes in physical and mental health status, lifestyle behavior, and nutritional status in adolescents, suggesting the need for targeted prevention and intervention for physical and mental well-being during the pandemic.

Change of dietary habits and the use of home meal replacement and delivered foods due to COVID-19 among college students in Chungcheong province, Korea (충청지역 일부 대학생의 코로나-19 이후 식생활 변화, 가정간편식과 배달음식 이용 실태)

  • Purpose: The purpose of this study was to evaluate the change in the use of home meal replacement (HMR) and delivered foods and food habits of college students due to coronavirus disease 2019 (COVID-19). Methods: A survey was conducted on 460 male and female college students in Chungcheong province in December 2020. Results: The methods of participation in classes in the 2nd semester of 2020 were 40.2% for '100% non-face-to-face' and 40.4% for 'more than 70.0% of non-face-to-face classes'. 52.8% of the subjects responded that their physical activity had decreased, while 36.1% of the subjects responded that their body weight had increased over the past 6 months. Regarding the use of HMR, 62.7% of the male students and 69.6% of the female students responded with '1-2 times a week or less' before the outbreak of COVID-19. After the outbreak, 57.4% of males and 46.7% of females responded with '3-4 times a week or more' (p < 0.05, p < 0.001). As for the use of delivered food, 58.3% of the females responded with '2-3 times a month or less' before the outbreak, whereas the rate of responding with '1 or more times a week' after the outbreak was 64.6% (p < 0.001). Negative changes in food habits caused by COVID-19 were in the order of irregular meals (56.7%), increased intake of delivered food (42.2%), increased intake of snacks (33.3%), and increased intake of processed foods such as HMR (30.4%). Conclusion: As mentioned above, many college students had non-face-to-face classes due to COVID-19. During this period, they experienced decreased physical activity, weight gain, and undesirable eating habits such as irregular meal time, and increased intake of processed foods, delivery foods, and snacks. Therefore, there is a high need for nutrition education and policy support for the prevention and management of health and nutrition problems of college students, which can be caused due to the COVID-19 pandemic.

Anthropometric Measurement, Dietary Behaviors, Health-related Behaviors and Nutrient Intake According to Lifestyles of College Students (대학생의 라이프스타일 유형에 따른 신체계측, 식행동, 건강관련 생활습관 및 영양소 섭취상태에 관한 연구)

  • The purpose of this study was to investigate the differences according to lifestyle in anthropometric measurement, dietary attitude, health-related behaviors and nutrient intake among the college students. The subjects were 994 nation-wide college students (male: 385, female: 609) and divided into 7 clusters (PEAO: passive economy/appearance-oriented type, NCPR: non-consumption/pursuit of relationship type, PTA: pursuit of traditional actuality type, PAT: pursuit of active health type, UO: utility-oriented type, POF: pursuit of open fashion type, PFR: pursuit of family relations type). A cross-sectional survey was conducted using a self administered questionnaire, and the data were collected via Internet or by mail. The nutrient intake data collected from food record were analyzed by the Computer Aided Nutritional Analysis Program. Data were analyzed by a SPSS 12.0 program. Average age of male and female college students were 23.7 years and 21.6 years, respectively. Most of the college students had poor eating habits. In particular, about 60% of the PEAO group has irregularity in meal time. The students in PAH and POF groups showed significantly higher consumption frequency of fruits, meat products and foods cooked with oil compared to the other groups. As for exercise, drinking and smoking, there were significant differences between PAH and the other groups. Asked for the reason for body weight control, 16.2% of NCPR group answered "for health", but 24.8% of PEAO group and 26.3% of POF group answered "for appearance". Calorie, vitamin A, vitamin $B_2$, calcium and iron intakes of all the groups were lower than the Korean DRIs. Female students in PTA group showed significantly lower vitamin $B_1$ and niacin intakes compared to the PFR group. Therefore, these results provide nation-wide information on health-related behaviors and nutrient intake according to lifestyles among Korean college students.
