Effects of Tracheal Suction and Method of Bronchodilator Inhalation on Vital Signs and Pulmonary functions in Patients with Open Heart Surgery(OHS) (심장수술 환아에게 기도흡인과 기관지 확장제의 투여 방법이 활력징후 및 폐기능에 미치는 효과)

  • Objective: The purpose of this study was to identify the effects of tracheal suction and the effects of different methods of bronchodilator inhalation (Ultrasonic nebulizer: MDI puff, MDI puff with spacer) in VSD surgery patients. Material & Method: From June 2001 to March 2002, sixty consecutive patients were randomly assigned to a control group (n= 15), ultrasonic nebulizer group (n=15), metered dose inhalation (MDI) puff group (n=15) and MDI with spacer group (n=15). Vital signs (HR, BP, CVP), ABGA and pulmonary functions were measured before suction (baseline for suction), after suction, 15 minutes after suction (base of bronchodilator inhalation), 30 minutes after bronchodilator inhalation, and 2 hours after bronchodilator inhalation. Stastistical analysis was performed using SPSS software. Repeated measure ANOVA was used to examine the effects of tracheal suction. One way ANOVA with Bonferroni's correction and multiple range test (the least significant difference test) were used to examine the effects of albuterol inhalation. Result: 1. Heart .ate increased significantly immediately after suction (p<.01) and recovered 15 minutes after suction. 2. $PaO_2$ and PH decreased significantly immediately after suction (p<.05) and $PaO_2$ recovered 15 minutes after suction. $PaCO_2$ increased immediately after suction and significantly 15 minutes after suction (p<.01). But changes in vital signs and ABGA were within the normal range. 3. Tidal volume decreased significantly 15 minutes after suction (p<.05). 4. Changes of HR and tidal volume were greater in the nebuizer group compared to the other groups (p<.05) 30 minutes after bronchodilator inhalation and recovered 2 hours after bronchodilator inhalation. 5. Changes of airway deadspace was greater in the nebulizer group compared to the control group and MDI puff group 30 minutes after albuterol inhalation (p<.05) and at 2 hours (p<.01). Conclusion: Tracheal suction did not have significant effect on vital signs and pulmonary functions after OHS. Although the methods of bronchodilator inhalation did not showed any significant difference on pulmonary function, the nebulizer method increased $PaO_2$ (20%) and tidal volume transiently. If the patient needs bronchodilator inhalation with bronchospasm after OHS, the nebulizer method is the best choice. More study on the effects of bronchodilator inhalation in bronchospasm group is needed.

Die $Gespr\"{a}chsverhalten$ von zwei Geschlechten bei Online- und Offline-Kommnunikation aus der $gespr\"{a}chsanalytischen$ Perspektive (담화분석적 입장에서 고찰한 실생활과 온라인 소통에서 나타나는 양성의 소통 관행)

  • Bei verschiedenen $Gespr\"{a}chen$ zwischen Frauen und $M\"{a}nnern$ sind mehrere Unterschiede in $Gespr\"{a}chsstrategien$ und -verhalten des jeweiligen Geschlechts zu beobachten. Herkemmlichen Ansichten nach $f\"{u}hrten$ $M\"{a}inner$ $Gespr\"{a}iche$ mit Frauen eher dominativ, $w\"{a}hrend$ Frauen eine deutliche Vorliebe $f\"{u}r$ kooperativ erlaufende $Gespr\"{a}che$ zeigen. Um die $G\"{u}ltigkeit$ dieser Perspektive zu $pr\"{u}fen$, wurden in dieser Albeit insgesamt $zw\"{o}lf$ Beratungsgesprache herangezogen. Daraus ergab sich, dass $M\"{a}nner$ im $Gespr\"{a}ich$ statistisch $h\"{a}ufiger$ die anderen $Gespr\"{a}chsteilnehmer$ unterbrechen. Da ein $Gespr\"{a}ch$ ein komplex vernetztes soziales Geschehen ist, sollte diese Statistik bei der Bestimmung der $Gespr\"{a}chsverhalten$ von zwei Geschiechtern keine entscheidende Rolle spielen. Im Anschluss daran habe ich mick mit Hilfe einiger im Internet gesammelten Daten, mit den $Gesp\"{a}chsverhalten$ bei der so genannten Online-Kommunikation befasst. Bei der Online-Kommunikation lassen sich zwei kontrastive verbale $Umg\"{a}nge$ der Geschlechtern beobachten, $n\"{a}mlich$ ein reibungslos ablaufendes IRC(Internet Relay Chatting) und eine $auff\"{a}llige$ Debatte, die mehrfache Konflikte zwischen den Beteiligten zeigt. Um diese $Ph\"{a}nomene$ auf der Basis der $Gespr\"{a}chsanalyse$ zu $erl\"{a}utern$, werden vol allem das Konzept der Kooperation beim $Gespr\"{a}ch$ und die Dynamik der $Gespr\"{a}chsstruktur$ erneut begriffen. $Schlie{\ss}lich$ werden die unterschiedlichen $Gespr\"{a}chsverhalten$ von Frauen und $M\"{a}nnern$ folgend wiedergegeben: Bei realen Face-To-Face-$Gespr\"{a}chen$ setzen Frauen den Schwerpunkt auf die emotionale und interaktive Kooperation, $w\"{a}hrend$ Manner ihn auf die kognitive Kooperation setzen. Dieser Sachverhait $\"{a}ndert$ sich aber bei der Online-Kommunikation. Auch Manner halten die emotionale und interaktive Kooperation $f\"{u}r$ wichtig und $f\"{u}hren$ mehr kooperative und reibungslose $Gespr\"{a}iche$ mit Frauen. Doch in einer Debatte um ein Thema $verh\"{a}lt$ sich jeder Beteiligte anders. Die moisten benutzen die Strategien, die den geringen Unterschied zwischen den Geschlechtern in Anfangsbedingungen $vergr\"{o}{\ss}ern$, und gleichzeitig die individuellen Unterschiede zwischen demselben Geschiecht verschleichen. Dadurch kann eine Debatte zwischen Geschlechten in Streit ohne Kompromisse geraten.

Stabilization Performance Evaluation of Filter(pH) Using Ionic Water Generator (이온수기 필터(pH)의 안정화 성능평가)

  • This study is about ionic water generator filter Recently, a lot of people feel deep interest in health and drinking water. And there are various types of water. Ionic water generator is a system with special function, and can be classified as a medical device and should be manufactured after approval from the Food and Drug Administration. Basically ionized water is different from the packaged and stored water. When the tap water or ground water passes through the various filters of ionic water generator, it turns to the purified water of pH7 ~ 7.5 and we can electrolyze that water into anion and cation by diaphragm. And in negative electrode side, we can get alkaline water with calcium ($Ca^+$), potassium ($K^+$), magnesium ($Mg^+$), sodium ($Na^+$) for body. In general, we can change pH value from 5 to 9 of ionizer by controlling the level of electrolysis voltage in the ionizer. In general, 1stage (pH8), 2stage (pH8.5), 3stage (pH9), 4stage (pH9.5) are used as the alkaline ionized water, -1Stage (pH6.0), -2 stage (pH5.0) are used as the acidic water. But in early stage, the water that passed through filter was weakly alkaline water and that was problem. Therefore, when filter condition is stable, the pH and ORP value of water is different with the early one. the initial setting pH value of the ionizer was confirmed that changes significantly. In order to resolve this problem we need to wash filter for some period time and neutralize by acidification treatment of the filter.

Analysis of crown size and morphology, and gingival shape in the maxillary anterior dentition in Korean young adults

  • PURPOSE. The aim of this investigation was to analyze the dimensions of clinical crowns and to classify the crown and the gingival type in the anterior teeth in Korean young adults. MATERIALS AND METHODS. Casts were obtained from 50 subjects ranging in age from 24 to 32. Measurements of length and width were made on the casts using a pair of digital calipers on the entire dentition. Crown thickness and papilla height were also measured and MDW/CL (mesiodistal width to clinical length) and CW/CL (cervical width to clinical length) ratios of the maxillary anterior teeth were calculated. The K-clustering method was used for CW/CL to classify the anterior tooth shape into three groups (tapered, ovoid, and square), and one-way analysis of variance and Duncan's post-hoc comparison were used to evaluate statistical significance between the groups. Pearson's correlation analysis was performed between tooth shape and papillary height (PH) to demonstrate the correlation between tooth shape and gingival morphological characteristics. RESULTS. The average length of the maxillary central incisors was 9.89 mm; the mesio-distal width was 8.54 mm; and the ratio of width/length was 0.86 in Korean young adults. The average bucco-palatal thickness of the central incisor was 3.14 mm at the incisal 1/3 aspect. Ovoid type was the most common tooth shape (48%), followed by square type (29%) and taper type (23%) in the central incisors of Korean young adults. Tooth shape and gingival type were correlated with each other. CONCLUSION. New reference data were established for tooth size in Korean young adults and the data show several patterns of tooth shape and gingival type. Clinicians should diagnose and treat based on these characteristics for better results in the Korean population.

A Effect on the Maintenance of Chicken Freshness through $\pi$-Water Treatment ($\pi$-Water 처리를 통한 닭고기의 신선도 유지 효과)

  • The present study attempted to obtain a basic data concerning the effect of $\pi$-water, a sort of physiological water, on the maintenance of chicken freshness. The meats were divided into three groups: the control, 20% - dilution, and 5% - dilution groups. In the experiment, the changes of the TBA-values of the meats started to appear on the 3rd day in the control group, and from the 4th day in the 20% - and 5% - dilution groups. The changes of pH didn't appear in the meats as the storage time passed. The number of general bacteria increased rapidly on the 3th day in the control, and on the 5th day in the 20% -, and 5% - dilution groups. As for the color, the meats turned greenish on the 5th day in the control, 7th, in 20% - dilution, and, l0th in the 5%-dilution groups. As for the order, the meats emitted the unpleasant order, such as that of ammonia, on the 3rd day in the control, 5th, in the 20% - dilution, and, 7th, in the 5ft-dilution group.

Retrospects and Prospects of Sixty Years of Geographical Studies in Korea (한국의 지리학연구 60년 회고와 전망)

  • In this article, major research trends are examined mainly from articles published in Journal of the Korean Geography, academic society journals of sub-fields in geography and Ph.D. dissertations in geography of universities in Korea. Based on the research trends up to current point, issues and research directions in order to raise the status of Korean Geography in the twenty first century are prospected. Sixty years of Korean Geography can be divided into 4 periods such as before 1970s, during the 1980s, during the 1990s, and the period after the 2001. Each period of time revealed distinct aspects of development both quantitatively and qualitatively. For the last 60 years, the study of Korean Geography has gone through enormous changes in the number of articles with dynamics and variety in contents and methodology, breathing with the changes of the Korean society. Experiencing urbanization and industrialization along with rapid economical growth, the study of Korean Geography has progressed by analyzing the issues and problems of national space and conducting various researches from the case studies for problem-solving to the theoretical and policy oriented studies. The key-words such as 'knowledge based information society', 'aged society', and 'the era of globalization' will be realized in the twenty flit century. And environment-friendly policies for sustainable development will be more stressed in a globalized world. In order to provide research methods and alternatives suitable for newly transformed global society and for utilizing the essence of Geography in the future, the studies on Korean Geography should actively focus on integrative studies between physical and human geography, on interdisciplinary studies, on regional studies related to foreign countries and North Korea, and on the establishment of Korean geographical theory and model.

Study for Multi Channel Radiation Detector Using of Microfilm and Carbon Electrode (탄소막 마이크로필름을 이용한 다채널 전리함 개발에 관한 연구)

  • We have designed the multi channel detector for the quality assurance of clinical photon beams. The detector was composed of solid phantom inserted by six plane-parallel ionization chambers at different depth. The chamber as a mini plane parallel chamber was made of carbon coated microfilms. In this study the electrical characteristics of the six chambers in the solid phantom were evaluated using 6 MV photon beam. The leakage currents were less than 0.5 pA, reproducibility was less than 0.5$\%$, linearity was less than 0.5$\%$, and dose rate effect was less than 0.7$\%$. In addition the effect of dose variation from other chambers was estimated to maximum 0.8$\%$ approximately. The developed detector can be used for quality determination in output dosimetry or measurement of percentage depth dose approximately for clinical photon beam.

Production of Nucleotide by Immobilized Cell (고정화 미생물에 의한 뉴크레오타이드 생산)

  • The effective p.eduction of 5'-GMP(5'-Guanylic acid) by enzymatic conversion of 5'-XMP(5'-Xanthyic acid) was investigated. The Iyophilized Brevibacterium ammoniagenes ATCC 19216 which were used as the XHP aminase source, was immobilized by entrapping in K-carrageenan, agar, polyacrylamide or Ca-alginate. $3\%$ K-carrageenan was selected as the most suitable matrix. In the production of 5'-GMP using the free cells of 3. ammoniagenes ATCC 19216, the optimum conditions were $42^{\circ}C$, PH 7.0, 100mg/ml glucose, 120mg/ml cell ,8mg/ml $MgSO_4\cdot7H_2O$, 5mg/ml POESA, 5mg/ml phytic acid. Under the conditions, $94.5\%$ of 5'-GMP was converted within 8 hours. In the production of 5'-GMP using the immobilized whole cells of B. ammoniagenes ATCC 19216, the optimum conditions were $37^{\circ}C$, pH 7.5, 50mg/ml glucose, 1mg/ml $KH_2PO_4$, 10mg/ml phytic acid, 60mg/ml cell, 8mg/ml $MgSO_4\;\cdot\;7H_2O$, 5mg/ml POESA. Under the conditions, $64.7\%$ of 5'-GMP was converted within 40 hours.

  • This study was carried out with sardine to develope a new type of fish protein concentrate. Chopped sardine meat was thermally treated in two different ways, autoclaved at $121^{\circ}C$ for 1 min and boiled at $95^{\circ}C$ for 5 min. The heat treated meat was pressed, controlled to PH 7.8 with $3\%$ (w/v) of $NaHCO_3$ and hot-air dried(at $40^{\circ}C$). The dried meat was powdered (50mesh), air and vacuum packed in laminated film bag(PET/AL. foil/CCP) and stored at room temperature for 60 days. The results of product quality analysis are as follows : 1. Proximate contents of moisture, crude lipid and protein of the autoclaved and boiled product were in the range of $10.0{\~}10.2\%,\;9.0{\~}9.1\%$ and $73.8{\~}74.4\%$, respectively. Yields of the both products were $40\%$ and $32.5\%$. 2. Values of emulsion activity, emulsion stability and foam expansion of the autoclaved product were $48.7\%$, $44.1\%\;and\;44.0\%$, respectively. These values were higher than those of boiled product. 3. Water holding capacity and digestibility of the both products were in the range of $5.0{\~}5.3\%$ and $78.0{\~}78.2\%$, respectively.

  • A survey was conducted on the isolation of lactic acid bacteria with antibacterial activity to extend kimchi shelf-life. Antibacterial activity was tested against bacteria associated with acidification of kimchi, including Lactobacillus plantarum, Pediococcus pentosaceus, and Lactobacillus sakei, using agar-well diffusion assay. Two isolates from kimchi were identified as Lactococcus lactis subsp. lactis and Lactobacillus brevis by 16S rRNA gene sequence analysis and API 50 CHL assay, and they showed antibacterial effects against indicator strains. The isolates displayed acid tolerance at pH 3.5, salt tolerance in 5% NaCl, and growth at $4^{\circ}C$. These result imply that the selected strains might be used to extend kimchi shelf-life as a potential starter.
