Epidemiology and outcomes of patients with penetrating trauma in Incheon Metropolitan City, Korea based on National Emergency Department Information System data: a retrsopective cohort study

  • Purpose: Patients with penetrating injuries are at a high risk of mortality, and many of them require emergency surgery. Proper triage and transfer of the patient to the emergency department (ED), where immediate definitive treatment is available, is key to improving survival. This study aimed to evaluate the epidemiology and outcomes of patients with penetrating torso injuries in Incheon Metropolitan City. Methods: Data from trauma patients between 2014 and 2018 (5 years) were extracted from the National Emergency Department Information System. In this study, patients with penetrating injuries to the torso (chest and abdomen) were selected, while those with superficial injuries were excluded. Results: Of 66,285 patients with penetrating trauma, 752 with injuries to the torso were enrolled in this study. In the study population, 345 patients (45.9%) were admitted to the ward or intensive care unit (ICU), 20 (2.7%) were transferred to other hospitals, and 10 (1.3%) died in the ED. Among the admitted patients, 173 (50.1%) underwent nonoperative management and 172 (49.9%) underwent operative management. There were no deaths in the nonoperative management group, but 10 patients (5.8%) died after operative management. The transferred patients showed a significantly longer time from injury to ED arrival, percentage of ICU admissions, and mortality. There were also significant differences in the percentage of operative management, ICU admissions, ED stay time, and mortality between hospitals. Conclusions: Proper triage guidelines need to be implemented so that patients with torso penetrating trauma in Incheon can be transferred directly to the regional trauma center for definitive treatment.

폐상피세포 장벽에 대한 $H_2O_2$의 영향 (Effect of $H_2O_2$ on Alveolar Epithelial Barrier Properties)

  • 연구배경 : 정상 폐상피세포에서는 항상 생성되고 있는 활성산소(oxygen radical)에 의한 유해작용에 노출되어 있고, 이들 유해 산소들은 폐기종과 같은 폐질환의 원인 기전으로 생각되고 있다. 본 연구에서는 이 방법을 이용하여 만든 폐상피세포 단일막에서 전기생리학적인 관점에서 물질의 이동지표인 short-circuit current(Isc)와 조직저항(R)에 대한 활성산소의 하나인 $H_2O_2$(hydrogen peroxide)가 어떤 영향을 미치는지를 연구함으로서 세포생리학적 기전을 구명하고자 한다. 방법 : Tissue culture-treated polycarbonant membrane filter 에서 배양시킨 쥐 제 2 형 폐상피세포 배양 단일막에서 $H_2O_2$의 능동적 이온 이동 (Isc) 과 수동적 용질이동에 대한 조직저항(R)에 미치는 효과를 관찰하였다. 배양 제 3 일과 제 4 일째 단일막을 수정된 Ussing chamber에 설치하고 막 양측에 HEPES-buffered Ringer 용액으로 incubation 하였다. 외부에서 0~100 mM 농도의 $H_2O_2$를 apical 또는 basolateral side에 작용시켜 Isc와 R의 변화를 관찰하였다. 폐상피세포 장벽이 외부의 $H_2O_2$에 대하여 방어작용을 가지는 세포내 catalase 활성도를 측정하고, catalase 억제제인 aminotriazol(ATAZ) 20 mM의 효과도 함께 관찰하였다. 결과 : 이 단일막은 형태학적으로 보아서 in vivo 에서의 포유류 제 1 형 폐상피세포 장벽의 특성을 나타내고 세포들 사이는 tight junction을 이루며(조직저항 R: 2,000 ohm-$cm^2$ 이상) sodium ion의 능동적 이동 (Isc: 5 ${\mu}A/cm^2$)을 보였다. $H_2O_2$는 dose-dependent 양식으로 Isc와 R 모두 감소시켰다. Apical side에 작용하는 $H_2O_2$에 있어서는 60분에 50% 억제하는 농도인 $ED_{50}$는 Isc와 R은 약 4mM 이었으나 basolateral side의 경우는 약 0.04mM 로서 그 작용 강도는 apical에 비하여 약 100배 정도 더 컸다. ATAZ 존재시 apical side의 $ED_{50}$는 0.4mM로 감소하였으나 basolateral side의 경우 변화가 없었다. $H_2O_2$의 제거율은 apical 또는 basolateral side 어느 쪽에 존재하든 같았으며, 세포내 catalase 활성도는세포배양기간이 길어짐에 따라 증가함을 보였다. 결론 : 이상의 실험결과는 basolateral side에 작용하는 $H_2O_2$는 세포내 막구성성분 중 basolateral 측에 존재하는 곳에(예, $Na^+,\;K^+$-APTase) 직접 장애를 미칠 것으로 생각된다. 한편 apical side에 작용하는 $H_2O_2$는 막성분에 도달하기 전에 세포내에 존재하는 catalase에 의하여 대부분 그 작용을 잃게 된다. 결론적으로 Isc와 R로 측정된 폐상피세포 장벽의 특성은 $H_2O_2$에 의하여 손상을 받고 apical side 보다는 basolateral side 측정이 더 손상을 잘 받게 된다.

Ni/3C-SiC 계면의Ohmic 특성 (Ohmic Characteristics of Ni/3C-SiC Interface)

  • 본 연구에서는 3C-SiC의 ohmic 접합에 대하여 그 전기적 특성과 미세구조의 상관관계에 대하여 분석하였다. 표준사진식각 공정을 통하여 ohmic접합 금속으로서 Ni을 진공증착시켜 일련의 TLM패턴으로 열처리에 따르는 전류-전압 특성을 조사하였고 TEM, SEM, AES, EDS를 사용하여 Ni/SiC 계면에 대한 미세구조, 화학적 특성을 분석하였다. 열처리 온도와 시간을 통한 thermal budget이 증가함에 따라서 접촉저항이 감소되었으며 그 값은 $10^{-2}$-$10^{-4}$$\textrm{cm}^2$의 범위에 속하였다. EDS와 AES를 통하여 7$50^{\circ}C$이상의 열처리 후 silicide(NiSi$_{2}$)의 주변에 carbon층이 형성되는 것을 확인하였으며, 열처리 온도가 증가함에 따라서 island형 silicide의 크기가 조밀해지며 SiC와의 접착성이 향상됨을 알 수 있었다. Ni/3C-SiC ohmic 접합의 전기적 특성은 계면에 생성되는 silicide와 carbon의 형성거동에 의하여 결정되는 것으로 믿어진다.

녹수초의 항암활성물질의 분리 및 항암력 평가 (Isolation and Evaluation of an Antitumor Constituent from Pyrolae Herba)

  • The cytotoixic effect of Pylorae Herba (Pyrola japonica Klenze) against L1210 and K-562 cells was studied in vitro. The methanolic extract of Pylorae Radix was add ed to the culture of L1210 cells and K-562 cells for the cytotoxic activity and the ED50 values of hexane, ethylacetate, buthanol and water fractions from methanolic extract were determined using MTT (3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyltetrazoliumbromide) assay. The active constituent isolated by bioassay guided fractionation followed by purification gave rise to a yellow needle crystal and was clarified to be chimaphilline by the comparison with the published data. The average life spans with it were not prolonged significantly on tumor growth in hybrid female mouse (BDF1-KIST) inoculated subcutaneously with P388 cells.

  • Various N'-substituted anilino-N-methyl-N'-nitrosoureas(2a-n) were easily prepared from the reaction of substituted phenylhydraines $(3, 4-CH_3, {\;} 3-, {\;} 4-OCH_3, {\;} 3-, {\;} 4-F, {\;} 3, {\;} 4-Cl, {\;} 4-Br, {\;} 2-, {\;} 3-, {\;} 4-NO_2, 4-(NO_2)_2)$ with methyl isocyanate, followed by the nitrosation with 99% HCOOH and dry sodium lnitrite powder. Surprisingly, of these series of analogus, the anilino-nitrocosureas substituted with eletron-withdrawing nitro groups (2k-a) showed significantly low $ED_{30}$ values of $1.4-3.4 {\mu}g/ml.$ In addition, none of these copounds subtituted with electron-donating groups exhibited cytotoxicities.

자연휴양림(自然休養林) 매력물(魅力物)의 기대불일치(期待不一致)가 이용만족도(利用滿足度)에 미치는 영향(影響) - 자연휴양림의 휴양자원과 시설을 중심으로 - (The Effects of Expectancy-Disconfirmation of Attraction Affecting Visitor's Satisfaction in Natural Recreation Forests - Focused on Recreational Resources and Facilities -)

  • 본 연구의 목적은 자연휴양림 매력물의 기대불일치가 이용만족도에 미치는 영향을 규명하는 것이다. 자연휴양림의 자원과 시설, 기대불일치, 이용만족도에 관한 이론적 고찰을 통해 자연휴양림의 매력물을 도출하고 개념적 틀을 설정하였다. 본 연구의 자료는 2004년 여름철에 3개의 자연휴양림 이용자 415명을 대상으로 한 설문조사를 통해 수집되었다. 분석결과, 1) 각 매력물 중 이용자 흥미자원의 기대도와 성취도 수준이 가장 높았다. 2) 자연휴양림 매력물에 대한 기대도와 성취도간에는 1% 유의 수준에서 통계적 차이가 있었다. 3) 기대불일치가 가장 심한 매력물은 안전시설이었으며, 기대불일치가 적은 매력물은 생물적 자원(식물)으로 나타났다 4) 이용만족도는 매력물의 기대불일치가 클수록 낮아지는 것으로 평가되었다. 5) 다변량 분석결과, 물리적 자원, 사회문화적 자원, 숙박시설, 기반시설, 안전시설, 위생시설, 교육시설의 기대불일치가 1% 유의수준에서 이용만족도에 영향을 미치는 변수로 규명되었으며, 매력물 중 이용만족도의 값의 변화에 가장 큰 영향을 미치는 변수는 기반시설의 기대불일치인 것으로 평가되었다.

천련자로부터 분리한 Limonoid 성분의 세포독성 (The Cytotoxic Limonoid From the Fruits of Melia Toosendan)

  • The MeOH extract of the fruits of Melia toosendan was selected for futher study by its cytotoxicity and effect on the human breast cancer cell line, MCF-7. The active principle obtained by activity guided fractionation followed by purification gave rise to a needle crystal. The structure was deduced by employing NMR and was determined to be identical with 28-deacetyl sendanin by comparison with published data. This compound induced morphological change of MCF-7 to be rounded with tubule at concentrations between $50\;{\mu}g/ml$ and $0.025\;{\mu}g/ml$. This compound, however, showed strong cytotoxic effect on Hepalclc7 and HepG2, and their $GI_{50}$ on the hepatoma cell lines were $0.238\;{\mu}g/ml$ and $0.805\;{\mu}g/ml$, respectively. Its effect on lymphocyte of mouse was stronger than hepatoma cell lines, and their $ED_{50}$ of polyclonal antibody response was $0.011\;{\mu}g/ml$, and $ED_{50}$ of cell viability was $0.039\;{\mu}g/ml$.

사백산(瀉白散)과 사백산가사려륵(瀉白散加訶藜勒)이 호흡기계(呼吸器系)에 미치는 영향(影響) (Effect of Sabaeksan and Sabaeksangakaryereuk on the Respiratory System in Experimental Animals)

  • Sabaeksan and Sabaeksangagaryureuk, a traditional prescription, has been used in Korea for many centuries as a treatment for chronic respiratory disease. The purpose of the present study was to determine the effect of Sabaeksan and Sabaeksangagaryureuk on acetylcholine-induced tracheal smooth muscle contraction in guinea pigs and norepinephrine-induced vascular smooth muscle contraction in pigs. Guinea pig (500g, female) were killed by CO2 exposure and a segment (8-10mm) of the thoracic trachea from each guinea pig and renal artery from each pig were cut into equal segments and mounted 'in pairs' in a tissue bath. Contractile force was measured with force displacement transducers under 0.5g loading tension. The dose of acetylcholine (Ach) and norepinephrine (NE) which evoked 50% of maximal response ($ED_{50}$) was obtained from cumulative dose response curves for acetylcholine (10-7-10-4M) and norepinephrine (10-7-10-4M). Contractions of tracheal smooth muscle evoked by Ach ($ED_{50}$) were inhibited significantly by Sabaeksan and Sabaeksangagaryureuk. Propranolol (10-7M) slightly but significantly attenuated the inhibitory effects of Sabaeksan and Sabaeksangagaryureuk. Indomethacin and methylene blue (10-7M) did not significantly alter the inhibitory effect of Sabaeksan and Sabaeksanga-garyureuk. Contractions of vascular smooth muscle evoked by NE (NE50) were inhibited significantly by Sabaeksan and Sabaeksangagaryureuk. Propranolol (10-7M) slightly but significantly attenuated the inhibitory effects of Sabaeksan and Sabaeksangagaryureuk. Indomethacin and methylene blue (10-7M) did not significantly alter the inhibitory effect of Sabaeksan and Sabaeksangagaryureuk. These results indicate that Sabaeksan and Sabaeksangagaryureuk can relax acetylcholine-induced contraction of guinea pig tracheal smooth muscle, and norepinephrine-induced contraction of pig vascular smooth muscle that this inhibition involves, in part, the relation of adrenergic receptor.

Pseudoalteromonas sp. Ju11-1과 Pseudoalteromonas sp. Ju14의 색소 추출물의 물리화학적 안정성과 기능성 (The Physicochemical Stabilities and Biological Activities of Pigment Extracts from Pseudoalteromonas sp. Ju11-1 and Pseudoalteromonas sp. Ju14)

  • 해양세균 Pseudoalteromonas sp. Ju11-1과 Pseudoalteromonas sp. Ju14의 에탄올 색소 추출물에 대한 안정성과 기능성을 검토한 결과, Ju11-1의 세균 색소는 pH 5.0의 조건과 $25^{\circ}C$ 이하에서 매우 안정하였으며, 금속이온첨가의 경우 $Ca^{2+}$$Mg^{2+}$에서 높은 안전성을 나타내었다. Free radical 소거 활성은 $IC_{50}$ $95.2{\mu}g$/ml, 인체세포에 대한 DNA 손상 회복능은 $ED_{50}$ $82.3{\mu}g$/ml으로 나타나 항산화능이 매우 우수한 것으로 나타났다. 한편 Pseudoalteromonas sp. Ju-14의 세균 색소의 경우 pH 4.0에서 pH 8.0 까지의 조건과 $40^{\circ}C$ 이하에서 매우 안정하였으며, $25^{\circ}C$, 14일간 90% 이상의 잔존율을 나타내어 빛에 대한 안정성이 매우 우수한 것으로 나타났다. 금속이온의 경우 $Fe^{2+}$, $Al^{+3}$, $Cu^{+2}$를 제외한 실험된 모든 금속이온에 대하여 매우 안정하였으며 특히, $Na^+$에 대한 안정성이 매우 높은 것으로 나타났다. Free radical 소거 활성은 $IC_{50}$ $208.6{\mu}g$/ml, 인체세포에 대한 DNA 손상 회복능은 $ED_{50}$ $96.4{\mu}g$/ml으로 항산화능이 우수한 것으로 나타났다. 해양세균 Pseudoalteromonas 속의 Ju11-1과 Ju14, 두 균주의 색소 추출물은 우수한 물리화학적 안정성을 갖으며, free radical 소거 활성 및 인체세포에 대한 DNA 손상 회복능에서 높은 활성을 나타내어 항산화 활성을 갖는 기능성 색소로의 적용을 검토할 수 있을 것으로 기대된다.

평폐산(平肺散)의 효능(效能)에 관(關)한 실험적(實驗的) 연구(硏究) (Experimental Studies on the Effects of Pyeongpaesan)

  • Pyeongpaesan (平肺散) has been used in Korea for many centuries as a treatment for respiratory disease. The effect of Pyeongpaesan (平肺散) on tracheal smooth muscle is not known. The purpose of the present study is to determine the effect of Pyeongpaesan (平肺散) on histamine and acetylcholine induced tracheal smooth muscle contraction in rats and guinea pigs. Guinea pig (500 g, male) and Sprague Dawley rats (200 g, male) were killed by $CO_2$ exposure and a segment (8-10 mm) of the thoracic trachea from each rat and guinea pig was cut into equal segments and mounted 'in pairs' in a tissue bath. Contractile force was measured with force displacement transducers under 0.5 g loading tension. The dose of histamine (His) and acetylcholine (Ach) which evoked 50% of maximal response ($ED_{50}$) was obtained from cumulative dose response curves for histamine and acetylcholine $(10^{-7}{\sim}10^{-4}M)$. Contractions evoked by His $(ED_{50})$ and Ach $(ED_{50})$ were inhibited significantly by Pyeongpaesan (平肺散). In guinea pig tracheal smooth muscle, the mean percent inhibition of acetylcholine induced contraction was 13.5% (p<0.05) after $10{\mu}l/ml$ Pyeongpaesan (平肺散), $64.6\(p<0.01)\;after\;30{\mu}l/ml$ Pyeongpaesan (平肺散), and $92.8\(p<0.01)\;after\;100{\mu}l/ml$ Pyeongpaesan (平肺散). In rat tracheal smooth muscle, the mean percent inhibition of acetylcholine induced contraction was $60.9\(p<0.01)\;after\;30{\mu}l/ml$ Pyeongpaesan (平肺散), and $91.2\(p<0.01)\;after\;100{\mu}l/ml$ Pyeongpaesan (平肺散). Also, in guinea pig tracheal smooth muscle, the mean percent inhibition of histamine induced contraction was $104.8\(p<0.01)\;after\;30{\mu}l/ml$ Pyeongpaesan (平肺散) and $142.3\(p<0.01)\;after\;100{\mu}l/ml$ Pyeongpaesan (平肺散). In rat tracheal smooth muscle, the mean percent inhibition of histamine induced contraction was $63.7\(p<0.01)\;after\;30{\mu}l/ml$ Pyeongpaesan (平肺散), and $107.5\(p<0.01)\;after\;100{\mu}l/ml$ Pyeongpaesan (平肺散). Propranolol $(10^{-7}M)$ slightly but significantly attenuated the inhibitory effects of Pyeongpaesan (平肺散). Following treatment with propranolol, the mean percent inhibition caused by $100{\mu}l/ml$ Pyeongpaesan (平肺散) fell to 15.7% (p<0.05) in guinea pig induced by acetylcholine contraction and the mean percent inhibition caused by $100{\mu}l/ml$ Pyeongpaesan (平肺散) fell to 22.3% (p<0.05) in guinea pig induced by histamine contraction and by $100{\mu}l/ml$ Pyeongpaesan (平肺散) fell to 28.7% (p<0.01) in rat induced by histamine contraction. Indomethacin and methylene blue $(10^{-7}\;M)$ did not significantly alter the inhibitory effect of Pyeongpaesan (平肺散). Also, I could find the effects of Pyeongpaesan (平肺散) and Pyeongpaesanga (平肺散加) morphine on the tracheal smooth muscle in guinea pig and rat did not change significantly. These results indicate that Pyeongpaesan. (平肺散) can relax histamine and acetylcholine-induced contraction of guinea pig and rat tracheal smooth muscle, and that this inhibition involves sympathetic effects and the release of cyclooxygenase products.
