Background: Although previously studies have reported that most patients with malignancy prefer to die at home, this is not the real situation in clinical practice. Aim: In this study, we aimed to determine the characteristics of Turkish cancer patients admitted to the emergency department (ED) within one month before death. Materials and Method: This descriptive retrospective study focused on questions about how often and why patients with cancer visited the ED before death. A total of 107 individuals with cancer were divided into 2 groups: Group 1, patients with at least one visit in the final 4 weeks; and Group 2, patients with no visit to ED. Demographic and clinical features were compared between the two groups. Statistical analyses: Descriptive statistical methods, statistical analysis for correlation, Student's t-test, chi-square tests and logistic regression were used. Results: At least one visit to ED within one month before death was reported for 64 (60%) of the 107 cases. Of these 64 (Group 1), 38% (n=24) were discharged and 9% (n=6) died in the ED. The most common site of the primary tumor was the lung (n=24, 38%) and the most common symptom was dyspnea (92%). With the other 43 (40%) cancer patients not presenting to the ED within one month before death, they were more likely to be female with another type of cancer. Conclusions: Guidelines are needed for better management of cancer patients benefiting from visits to ED within the last month of life.

Purpose: The purpose of this study was to identify the relationship between empowerment and performance of infection control by emergency department nurses. Method: Participants were 190 nurses working in emergency departments in 14 hospitals located in Busan. Descriptive statistics, t-test, ANOVA, and Pearson's correlation coefficients with SPSS PC+ WIN 12.0 were used to analyze the data. Results: There were significant differences in empowerment by years in ED (emergency departments) and position. There was a significant differences in performance of infection control by position. There were significant differences in empowerment by infection control-related characteristics and by regular conference for infection control. There was a significant and positive relationship between empowerment and performance of infection control in ED nurses. Conclusion: The results of this study indicate that improvement in the level of ED nurse's empowerment would lead to an increase in the performances of infection control.

Objective : Few studies exist on primary spinal cord tumors (PSCTs) in pediatric patients. The purpose of this study was to perform descriptive analysis and detailed survival analysis for PSCTs. Methods : Between 1985 and 2017, 126 pediatric patients (male : female, 56 : 70) with PSCTs underwent surgery in a single institution. We retrospectively analyzed data regarding demographics, tumor characteristics, outcomes, and survival statistics. Subgroup analysis was performed for the intramedullary (IM) tumors and extradural (ED) tumors separately. Results : The mean age of the participants was 6.4±5.04 years, and the mean follow-up time was 69.5±46.30 months. The most common compartment was the ED compartment (n=57, 45.2%), followed by the IM (n=43, 34.1%) and intradural extramedullary (IDEM; n=16, 12.7%) compartments. Approximately half of PSCTs were malignant (n=69, 54.8%). The most common pathologies were schwannomas (n=14) and neuroblastomas (n=14). Twenty-two patients (17.5%) died from the disease, with a mean disease duration of 15.8±15.85 months. Thirty-six patients (28.6%) suffered from progression, with a mean period of 22.6±30.81 months. The 10-year overall survival (OS) rates and progression-free survival (PFS) rates were 81% and 66%, respectively. Regarding IM tumors, the 10-year OS rates and PFS rates were 79% and 57%, respectively. In ED tumors, the 10-year OS rates and PFS rates were 80% and 81%, respectively. Pathology and the extent of resection showed beneficial effects on OS for total PSCTs, IM tumors, and ED tumors. PFS was affected by both the extent of removal and pathology in total PSCTs and ED tumors; however, pathology was a main determinant of PFS rather than the extent of removal in IM tumors. The degree of improvement in the modified McCormick scale showed a trend towards improvement in patients with IM tumors who achieved gross total removal (p=0.447). Conclusion : Approximately half of PSCTs were malignant, and ED tumors were most common. The most common pathologies were schwannomas and neuroblastomas. Both the pathology and extent of resection had a decisive effect on OS. For IM tumors, pathology was a main determinant of PFS rather than the extent of removal. Radical excision of IM tumors could be a viable option for better survival without an increased risk of worse functional outcomes.
Background: Factors associated with the prognosis of patients with small cell lung cancer (SCLC) is relatively unknown, than of those with non-small cell lung cancer. This study was undertaken to identify the prognostic factors of SCLC. Methods: The medical records of 333 patients diagnosed with SCLC at tertiary hospital from January 1, 2008, to December 31, 2012 were retrospectively reviewed. Patients were categorized by age (${\leq}65$ years vs. >65 years) and by extent of disease (limited disease [LD] vs extensive disease [ED]). Overall survival and progression free survival rates were determined. Factors associated with prognosis were calculated using Cox's proportional hazard regression model. Results: Most baseline characteristics were similar in the LD and ED groups. Eastern Cooperative Oncology Group (ECOG) performance status (PS), first chemotherapy regimen, and prophylactic cranial irradiation (PCI) differed significantly in patients with LD and ED. Mean ECOG PS was significantly lower (p<0.001), first-line chemotherapy with etoposide-cisplatin was more frequent than with etoposide-carboplatin (p<0.001), and PCI was performed more frequently (p=0.019) in LD-SCLC than in ED-SCLC. Prognosis in the LD group was better in younger (${\leq}65$ years) than in older (>65 years) patients, but prognosis in the ED group was unrelated to age. Conclusion: This study showed that overall survival (OS) was significantly improved in younger than in older patients with LD-SCLC. Univariate and multivariate analyses showed that age, PCI and the sum of cycles were significant predictors of OS in patients with LD-SCLC. However, prognosis in the ED group was unrelated to age.
In order to increase the persistence of plant growth promoting rhizobacteria (PGPR) in rhizpsphere soil, the growth of tomato was examined after the application of Arthrobacter woluwensis ED immobilized in alginate bead, which was known as PGPR. When tomato seedlings were treated with A. woluwensis ED of $1{\times}10^6$ cells g $soil^{-1}$ and incubated for 30 days in a plant growth chamber, the shoot length, root length, fresh weight and dry weight of the grown tomato plants treated with the suspended inoculants significantly increased by 36.2, 59, 51.1, and 37.5%, respectively compared to those of the uninoculated control. The treatment of the immobilized bacteria increased those by 42, 67.4, 62.5, and 60.4%, respectively compared to those of the uninoculated control. Therefore, the enhancement of tomato growth by the treatment of the immobilized bacteria was higher than those by the suspended inoculants. The effects of the inoculation on indigenous bacterial community and the fate of the inoculated bacteria were monitored by denaturing gradient gel electrophoresis analysis. The DNA band intensity of A. woluwensis ED in the tomato rhizosphere treated with the suspended inoculants continuously decreased after the inoculation, but the band intensity in the tomato rhizosphere soils treated with the immobilized inoculants showed the maximum at 1 week after inoculation and the decreasing rate was less than that of the suspended inoculants, which indicated the longer maintenance of the immobilized bacteria at rhizosphere soils. Therefore, encapsulation of PGPR in alginate beads may be more effective than liquid inoculant for the plant growth promotion and survival of PGPR at plant rhizosphere.
The purpose of this study was to make new suggestions for the production of high value-added, fashion goods by reviewing and synthesizing the design of dress shirts. (Ed-I've combined paragraphs here) For this study, 1491 photographs on the dress shirts were collected for analysis from men's collection$(2001\sim2006)$. Concerning the actual characteristics of men's fashion design, the following conclusions were obtained from the analysis of the dress shirts of men's collection. (Ed-I've combined paragraphs here) First, feminine ornaments such as frill, pin tuck, and embroidery can be seen in men's dress shirts and men's sexuality has changed into a softer men's image. Second, asymmetry balance using detachability are expressed in the collars, sleeves, and plackets of men's dress shirts. Third, in a very novel suggestion, a combination of two items was designed in a dress shirt and variant shirt forms were created. Purpose: A few studies have assessed the factors affecting the prognoses for major trauma patients and those improving the circumstances when dealing with the trauma system. In that light, we analyzed factors, such as pre-hospital factors, the time to admission, the length of stay in the emergency department (ED) and emergency operation, influencing the outcomes for trauma patients. Methods: The patients who visited our emergency department from April 1, 2009, to February 29, 2011, due to major trauma were enrolled in the study. The inclusion criterion was a revised trauma score (RTS) < 7 or injury severity score (ISS) ${\geq}$ 16. We used reviews of medical records, to analyze each step of emergency medical care with respect to patients' sex, age, visit time and visit date. Continuous variables were described as a median with an interquartile range, and we compared the variables between the survival and the mortality groups by using the Mann-Whitney U test. Fisher's exact test was used for nominal variables. Using the variables that showed statistical significance in univariate comparisons, we performed a logistic regression analysis, and we tested the model's adequacy by the using the Hosmer-Lemeshow method. Results: A total of 261 patients with major trauma satisfied either the RTS score criterion or the ISS score criterion. Excluding 12 patients with missing data, 249 patients were included in this study. The overall mortality rate was 16.9%. Time to ED arrival, time to admission, time of ED stay, RTS, ISS, and visit date being a holiday showed statistically significant differences between the survival and the mortality groups in the univariate analysis. RTS, ISS, length of ED stay, and visit date being a holiday showed statistical significance in the multivariate analysis. Conclusion: The mortality rate did not show a significant relationship with the time to ED arrival, use of 119, on time to admission. Rather, it elicited a quite significant correlation with the trauma scoring system (RTS and ISS), the time of ED stay, and the visit date being a holiday.
The effects of choline and choline esters(acetylcholine, methacholine, carbachol, bethanechol) on motility of isolated rabbit jejunum segment were examined and $pD_2$ values of each drugs were compared. The results were as follows. In choline, there were revealed that maximum effective concentration was $10^{-2}M$, $ED_{50}$ was $2.4{\times}10^{-3}M$, and $pD_2$ value was 2.619. In acetylcholine, there were revealed that maximum effective concentration was $10^{-4}M$, effect was hardly showin in $10^{-9}M$ concentration, $ED_{50}$ was $0.5{\times}10^{-5}M$, and $pD_2$ value was 5.154. In methacholine, there were revealed that maximum effective concentration was $10^{-5}M$, effect was hardly shown in $10^{-9}M$ concentration, $ED_{50}$ was $9{\times}10^{-7}M$, and $pD_2$ value was 6.045. In carbachol, there were revealed that maximum effective concentration was $10^{-5}M$, effect was hardly shown in $10^{-11}M$ concentration, $ED_{50}$ was $5.7{\times}10^{-7}M$, and $pD_2$ value was 6.244. In bethanechol, there were revealed that maximum effective concentration was $10^{-4}M$, effect was hardly shown in $10^{-8}M$ concentration, $ED_{50}$ was $3.3{\times}10^{-6}M$, and $pD_2$ value was 5.480. Choline and choline esters caused contraction on motility of isolated rabbit jejunum segment. The order of $pD_2$ values of drugs was carbachol, methacholine, bethanechol, acetylcholine and choline (in the descending order of potency).
Hemoptysis is a major reason for emergency department (ED) visits. Catamenial hemoptysis (CH), a rare condition of thoracic endometriosis, can cause recurrent hemoptysis but is difficult to diagnose in the ED due to the scarcity of cases and nonspecific clinical findings. We report a case of a 26-year-old woman who presented to the ED with recurrent hemoptysis since 2 years without a definite cause. Her vital signs and blood test findings were unremarkable. Chest computed tomography (CT) did not show any specific lesions other than a non-specific ground-glass opacity pattern in her right lung. She was on day 4 of her menstrual cycle and her hemoptysis frequently occurred during menstruation. Although there was no histological confirmation, based on her history of hemoptysis during menstruation and no other cause of the hemoptysis, the patient was tentatively diagnosed with CH and was administered gonadotropin-releasing hormone. She had no recurrence of hemoptysis for 3 months. While CH is difficult to diagnose in the ED, the patient's recurrent hemoptysis related to menstruation was a clue to the presence of CH. Therefore, physicians should determine the relationship between hemoptysis and menstruation for women of childbearing age presenting with repeated hemoptysis without a definite cause.
Purpose : This study examined factors influencing the attitudes towards nurse-led defibrillation in the emergency department (ED) and intensive care unit (ICU). Methods : This was a cross-sectional study. A total of 212 nurses in the ED and ICU from three general hospitals responded to the survey. Data were collected between September and October 2015 using structured questionnaires. Independent t-test, Mann-Whitney U test, one-way analysis of variance (ANOVA), Pearson's correlation coefficient, and hierarchical multiple regression were used to analyze factors affecting nurses' attitudes towards nurse-led defibrillation. Results : The mean score of nurses' attitudes towards nurse-led defibrillation was 61.37 of a possible 91. Hierarchical multiple regression showed that nurses' attitudes towards nurse-led defibrillation were significantly affected by their experiences regarding cardiopulmonary resuscitation (p=.003), knowledge regarding electrocardiogram interpretation and defibrillation (p=.007), and tenure in ED or ICU (p=.043). Conclusion : Results indicate that employers should set policies to support nurses in maintaining their careers in the ED or ICU and conduct systematic educational programs for them. This will increase nurses' confidence in performing defibrillation, which will lead to early defibrillation during in-hospital cardiac arrest. Consequently, it can be a strategy to increase the survival rate of cardiac arrest patients.
