Rabbit Model for in vivo Study of Intervertebral Disc Degeneration and Regeneration

  • Objective: The purpose of this study is to verify the usefulness of the rabbit model for disc degeneration study. Materials: The L1-L2, L2-L3, L3-L4. or L4-L5 lumbar intervertebral disc (IVD) of 9 mature male New Zealand White rabbits were injured by inserting a 16-gauge needle to a depth of 5 mm in the left anterolateral annulus fibrosus while leaving L5-L6 IVD uninjured. Three other rabbits also received intradiscal injections of rabbit disc cells transfected with adenovirus and bone morphogenetic protein-2 (ad-BMP-2) at L4-L5 in addition to injury by 16-gauge needle at the L1-L2 level. Using digitized radiographs, measurements of IVD height were made and analyzed by using the disc height index (DHI). Magnetic resonance imaging (MRI) scans of the injured discs, injected discs, and uninjured L5-L6 discs were performed at 15 weeks post surgery and compared with preoperative MRI scans. Results: All twelve rabbits showed consistent results of disc degeneration within 15 weeks following annular puncture. DHIs of injured discs were significantly lower than that of the uninjured L5-L6 discs (p<0.05). The mean value of disc degeneration grade of injured discs was significantly higher than that of uninjured discs (p<0.05). The injection of disc cell transfected with ad-BMP-2 did not induce disc regeneration at 15 weeks after injection. Conclusion: This study showed that the injured disc had a significant change in DHI on simple lateral radiograph and disc degeneration grade on MRI scans within 15 weeks in all rabbits. Rabbit annular puncture model can be useful as a disc degeneration model in vivo.

Preparation of silica-coated gadolinium compound particle colloid solution and its application in imaging

  • A preparation method for gadolinium compound (GdC) nanoparticles coated with silica ($GdC/SiO_2$) is proposed. GdC nanoparticles were prepared with a homogeneous precipitation method at $80^{\circ}C$ using $1.0{\times}10^{-3}$ M $Gd(NO_3)_3$, 0.5 M urea and $0-3.0{\times}10^{-4}$ M ethylenediarinnetetraacetic acid disodium salt dihydrate (ETDA) in water. As a result of preparation at various EDTA concentrations, GdC nanoparticles with a size as small as $40.5{\pm}6.2$ nm, which were colloidally stable, were prepared at an EDTA concentration of $2.0{\times}10^{-4}$ M. Silica-coating of the GdC nanoparticles was performed by a St$\ddot{o}$ber method at $35^{\circ}C$ using $1.0-10.0{\times}10^{-3}$ M tetraethylorthosilicate (TEOS), 11 M $H_2O$ and $1.5{\times}10^{-3}$ M NaOH in ethanol in the presence of $1.0{\times}10^{-3}$ M GdC nanoparticles. Performance of preparation at various TEOS concentrations resulted in production of $GdC/SiO_2$ particles with an average size of $106.1{\pm}11.2$ nm at a TEOS concentration of $5.0{\times}10^{-3}$ M. The gadolinium (Gd) concentration of $1.0{\times}10^{-3}$ M in the as-prepared $GdC/SiO_2$ particle colloid solution was increased up to a Gd concentration of 0.2 M by concentrating with centrifugation. The core-shell structure of $GdC/SiO_2$ particles was undamaged, and the colloid solution was still colloidally stable, even after the concentrating process. The concentrated $GdC/SiO_2$ colloid solution showed images of X-ray and magnetic resonance with contrast as high as commercial Gd complex contrast agents.

Establishment of a [18F]-FDG-PET/MRI Imaging Protocol for Gastric Cancer PDX as a Preclinical Research Tool

  • Purpose: The utility of 18-fluordesoxyglucose positron emission tomography ([18F]-FDG-PET) combined with computer tomography or magnetic resonance imaging (MRI) in gastric cancer remains controversial and a rationale for patient selection is desired. This study aims to establish a preclinical patient-derived xenograft (PDX) based [18F]-FDG-PET/MRI protocol for gastric cancer and compare different PDX models regarding tumor growth and FDG uptake. Materials and Methods: Female BALB/c nu/nu mice were implanted orthotopically and subcutaneously with gastric cancer PDX. [18F]-FDG-PET/MRI scanning protocol evaluation included different tumor sizes, FDG doses, scanning intervals, and organ-specific uptake. FDG avidity of similar PDX cases were compared between ortho- and heterotopic tumor implantation methods. Microscopic and immunohistochemical investigations were performed to confirm tumor growth and correlate the glycolysis markers glucose transporter 1 (GLUT1) and hexokinase 2 (HK2) with FDG uptake. Results: Organ-specific uptake analysis showed specific FDG avidity of the tumor tissue. Standard scanning protocol was determined to include 150 μCi FDG injection dose and scanning after one hour. Comparison of heterotopic and orthotopic implanted mice revealed a long growth interval for orthotopic models with a high uptake in similar PDX tissues. The H-score of GLUT1 and HK2 expression in tumor cells correlated with the measured maximal standardized uptake value values (GLUT1: Pearson r=0.743, P=0.009; HK2: Pearson r=0.605, P=0.049). Conclusions: This preclinical gastric cancer PDX based [18F]-FDG-PET/MRI protocol reveals tumor specific FDG uptake and shows correlation to glucose metabolic proteins. Our findings provide a PET/MRI PDX model that can be applicable for translational gastric cancer research.

$^{17}O$ NMR 기법을 이용한 상자성 자기공명조영제의 물분자 교환에 관한 연구 ($^{17}O$ NMR Study On Water Excharge Rate of Paramagnetic Contrast Agents)

  • 목적 : 자성 자기공명조명제의 효율을 결정하는데는 상자성물질의 물분자 결합위치에 구속되어 있는 물분자와 자유 물분자사이의 물분자 교환율이 매우 중요한 역할을 담당한다. 따라서 본 연구에서는 $^{17}O-NMR$기법을 사용하여 현재 상용화 되어 있는 Gd 자기공명조영제 및 최근 간특이성 자기공명조영제로 제안되고 있는 Gd-EOB-DTPA의 물분자 교율을 측정하고자 하였다. 대상 및 방법 본 연구에 사용된 조영제는 Gd-DTPA, Gd-DTPA-BMA, Gd-DOTA, Gd-EOB-DTPA 이며 여기에 Isotech 사의 5% $^{17}O$로 치환된 증류수를 혼합하여 사용하였다. 결과적인 시료의 pH는 buffer용액을 사용하여 pH=7로 고정하였으며 다양한 온도에서 Bruker-600 (14.1 T, 81.3 MHz)모델의 NMR장비를 사용하여 측정하였다. 에코열 24개의 Carr-Purcell-Meiboom-Gill (CPMG) 펄스 시권스를 사용하여 $^{17}O$의 스핀-스핀 이완시간(T2)을 측정하고 이렇게 얻어진 T2 데이터는 최소자승법을 이용하여 Solomon-Bloembergen방정식에 fitting시켜서 최종적으로 각 조명제의 물분자 교환율을 계산하였다. 결과 : 측정된 각 조영제의 물분자 교환시간은 300k의 온도에서는 Gd-DTPA의 경우 0.427, Gd-DTPA-BMA의 경우 $1.99{\;}{\mu}s$, Gd-DOTA의 경우 $0.27{\;}{\mu}s$, Gd-EOB-DTPA의 경우 $0.11{\;}{\mu}s$로 나타났으며 이러한 물분자 교환시간은 온도에 따라 변화함을 알았다. 물분자 교환시간의 온도 의존성은 모든 조영제에서 지수함수의 형태로 나타났으나 조영제에 따라 온도가 올라감에 따라 물분자 교환시간이 감소하는 감소율에서는 차이를 나타내었다. 결론 : 상자성 조영제의 relaxation enhancement 기전을 이해하는데는 물분자 교환율에 대한 정보가 매우 중요하며 이러한 물분자 교환율을 정확히 측정하는데는 $^{17}O-NMR$기법이 매우 유용함을 알 수 있었다.

Engraftment of Human Mesenchymal Stem Cells in a Rat Photothrombotic Cerebral Infarction Model : Comparison of Intra-Arterial and Intravenous Infusion Using MRI and Histological Analysis

  • Objective : This study aimed to evaluate the hypotheses that administration routes [intra-arterial (IA) vs. intravenous (IV)] affect the early stage migration of transplanted human bone marrow-derived mesenchymal stem cells (hBM-MSCs) in acute brain infarction. Methods : Male Sprague-Dawley rats (n=40) were subjected to photothrombotic infarction. Three days after photothrombotic infarction, rats were randomly allocated to one of four experimental groups [IA group : n=12, IV group : n=12, superparamagnetic iron oxide (SPIO) group : n=8, control group : n=8]. All groups were subdivided into 1, 6, 24, and 48 hours groups according to time point of sacrifice. Magnetic resonance imaging (MRI) consisting of T2 weighted image (T2WI), $T2^*$ weighted image ($T2^*WI$), susceptibility weighted image (SWI), and diffusion weighted image of rat brain were obtained prior to and at 1, 6, 24, and 48 hours post-implantation. After final MRI, rats were sacrificed and grafted cells were analyzed in brain and lung specimen using Prussian blue and immunohistochemical staining. Results : Grafted cells appeared as dark signal intensity regions at the peri-lesional zone. In IA group, dark signals in peri-lesional zone were more prominent compared with IV group. SWI showed largest dark signal followed by $T2^*WI$ and T2WI in both IA and IV groups. On Prussian blue staining, IA administration showed substantially increased migration and a large number of transplanted hBM-MSCs in the target brain than IV administration. The Prussian blue-positive cells were not detected in SPIO and control groups. Conclusion : In a rat photothrombotic model of ischemic stroke, selective IA administration of human mesenchymal stem cells is more effective than IV administration. MRI and histological analyses revealed the time course of cell migration, and the numbers and distribution of hBM-MSCs delivered into the brain.

단위용적 및 다용적 기법 자기공명분광 신호처리 분석 소프트웨어의 개발 (The Development of Signal Processing Software for Single-and Multi-Voxel MR Spectroscopy)

  • 단위용적기법(Single-Voxel Technique) 및 다용적기법(Multi-Voxel Technique)으로 얻어진 수소원자핵 자기공명분광(Proton Magnetic Resonance Spectroscopy : 1H-MRS) 데이터를 고가의 특정 workstation이 아닌 일반 PC에서 처리${\cdot}$분석 할 수 있는 GUI(Graphical User Interface) 기반의 분광분석용 소프트웨어(S/W)를 개발하고 MRS 임상 데이터로 기초적인 시험을 하였다. 단위용적기법의 MRS 경우에는 시간 영역의 측정신호와 주파수 영역의 분광정보를 한 화면에 동시에 표시할 수 있게 하였고 필요에 따라 위상보정, 필터링(filtering), 퓨리에변환 등으로 후처리 한 후 생체 대사물질(metabolite) 별 MRS 피크 면적 비를 계산하여 정성분석을 하였다. 다용적기법의 MRS에서는 측정된 신호의 재배열 및 3차원 FFT로 분광영상(spectroscopic image)을 재구성하고 이를 T1-강조 또는 T2-강조 영상 위에 중첩시켜 해부학적 정보와 함께 표시할 수 있도록 하였다. 또한 생체에서 대사물질에 해당하는 분광정보의 각 피크의 분포도(metabolite image)를 만들고 해부학적 영상과 중첩시켰다. 병원에 설치되어있는 MR 장치에서 정상인과 뇌에 이상이 있는 환자를 대상으로 얻은 MR 분광 데이터를 처리·분석한 결과 사용자의 편의성이 높을 뿐만 아니라 신뢰성이 우수하여 임상적 활용도가 높을 것으로 기대된다.

Software development for the visualization of brain fiber tract by using 24-bit color coding in diffusion tensor image

  • Purpose: The purpose of paper is to implement software to visualize brain fiber tract using a 24-bit color coding scheme and to test its feasibility. Materials and Methods: MR imaging was performed on GE 1.5 T Signa scanner. For diffusion tensor image, we used a single shot spin-echo EPI sequence with 7 non-colinear pulsed-field gradient directions: (x, y, z):(1,1,0),(-1,1,0),(1,0,1),(-1,0,1),(0,1,1),(0,1,-1) and without diffusion gradient. B-factor was 500 sec/$\textrm{mm}^2$. Acquisition parameters are as follows: TUTE=10000ms/99ms, FOV=240mm, matrix=128${\times}$128, slice thickness/gap=6mm/0mm, total slice number=30. Subjects consisted of 10 normal young volunteers (age:21∼26 yrs, 5 men, 5 women). All DTI images were smoothed with Gaussian kernel with the FWHM of 2 pixels. Color coding schemes for visualization of directional information was as follows. HSV(Hue, Saturation, Value) color system is appropriate for assigning RGB(Red, Green, and Blue) value for every different directions because of its volumetric directional expression. Each of HSV are assigned due to (r,$\theta$,${\Phi}$) in spherical coordinate. HSV calculated by this way can be transformed into RGB color system by general HSV to RGB conversion formula. Symmetry schemes: It is natural to code the antipodal direction to be same color(antipodal symmetry). So even with no symmetry scheme, the antipodal symmetry must be included. With no symmetry scheme, we can assign every different colors for every different orientation.(H =${\Phi}$, S=2$\theta$/$\pi$, V=λw, where λw is anisotropy). But that may assign very discontinuous color even between adjacent yokels. On the other hand, Full symmetry or absolute value scheme includes symmetry for 180$^{\circ}$ rotation about xy-plane of color coordinate (rotational symmetry) and for both hemisphere (mirror symmetry). In absolute value scheme, each of RGB value can be expressed as follows. R=λw|Vx|, G=λw|Vy|, B=λw|Vz|, where (Vx, Vy, Vz) is eigenvector corresponding to the largest eigenvalue of diffusion tensor. With applying full symmetry or absolute value scheme, we can get more continuous color coding at the expense of coding same color for symmetric direction. For better visualization of fiber tract directions, Gamma and brightness correction had done. All of these implementations were done on the IDL 5.4 platform.

3.0T 자기공명영상기기에서 시행한 3D-FLAIR 영상에서의 정상 안면신경의 조영증강 양상 (Enhancement Pattern of the Normal Facial Nerve on Three - Dimensional (3D) - Fluid Attenuated Inversion Recovery (FLAIR) Sequence at 3.0 T MR Units)

  • 목적: 3-T 자기공명영상장치에서 시행한 3D-FLAIR 및 3D-T1-FFE-FS영상에서의 정상안면신경의 조영증강 양상의 비교가 그 목적이다. 대상과 방법: 측두골 자기공명영상을 촬영한 환자 중 안면신경이상의 과거력이 없는 20명의 환자를 대상으로 하였다. 두 명의 영상의학전문의가 정상안면신경의 조영증강을 평가하기 위해 조영증강 전후의 3D-T1-FFE-FS영상과 3D-FLAIR영상을 2주 간격으로 독립적으로 분석하였다. 안면신경은 5개의 분절로 나누었고 조영증강의 정도는 없거나 약하게 혹은 강하게 되는 경우로 나누어 분석하였다. 결과: 3D-FLAIR영상에서 한 명의 평가자가 두 예의 안면신경 슬부에 약한 조영증강이 있다고 보았다. 3D-T1-FFE-FS영상에서는 13예의 안면신경에서 최소 한 분절 이상 조영증강이 있다고 하였고, 최소 한 명의 평가자가 안면신경의 총 100개 분절 중 17개의 분절에서 조영증강이 있다고 분석하였으며, 유양분절의 빈도가 가장 높았다. 3D-T1-FFE-FS영상에서 정상안면신경의 조영증강에 대한 평가자간의 일치도는 ${\kappa}$값 0.589로 우수하였다. 결론: 3D-T1-FFE-FS영상과 달리 3D-FLAIR영상에서 정상안면신경은 거의 조영증강되지 않는다.

Comparative Analysis of Spontaneous Infectious Spondylitis : Pyogenic versus Tuberculous

  • Objective : Spondylitis is often chemotherapy resistant and requires long-term treatment. Without adequate chemotherapy, the outcome can be fatal or result in severe neurologic damage. Therefore, differentiating the etiology of spondylitis is very important, particularly in spontaneous cases. As the prevalence of tuberculosis in Korea has decreased in recent years, updated clinical research about spondylitis is warranted. Methods : From April 2010 to March 2016, data from spondylitis patients were collected retrospectively. In total, 69 patients (51 with pyogenic spondylitis and 18 with tuberculous spondylitis) were included. Clinical data, laboratory findings including erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) level, measurements of Cobb angles at the initial and final follow-up, and radiologic features on magnetic resonance imaging (MRI) scans were evaluated. To test differences between the pyogenic and tuberculous groups, numerical data were compared using the student's t-test and Mann-Whitney U test, and categorical data were compared using the chi-square test and Fisher's exact test. Results : The patients' mean age was 60.0 years. Male sex was slightly predominant (56.5%). There was no difference in mean age and sex between the two groups. The pyogenic group had a relatively higher proportion of immunocompromised patients. The peak CRP value was higher in the pyogenic group than in the tuberculous group (14.08 mg/dL and 8.50 mg/dL, respectively, p=0.009), whereas the ESR was not significantly different between the groups (81.5 mm/h and 75.6 mm/h, respectively, p=0.442). Radiologically, the presence of disc space sparing and vertebral body collapse differed between the groups. In the tuberculous group, the disc was more commonly preserved on contrast-enhanced MRI (50% and 23.5%, respectively, p=0.044), and vertebral body collapse was more common (66.6% and 15.7%, respectively, p<0.001). The mean length of hospitalization was longer in the pyogenic group (56.5 days and 41.2 days, respectively, p=0.001). Four mortality cases were observed only in the pyogenic group. The most commonly isolated microorganism in the pyogenic group was Staphylococcus aureus(S. aureus) (methicillin susceptible S. aureus and methicillin resistant S. aureus [MRSA] in 8 and 4 cases, respectively). Conclusion : The clinical and radiological manifestations of spontaneous spondylitis differ based on the causative organism. Pyogenic spondylitis patients tend to have a higher CRP level and a more severe clinical course, whereas tuberculous spondylitis patients present with destruction of the vertebral body with disc sparing more frequently. The presence of MRSA is increasing in community-acquired spondylitis cases.

흰쥐 광 혈전 뇌경색 모델에서 줄기세포 추적을 위한 자화강조영상 (Susceptibility Weighted Image for Stem Cell Tracking in Rat Photothrombotic Infarction)

  • 사람 중간엽 줄기세포(hMSCs)를 초상자성산화철(SPIO)로 표지하여, 체외에서 세포 농도에 따른 신호의 변화를 알아보고, 흰쥐 광 혈전 뇌경색 모델에 이식하여 자기공명영상 기법 중, T2강조영상과 $T2^*$강조영상 그리고 자화강조영상의 줄기세포 검출능을 분석하고, 병리 소견과 비교하고자 하였다. 체외실험은 SPIO(Feridex I.V.$^{(R)}$)로 표지한 줄기세포 $1.56{\times}10^4$, $3.13{\times}10^4$, $6.25{\times}10^4$, $1.25{\times}10^5$, $2.5{\times}10^5$, $5{\times}10^5$ 개/$m{\ell}$, SPIO로 표지 하지 않은 $5{\times}10^5$ 개/$m{\ell}$의 세포를 Control로 준비하여 $0.5m{\ell}$튜브에 담아 T2WI, $T2^*WI$, SWI sequence로 자기공명영상을 얻었다. 체내실험은 8마리의 흰쥐를 대상으로 정수리점(bregma)에서 우측으로 2.5mm, 뒤쪽으로 2.5mm 부위에 광 혈전 뇌경색을 만들고, SPIO-hMSC $2.5{\times}10^5$ 개/$m{\ell}$, $1m{\ell}$을 꼬리 정맥에 주입하여 실험하였다. 자기공명영상은 주입 전과 주입 후 1, 3, 7 그리고, 14일째 T2WI, $T2^*$WI, SWI 영상을 얻고 정량적으로 평가하였다. 자기공명영상검사 후 흰쥐는 조직검사를 위해 희생시켰다. 체외실험 결과 CNR은 SWI, $T2^*WI$, T2WI 순으로의 높게 나왔고, 세포 농도에 따라서는 $2.5{\times}10^5$ 개/$m{\ell}$의 농도에서 가장 높게 나왔다. 체내실험에서 normal, infarction, SPIO 상호간의 CNR을 분석해본 결과 normal-infarction의 CNR은 T2WI가 가장 높게 나왔고, normal-SPIO 그리고 infarction-SPIO에서 CNR은 SWI, $T2^*WI$, T2WI순으로 나왔다. 따라서 흰쥐를 이용한 실험에서 T2WI sequence는 infarction을 잘 표현해 주고, SWI는 SPIO로 표지한 줄기세포를 normal과 infarction으로 부터 잘 구별해 주었다. 현재 임상에서 SWI는 출혈, 석회화 병변등을 쉽게 찾아내는 sequence로 사용 되고 있지만 향후에 줄기세포 치료가 인간의 질병을 치료하는 주요방안으로 자리 잡을 때에도 이식된 줄기 세포를 관찰 할 수 있는 좋은 도구가 될 것으로 기대 된다.
