Inferential structure and reality problem is a serious issue to O.M.(oriental medicine). The study will analyze this issue through a philosophical and historical comparative study of W.M.M(Western modern medicine) and O.M. First, I presuppose some basic ideas. The first is the division of the 'the philosophy of medicine' and 'the medicine itself'. Second, there is a 'visibility' that discriminate between 'the abstractive concept' and 'the concrete object' in diagnostic terminology. The third is the separation of disease, the entity and disease, the phenomenon. Finally, the distinction between the cause of disease and the nature of disease. Through these basic concepts, this study will analyze O.M's diagnostic methodology, 'Pattern identification of the S.A.S(sign and symptom)'. The results are follows: 1. O.M's views disease as a phenomenon. So, the S.A.S, which is visible, is the disease itself. Tough the analysis and inference of the S.A.S, 證(zheng) the essence is derived. 2. 證(zheng) can be considered as 'the abstractive concept' reflecting the essence of a disease. 3. 證(zheng) is not arrived through causal sequence reasoning but rather by analogical reasoning. 4. 證(zheng) is 'the non-random correlative combination of S.A.S', pattern. These patterns secure the abstractive deduction in reality. that is, The causality, the positivism, the view of disease as entity, and anatomical knowledge are the traits peculiar to W.M.M. But, these properties can not be applied universally to every medical systems. Also, these properties do not indicate the superiority or inferiority of any medical system. 5. 證(zheng) summarizes the patients condition simultaneously with the S.A.S. However, 證(zheng) doesn't necessarily indicate the knowledge about the actual internal organ. That is, Early in O.M.'s history, the diagnostic terminologies including 證(zheng) were analogical reflections of a naive knowledge of internal organs and external environmental factors. Later, the naive knowledge in 證(zheng) changed int new nature, an abstractive concept. The confusion of the concept of disease, the indiscriminate acceptance of Western anatomical knowledge, and the O.M.'s theoretical evolution et are the challenge facing modern O.M. To find solutions, this study looks at the sequence of the birth of W.M.M. and then compares it's system with the O.M. system. Objective : The purpose of this study was to report the improvements of patients with globus pharyngeus treated with herbal medication from Shanghanlun. Methods : Each patient was diagnosed with Greater-yang disease 29th provision of Shanghanlun, and Gamchogungang-tang was used to treat. The result was evaluated by Glasgow-Edinburgh Throat Scale(GETS). Results : The GETS score decreased from 23 to 4 after 40 days of administration in case 1 and from 31 to 1 after 15 days of administration in case 2. Conclusions : Globus pharyngeus improved in each case. This case report suggests that '急' based on the 29th provision of Shanghanlun, can be a direct psychological cause of globus pharyngeus.
Objectives : This study is to propose a new hypothesis about the interpretation of 'Ming (明)' in Shanghanlun and the direction of future research about Yangmingbing(陽明病). Methods : We analyzed the original form of the chinese characters 'Ming(明)' and tried to understand their contexts in Shanghanlun. At the same time, we analyzed 2 clinical cases diagnosed and treated as Yangmingbing according to newly hypothesized definition. Results : From etymology, 'Ming(明)' means becoming distinct. Patients in 2 cases suffered from cold hypersensitivity of hands and feet, overactive bladder syndrome and dizziness, all of which became distinct and problematic. For this reason, we diagnosed patients with Yangmingbing(陽明病) and administered herbal medicine then they improved. Conclusions : The disease names in Shanghanlun have clinical message by the name itself. From etymology, at least in 15 characters provision of Shanghanlun, Yanmingbing means the disease made by becoming conscious about symptoms. Future research about Yangmingbing (陽明病) should start from here.
Objective : The purpose of this paper is to report the effect of Sosiho-tang gagambang on Obesity based on Shanghanlun provisions Methods : According to a disease pattern identification diagnostic system based on Shanghanlun provisions, the patient was diagnosed as Yangmyung-byung, and treated for 90 days with Soshiho-tang gagambang that Scutellariae Radix was substituted with Salviae Miltiorrhizae Radix. The result of treatment was evaluated by Body Mass Index(BMI). Results : After taking Soshiho-tang gagambang for 90 days, the patient's BMI was decreased, and the patient is cured of Obesity. Conclusions : This study suggests that the treatment based on Shanghanlun provision has efficacy on Obesity. In addition, further studies are necessary in order to discover the roles of flavor of Shanghanlun herbal decoctions, and ensure the safety of it.
Objective : To explore the meaning of Eumyangyeokchahunobok-byung (陰陽易差後勞復病) through two cases and finding the relationship with the DSM-5 sleep-wake disorder. Methods : Following a disease pattern identification diagnostic system based on Shanghanlun provisions (DPIDS), we analyzed cases with chronic fatigue and diplopia caused after sleep disorders. The patients were treated with the Soshiho-tang and Yijoong-tang. Results : The two herbal medicines showed remarkable improvement in the patients. The sleep disorders and the secondary pathologies were rapidly relieved within a month. Conclusions : The patterns of sleep disorders of the two patients were found to be very similar to the circadian rhythm sleep-wake disorders, as described in DSM-5. Although the etymology of the Chinese characters has not been clearly known yet, it was possible to present the original hypotheses on 更and 了due to clinical inference.
Objective : The purpose of this study is to report the improvement in a patient who had Tourette syndrome treated with Hoeyeoksan based on Shanghanlun provisions. Methods: The symptoms of the patient were checked on days 14, 34, 62, 82, 103, and 119 after administration of Hoeyeoksan. Tourette syndrome was evaluated using Yale Global Tic Severity Scale (YGTSS), and oversleep and constipation were evaluated using visual analog scale (VAS). In addition, new clinical implications for the interpretation of Shanghanlun provision 318 were reviewed. Results: Daily tics occurred ten times a day for 14 days after the initial diagnosis but decreased to eight times a day after 34 days, six times a day after 62 days, five times a day after 82 days, twice a day after 103 days, and once a day after 119 days after administration of Hoeyeoksan. Oversleep and constipation improved together with Tourette syndrome. Conclusions: In this study, we observed that Hoeyeoksan had a therapeutic effect by improving the chief complaint of the patient. Hence, the interpretation of Shanghanlun provision 318 must include a patient's behavioral and sleep patterns as predisposing factors of Tourette syndrome.
Objective : This study is to propose a new hypothesis about the interpretation of Jueyinbing in Shanghanlun by studying 2 Jueyinbing cases. Methods : We analyzed the original form of the chinese characters 'Jue(厥)', 'Jueyin(厥陰)' and 'Huoluan(?亂)' and tried to understand their contexts in Shanghanlun. At the same time, we analyzed 2 clinical cases diagnosed and treated as Jueyinbing according to newly hypothesized definition. Results : Both patients suffered from eye diseases which caused by concentrating on a sophisticated work by straining their hands, feet and chest during the night time. After the herbal intervention, their eye diseases are improved firstly. The National Eye Institute 25-item Visual Function Questionnaire (NEI-VFQ) score increased on both patients. Not only all the patients' chief complaints relieved but their other clinical problems such as limb pain, hand tremble also improved. Conclusions : These results highly insist that, at least in 15 characters provision of Shanghanlun, Jueyinbing had had the different usage from the conventional terminology, and it is assumed that Jueyinbing means the disease made by concentrating on a sophisticated work by straining hands, feet and chest during night time.
Objective : The purpose of this study was to report the improvement in two patients with different complaints of sleep disorder treated with herbal medication based on the Shanghanlun disease pattern identification diagnostic system (DPIDS), and hypothesize the meaning of word '更'. Methods : According to DPIDS, patients were diagnosed with Taeyang-byung no. 29 provision, and took Gamchogungang-tang herb medication 15 or 90days. The results of administration were evaluated by the hyperhidrosis disease severity scale (HDSS) and the visual analog scale (VAS). Results : After administration of Gamchogungang-tang 15 or 90days, based on 29 provision of Gamchogungang-tang diagnosed according to Shanghanlun provisions, in one case the HDSS decreased from 3 to 1 and the VAS decreased from 10 to 1. In the other case, the VAS decreased from 10 to 1. Conclusions : Each patient not only recovered from symptoms of hyperhidrosis and alopecia areata, but also from sleep disorders following administration of a single medication of Gamchogungang-tang. This case report suggests that the word '更' in the 29 provisions of Shanghanlun means sleep disorder in this case.
Background: Lung cancer is the leading cause of cancer death worldwide Therefore, identification of genetic as well as environmental factors is very important in developing novel methods of lung cancer prevention. However, this is a multi-layered problem. Therefore a lung cancer risk prediction system is here proposed which is easy, cost effective and time saving. Materials and Methods: Initially 400 cancer and non-cancer patients' data were collected from different diagnostic centres, pre-processed and clustered using a K-means clustering algorithm for identifying relevant and non-relevant data. Next significant frequent patterns are discovered using AprioriTid and a decision tree algorithm. Results: Finally using the significant pattern prediction tools for a lung cancer prediction system were developed. This lung cancer risk prediction system should prove helpful in detection of a person's predisposition for lung cancer. Conclusions: Most of people of Bangladesh do not even know they have lung cancer and the majority of cases are diagnosed at late stages when cure is impossible. Therefore early prediction of lung cancer should play a pivotal role in the diagnosis process and for an effective preventive strategy.
Objectives: We aimed to confirm the safety and effectiveness of the decoction of Sibjo-tang, which is a powder type purgative. In addition, we checked whether the '發作' of 『Shanghanlun』 can be interpreted to have the same meaning as "seizure" in English. By confirming these objectives, we intend to lead the progress in the application of Sibjo-tang and to expand the clinical application of the 152nd provision and Sibjo-tang. Methods: We analyzed the medical records of patients who visited Apgujeongjeongin Oriental Clinic and Almyeon Oriental Clinic for seizures. We confirmed side effects in patients who took Sibjo-tang for a long time. Sibjo-tang was prepared as a hot water extract by using 5 g each of Euphorbiae Pekinensis Radix, Eurphobiae Kansui Radix, and Genkwae Flos and 15 g of Zizyphi Fructus. The "seizure" recorded in the 152nd provision was interpreted through the shape analysis of Oracle bone scripts. The seizure frequency of patients diagnosed with epilepsy after taking Sibjo-tang was compared, and we analyzed other symptoms and psychosocial conditions associated with seizures at the first episode. Results: No side effects were found in 7 patients who took Sibjo-tang for 16.57 ± 14.10 months. Seizures were eliminated in 6 cases while taking Sibjo-tang and significantly decreased in 1 case. Frequent indigestion (in 7 cases), mild exhaustion (in 5 cases), and inferiority complex (in 5 cases) were identified as accompanying pathogenic conditions. The "seizure" in the 152nd provision was interpreted as "a state of being stabbed by a sharp substance in clothes", confirming a deep connection with epileptic seizures. Conclusions: The results suggest that using a decoction of Sibjo-tang is safe and efficacious. In addition, the necessity of conducting an advanced study on epileptic seizures accompanied by frequent indigestion using Sibjo-tang was confirmed.
