The Findings of $^{99m}Tc$-MDP Bone Scan in Primary Malignant Bone Tumors (원발성 악성 골종양의 $^{99m}Tc$-MDP 골스캔 소견)

  • [ $^{99m}Tc$ ]-MDP bone scan was performed in 31 patients with primary malignant bone tumors, 22 patients with osteogenic sarcoma, 5 patients with chondrosarcoma and 4 patients with Ewing's sarcoma. The findings were classified by isotope intensity of accumulation in tumor as grade 1 to 3, overall pattern of isotope distribution in tumor as grade 1 to 3, and distortion of bony outline as grade 1 to 3. Histologic classifications were cor related with scan findings in 22 patients with osteogenic sarcoma. The results were as follows 1) In 22 patients with osteogenic sarcoma, markedly increased isotope intensity higher than sacroiliac joint with patchy areas of decreased intensity and severe bony distortion were found in 16 patients. The correlations between histologic classification and scan findings were not discovered. 2) In 5 patients with chondrosarcoma, mildly increased isotope Intensity with patchy areas of increased intensity and mild bony distortion were found in 4 patients.. 3) In 4 patients with Ewing's sarcoma, markedly increased homogenous intensity with moderate bony distortion were found in 3 patients. Conclusively there were common findings in each 3 primary malignant bone tumors and $^{99m}Tc$-MDP bone scan was complemented with radiologic studies in differentiating primary malignant bone tumors.

FDG-PET and MDP scan findings in chronic osteomyelitis of the left femur (좌측 대퇴골에 발생한 만성골수염의 PET와 MDP scan 영상)

  • A 49-year-old male patient with a carcinoma of the right pyriform sinus had a whole-body bone scan and gamma camera based F-18 FDG-PET for staging. Tc-99m MDP bone scan depicted diffuse increased uptake in the left femur due to chronic osteomyelitis but no skeletal metastasis. F-18-FDG-PET revealed increased focal bone uptake and uptake in the draining sinus due to chronic osteomyelitis in addition to visualization of the right pyriform sinus carcinoma and right neck nodal uptake. Fluorine-18 fluorodeoxyglucose-positron emission tomography is significantly more accurate than the bone scan in pinpointing chronic osteomyelitis focus and draining soft tissue infection.

$^{99m}Tc$-MDP Bone Scan in the Cases of Muscle Necrosis Associated with Acute Renal Failure (급성(急性) 신부전(腎不全)이 동반(同伴)된 근양사(筋壤死)에 있어서의 $^{99m}Tc$-MDP 골주사(骨走査))

  • We studied four patients with muscle necrosis associated with acute renal failure to evaluate the diagnostic value of the bone scan in this disease. The illness followed carbon monoxide poisoning in two patients, acute physical exertion in one and contaminated intramuscular injection in the other. Whole-body rectilinear bone scans using technetium 99m-methyldiphosphonate were done. In all patients, increased muscle labelling at the regions of suspected muscle injury was showed, and in one, it was after normalization of serum muscle enzyme levels. In one patient, the bone scan was rechecked 8 months later and showed no residual abnormality. Above all, the site and precise extent of muscle injury could be detected and the degree of muscle labelling seemed to correlate with the severy of muscle injury. These findings suggest that isotope scanning may be useful in the diagnosis of patients with acute muscle necrosis.

Evaluation of Vascularization of Hydroxyapatite Ocular Implants by $^{99m}Tc-MDP$ Bone Scan : Preliminary Study ($^{99m}Tc-MDP$ 골주사를 이용한 Hydroxyapatite 안구보충물의 혈관신생에 관한 예비연구)

  • 목적 : 안구제거술 후에 삽입한 hydroxyapatite 안구보충물내로 혈관신생이 완전하게 일어 나는 시기를 알아보기 위함이다. 방법 : 안구제거술을 받은 43예의 환자(남자 33예, 여자 10예, 14-56세)에게 대부분 변경되지 않은 방법으로 hydroxyapatite 안구보충물을 삽입한 후 10-23주 사이에 다양한 간격(10-12 주 2예, 13-16주 18예, 17-20주 20예, 21-23주 3예)을 두고 $^{99m}Tc-MDP$ 골주사를 시행하여 보충물내로의 혈관신생 정도를 전향적으로 측정하였다. 정면 및 측면촬영의 정적영상에서 안구보충물의 방사능섭취가 비교(nasal bridge)의 방사능섭취보다 약할 경우를 grade 1, 같을 경을 grade 2, 강할 경우를 grade 3로 나누었으며 grade 2와 3을 혈관신생이 완전하다고 정의하였다. 또한 43예 모두에서 방사능섭취 정도와 관계없이 안구보충물 삽입후 평균 21주되는 시기에 안구보충물에 구멍을 내어 이 구멍으로부터의 출혈여부를 확인한 후 운동성나사(motility peg)를 끼워 의안(prosthesis)과 연결하였다. 43예중 7예에서는 $^{99m}Tc-MDP$ 골주사와 조영제주입후의 자기공명영상(Gd-DTPA T1-weighted images)을 같은 날짜에 시행하여 각각의 예에서 방사능섭취 정도와 조영제증강 정도를 비교하였다. 결과 : 안구보충물내로 완전하게 혈관신생이 일어났던 grade 2와 3의 비율은 안구보충물삽입후 10-12주에서는 0%, 13-16주 33%, 17-20주 50%, 21-23주 67%로써 시간경과에 따라 점차 증가하였다. 골주사소견에 관계없이 안구보충물 삽입후 평균 21주되는 시기에 보충물 앞쪽에 구멍을 내었을 때 43예 모두에서 완전한 혈관신생을 의미하는 출혈을 볼 수 있었다. $^{99m}Tc-MDP$ 골주사와 조영제주입후의 자기공명영상을 둘 다 시행한 7예 각각에서 방사능섭취의 정도와 조영제증강의 정도가 거의 일치하였다. 결론 : 본 예비연구의 결과로써 hydroryapatite 안구보충물 삽입후 21주째에 $^{99m}Tc-MDP$ 골주사를 시행하는 것이 좋을 것으로 생각되며, hydroxyapatite 안구보충물내로의 완전한 혈관신생 여부를 보는 데에는 $^{99m}Tc-MDP$ 골주사가 조영제주입후의 자기공명영상보다 더 경제적이라고 할 수 있겠다.

Cold Lesions in $^{99m}Tc$-MDP Bone Scans of Patients with Skeletal Metastases ($^{99m}Tc$-MDP 골스캔에서 냉소로 나타난 전이 암의 분석)

  • The present study was purposed to evaluate the incidence and the characteristics of metastatic "cold" lesions in $^{99m}Tc$-MDP bone scans of adult patients with solid malignancies. There were 29 cold lesions in 24 patients. The incidence of cold lesions was about 1% of total cases of bone scans for the patients with malignancy, or 2.5% of cases with bone metastases. Th primary sites of malignancies were lung (four cases), uterine cervix (three cases), kidney, nasopharynx, thyroid, urinary bladder, prostate, lymphoma (two cases each other), liver, breast and others (one case each other). But the relative incidence of cold lesion in lung cancer and breast cancer was low. The most frequent site of cold lesion was spine, and pelvis, skull and rib were followed. The incidence of cold lesion was related to the regional incidence of bone metastases. The size of the cold lesions was greater than that of the hot. There were six cases of single cold lesion without any other abnormalities and two cases of cold lesion which were initially hot. So it should be considered that bone metastases might be presented as cold lesions in bone scan.

Experiment to Calculate the Dosage of Radiopharmaceutical Products during the Bone Scan Tests (Bone Scan 검사 시 실제 환자에게 투여되는 용량 계산을 위한 실험)

  • In this experiment, we are to know the administered radioactivity in the actual patients by measuring the remained radioactivity when administering the isotope with noted MDP, the radiopharmaceutical product, to 50 visited patients for the bone scan, confirmed the radioactivity administered to actual patients. We confirmed the actual administered amount of remained radioactivity in the syringe and 3-way by using a gauger after administering the isotope with MDP noted via 3-way with 50 patients maintaining 3-way after CT or MRI among the visited patients for the bone scan in the department of nuclear medicine. As a result of radioactivity in the 3-way and syringe pre and post injection of radiopharmaceutical products in 50 patients, average radioactivity of pre-injection is 31.75 mCi, average remained radioactivity in the syringes after injection is 1.22 mCi, and the average remained radioactivity in 3-way after injection is 0.95 mCi. The average of actual administered radioactivity is 29.57, so it is obvious that average 2.18 mCi was administered for less than the dosage that we initially intended to inject. When determining the dosage in view of the radioactivity that remains in the 3-way with the syringe, it would be possible to accurately dose the desired dosage to be administered to actual patients.

Bone Scintigraphic Findings in Leukemic Patients (백혈병 환자의 골스캔 소견)

  • The leukemias represent diffuse lesions of the bone marrow and significant bony abnormalities are associated with the more aggressive varieties of leukemias and with the younger age groups. On plain film, several roentgenographic findings are observed such as diffuse osteopenia, radiolucent metaphyseal bands, osteolytic lesions and periostitis. We evaluated bone scintigraphic findings using $^{99m}Tc-methylene$ diphosphonate $(^{99m}Tc-MDP)$ in ten patients with proven leukemia. The scan is abnormal in 90%. We classified abnormal scintigraphic findings to 3 types and these are increased diffuse juxta-articular uptake, focal increase of uptake and combined type. The common sites of focal uptake were femur, humerus, tibia, spine, ribs, calvarium, scapula and mandible. We concluded that $(^{99m}Tc-MDP)$ bone scintigraphy is sensitive imaging modality in leukemic patients with bone pain.

  • Patients with diffusely increased uptake in both kindeys("hot kidney") on the $^{99m}Tc-MDP$ bone imagings were evaluated. Total incidence of the "hot kidney" was 2.7% (56/2053). Among 56 patients with "hot kidney", the most common diseases were malignancies (30), and the remainders were composed of renal diseases(11) and other disease group(16). We report the possible factors for the "hot kidney" with brief review of some literatures.

Normal Variants and Artifacts in Bone Scan: Potential for Errors in Interpretation (골스캔 판독시 오류를 범할 수 있는 정상 변이소견 및 인공물)

  • Bone scan is one of the most frequently peformed studios in nuclear medicine. In bone scan, the amount of radiolsotope taken up by lesion depends primarily on the local rate of bone turnover rather than on the bone mass. Bone scan is extremely sensitive for defecting bony abnormalities. However, abnormalities that appear on bone scan may not always represent disease. The normal scan appearances may be affected not only by skeletal physiology and anatomy but also by a variety of technical factors which can influence image quality. Many normal variants and artifacts may appear on bone scan. They could simulate a pathologic process and could mislead into the wrong diagnostic interpretation. Therefore, their recognition is necessary to avoid misdiagnosis. A nuclear medicine physician should be aware of variable appearance of the normal variants and artifacts on bone scan. In this article, a variety of normal variants and artifacts mimicking real pathologic lesion in bone scan interpretation are discussed and illustrated.
