Prognostic value of $^{18}F$-fluorodeoxyglucose positron emission tomography, computed tomography and magnetic resonance imaging in oral cavity squamous cell carcinoma with pathologically positive neck lymph node

  • Purpose: To evaluate the prognostic value of preoperative neck lymph node (LN) assessment with $^{18}F$-fluorodeoxyglucose positron emission tomography ($^{18}F$-FDG PET), computed tomography (CT), and magnetic resonance imaging (MRI) in oral cavity squamous cell carcinoma (OSCC) patients with pathologically positive LN. Materials and Methods: In total, 47 OSCC patients with pathologically positive LN were retrospectively reviewed with preoperative $^{18}F$-FDG PET and CT/MRI. All patients underwent surgical resection, neck dissection and postoperative adjuvant radiotherapy and/or chemotherapy between March 2002 and October 2010. Histologic correlation was performed for findings of $^{18}F$-FDG PET and CT/MRI. Results: Thirty-six (76.6%) of 47 cases were correctly diagnosed with neck LN metastasis by $^{18}F$-FDG PET and 32 (68.1%) of 47 cases were correctly diagnosed by CT/MRI. Follow-up ranged from 20 to 114 months (median, 56 months). Clinically negative nodal status evaluated by $^{18}F$-FDG PET or CT/MRI revealed a trend toward better clinical outcomes in terms of overall survival, disease-free survival, local recurrence-free survival, regional nodal recurrence-free survival, and distant metastasis-free survival rates even though the trends were not statistically significant. However, there was no impact of neck node standardized uptake value ($SUV_{max}$) on clinical outcomes. Notably, $SUV_{max}$ showed significant correlation with tumor size in LN (p < 0.01, $R^2$ = 0.62). PET and CT/MRI status of LN also had significant correlation with the size of intranodal tumor deposit (p < 0.05, $R^2$ = 0.37 and p < 0.01, $R^2$ = 0.48, respectively). Conclusion: $^{18}F$-FDG PET and CT/MRI at the neck LNs might improve risk stratification in OSCC patients with pathologically positive neck LN in this study, even without significant prognostic value of $SUV_{max}$.

FDG-PET을 이용한 암 치료 효과의 평가 (Evaluation of Cancer Treatment Using FDG-PET)

  • FDG-PET has potential as an effective, non-invasive tool to measure tumor response to anticancer therapy. The changes in tumor FDG uptake may provide an early, sensitive guide to the clinical and subclinical response of tumors to cancer treatment, as well as functional assessment of residual viable tumor. This may allow the evaluation of subclinical response to anticancer drugs in early clinical trials and improvements in patients management. However, monitoring tumor responses with FDG-PET is still in its infancy. The methods of measurement of FDG uptake are currently diverse and timing with respect to anti cancer therapy variable. Therefore, there is a need for larger-scale trials along with standardized methodology and a collection of reproducibility data. The recent guideline from the European group seems to be the most comprehensive. In future, the combination of morphological and metabolic images may improve the quantitative nature of these measurements by relating tumor viability to total tumor mass. More data on sensitivity and specificity of FDG-PET technique are needed along with continued advancement of PET methodology.

부인암에서 양전자방출단층촬영의 이용 (Utility of PET in Gynecological Cancer)

  • Clinical application of positron emission tomography (PET) is rapidly increasing for the detection and staging of cancer at whole-body studios performed with 2-[fluorine-18]fluoro-2-deoxy-D-glucose (FDG). Although many cancers can be detected by FDG-PET, there has been limited clinical experience with FDG-PET for the defection of gynecological cancers including malignancies in uterus and ovary. FDG-PET can show foci of metastatic disease that may not be apparent at conventional anatomic imaging and can and in the characterization of indeterminate soft-tissue masses. Most gynecological cancers need to surgical management. FDG-PET can improve the selection of patients for surgical treatment and thereby reduce the morbidity and mortality associated with inappropriate surgery. FDG-PET is also useful for the early detection of recurrence and the monitoring of therapeutic effect. In this review, I discuss the clinical feasibility and limitations of this imaging modality in patients with gynecological cancers.

경화성 혈관종에서의 높은 FDG 섭취 (High FDG Uptake in Sclerosing Hemangioma)

  • A 42 years old woman underwent F-18 FDG PET because of the incidentally detected lung mass on chest X-ray. PET/CT showed hypermetabolic lesion in the lung right upper lobe and the lung cancer was suspected because of the high FDG uptake. However, pathologic diagnosis was sclerosing hemangioma. There are few reports on the evaluation of sclerosing hemangioma using FDG PET. A report showed a slightly increased uptake (standardized uptake ratio of 1.8) (1), and another report showed unsatisfactory result (2). We suggest that sclerosing hemangioma could be seen as hypermetabolic lesion on the FDG PET.

국소 진행된 직장암의 $^{18}F$-FDG PET/CT를 이용한 항암방사선치료의 반응성 예측 (The Usefulness of $^{18}F$-FDG PET/CT for Predicting the Response of Locally Advanced Rectal Cancer to Neoadjuvant Chemoradiation Therapy)

  • 목 적: 국소 진행된 직장암에서 수술 전 동시항암방사선치료 후 정확한 치료 반응 평가는 매우 중요하다. 본 연구는 국소 진행된 직장암에서 $^{18}F$-FDG PET/CT를 이용한 수술 전 항암방사선치료의 반응성 예측에 대해 알아보고자 하였다. 대상 및 방법: 2006년 8월부터 2008년 1월까지 원자력의학원에서 국소 진행된 직장암으로 수술 전 항암방사선치료를 시행한 20명의 환자를 대상으로 전향적 연구를 시행하였다. 방사선 치료는 한 회에 1.8 Gy씩 하루 1회 주 5회의 일정으로 총 50.4 Gy 시행되었고, 항암 치료는 3주 간격으로 3회 시행되었다. 모든 환자는 치료 전과 항암방사선치료 종료 6주 후에 각각 $^{18}F$-FDG PET/CT 촬영을 하였다. 모든 환자는 수술을 시행 받았고 수술 후 절제된 조직을 통해 Dworak 분류에 따른 종양의 치료에 대한 반응 정도를 평가하였다. 결 과: 20명 중 11명의 환자가 Dworak 분류에 따라 반응군에 속했으며, 나머지 9명은 비반응군으로 분류되었다. 수술 후 $SUV_{max}$값과 metabolic volume (SUV가 2.5 이상인 병변의 용적)은 수술 전 $SUV_{max}$값과 metabolic volume보다 현저히 낮았다. 그러나 반응군에서 metabolic volume의 변화를 제외하고는 통계적으로 유의한 차이는 보이지 않았다. 이 연구에서는 치료 후 반응군과 비반응군을 구분하는 cut-off value는 $SUV_{max}$가 67% 감소하는 지점으로 나타났다. 이 cut-off value를 이용하여 반응군과 비반응군을 구분하는 경우 민감도는 45.5%, 특이도는 88.9%였으며, 양성 예측률과 음성 예측률은 각각 71.4% 및 53.8%였다. 결 론: 비록 본 연구에서는 통계적으로 유의한 결과를 얻지 못하였지만, 다른 연구들을 통해 봤을 때 국소 진행된 직장암 환자에서 수술 전 방사선 및 항암 병합 요법의 효과를 판정하고 예측하는데 있어서 $^{18}F$-FDG PET/CT를 이용한 검사가 유용할 것으로 생각된다.

공기주입 직장 확장 F-18 FDG PET/CT의 임상적 유용성 (The Clinical Utility of Rectal Gas Distension F-18 FDG PET/CT)

  • 목적: F-18 FDG PET/CT에서 직장에 국소적 섭취 증가를 보이는 환자에서 공기주입 직장확장 영상(rectal gas distension image)의 임상적 유용성을 알아보았다. 대상 및 방법: 2008년 1월부터 2008년 7월까지 F-18 FDG PET/CT를 촬영한 환자 중 공기주입 직장확장 영상을 추가 촬영한 24명의 환자를 대상으로 하였다(남:여=11:13 나이 $62.8{\pm}12.4$세). F-18 FDG 주사 후 1시간째에 촬영한 영상에서 직장 내 국소적인 섭취증가를 보인 환자는 직장 내로 공기주입 후 복부 추가 PET/CT영상을 얻었다. 공기주입 직장확장 영상에서도 지속적으로 관찰되는 섭취에 대해서는 크기와 SUVmax를 측정하였고, 대장내시경과 조직검사로 확진하였다. 결과: 24명의 직장 내 국소 섭취증가를 보인 환자 중 16명은 하부 위장관 악성 종양 환자였고, 2명은 상부 위장관 악성 종양, 2명은 난소암, 3명은 다른 악성 종양(유방암, 갑상선암, 담낭암), 1명은 양성 질환 이었다. 이중 7명은 공기주입 직장확장 영상에서 보이지 않아 생리적 섭취로 간주하였다. 또한 3명의 환자는 직장이 아닌 인접한 다른 장기의 섭취로 확인되었다(2명: 직장방관공간, 1명: 자궁 근종). 공기주입 직장확장 영상에서 지속적으로 섭취를 보였던 17병소 중에서 15병소는 악성 종양으로 확진되었고, 이들의 크기는 $4.7{\pm}2.9\;cm$ (1-12.1 cm), SUVmax는 $16.1{\pm}7.7$ (5.7-28.9)이었다. 2명은 양성질환(1: 선종, 1명: 염증성질환)으로 확인되었으며, 선종으로 확인된 병변은 직장확장 영상에서 1 cm 크기의 유경성(pedunculated) 용종 모양으로 관찰되었다. 염증성 질환으로 확인 되었던 병변 중 하나는 직장확장 영상에서 이전 직장 수술 부분에 국소적으로 섭취가 남아있었으나 섭취정도가 감소하였다. 결론: 공기주입 직장확장 영상은 직장의 국소 병변에 대해 생리적 섭취와 병리적 섭취를 감별하는데 도움을 주며, 직장 병변과 직장 외 병변을 감별해 준다. 또 병변의 직접적인 형태를 관찰할 수 있어 판독의 특이도를 높이는데 도움을 줄 수 있다.

Imaging of Gastric Cancer Metabolism Using 18 F-FDG PET/CT

  • Aerobic glycolysis has been the most important hypothesis in cancer metabolism. It seems to be related to increased bioenergetic and biosynthetic needs in rapidly proliferating cancer cells. To this end, F-18 fluorodeoxyglucose (FDG), a glucose analog, became widely popular for the detection of malignancies combined with positron emission tomography/computed tomography (PET/CT). Although the potential roles of FDG PET/CT in primary tumor detection are not fully established, it seems to have a limited sensitivity in detecting early gastric cancer and mainly signet ring or non-solid types of advanced gastric cancer. In evaluating lymph node metastases, the location of lymph nodes and the degree of FDG uptake in primary tumors appear to be important factors affecting the diagnostic accuracy of PET/CT. In spite of the limited sensitivity, the high specificity of PET/CT for lymph node metastases may play an important role in changing the extent of lymphadenectomy or reducing futile laparotomies. For peritoneal metastases, PET/CT seems to have a poorer sensitivity but a better specificity than CT. The roles of PET/CT in the evaluation of other distant metastases are yet to be known. Studies including primary tumors with low FDG uptake or peritoneal recurrence seem suffer from poorer diagnostic performance for the detection of recurrent gastric cancer. There are only a few reports using FDG PET/CT to predict response to neoadjuvant or adjuvant chemotherapy. A complete metabolic response seems to be predictive of more favorable prognosis.

18F-FDOPA Brain PET/CT 검사의 영상 대조도 분석 및 섭취 시간에 따른 SUV변화 고찰 (A Study on analysis of contrasts and variation in SUV with the passage of uptake time in 18F-FDOPA Brain PET/CT)

  • $^{18}F$-FDOPA는 뇌 종양의 아미노산 대사를 추적하는 방사성 의약품이다. 본 연구의 목적은 뇌 종양의 아미노산 대사를 영상화 하여 악성 종양을 진단하는 $^{18}F$-FDOPA와 포도당 대사를 통한 $^{18}F$-FDG의 Brain PET/CT 검사 영상의 대조도 분석을 통해 병변의 검출 능력을 비교하고, $^{18}F$-FDOPA Brain PET/CT 검사에서 섭취 시간에 따른 SUV의 변화를 분석하여 최적의 영상 획득 시간을 알아보기 위함이다. $^{18}F$-FDOPA 와 $^{18}F$-FDG 두 영상에서 종양(Tumor)과 소뇌(Cerebellum)의 중심에 각각 약 $350mm^2$의 관심 영역을 설정하여 $SUV_{max}$를 측정하였고, 종양과 소뇌의 $SUV_{max}$ 비율(T/C ratio)을 산출하였고, $^{18}F$-FDOPA 투여 직후 30분 동안 획득한 리스트 수집 방식 데이터(List mode data)를 활용해 2분씩 15프레임으로 나눈 뒤 각 프레임 별로 종양과 소뇌 중심에 $SUV_{max}$를 측정하여 위와 동일한 방법으로 T/C ratio를 산출하여 분석하였다. 종양의 평균 $SUV_{max}$를 비교해 본 결과, $^{18}F$-FDOPA Brain PET/CT 검사에서 $4.2{\pm}0.8$, $^{18}F$-FDG Brain PET/CT 검사에서는 $5.6{\pm}0.7$ 이었다. 또한, T/C ratio는 $^{18}F$-FDOPA 검사에서 $2.1{\pm}0.7$, $^{18}F$-FDG 검사에서는 $1.1{\pm}0.4$ 이었으며, $^{18}F$-FDOPA의 $SUV_{max}$$^{18}F$-FDG보다 낮지만 T/C ratio는 높게 나타나 종양 구별 능력이 더욱 뛰어난 것을 알 수 있었다(t=-5.214, p=0.000). $^{18}F$-FDOPA의 섭취 시간에 따른 $SUV_{max}$와 T/C ratio를 분석한 결과, $SUV_{max}$와 T/C ratio의 Peak는 모두 6~8분에서 나타났다. 이를 토대로 본원에서 $^{18}F$-FDOPA Brain PET/CT 검사에서 활용하는 10~30분의 영상과 Peak가 나타나기 시작한 6~26분의 영상을 비교한 결과 SUV와 T/C ratio가 각각 0.2, 0.1 증가하였다. 추후 지속적인 연구를 통해 검사 소요시간의 단축 가능성과 추가적인 스캔 정보 활용을 통한 정확한 진단에도 도움이 될 것으로 사료된다.

주기성 구토 증후군 환아에서 보인 F-18 FDG PET에서의 뇌 포도당 대사 소견 (Pattern of Cerebral Glucose Metabolism on F-18 FDG Brain PET during Vomiting and Symptom Free Periods in Cyclic Vomiting Syndrome)

  • Cyclic Vomiting Syndrome (CVS) is characterized by recurrent, periodic, self-limiting vomiting. However, its pathogenesis is not yet established. We investigated the changes of the cerebral glucose metabolism using F-18 FDG during the vomiting attack and symptom free period in two children with CVS. FDG PET study showed the markedly increased metabolism in both temporal lobes and also in the medulla and cerebellum during the vomiting period. Also, FDG PET showed the decreased metabolism un the parieto-occipital and occipital areas during the vomiting period. The area with decreased metabolism seemed to be related with the region showing abnormalities in EEG and perfusion SPECT studies. We expect that what we observed would be a helpful finding in clarifying the pathogenesis of the CVS.

Kikuchi-Fujimoto disease mimicking malignant lymphoma with 2-[$^{18}F$]fluoro-2-deoxy-D-glucose PET/CT in children

  • Purpose: Kikuchi-Fujimoto disease (KFD) is a benign disease, which is characterized by a cervical lymphadenopathy with fever, and it often mimics malignant lymphoma (ML). 2-[$^{18}F$]fluoro-2-deoxy-D-glucose positron emission tomography/computed tomography ($^{18}F$-FDG PET/CT) is a powerful imaging modality for the diagnosis, staging and monitoring of ML, with the limitations including the nonspecific FDG uptake in infectious or inflammatory processes. This study compared clinical manifestations and PET/CT findings between KFD and ML patients. Methods: We retrospectively reviewed the medical records of 23 patients with KFD and 33 patients with ML, diagnosed histopathologically, between January 2000 and May 2013 at the Department of Pediatrics, Yeungnam University Medical Center. Among them, we analyzed the clinical manifestations, laboratory findings and characteristics, and the amount of $^{18}F$-FDG uptake between 8 KFD and 9 ML patients who had $^{18}F$-FDG PET/CT. Results: The $^{18}F$-FDG PET/CT maximum standardized uptake values ($SUV_{max}$) ranged from 8.3 to 22.5 (mean, 12.0) in KFDs, and from 5.8 to 34.3 (mean, 15.9) in MLs. There were no significant differences in $SUV_{max}$ between KFDs and MLs. $^{18}F$-FDG PET/CT with ML patients showed hot uptakes in the extranodal organs, such as bone marrow, small bowel, thymus, kidney, orbit and pleura. However, none of the KFD cases showed extranodal uptake (P<0.001). $^{18}F$-FDG PET/CT findings of KFD with nodal involvement only were indistinguishable from those of ML. Conclusion: Patients who had extranodal involvement on PET/CT were more likely to have malignancy than KFD.
