Serum thyroglobulin measurement and I-131 whole-body scintigraphy (WBS) are well-established methods for the detection of recurrence in the follow-up of patients with thyroid carcinoma. However, inconsistent results are observed frequently, and these two methods are not always able to detect recurrence. In some patients, serum thyroglobulin level is elevated but the WBS is negative, because the recurrent tumor is too small and below the sensitivity of the diagnostic scan, or there is a dissociation between thyroglobulin synthesis and the iodine frapping mechanism. In such cases, various nuclear imaging methods including Tl-201 Tc-99m-sestamibi, and F-18-FDG PET can be used besides anatomical imaging methods. Among them, FDG PET localizes recurrent lesions in WBS-negative thyroid carcinoma with high accuracy. Several studies have suggested that empirical high-dose I-131 therapy resulted in a high rate of visualization in post-therapy scans with evidence of subsequent improvement. An important question is when to operate on patients with recurrent tumor. We believe that surgical removal is the best means of treatment for patients with localized persistent tumor, despite the high-dose I-131 therapy. with tumor in thyroid remnant, and with isolated recurrence in the lymph node, lung or bone. In addition, we recommend palliative resection of locally unresectable mass with subsequent treatment with high-dose I-131 therapy. Before I-131 therapy, the evaluation of sodium-iodide symporter expression in thyroid carcinoma can predict iodine uptake. Retinoic acid is known to induce redifferentiation, and to enhance I-131 uptake in thyroid carcinoma. Retinoic acid therapy may represent an alternative approach before high-dose I-131 therapy.
During the maintenance period at Korean nuclear power plants, internal exposure of radiation workers occurred by the inhalation of $^{131}I$ released to the reactor building when primary system was opened. The internal radioactivity of radiation workers contaminated by $^{131}I$ was immediately measured using a whole body counter and the whole body counting was performed again after a few days. In this study, the intake estimated from the record history of entrance to radiation control areas and the measurement results of air sampling for $^{131}I$ in those areas, were compared with that from the results of whole body counting. As a result, it was concluded that the intake estimation using whole body counting and air sampling showed similar results.

A 39-year-old female patient who had undergone a total thyroidectomy for a papillary thyroid carcinoma underwent a whole body scan with I-131. The I-131 scan was performed 72 hours after administering 185 MBq (5 mCi) of an I-131 solution. The anterior image of the head, neck, and upper chest showed multiple areas of increased uptake in the mediastinal area considering of functional metastasis. However, radioactivity was not evident in the image taken after removing her clothes and muffler. The image obtained after placing the muffler on the pallet showed that the radioactivity was still present. It is well known that artifacts on an I-131 scan can be produced by styling hair sputum, drooling during sleep, chewing gum, and paper or a cloth handkerchief that is contaminated with the radioactive iodine from either perspiration or saliva. This activity might be mistaken for a functional metastasis. Therefore, it is essential that an image be obtained after removing the patient's clothes. In this study, artifacts due to a contaminated muffler on the I-131 scan were found. These mimicked a functional metastasis of the mediastinal area in a patient with a papillary thyroid carcinoma.
In order to compare the therapeutic effect as well as side effects between antithyroid therapy and radioiodine therapy in hyperthyroidism, the author evaluated 111 cases of hyperthyroidism which were composed of 57 patients with antithyroid treatment, 23 patients with combined treatment comprising of antithyroid and radioactive iodine ($^{131}I$) and 31 patients with treatment of $^{131}I$ alone. The $^{131}I$ treatment was limited to older age, above 20 years of age, and not employed in patients with pregnancy and lactation. The patients treated with antithyroid belonged to relatively younger age, and also milder in symptoms and thyroid function tests. The remission rate of antithyroid therapy group was 97.8% within 16 months. Among them the early remission rate occured within 4 months was found to be 21.7%. The combined therapy group, having prescribed more severe cases, showed the remission rate to be 75% within 16 months. The remission rate of $^{131}I$ therapy group revealed 90.9% within 16 months. Among them the early remission rate taking place within 4 months disclosed 16.7%. The side effects of antithyroid drug, propylthiouracil, were noted as follows: itching (2.7%) skin rashes (1.3%), and adverse enlargement of goiter (10.7%). The side effects $^{131}I$ therapy were transient hypothyroidism(3.9%) permanent hypothyroidism (7.8%) and itching (2.0%).
T1-201 and I-131 scintigraphy were performed to visualize malignant lesions postoperatively in 26 patients with well-differentiated thyroid cancer. Anyone of uptake index and retention index was higher in malignant lesions than that in benign lesions. T1-201 scintigraphy showed uptake ininflammatory tissues. T1-201 concentration was observed in fourteen patients and six of them had malignant lesions, while I-131 concentration was observed in six patients and all had malignant lesions. In sensitivity, combination of T1-201 and I-131 scintigraphy is higher than I-131 or T1-201 scintigraphy only. In postoperative patients with well-differentiated thyroid cancer, it may be useful that T1-201 scintigraphy will be performed simultaneously with I-131 scintigraphy for detecting recurrence.
Many radionuclides exist in normal environment and artificial radionuclides also can be detected. The radionuclides ($^{131}I$) are widely used for labeling compounds and radiation therapy. In Korea, the radionuclide ($^{131}I$) is produced at the Radioisotope Production Facility (RIPF) at the Korea Atomic Energy Research Institute in Daejeon. The residents around the RIPF assume that $^{131}I$ detected in environmental samples is produced from RIPF. To ensure the safety of the residents, the radioactive concentration of $^{131}I$ near the RIPF was investigated by monitoring environmental samples along the Gap River. The selected geographical places are near the nuclear installation, another possible location for $^{131}I$ detection, and downstream of the Gap River. The first selected places are the "front gate of KAERI", and the "Donghwa bridge". The second selected place is the sewage treatment plant. Therefore, the Wonchon bridge is selected for the upstream of the plant and the sewage treatment plant is selected for the downstream of the plant. The last selected places are the downstream where the two paths converged, which is Yongshin bridge (in front of the cogeneration plant). In these places, environmental samples, including sediment, fish, surface water, and aquatic plants, were collected. In this study, the radioactive iodine ($^{131}I$) detection along the Gap River will be investigated.
BACKGROUND: Recently, the use of $^{131}I$ for diagnosis and treatment of thyroid cancer has been increasing, and the radionuclide is continuously released into aquatic ecosystem. This study was carried out to investigate the $^{131}I$ concentrations in mainstreams, tributaries, and sewage wastewater treatment plants (SWTPs) of the Yeongsan River Basin and to identify their origins from the assessment of behaviors in the rivers. METHODS AND RESULTS: The water samples were collected from 19 sites including mainstreams (13), tributaries (4) and SWTPs (2). The $^{131}I$ concentration was measured using a gamma-ray spectrometry with a HPGe detector. The $^{131}I$ in SWTPs was detected mostly in the discharged effluent at the sampling sites. However, from the surface water of the rivers, $^{131}I$ was found only at two sites from each sampling period of the first (MS4 and MS10) and the second half (MS4 and MS7) of the year 2017. The concentrations of $^{131}I$ in the effluent discharged from SWTPs were in the range of 0.0870 to 3.87 Bq/L for SWTP1, and $^{131}I$ in the river revealed that it was not detected in the upper streams of the mainstreams and tributaries, while continuous detection was found in the SWTPs and downstream sites affected by the effluent. However, the concentration of $^{131}I$ decreased downstream, eventually becoming undetectable. Such behavior was closely related to the behavior found in the SWTPs. CONCLUSION: These results indicated that medically-derived $^{131}I$ was discharged to the river via sewage effluent at the SWTPs. It is necessary to evaluate the influence of aquatic ecosystems through continuous monitoring in the future.

Over the past 6 years, from May 1960 to June 1966, 1,716 patients with various diseases of thyroid were examined and thyroid function tests with $^{131}I$ were done. Among them, 545 patients with hyperthyroidism were treated with $^{131}I$. A summary of the clinical data of the $^{131}I$-thyroid function tests and the therapeutic results of $^{131}I$ were presented and discussed. 1. The patients examined consisted of; 596 cases(34.7%) with toxic diffuse goiter, 412 cases(24.0%) with non-toxic nodular goiter, 278 cases(16.2%) with euthyroidism, 236 cases(13.8%) with non-toxic diffuse goiter, 89 cases(5.2%) with hypothyroidism, 53 cases(3.1%) with toxic nodular goiter, 32 cases(1.9%) with thyroiditis and 20 cases(1.2%) with dyshormonogenesis. 2. There were 218(12.7%) male patients and 1,498(87.3%) female patients, showing a ratio of 1:6.9. female predominantly. 3. The majority of patients(79.6%) were in the 3rd through 5th decades of their lives showing the peak in the 4th decades(35.9%). 4. The diagnostic values and normal ranges of $^{131}I$ uptake test, 48 hour serum activity, $T_3$ red blood cell uptake and $PB^{131}I$ conversion ratio were discussed. 5. An attention was given to dyshormonogenesis, a qualitative hypothyroidism, due to its characteristic findings of clinical and $^{131}I$ thyroid function tests, and its pathogenesis was briefly reviewed. 6. Among 545 patients with hyperthyroidism treated with $^{131}I$, 68.3% was cured after single. therapeutic dose and another 24.0% was cured after second dose. 7. The complications of $^{131}I$ therapy were discussed in some details and myxedema had developed. in 3.9% of our cases. No thyroid cancer was found after $^{131}I$ therapy.
To evaluate the genotoxic effect of $^{131}I$, lymphocytes from 9 patients who underwent large dose (150 mCi) $^{131}I$ therapy after total thyroidectomy were studied for sister chromatid exchanges (SCE) before and after their first radioiodine treatments. Frequency of SCE (FSCE) was counted in chromosomes of 30 lymphocytes in each patients, and was expressed as numbers of SCE per cell. Numbers of leukocytes were also observed during $^{131}I$ therapy. Pretreatment FSCE ($4.2{\pm}0.7$) was not different from the control ($3.8{\pm}0.4$, p=0.17). However, the frequency was significantly elevated after $^{131}I$ administration (at the second day, $7.9{\pm}0.8$, p<0.001) and was diminished but still significantly elevated after a week (at 9th day, $6.4{\pm}0.6$, p<0.001). While counts of leukocytes in the peripheral blood showed no change (p> 0.05). In conclusion, chromatids of human lymphocytes were significantly damaged after $^{131}I$ treatment without any bone marrow supression. And the repair of SCE was not complete within one week.
