Effects of adherence to Korean diets on serum GGT and cardiovascular disease risk factors in patients with hypertension and diabetes (고혈압 및 당뇨병 환자에서 한식 섭취가 혈청 GGT와 심혈관질환 위험인자에 미치는 영향)

  • Purpose: This study examined whether the supply of healthy Korean diets for 12 weeks is effective in improving the risk factors related to serum GGT and cardiovascular diseases in patients with hypertension and diabetes. Methods: This study selected 41 patients, who were treated with hypertension and diabetes. The Korean diet was composed of cooked-rice, soup, kimchi, and various banchan with one serving called bapsang, which emphasize proportionally high consumption of vegetables and fermented foods, moderate to high consumption of legumes and fish, and low consumption of animal foods. The control group was instead instructed to "eat and exercise as usual" while following the Korean Diabetes Association's dietary guidelines with an intake that can assist in glycemic control, maintain adequate weight, and meet the nutritional requirements. The Korean diet group (21 patients) were served three healthy Korean meals a day for 12 weeks, and the control group (20 patients, who trained in the diet guideline of diabetes) maintained their usual diabetic diet. The serum GGT, blood pressure, heart rate, glycemic control data, cardiovascular risk indicators, and changes in diet measured at the four visits (week 0, 4, 8, and 12) during the course of 12 weeks were compared and evaluated. Results: The serum GGT (p < 0.001), HbA1c (p = 0.004), heart rate (p = 0.007), weight (p = 0.002), Body Mass Index (p = 0.002), body fat mass (p < 0.001), body fat (%) (p < 0.001), and free fatty acid (p = 0.007) in the Korean diet group decreased significantly after the dietary intervention compared to the control group. The amount of intake of rice, whole grains, green vegetables, Kimchi, and soybean fermented food were increased significantly compared to the control group (p < 0.001). The Korean diet group showed significant decreases (p < 0.001) in the intake of animal protein, lipid, and cholesterol derived from animal foods compared to the control group but significant increases (p < 0.001) in the intake of total calories, folic acid, dietary fiber, sodium, potassium, and vitamins A, E, and C. Conclusion: In patients with hypertension and diabetes, it was confirmed that regular eating of a healthy Korean diet helps improve the risk factors for GGT and cardiovascular diseases.

Clinical Study of Moxibustion on The Complication of Diabetes Mellitus (당뇨 합병증에 대한 구치료의 임상적 연구)

  • The purpose of this study was to investigate the effects of oxibustion on complications of Diabetes Millitus. We collected 23 patients with glycosurea complications in Songeui Oriental Hospital from April, 2003 to July, 2003, and then expremented Moxibustion, medical examinations and laboratory test for 4 weeks. At the symptons change : Among the symptons which the number over 5 persons of patients is reserved, dysaesthesia of limbs, neuralgic pain, urinary frequent urination, dpontaneous perspiration, thirst, decrease of body weight and malaise showed the improvement over 60%(n=23). Level of FBS was don't have the significant meaning however showed the tendency of the desent, and level of HbA1c significantly decreased after treatment(P&0.001)(n=19).

The Hematological Effect of Excessive Crude Allium Tuberosum Extract Administrated in Large Dogs (과량의 부추 생즙 투여가 대형견의 혈액상에 미치는 영향)

  • The Allium tuberosum is a perennial herb. From the ancient times, it has been used for food or medical purpose. Allium tuberosum is widely distributed in Korea, China, Japan and other countries. As herbal medicine, the Allium tuberosum has medical effects on lumbago, sthenia, diuresis, nocturia, hematemesis, anemia, haemorrhoid, hiccups, diarrhea, eczema, sunstroke, pollution and so on. Recently, the Allium tuberosum has been studied on its components, anti-microorganism action, anti-fungal action, anti-arteriosclerosis action, anti-cancer action. However, there has not been any study on hematological effect of allium tuberosum extract on the blood in large dogs whereas the effects of excess garlic and onion on the blood of cattle, horses, dogs, cats and sheep has been reported by many researchers. Thus, this study was performed to observe the hematological effect of excessive Allium tuberosum extract administrated in large dogs. 1. The Red Blood Cell(R3C) count was significantly decreased(p<0.05), Packed Cell Volume (PCV) count were significantly decreased(P<0.05), and Reticulocyte count was significantly increased(p<0.05) 2. The White Blood Cell(WBC) count was significantly increased(p<0.05).3. The Hemoglobin concentration was significantly decreased(p<0.05) and Mean Corpuscular Hemoglobin Concentration (MCHC) was increased. and Mean Corpuscular Volume(MCV) was significantly increased.(p<0.05). 4. The Reduced glutathione(GSH) and Met-Hemoglobin were significantly increased (p<0.05).

Health Literacy, Diabetic Knowledge, and Diabetic Self-care among Foreign Diabetic Patients at a Hospital in South Korea (일개 상급종합병원을 이용하는 미국과 유럽계 외국인 당뇨 환자의 건강정보이해능력, 당뇨지식, 당뇨자가간호)

  • Purpose: The purpose of this study was to investigate health literacy, diabetic knowledge, and diabetic self-care among foreign diabetic patients at a hospital in South Korea. Methods: Participants was 134 foreign patients diagnosed with diabetes and who understand English. Data were analyzed by SPSS version 20.0. Results: Health literacy significantly differed with HbA1c levels, whereas diabetic knowledge significantly differed with education levels, and time since diagnosed. Diabetic self-care significantly differed with time since the diagnosis. Health literacy significantly differed with diabetic knowledge and diabetic self-care. There was a correlation between health literacy and diabetic knowledge and between diabetic knowledge and diabetic self-care but not between health literacy and diabetic self-care. Conclusion: There are significant results on health literacy, diabetic knowledge and diabetic self-care. Subject with adequate health literacy had high scores on diabetic knowledge and self-care.

Studies on the Factors Influencing the Transformation in Escherichia with pBR322 DNA (Escherichia coli의 pBR322 DNA 형질전환에 관여하는 인자에 관한 연구)

  • To investigate the factors influencing the artifical transformation in Escherichia coli, E. coli C600 was transformed by pBR322 DNA with tetracycline and ampicillin resistant gene purified by CsCl-Etbr equilibrium density gradient centrifugation from E.coli HB 101. The influencing factors in the transformation such as concentration of calcium chloride, time of ice incubation, temperature and time of heat shock, time of gene expression, effects of plasmid DNA concentration and adding time were examined in these experiments. The results obtained were as follows; 1. The highest transformation frequency was observed in the treatments of 100 mM $CaCl_2$ before heat shock and the treatment of $CaCl_2$ was essential step in the process of E. coli transformation. 2. The highest transformation frequency was observed in the treatment of heat shock at $42^{\circ}C$ for 4 min. or $37^{\circ}C$ for 6 min., but the prolonged heat shock resulted a decreased transformation frequency. 3. Treatments of ice incubation at $0^{\circ}C$ for 45 min. before heat stocks or at $0^{\circ}C$ for 30min. after heat shock resulted an increased transformation frequency. 4. There was a linear relationship between DNA concentration and transformation frequency at the concentration of $8{\times}10^3$ recipient cells. The highest transformation frequency reached in carte of 7 mcg of donor DNA, but above 1 mcg of DNA concentration, transformation frequency was not remarkably increased. Addition of donor DNA just after the treatment of $CaCl_2$ was the best. 5. The best condition of gene expression at $37^{\circ}C$ were 40min. for TC-resistant gene and 100min. for AP-resistant gene. TC-resistant gene was higher in the transformation frequency and faster in the gene expression time than AP-resistant gene. In these results, the best conditions for the transformation of E. coli C 600 with pBR322 DNA were: treatment with 100mM $CaCl_2$, ice incubation at $0^{\circ}C$ for 45 min, heat shock at $42^{\circ}C$ for 4 min., 30 min. of ice incubation and incubation at $37^{\circ}C$ for 100min. for gene expression in that order.

Pharmacological evaluation of HM41322, a novel SGLT1/2 dual inhibitor, in vitro and in vivo

  • HM41322 is a novel oral sodium-glucose cotransporter (SGLT) 1/2 dual inhibitor. In this study, the in vitro and in vivo pharmacokinetic and pharmacologic profiles of HM41322 were compared to those of dapagliflozin. HM41322 showed a 10-fold selectivity for SGLT2 over SGLT1. HM41322 showed an inhibitory effect on SGLT2 similar to dapagliflozin, but showed a more potent inhibitory effect on SGLT1 than dapagliflozin. The maximum plasma HM41322 level after single oral doses at 0.1, 1, and 3 mg/kg were 142, 439, and 1830 ng/ml, respectively, and the $T_{1/2}$ was 3.1 h. HM41322 was rapidly absorbed and reached the circulation within 15 min. HM41322 maximized urinary glucose excretion by inhibiting both SGLT1 and SGLT2 in the kidney. HM41322 3 mg/kg caused the maximum urinary glucose excretion in normoglycemic mice ($19.32{\pm}1.16mg/g$) at 24 h. In normal and diabetic mice, HM41322 significantly reduced glucose excursion. Four-week administration of HM41322 in db/db mice reduced HbA1c in a dose dependent manner. Taken together, HM41322 showed a favorable preclinical profile of postprandial glucose control through dual inhibitory activities against SGLT1 and SGLT2.

An Experimental Study for Radiation Nephritis in Rabbits (가토(家兎)를 이용(利用)한 방사선신염(放射線腎炎)의 실험적(實驗的) 연구(硏究))

  • Experimental radiation nephritis was produced in 15 rabbits by X-irradiation. About $2,000{\gamma}$ (tissue doses) were given to both kidneys of a rabbit in 5 days. Other tissues and organs except both kidneys were protected with 2mm thickened lead plates. 5 weeks after the last irradiation, blood pictures, blood pressures, B.U.N., serum creatinine, Ca, Mg, Fe levels and serum erythropoietin activity of the irradiated rabbits were studied. After finishing above studies, rabbits were sacrificed and both kidneys were removed and examined histopathologically. Same laboratory and pathological studies were performed in 6 control rabbits. In this study, the author obtained following results. 1) Both kidneys of rabbitis with experimental raditation nephritis showed marked histopathological changes, i.e.: renal tubules showed diffuse cloudy swelling, impacted intraluminal hyaline casts and focal precipitations of lime salts on the tubular epithelium. Diffuse interstitial fatty necrosis and various degrees of fibrotic infiltrations on the interstitium were also seen in association with focal lymphocytic infiltrations. Hyaline degenerations were observed on the glomeruli and small vessels. 2) Experimental radiation nephritis rabbits showed marked lowering in R.B.C. counts, decreased hemoglobin levels, low hematocrit values and leucopenia in comparison with those of control rabbits. (P<0.01). (Table 1 & 2). 3) Mild proteinuria were observed in experimental radiation nephritis in rabbits. 4) The levels of B.U.N. and serum creatinine increased in experimental radiation nephritis. (P<0.01). (Table 1,3 & 4) 5) The levels of serum Ca and Mg Showed no statistical difference in comparison with those of control rabbits. (P>0.05) (Table 3 & 4). 6) No statistical correlations were observable between the levels of B.U.N. and Hb. values. (${\gamma}=-0.223$) No close correlations (${\gamma}=-0.328$) were noticed between the levels of B.U.N. and serum iron levels. 7) Erythropoietin activity (R.B.C. $^{59}Fe$ Incorporation) was measured by the modified Fried method. No change in its activity was noticed in radiation nephritis group comparing with that of the control group. (P>0.05). (Table 1,3 & 5). 8) Carotid artery blood pressures showed also no difference. Analysis of Dietary Behavior of Type 2 Diabetic Patients Visiting Public Health Center (보건소 방문 제2형 당뇨병환자의 식행동 영향 요인 분석)

  • The purpose of this study was to investigate the dietary behavior of people with type 2 diabetes mellitus and to improve their quality of life through medical nutrition therapy. The subjects were 38 persons with type 2 diabetes mellitus visiting a public health center to participate in a dietary education program from Jun, 2003 to Nov. 2003 in Daegu, Korea. The interviews were tape-recorded and analyzed attitude, knowledge, and awareness of patients by focus group interview. Most of the patients were mainly dependent on drug therapy and had little experience of diet education. Barriers to dietary practice adherences were limitations in food selection, lack of will and feel of burden. Barriers to follow guidelines were lack of self-control, confliction with food habits of their family, accessibility, economical problems, fear for the change after dietary practice, food difficulties in meal distribution and difficulties for eating out. After 4 weeks of intensive nutrition education, fasting blood sugar levels were decreased and postprandial and waist circumference were significantly decreased in all patients and 26.9% of patients were under decreased oral hypoglycemic agent dosage due to improved blood sugar level. dietary knowledge of subjects were greatly improved in such items as dietary intake, saturated fat, HbA1C, ideal body weight, and waist circumference.

The Risk Factors Associated with Foot Re-Ulceration in Diabetes: A Retrospective Study Including Psychosocial Risk Factors (당뇨병성 족부궤양의 재발과 연관된 위험인자: 심리사회적 위험인자를 포함한 후향적 연구)

  • Purpose: To evaluate several risk factors related to re-ulceration of diabetic foot including psychosocial aspects such as familial support and degree of independence of patients' activity. Materials and Methods: We reviewed medical records and performed telephone interview with eighty-five patients who had a history of hospitalization in our hospital due to diabetic foot ulceration from year 2002 to 2010. Based on the collected data, we analyzed several factors such as age, gender, prevalence duration, accompanying diseases, HbA1c level, degree of independence and familial support. Results: The mean age was 61.4 years and most common in the 4th decade. There were 57 cases (67%) of recurrence, predominance of male. Eleven patients with recurrent diabetic foot ulceration had undergone major amputations. Psychosocial problems such as depression, insufficient familial support and mortality were more frequently observed in recurrent group. Conclusion: This study shows that psychosocial factor such as familial support for patient with diabetic foot could be important to reduce the recurrence rate of diabetic foot ulceration. Therefore, we should pay attention to strategic plans for prevention, screening, treatment, and aftercare through the prospective studies including psychosocial risk factor in diabetic foot ulceration.

A Study on the Performance and Utilization of Results of Self-monitoring of Blood Glucose in Type Ⅱ Diabetes (일 병원 제2형 당뇨병환자의 자가혈당검사 수행 및 활용 실태)

  • Purpose: This study was conducted to investigate performance of self-monitoring of blood glucose and the utilization of results for people with type II diabetes. Method: Data was collected from 177 patients with type II diabetes using a questionnaire from August to September, 2009. Result: Most participants performed self-monitoring of blood glucose in the correct way while some misused the lancet or test strips. In the utilization of results for self-monitoring blood glucose data, 62% of participants always recorded the data, 46% always understood the cause for hyperglycemia or hypoglycemia, 25% changed their diet, 23% regulated their exercise-amount, and 11% of participants adjusted the drug dosage. There were significant differences in the performance of self-monitoring of blood glucose and the utilization of results according to the characteristics in the self-monitoring subjects such as awareness of HbA1c, target fasting glucose level, target fasting glucose level 2 hours after meal etc. Conclusion: Systemic self-monitoring of blood glucose education which includes the utilization of self-monitoring of blood glucose needs to be developed for type II diabetic patients.
