Carpal Tunnel Syndrome : Correlation between Magnetic Resonance Imaging and Nerve Conduction Study (수근관증후군 : 자기공명영상과신경전도검사의 상관 관계)

  • Purpose : Carpal tunnel syndrome (CTS) is a disorder of median nerve at wrist. It is usually diagnosed through clinical manifestation and nerve conduction study (NCS). However, sometimes, NCS does not provide a reliable evidence to reach the diagnosis. Thus, authors performed this study to determine whether NCS was correlated with specific parameters measured on magnetic resonance imaging (MRI) which might become a potential complemental diagnostic tool. Methods : We performed MRI in 34 wrists of 18 patients with clinical manifestations of CTS and pathologic nerve conduction values and analyzed them at levels of the distal radioulnar joint, pisiform and hook of hamate, Results : Increase in the cross-sectional area of the median nerve at the pisiform level and flattening, increased signal intensity, and contrast enhancement of the median nerve at levels of the pisiform and hook of hamate were statistically significant. Change in cross sectional areas between the distal radioulnar joint and hamate and the signal intensities at levels of pisiform and hamate were well correlated with the median nerve conduction velocity. Conclusions : Characteristic MRI findings in CTS reported previously were well demonstrated and some of MRI parameters are well correlated with nerve conduction study. MRI, despite cost, may help in evaluating CTS.

  • A 2-year-old, domestic short hair cat presented because of a 2-year history of chronic nasal discharge and chronic otitis. Examination of the oral cavity revealed a mass in the nasopharynx. For further examination, computed tomography (CT) was performed and large polyp was revealed on the nasopharyngeal area. Traction removal of the polyp was performed using a spay hook. After removal of the mass, Horner's syndrome was developed but resolved spontaneously within 14 days.

  • The analysis of serum free light chains (sFLCs) can improve the diagnosis and monitoring of multiple myeloma and other plasma cell dyscrasias. As with other immunoassays, sFLCstests are subject to potential antigen excess and heterophilic antibody interference. We describe 9 cases of sFLCs antigen excess in patients with multiple myeloma using the FreeliteTM Human Kappa and Lambda Free Kits (The Binding Site ltd., Birmingham, UK) and the Hitachi7600 P module turbidimetric system. A total of 1,247 consecutive samples from 250 patients with multiple myeloma were assayed for sFLCs from April to September, 2009. The samples were assayed using an initial dilution of 1 :5and subsequent dilutions of 1 :50 and 1: 100. The same samples were analyzed for the presence of monoclonal gammopathies using serum protein electrophoresis (SPE) and immunofixation electrophoresis (IFE). There were 9 samples (0.72%) of antigen excess with 3 cases of kappa (0.24%) and 6 cases of lambda (0.48%). These cases represents an example of antigen excess or "hook effect" using the serum free light chain assays and mandates high level of attention to falsely low sFLC levels due to antigen excess, especially when it is disaccordant to other assay results or clinical manifestations.

Tae-Geuk-Guan(太極拳) is military arts developed before Song empire(宋). Tae-Geuk-Guan(太極拳) has many branch.(Jin-sik陳式, Yang-sik楊式, Mu-sik武式 O-sik吳式, Son-sik孫式) Tae-Geuk-Guan'(太極拳) manual movement use fist(拳), palm(掌), hook shape(鉤) and its using form has many type like Bung, Yi, Jae, An, Chae, Yul, Ju, Go. Its gait has many type like Sang-bo(上步), Tae-bo(退步), Jin-bo(進步), Deng-gak, Bun-gak(分脚), Bak-gak(拍脚). Essential theory of Tae-Geuk-Guan(太極拳) is Yi-Sim-Hang-Gi(以心行氣) & Yi-Gi-Un-Sin(以氣運身). It means mind(心) moves qi(氣) and qi(氣) moves body(身).

First Record of the Oblique-banded Grouper, Epinephelus radiatus (Perciformes: Serranidae) from Korea (한국산 바리과(family Serranidae) 어류 1 미기록종, Epinephelus radiatus)

  • A single serranid specimen of Epinephelus radiatus was collected by a hook for the commercial longline fisheries occurred near Marado, Jeju Island, Korea. The present specimen was characterized by five irregular dark brown bands passing downward and forward from upper edge of body, scales in longitudinal row 107, and pored lateral line scales 55. This species is easily distinguishable from the morphologically similar Korean serranid species of E. poecilonotus based on band patterns on body. That is, the former has five irregular oblique dark-edged brown bands, and the latter has several long horizontal bands on lateral body. We propose a new Korean name, "Ma-ra-bari," for Epinephelus radiatus.

  • Excavators are construction machines used for digging soil, transporting soil, dismantling buildings and clearing the ground at construction sites. There are various attachments attached to the arm end of the excavator and used for various operations. There are the most common types of buckets attached for excavation and other types of attachments used for foresting, road cleaning, snow removal, and waste disposal. When multiple types of work are done at the same time, several attachments must be replaced and a device called a quick coupler is used to reduce replacement time. Although a quick coupler reduces the replacement time of the Attachment, it is necessary to attach safety devices to prevent unintentional detachment of attached attachments during the operation. To prevent the attachment from leaving or falling regardless of the operator's intention, support is installed in the hook of the bucket pin and controlled through a separate hydraulic cylinder to ensure safety. When attaching an attachment, it shall be attached without any action. This study is intended to verify the design validity of safety devices to prevent falls of attachments occurring at construction sites.

First record of Snyderina yamanokami (Pisces: Scorpaeniformes) from Korea

  • This is the first report of Snyderina yamanokami, from the subfamily Tetraroginae of the family Scorpaenidae, collected in Korea. The specimen (175.3 mm long) was caught in the coastal waters of Jeju Island on a commercial longline hook on 9 September 2009. This species is characterized by the following morphological traits: 9-11 dorsal fin rays, 5-6 anal fin rays, 13-15 pectoral fin rays, 5 ventral fin rays, pectoral fin tip reaching or extending past the origin of the anal fin, palatines toothless, and four dark spots on the middle of the body. We suggest a new Korean name, "Keun-mi-yeok-chi," for S. yamanokami.

  • The objective of this paper is to investigate the Intelligence control characteristics of a digital damper. This paper deals with a two-degree-of-freedom suspension using the damper with ER fluid for a quarter vehicle system. The control law for semi-active suspensions modeled in this study is developed using passive and ANFIS control method. Computer simulation results show that the semi-active suspension with ER damper has good performances of ride quality.

  • Unrecorded species from the beach at Gangnung City is reported: Cakile edentula, Brassicaceae. The genus Cakile is unrecorded in Korea and easily distinguished from other genera in Brassicaceae by the upper member of fruits distinctly longer or thicker than the lower part, petiole white or anthocyanic, style absent and leaves glabrous. The local name is Seo-yang-gaet-naeng-i and the morphological characters and illustrations of the species are provided.

  • Anglicae dahuricae Radix (ADR), the dried roots of Angelica Dahurica Benth et Hook (Umbelliferae), is a traditional herbal medicine used to treat colds, headache, skin diseases such as acne and possess analgesic, antipyretic and drainage effects. In the present study, the author evaluated the effect of ADR on regulation of inflammatory reaction. ADR reduced the ear-swelling responses derived from compound 48/80 in dose-dependent manner significantly. ADR inhibited the PMA plus A23187-induced productions of IL (Interleukin)-8, IL-1β, granulocyte macrophage-colony stimulating factor (GM-CSF) and tumor necrosis factor (TNF)-α from human mast cells (HMC)-1. In addition, ADR blocked PMA plus A23187-induced ERK1/2 phosphorylation. Ⅰ suggest that ADR regulates inflammatory reaction through inhibition of inflammatory cytokines such as IL-8, IL-1β and GM-CSF.
