A Study on the Situations and Satisfaction of Home-based Occupational Therapy Service for Children (아동 가정방문 작업치료서비스 실태와 만족도 조사)

  • This study investigated the situations and satisfaction of home-based occupational therapy service for children under eighteen residing in Suncheon and Gwangyang. Arithmetic statistics were conducted by dividing the general facts of home-based occupational therapy service, the kinds of service used in home-based therapy, the satisfaction of home-based occupational therapy service, facts helpful for children, and facts helpful for parents or home. As a result, the most common source of home-based occupational therapy information was the association for children with disabilities and its satisfaction was high. This study will provide basic materials to build up systematic, stable, and continuous rehabilitation therapy support projects by the government.

Effects of Home-based Cognitive Occupational Therapy Applied to Dementia Patients in the Initial Stages in Gangwon-do (강원도 지역 초기 치매환자에게 적용한 가정방문 인지작업치료의 효과)

  • Objective : The purpose of this study is to find out the effects of a Home-based cognitive occupational therapy program conducted for the dementia patients in the initial stages living in Taebaek region in Gangwon-do. Methods : This study carried out Home-based cognitive occupational therapy intervention by 30 Occupational Therapy Dept. students and volunteers targeting 30 dementia patients in the initiative stages for eight sessions, once a week. To identify the change of cognitive function of the participants before and after the program, MMSE-K was used for measuring. Results : As a result of applying Home-based cognitive occupational therapy to the dementia patients in the initial stages, overall cognitive function improvement was demonstrated. Especially, statistically significant improvement was exhibited in orientation, memory retrieval, the concentration of attention, and language ability. Conclusion : The program for cognitive function improvement is considered to be usefully applied to intervention in the dementia patients. The development of various Home-based occupational therapy intervention programs is required in order to adopt the Home-based occupational therapy service in the future.

Effect of Home-Visit Occupational Therapy on Community Dwelling Stroke Survivors: A Case Study (지역사회 거주 뇌졸중 환자의 가정방문 작업치료 효과: 사례 연구)

  • Objective : The purpose of this study was to verify the effectiveness of home-visit occupational therapy in stroke patients. Methods : Two patients with stroke who applied for home-based occupational therapy services at a health center in Seoul were enrolled. The home-visit occupational therapy program evaluates the subject's daily living, task performance, cognitive, and emotional functions, sets occupational therapy goals and plans interventions based on a client-centered approach. Occupational therapy programs consisted of 12 sessions based on the client's major problems. Results : COPM scores improved in both cases, there was an improvement in COPM scores, and in Case 2 there were improvements in MBI and K-MMSE scores. There was also an improvement in KGDS scores in Case 1. Conclusion : Home-visit occupational therapy was found to be effective in improving daily activities, cognition, and mental function of stroke patients. During home and community integration, continuous and continuous rehabilitation services need to be activated from institutional rehabilitation to community-based rehabilitation. Active home-visit occupational therapy is needed to promote physical, cognitive, mental and social access in stroke patients discharged from hospitals.

Current State and the Future Tasks of Home Visit Nursing Care in South Korea (우리나라 가정방문간호의 현황과 향후 과제)

  • Objectives: We searched and reviewed the literature including the laws or acts, statistics, guidelines, papers and conference proceedings related to home visit nursing care in South Korea. Method: We searched and reviewed the literature including the laws or acts, statistics, guidelines, papers and conference proceedings related to home visit nursing care in Korea. Results: There are three types of home care nursing in Korea. Public health center provides home visit nursing to vulnerable population by registered nurses for free, based on community health act in public health center. As of 2017, 1,261,208 people were enrolled in the visiting health program of public health center. Health behavior and disease management has been improved and showed having cost-benefit effect among the enrolled people in visiting health program. Visiting nursing care in long-term care services is provided by registered nurses or nurse aid, based on long-term care act. The cost is paid as the unit price according to service time. 1,095,764 older people used long-term care services in 2017, only 0.2% of total cost used for home visiting nursing. Even though the number of user of home visiting nursing, it was reported that users spent less medical cost and hospitalized shorter. Hospital-based home care nursing is provided to patients and their families under the prescription of a doctor by family nurse specialists who are employed by medical institute based on medical law. Four hundred sixty family nurse specialists worked for hospital-based home care nursing and hospital-based home care services accounted for 0.038% of total medical expenses in 2017. Conclusion: Even though home visit nursing care services are different in aspect of legal basis, personnel, running institutes, and cost basis, home visit nursing care showed cost-benefit effect and good health outcomes. In order to advance home visit nursing care, the integrated home visiting care, improvement of working condition, and revision of legal basis should be considered.

A Convergence Study on Perception of Music Therapists of Home-visiting Music Therapy Service (가정방문형 음악치료 서비스에 대한 음악치료사 인식의 융복합적 연구)

  • The purpose of this study was to convergently investigate the perception of home-visiting music therapy services. For this, a survey was conducted on 74 persons divided into two groups, which are pre-service music therapists attending the graduate schools of music therapy and professional music therapists with certifications. As a result, first, the awareness of home-visiting music therapy service was lower than that of preexisting home-visiting services, but the necessity and expected effect were similar to them. Second, in the operational plans, there was shown a higher ratio of 40-minute services twice a week held by governmental or public organizations. Third, there were significant differences in subitems of trends and expected effects of the home-visiting music therapy service according to whether or not to hold a certification and to have experiences in providing the services. This study could be expected to provide basic data for the home-visiting music therapy being utilized as a field of music therapy. Based on the study, the establishment of practical service strategy could be suggested.

Hospital-based home care reinbursement and service use for the elderly (노인의 의료기관 가정간호 급여청구 및 서비스 이용 현황)

  • The purpose of this study was to address the current status of hospital-based home care(HBHC). We analyzed the data on HBHC from national electronic data information of Health Insurance Review Agency. Beside, we surveyed 75 hospital-based home care agency. In 2006, 20,343 elderly(64.0% from all HBHC user) used 333,889 visits(76.8%from all visits). Medical diagnosis was composed of circulatory disease including cerebrovascular diseases 41.3%, endocrine system disease including Diabetes mellitus 10.4%, neoplasm 9.7%. Some of subjects used HBHC in excess of maximum covered 8 visits a month by National Health Insurance, decubitus 7.0%, the cancer 5.4%, the diabetes 2.5%, the hypertension 1.1%, and the stroke 0.9%. This results will contribute to expand the coverage of hospital-based home care by National Health Insurance. There was distribution difference in medical diagnosis and nursing intervention between HBHC and Public health center-based home care(PBHC) subjects. Therefore, HBHC subjects had more severe medical diagnosis, and were intervened more injections, examinations, than PBHC subjects. These differences must be considered to set up functional role among the three types of home visit care.

  • Home health care system in Korea has been classified into three types of home care programs based on different laws and regulations; for example, home health care nursing(HHCN) is based on medical laws, visiting health care nursing (VHCN) is based on long-term health care insurance, and visiting health care(VHC) is based on the regional health care act. HHCN in Korea has taken on an important role under the mandate of the national health care system since 2000. VHCN will commence its role under the long term health care insurance system in 2008. The strengthening of VHC commanded health promotion and prevention for vulnerable families in the community in 2007. This is an important turning point for increasing quality management for home health care program; it suggests certain possibilities for building a foundation for further changes in the service delivery structure. Accordingly, the home health care policy makers in Korea have a major function and role that consists of developing an agenda and alternatives for policy making in a systematic manner and clearly presenting implementation strategies for elderly health care system.

Changes on Hospital-based Home Care Services Utilization After Long-term Care Insurance Launch (노인장기요양보험제도 도입 후 의료기관 가정간호 이용실태 변화)

  • This study was to address changes on hospital-based home care utilization after long-term care insurance(LTCI) was launched. National electronic data information(EDI) on hospital-based home care from Health Insurance Review Agency in 2007.7~2008.6(prior to LTCI) and in 2009(posterior to LTCI) was analyzed. After the launch of long-term care insurance, 40 hospital-based home health care agencies(HHCA) were diminished and regions not having any HHCA were increased from 53% to 59%. Hospital-based home care utilization was decreased in the elderly(clients 13.4%, visits 20.9%) as well as non-elderly(clients 3.5%, visits 3.9%). It is presumed that diminished HHCAs result in decreased accessibility to hospital-based home health care for non-elderly. The clients, visits, and reimbursed cost per agency were not changed. It is presumed that small agencies were closed already. The total reimbursed cost per agency in 2009 was 121,850,000 won. Results suggest that the government has to give support to open more HHCA to increase the accessibility for non-elderly. Also, hospital-based home care services utilization has to be monitoring regularly.

  • The aim of this study was to understand home hospice care status and problem in Korea, and ultimately to develop the home hospice standard. This study was conducted as a part of a study on the institutionalization of the home hospice in Korea. A focus group interview with representatives of seven home hospice agency where have provided home hospice service for years was conducted. All of the participants agreed to the essential components for home hospice service including 24 hour on call service, multidisciplinary team visiting, and periodical team meeting. Visiting frequency was 1-3 times per week mostly by nurses. And they agreed requisitely to fulfill an office for home visiting nurses, storage space, and home visiting bags. The obstacles of providing home hospice were 1) no reimbursement system, 2) difficulties to change medication at home, 3) lack of inpatient beds for symptom control. Standardization of home hospice is critical to improve service quality and to develop reimbursement system. The findings of this study could be used as a basic data to develop home hospice standards and guidelines.
