The Neurotransmitter Pathway of Itching (가려움증의 신경전달 경로)

  • It was common that the classification of itching was classified into four categories according to the neurophysiological mechanisms of pruritoceptive itching, neuropathic itching, neurogenic itching and psychogenic itching. Recently it was classified by clinical criteria. The neurotransmission pathway of itch is divided into histamine-dependent pathway and histamine-independent pathway. Different receptors and neuropeptides act on each itch mediator. Itch mediators such as histamine, BAM8-22, and chloroquine are transmitted through the histamine-dependent pathway. Cowhage spicule, protease, and TSLP (Thymic stromal lymphopoietin) have been reported to be related to the histamine-independent pathway. These itch mediators, receptors, and neuropeptides are the targets of treatment for itching. Although itching and pain are typical noxious stimuli, and in the past, it was argued that two senses were transmitted through one noxious stimulus receptor. It has recently been shown that itching and pain have an independent neurotransmitter system and both neuronal systems inhibit each other. In addition, the mutual antagonism between itching and pain is explained by various mechanisms. Recently, many new mediators and receptors are being studied. The studies on histamine 4 receptor (H4 receptor) have been actively conducted. And the H4 receptors are expressed in immune cells such as T cells. The therapeutic agent for blocking the H4 receptor can inhibit the inflammatory reaction itself, which is important for the itching and chronicization. Understanding the underlying mechanisms of itching and studying new itch mediators will lead to the development of effective therapies, and this is what I think the itching study will go on.

Effects of a Pruritus Intervention Program on Pruritus and Sleep Satisfaction for Hemodialysis Patients (가려움증 중재 프로그램이 혈액투석환자의 가려움증과 수면에 미치는 효과)

  • Purpose: The purpose of this study was to analyze the effects of a pruritus intervention program on pruritus and sleep satisfaction in patients undergoing hemodialysis. Method: This study was designed for a nonequivalent control group repeated measures quasi-experimental study. Hemodialysis patients were recruited from a kidney center at a university hospital located in G city, Korea. The subjects consisted of 43 patients 21 for the experimental group and 22 for the control group. The 2-week pruritus intervention program was given to the experimental group only. An analytical survey was done before and after the pruritus intervention program. Result: There was a significant decrease in pruritus and increase in sleep satisfaction in the experimental group compared to the control group. There was significant improvement in clinical symptoms and sleep satisfaction after completing the pruritus intervention program in the experimental group. Conclusion: Pruritus intervention program in hemodialysis patients decreased pruritus severity and increased sleep satisfaction, which can be used as an efficient nursing intervention for hemodialysis patients.

Effect of Seomaeyakssuk (Artemisia argyi H.) Extracts on Anti-pruritic Activities (항가려움증 활성에 섬애약쑥(Artemisia argyi H.) 추출물이 미치는 영향)

  • The purpose of this study was to investigate the anti-pruritic activities such as cell viability and pruritus-related factor using seomaeyakssuk (Artemisia argyi H.; Seomaeyakssuk) extract on MC/9 (mouse mast cell line). Seomaeyakssuk was extracted from hot distilled water. Cell viability was assessed using MTT assay on MC/9 cells. Anti-pruritic activities were measured through changes in the levels of transcription factor (IL4 and IL31) on MC/9 cells. In addition, the expression of linked proteins and histamine was measured. The results confirmed that significant cytotoxicity does not appear in the concentration range of 25, 50, and 100 ㎍/㎖. The levels of IL4 was reduced to 12% (25 ㎍/㎖), 26% (50 ㎍/㎖) and 61% (100 ㎍/㎖). Also, level of IL31 was decreased 33% (50 ㎍/㎖) and 36% (100 ㎍/㎖). In the case of proteins levels decreased significantly IL-4 34%/69% and IL-31 36%/37% at 50 ㎍/㎖ and 100 ㎍/㎖. Histamine decreased by 22, 58% and 61%, respectively, at concentrations of 25, 50, and 100 ㎍/㎖. This results shows possibility of ADD as raw material in anti-pruritic products.

Rooibos Extract Effect on the Seborrheic Scalp Improvement of Middle-Aged Men (루이보스 추출물이 중년 남성의 지루성 두피 개선에 미치는 효과)

  • This study conducted a 4-week clinical trial on 30 males suffering from seborrheic dermatitis and hair loss symptoms to examine the effects of rooibos extract on the seborrheic dermatitis improvement of 30-50 year old middle aged males. The comparison group used shampoo with no rooibos extract while experiment group 1 used shampoo including rooibos extracts, and experiment group 2 used shampoo and scalp enhancer including the rooibos extracts. The results showed that there were no meaningful changes on the scalps of the comparison group while experiment group 2, which used both shampoo and scalp enhancer including rooibos, showed better improvement on seborrheic dermatitis than experiment group 1 which only used shampoo including rooibos. These results imply that shampoo and scalp enhancer including rooibos extracts effectively improve the oil-moisture balance on seborrheic dermatitis and effectively deals with scalp itchiness redness and the color tone of the scalp, suggesting its use in scalp cosmetics that include antioxidation activation elements, which are effective in improving seborrheic dermatitis.

A Study on Inflammation and Itching of Cyperus rotundus Ethyl Acetate Fractions (향부자 에틸아세테이트 분획물의 염증 및 가려움증에 관한 연구)

  • In this study, Cyperus rotundus (C. rotundus) was fractionated into ethyl acetate to identify α-cyperone, a representative indicator, and its ethyl acetate fraction were evaluated to confirm the possibilityas a functional cosmetic ingredient. As a result of HPLC analysis, it was confirmed that the content of α-cyperone was 5.243%. In order to verify the inflammatory relief effect of the ethyl acetate fraction from C. rotundus (EAFC) and α-cyperone, it was confirmed that nitric oxide (NO) production inhibitory ability in RAW 264.7 macrophages induced an inflammatory reaction with lipopolysaccharide (LPS). Real-time qPCR analysis confirmed inhibition of mRNA expression level of inflammatory factors, IL-1β, IL-6, TNF-α, COX-2 and iNOS. As a results of conducting a clinical study using a simple cosmetic formulation containing EAFC, it was confirmed that the skin irritation stimulated by sodium lauryl sulfate (SLS) was calming and relieving itching. Through these results, it is believed that the C. rotundus can be used as a natural cosmetic ingredient that has the effect of inhibiting inflammation and relieving itching.

건강한 여성-질염

  • 질염은 많은 여성들이 한 번쯤은 이로 인해 고생한 적이 있을 만큼 아주 흔한 염증이다. 대개 미혼여성보다는 성생활을 하는 기혼 여성에게서 주로 나타나며 증상은 우선 냉이 많아지는데 냄새가 나거나 심하게 가려움증까지 나타난다. 원래 정상적인 질분비물은 흰색이거나 연한 노란빛으로 양이 그다지 많지 않고 다만 배란기 전후로 해서 맑은 물처럼 잠시 양이 증가하는 정도이지만 질염이 있을 때는 질 입구의 가려움, 화끈거림 성관계 시 통증, 소변시 쓰라림 등이 동반된다.
