Anterior and Posterior Overjet for Clinical Arch Coordination using 3-dimensional Analysis

  • Introduction : The purposes of this study were to analyze the differences between the anterior and posterior overjets using bracket slot points, and compare two methods of overjet calculation according to different reference points using clinical bracket points on three-dimensional digital models. Methods : A total of 35 normal occlusion models were scanned using a three-dimensional scanner (Orapix$^{(R)}$, Orapix Co., Ltd, Seoul, Korea) and then, virtual brackets (0.022" Slot MBT preadjusted brackets, 3 M Co.CA. USA) were placed on the digital models using virtual setup program (3Txer$^{(R)}$ ver. 1.9.6, Orapix co., Ltd). Archwire-like curves were designed to analyze labial and buccal overjet. Results : There were no statistically significance differences between the right and left overjet and between genders. The average overjet was found to be $1.67{\pm}0.85mm$ at the central incisor area, $2.16{\pm}0.88mm$ at the second premolar and $1.53{\pm}0.71mm$ at the first molar. Conclusion : It is recommended that overjet of individualized upper and lower arch to be 2.0mm at the anterior and posterior teeth.

자연 치아와 골유착성 임플랜트의 동요도에 관한 연구 (MOBILITY OF NATURAL TEETH AND OSSEOINTEGRATED IMPLANTS)

  • It is well known that implants showing no clinical mobility are successfully osseointegrated and have good prognosis. When implants are under load, their mobility begins to increase. It is of necessity to substantiate whether excessive load is on or premature occlusal force is acting prior to desirable osseointegration. Using Periotest unit, we could measure the pattern of mobility change. Consequently, osseointegrated treatment has come to success by intercepting progressive mobility and doing perceptive treatment according to the result of Periotest Value(PTV). In this study, we took records of intangible mobility of 70 osseointegrated implants. And we also measured the mobility of periodontally sound natural teeth as a standard from 30 dental personnel. Conclusions were summarized as followings ; 1. Lower lateral incisor has the highest PTV, whereas lower canine, upper canine, lower premolars and lower 1st molar have the lowest PTV in natural dentition. 2. There are little significant statistical difference of PTV between men and women in both(natural and implant) dentition. 3. In general, lower natural teeth show lower PTV than upper counterpart. 4. Mandibular implants have lower PTV than those of maxillary implants. 5. All of the successfully osseointegrated implants have lower PTV than those of periodontally healthy teeth.

Analysis of location and prevalence of maxillary sinus septa

  • Purpose: The sinus lift procedure requires detailed knowledge of maxillary sinus anatomy and the possible anatomical variations. This study evaluated the location and prevalence of maxillary sinus septa using computed tomography (CT). Methods: This study was based on the analysis of CT images for posterior maxilla which were obtained from patients who visited Chonbuk National University Dental Hospital during the period of June 2007 to December 2008. With the exclusion of cases presenting any pathological changes, 236 maxillary sinuses in 204 patients were retrospectively analyzed. The average age of the patients was 50.9. The cases were divided into two groups, an atrophy/edentulous segment and a non-atrophy/dentate segment, and maxillary sinus septa of less than 2.5 mm were not taken in-to consideration. The location of septa was also divided for analysis into 3 regions: the anterior (1st and 2nd premolar), middle (1st and 2nd molar) and posterior (behind 2ndmolar) regions. Results: In 54 (20.9%) of the 204 patients there were pathologic findings, and those patients were excluded from the analysis. Sinus septa were present in 58 (24.6%) of the 236 maxillary sinuses and in 55 (27%) of the 204 total patients. In the atrophy/ edentulous ridge group (148 maxillary sinuses), 41 cases (27.7%) were found, and 17 cases (19.3%) were found in the non-atrophy/ dentulous ridge group (88 maxillary sinuses). In terms of location, septa were found in 18 cases (27.3%) in the anterior, in 33 cases (50%) in the middle and in 15 cases (22.7%) in the posterior regions. Conclusions: In the posterior maxilla, regardless of type of ridge (atrophy/edentulous or non-atrophy/dentate), the anatomical variation of sinus septa is diverse in its prevalence and location. Thus, accurate information on the maxillary sinus of thepatient is essential and should be clearly understood by the surgeon to prevent possible complications during sinus lifting.

하악전돌자에서 3차원영상을 이용한 하악지시상분할골절단술과 관련된 하악골의 해부학적 연구 (MORPHOLOGIC STUDY FOR SAGITTAL SPLIT RAMUS OSTEOTOMY USING 3-D IMAGE IN MANDIBULAR PROGNATHISM)

  • Sagittal split ramus osteotomy(SSRO) has been commonly performed in the mandibular prognathism. The previous studies of the mandibular anatomy for SSRO have mostly been used in dry skull without consideration of age, sex or jaw relationship of patients. This study was performed to evaluate the location of mandibular canal and the anatomy of ramus, such as the location of mandibular lingula and the ramal bone marrow, which were associated with SSRO procedures, in the patients with mandibular prognathism and normal young adults by using computerized tomographs(CT) and 3D images. The young adults at their twenties, who were considered to complete their skeletal growth, and seen in the Department of Orthodontics and Oral and Maxillofacial Surgery in Chonnam National University Hospital between March 2000 and May 2003, were selected. This study was performed in 30 patients (15men, 15women) who were diagnosed as skeletal class I normal relationship, and another 30 patients (15men, 15women) who were diagnosed as skeletal class III relationship upon clinical examination and lateral cephalometric radiographs. The patients were divided into 2 groups : Class I group, the patients who had skeletal class Ⅰ normal relationship(n=30, 15men, 15women), and Class III group, the patients who had skeletal class III relationship(n=30, 15men, 15women). Facial CT was taken in all patients, and pure 3D mandibular model was constructed by V-works version 4.0. The occlusal plane was designed by three points, such as the mesiobuccal cusp of both mandibular 1st molar and the incisal edge of the right mandibular central incisor, and used as a reference plane. Distances between the tip of mandibular lingula and the occlusal plane, the sigmoid notch, the anterior and the posterior borders of ramus were measured. The height of ramal bone marrow from the occlusal plane and the distance between mid-point of mandibular canal and the buccal or lingual cortex of the mandible in the 1st and 2nd molars were measured by V-works version 4.0. Distance(Li-OP) between the occlusal plane and the tip of mandibular lingula of Class III Group was longer than that of Class I Group in men(p<0.01), but there was no significant difference in women between both groups. Distance(Li-SN) between the sigmoid notch and the tip of mandibular ligula of Class III group was longer than that of Class I Group in men(p<0.05), but there was no significant difference in women between both groups. Distance(Li-RA) between the anterior border of ramus and the tip of mandibular lingula of Class III Group was shorter than that of Class I Group in men and women(p<0.01). Distance(Li-RP) between the posterior border of ramus and the tip of mandibular lingula of Class III Group was slightly shorter than that of Class I Group in men(p<0.05), but there was no significant difference in women between both groups. Distance(RA-RP) between the anterior and the posterior borders of ramus of Class III Group was shorter than that of Class I Group in men and women(p<0.01). Longer the distance(SN-AN) between the sigmoid notch and the antegonial notch was, longer the vertical ramal length above occlusal plane, higher the location of mandibular lingula, and shorter the antero-posterior ramal length were observed(p<0.01). Height of ramal bone marrow of Class III Group was higher than that of Class I Group in men and women(p<0.01). Distance between mandibular canal and buccal cortex of Class III Group in 1st and 2nd lower molars was shorter than that of Class I Group in men and women (p<0.05 in 1st lower molar in men, p<0.01 in others). These results indicate that there are some anatomical differences between the normal occlusal patients and the mandibular prognathic patients, such as the anterior-posterior length of ramus, the height of ramal bone marrow, and the location of mandibular canal.

하악 완전 무치악 환자에서 solitary attachment를 연결한 임시 보철물로 조기 부하를 가한 후, 조립식 바를 이용한 최종 임플란트 지지 피개의치 제작증례 (Early loading using tempo denture with solitary attachment system, implant supported overdenture with prefabricated bar attachment system on Mandibular edentulous patient: A case report)

  • SFI-bar is prefabricated bar system and can be assembled at chairside without soldering or welding, thus reducing bone loss, costs and time. A 53-year-old male patient, who had severely absorbed mandible, hoped to wear a stable mandiblular denture. Four implants were placed in the extraction site of canine and 1st molar. Early loaded temporary denture with solitary type attachment was delivered 3 weeks after surgery. 3 month later, SFI-bar was connected and adjusted at chairside. Then, implant overdenture using SFI-bar was delivered. This case report showed that a satisfactory clinical result was achieved by 4-implant-supported overdenture using the SFI-Bar system in a mandibular edentulous patient.

성견에서 Titanium Plasma Sprayed IMZ 임플란트에 가해진 조기 기능하중의 골유착 및 골형성에 미치는 영향 (EFFECTS OF EARLY FUNCTIONAL LOADING ON BONE FORMATION AROUND TITANIUM PLASMA SPRAYED IMZ IMPLANTS IN DOGS)

  • The purpose of this study was to observe the effects of early functional on bone formation around titanium plsama sprayed IMZ implants. 15 IMZ implants were inserted on the mandibular premolar areaa in 5 mondgrel dogs. The inplants were divided into 4 groups according to it’s loading schedule : Temporary abutments were connected for functional loading 6weeks, 9weeks, and 12weeks after implants installation in group I, II, and III, respectively. Abutments were not connected in the control group. After polyfluorochrome sequential labeling, all animals were sacrificed and bone sections includig implants and the 1st. molar were removed and processed for histologic evaluation. From light and fluorescence microscopic evaluation as well as histomorphometric and EPMA analysis, following results were obtained : 1. Light microscopically, the osteoid tissues were observed much in the compact bone of unloaded group, and cancellous bone formation observed more in early loaded group. 2. Calcification and remodeling of bone took place faster in loaded group, especially in 6 weeks group. 3. Moderate to severe marginal bone resorption was observed in 6 weeks group, mainly on buccal crest. 4. There was no difference in the percentages of bone-to-metal contact among the experimental groups. 5. In quantitative analysis using EPMA, there was no difference in the content of calcium and phosphorus among the experimental groups, regardless of distance.

Alginate와 Agar-Agar에 의한 연합인상법의 촌법정도에 관한 임상적 연구 (A Clinical Study on the Dimensional Accuracy of the Combined Impression (Alginate & Agar-Agar))

  • A series of combined impressions, alginate impressions and rubber base impression made of the lower left quadrant of one subject having a cast gold crown with six reference marks on lower let 1st molar. the combined impressions of hydrocolloid were removed from the mouth at 2.5min, after the insertion of the tray. The alginate impressions were removed from the mouth at 2min, after the insertion of the tray. The rubber base impressions were removed from the mouth at 10min. The effect on the accuracy of the combined impressions, alginate impressions and rubber base impressions was obtained following results by comparing the distances between the reference marks in indirect stone cast with the standard measurements of the cast gold crown prior to cementation. The findings through this study could be summarized as follows; 1. The accuracy of combined impression was superior to that of alginate impression, but it was inferior to that of rubber base impressions. 2. Combined impressions produced a little distortion of the mesio-distal length, alginate impressions produced the largest distortion, ut rubber base impressions produced the least distortion.

Cone beam CT 영상과 석고모형 레이저 스캔 영상의 결합을 이용한 안면비대칭자의 치성보상 평가 (The assessment of dentoalveolar compensation in facial asymmetry individuals: integration of cone beam CT and laser scanned dental cast images)

  • 본 연구에서는 CBCT 영상파 석고모형 레이저 스캔 영상의 결합하여 안면비대칭자의 치성보상을 3차원적으로 계측하고 비교하였다. 안면비대칭자 30명과 정상교합자 20명을 대상으로 하여 CBCT 영상에서 두개악안면 골격에 대한 기준 좌표계를 설정하고 석고모형 레이저 스캔 영상에서 견치와 제1대구치의 3차원적 위치와 각도를 계측하여 그 차이(dev.-ndev.)를 구하였다. 두 집단을 비교 분석하고 이부 편위랑과의 상관 분석을 시행하여 다음과 같은 결과를 얻었다. 안면비대칭군의 편위측 상악 견치는 비편위측에 비해 $2.73{\pm}4.07\;mm$ 협측에 위치하였고, 편위측 상악 제1대구치는 비편위 측에 비해 $3.83{\pm}0.12\;mm$ 협측에 위치하였으며, $8.26{\pm}6.82^{\circ}$ 협측 경사되어 있었다. 비편위측 상악 제1대구치의 협측교두는 편위측에 비해 $1.76{\pm}0.11\;mm$ 정출되어 있었다. 편위측 하악 제1대구치는 비편위측에 비해 $6.01{\pm}0.14\;mm$ 설측에 위치하였고, 비편위측에 비해 $8.31{\pm}7.07^{\circ}$ 설측경사되어 있었다 (p < 0.01). 안면비대칭자는 이부 펀위량이 증가할수록 상악 제1대구치는 비편위측에 비해 편위측에서 협측 경사가 증가하는 양상을 보였고, 하악 제1대구치는 편위측에서 설측 경사가 증가하는 양상을 보였다. 또한 상악 제1대구치의 모든 교두와 하악 제1대구치의 근심설측 교두의 수직적 위치는 비편위측에서 더 정출되는 경향을 보이며, 하악 견치와 제1대구치의 횡적 위치도 편위측에서 좀 더 설측으로 위치하는 양상을 보였다. 이상의 결과에서 안면비대칭군에서 편위측의 견치와 대구치가 비편위측에 비해 횡적으로 기울어져 있고, 수직적 위치 및 각도의 차이를 보임을 확인하였다.

Multiloop Edgewise Arch Wire의 tip back 정도에 따른 응력 분포에 관한 유한요소법적 연구 (Stress analysis of Multiloop Edgewise Arch Wire with various degree of tip back bend : a study using the finite element method)

  • 임상에서 다양하게 사용되는 Multiloop Edgewise Arch Wire(MEAW)는 구치부 치아의 치축 직립을 위해 II급 고무줄이나 수직고무줄과 함께 사용된다. 이때 tip back bend의 정도는 각 환자의 구치치축의 근심경사의 정도에 따라 가감이 결정되나 이 장치가 실제 치근막에서 어떤 효과를 나타내는지 정확히 알려진 바는 없다. 이에 저자는 MEAW의 역학적 효과를 알아보기 위해 tip back bend의 정도를 변화시킨 후 II급 고무줄, 수직고무줄을 적용시켜 유한요소법을 통해 치근막에서 발생되는 응력의 분포와 크기를 정량적으로 비교, 관찰하여 장치의 역계를 이해하고 치료효과를 예측해 보고자 실험을 고안하였다. 발치나 교정치료의 경험이 없는 25세 남자 정상교합자를 대상으로 컴퓨터 단층촬영을 시행하여 얻은 방사선필림을 기초로 하여 유한요소 상악치아모델과, 치근막모델을 제작하였으며 여기에 beam element로 제작된 MEAW의 tip back bend를 5도, 10도, 15도로 달리하여 II급 고무줄 및 수직고무줄을 적용시켜 각 치근막에서의 압축, 인장력의 분포와 응력의 크기를 상호 비교한 결과 다음과 같은 결론을 얻었다. 1. MEAW에 tip back bend를 5도, 10도, 15도로 부여하고 II급 고무줄이나 수직고무줄을 적용시켰을때 전체 치근막에 나타나는 압축, 인장의 분포는 II급 고무줄, 수직고무줄 각각의 경우에서 tip back bend의 정도에 상관없이 유사한 양상을 나타냈다. 2. 각 치아의 치근막에 나타난 압축, 인장력의 크기는 5도,10도의 경우보다 15도 tip back bend 부여시 더 크게 나타났다. 3. 제1, 2대구치 원심측 치근막에서는 압축력이 나타났으며 특히 제2대구치 치근막의 압축부위가 제1대구치 치근막 압축부위보다 더 넓고 압축력의 크기도 더 크게 나타났다. 또한 tip back bend 정도가 클수록 압축력도 크고 압축부위도 더 넓게 나타났다. 4. 대구치 치근막의 압축력은 소구치 치근막의 압축력보다 상대적으로 더 작게 나타났다. 5. II급고무줄과 수직고무줄 적용 시를 비교해 보면 수직고무줄 장착시 전치부 치근막에 인장력이 더 넓게, 그 크기는 더 작게 나타났다. 반면에 구치부 치근막에 나타나는 응력의 분포와 크기는 별 차이를 보이지 않았다. 5. 전치부 치근막 인장부위에서 인장력은 견치에서 제일 컸다.

TMA wire로 제작된 3종류의 MVLAW(Multi-Vertical Loop Arch Wire)의 초기응력분포에 관한 광탄성학적 연구 (A PHOTOELASTIC STUDY ON THE INITIAL STRESS DISTRIBUTION OF 3 TYPES TMA MULTI-VERTICAL LOOP ARCH WIRE)

  • 교정치료시 치축의 개선을 위한 많은 연구와 노력이 이루어져 왔다. 본 교실에서는 치축개선을 위해 MEAW를 응용한 Multi-Vertical Loop Arch Wire(MVLAW : 엠빌로)를 수년 전부터 다수치아의 치축을 동시에 개선코자 사용해 왔다. MVLAW는 일종의 근심경사된 치아를 세우는 장치로써 open vertical loop를 3가지 종류로 변형시켜 적용시켜 왔으며 이를 각각 A, B, C형으로 명명하였다. 각 MVLAW는 .017"x.025" TMAwire로 제작되며, A형 MVLAW의 경우 각 open vertical loop의 apex를 10도 정도 벌려주어 활성화시키며(electric welding stop을 각 loop의 근심측에 위치시킨다), B형과 C형 MVLAW의 경우 open vertical loop의 원심측에 10도의 tip back bend를 형성함으로써 활성화시킨다(B형의 경우 electric welding stop이 각 loop의 원심측에 형성되는 반면 C형의 경우 electric welding stop을 형성하지 않는다). 본 연구는 구치부의 치축을 동시에 개선코자할 때 MVLAW를 3가지 종류로 분류한 후 각 MVLAW의 효과를 알아보고자 상하악 1/2악을 광탄성 모형에 재현하여 그 응력분포를 관찰하여 다음과 같은 결론을 얻었기에 보고하는 바이다. 1. A형 MVLAW의 경우 3급 견인고무를 걸지 않았을 경우 하악 제1대구치와 제2대구치 치근의 원심면에서 강한 응력분포를 나타내나 전치부에 발생한 미약한 응력분포는 3급 견인고무의 장착으로 상쇄되었다. 따라서 A형 MVLAW는 초기에 치관의 직립효과가 있는 것으로 보인다. 2. B형 MVLAW의 경우 3급 견인고무를 걸지 않았을 경우 각 치아의 근심측에 응력분포를 보이며 견인고무를 걸었을 때와 비교시 전치부에 증가된 응력분포를 보였다. 따라서 B형의 경우 초기에 치근의 직립효과가 있는 것으로 보인다. 3. C형 MVLAW의 경우 3급 견인고무를 걸지 않았을 경우 하악전치부를 제외하고는 응력분포를 나타내지 않았으나 견인고무를 걸었을 경우 주로 견치와 소구치 근심면에서 응력분포를 보였다. 따라서 C형의 경우 견인고무 사용시 초기에 견치와 소구치에서 치근의 직립효과가 있는 것으로 보인다.
