초록
목적 : 폐경기 여성에서 호르몬 대치요법과 안구 건조 증상과의 관계에 대해 알아보았다. 대상과 방법 : 호르몬 대치요법을 받고 있는 폐경기 여성에서 안구 건조증으로 안과 외래를 방문한 환자 100명과 호르몬 대치요법을 받지 않는 폐경기 여성 25명에서 안구 건조 증상의 정도 차이 등을 후향적으로 비교 분석하였다. 결과 : 57명에서 호르몬 대치요법을 받은 총 기간은 평균 41.4개월이었으며, 에스트로겐 단독요법 16명, 에스트로겐과 프로게스테론 병합요법 41명으로 각각 쉬르머 검사는 평균 7.29${\pm}$4.29mm, 6.59${\pm}$4.64mm, 눈물막 파괴시간은 5.00${\pm}$2.47초, 4.95${\pm}$2.35초를 보였다. 이들은 모두 유의한 변화를 보이지 않았다. 호르몬 치료를 받고 있는 폐경기 여성의 쉬르머 검사는 평균 6.75${\pm}$4.50mm, 눈물막 파괴시간은 평균 4.95${\pm}$2.37초로, 호르몬 치료를 받지 않는 폐경기 여성의 11.73${\pm}$7.09mm, 8.65${\pm}$4.50초에 비해 통계적으로 유의하게 감소한 것으로 나타났다(p<0.05). 결론 : 호르몬 대치요법을 받고 있는 폐경기 여성들에서 호르몬 대치요법의 총 기간과 호르몬의 종류가 안구 건조 증상의 정도에는 영향을 미치지 않으나, 호르몬 대치요법을 받지 않는 폐경기 여성과 비교하면 안구 건조 증상의 발현 정도가 유의하게 증가함을 알 수 있었다.
Purpose: An investigation was carried out into the relationship between dry eye syndrome and hormone replacement therapy(HRT) in postmenopausal women Methods: A retrospective analysis was made of the symptomatic differences between 100 and 25 postmenopausal women suffering from dry eye symptoms who had either undergone, or not, HRT, respectively. Results: The average total time period of HRT in 57 of the patients was 41.4months, (ranging form 2 to 180 months) in 57 patients. 16 patients had received an estrogen only regimen, and 41 a combination therapy, consisting of both estrogen and progesterone. The Schirmer test results and the tear film break-up times(BUT) in these two groups were 7.29${\pm}$4.29 and 6.59${\pm}$4.64 mm, and 5.00${\pm}$2.47 and 4.95${\pm}$2.35 seconds, respectively. No significant difference in the results was found between the two groups. The Schirmer test and BUT results in the HRT patients averaged 6.75${\pm}$4.50 mm, and 4.95${\pm}$2.37 seconds, respectively. The results showed a statistically significant decrease (p<0.05) for the non-HRT patients, with Schirmer test and BUT results of 11.73${\pm}$7.09 mm, and 8.65${\pm}$4.50 seconds, respectively. Conclusions: The total time period and the type of HRT regimen had no influence on the severity of the dry eye symptoms in postmenopausal women. However, when compared to non-HRT patients, the degree of symptomatic manifestations showed statistically significant increases. Further observations and follow-up on these subjects should be pursued.
