목적: 본 연구는 지르코니아 도재 표면의 나노구조 알루미나 코팅이 지르코니아와 레진 시멘트와의 전단결합강도에 미치는 영향을 알아보고자 하였다. 재료 및 방법: 지르코니아 원판 80개를 표면처리방법(산화알루미늄 분사처리(A), 산화알루미늄 분사 후 Rocatec 처리(R), 연마 후 나노구조 알루미나 코팅(PC), 산화알루미늄 분사 후 나노구조 알루미나 코팅(AC))에 따라 4개의 군으로 나누었다. 알루미나 코팅은 질산 알루미늄을 가수분해시킨 용액에 침적 후 $900^{\circ}C$에서 열처리 하여 시행하였다. 지르코니아 표면 코팅은 주사전자 현미경을 이용하여 관찰하였다. 레진 블럭을 레진 시멘트를 이용하여 각 실험군의 지르코니아 표면에 합착하고 열순환처리 전, 후의 전단결합강도를 측정하였다. 결과: 알루미나 코팅을 한 지르코니아 표면은 균일하고 치밀한 나노구조 알루미나가 관찰되었다. PC, AC 군은 열순환처리 전과 후 모두 A와 R 군에 비해 현저하게 높은 전단결합 강도를 보였다. A, R 군은 열순환처리 후에 급격한 결합강도의 감소를 보였으나, PC와 AC군은 열순환처리에 의해 유의할만한 결합강도의 감소를 보이지 않았다. 결론: 지르코니아 표면에 나노구조 알루미나 코팅처리하는 것은 레진시멘트와의 결합강도를 증가시키는 방법이다.
The purpose of the present study is to evaluate the biological stability of the zirconia/alumina composite abutment by histologic and radiographic examination in clinical cases. 17 partially edentulous patients (5 men and 12 women, mean age 47) were treated with 37 implants. The implants were placed following the standard two-stage protocol. After a healing period of 3 to 6 months, zirconia/alumina composite abutments were connected. All radiographs were taken using paralleling technique with individually fabricated impression bite block, following insertion of the prosthesis and at the 3-, 6-, 12 month re-examinations. After processing the obtained images, the osseous level was calculated using the digital image in the mesial and distal aspect in each implant. An ANOVA and t-test were used to test for difference between the baseline and 3-, 6-, 12 months re-examinations, and for difference between maxilla and mandible. Differences at P <0.05 were considered statistically significant. For histologic examination, sample was obtained from the palatal gingiva which implant functioned for 12 months. Sections were examined under a light microscope under various magnifications. Clinically, no abutment fracture or crack as well as periimplantitis was observed during the period of study. The mean bone level reduction(${\pm}standard$ deviation) was 0.34 rom(${\pm}\;0.26$) at 3-months, 0.4 2mm(${\pm}\;0.30$) at 6-months, 0.62 mm(${\pm}\;0.28$) at 12-months respectively. No statistically significant difference was found between baseline and 3-, 6-, 12-months re-examinations (p > 0.05). The mean bone level reduction in maxilla was 0.33(${\pm}0.25$) at 3-months, 0.36(${\pm}0.33$) at 6-months, 0.56(${\pm}0.26$) at 12-months. And the mean bone level reduction in mandible was 0.35(${\pm}0.27$) at 3-months, 0,49(${\pm}0.27$) at 6-months, 0.68(${\pm}0.30$) at 12-months. No statistical difference in bone level reduction between implants placed in the maxilla and mandible. Histologically, the height of the junctional epithelium was about 2.09 mm. And the width was about 0.51 mm. Scattered fibroblasts and inflammatory cells, and dense collagen network with few vascular structures characterized the portion of connective tissue. The inflammatory cell infiltration was observed just beneath the apical end of junctional epithelium and the area of direct in contact with zirconia/alumina abutment. These results suggest the zirconia/alumina composite abutment can be used in variable intraoral condition, in posterior segment as well as anterior segment without adverse effects.
치과의사가 수복재료를 선택하고 환자에게 추천할 때는 수복할 치아 위치, 결손 정도, 환자의 심미적 요구도, 저작력, 나이, 경제적 여건 등 여러 가지의 요인을 고려한다. 그렇지만, 수복재료를 선택할 때의 일차적인 기준은 구강 내에서 기능 시 작용하는 교합력을 최우선으로 고려해야 한다. 미국 국립보건원(NIH) 지원으로 조사된 연구 결과에 의하면, 전치부 수복재료로서는 리튬 디실리케이트계 글라스-세라믹이 54%, layered zirconia가 17%, 루사이트 강화 글라스-세라믹이 13% 순을 보여, 전치부 수복재료로서 리튬 디실리케이트계 글라스-세라믹이 가장 많이 사용었다. 또한 구치부 수복재료로서는 단일구조 지르코니아가 32%, 금속-세라믹이 31%, 리튬 디실리케이트계 글라스-세라믹이 21% 순을 나타냈다. 세라믹 수복 재료의 특성을 살펴보면, 단일구조 지르코니아는 강도는 높지만 명도가 높고 저온열화로 인한 강도 저하가 일어날 수 있다. layered zirconia는 심미성은 우수하지만 강도가 낮은 비니어 세라믹의 칩핑과 박리가 문제가 되고 있다. 리튬 디실리케이트계 글라스-세라믹은 심미성은 우수하지만 지르코니아에 비해 강도가 낮으므로 구치부에 적용 시 파절이 일어날 위험성이 있다. 루사이트계 글라스-세라믹은 심미성은 우수하지만 기본적으로 강도가 낮기 때문에 전치부에 한정하여 적용한다. 세라믹 크라운이 구치부 교합력에 저항하기 위해서는 350 MPa 이상의 굴곡강도를 가져야 한다. 리튬 디실리케이트계 글라스-세라믹은 광투과성이 양호한 심미성이 있는 재료이므로 비니어 없이 전치부에 적용할 수 있고, 굴곡강도가 400 MPa 이상이므로 구치부 교합력에 저항할 수 있고, HF에 의한 산부식과 실란(silane) 처리가 가능하므로 레진과 강한 결합력을 얻을 수 있고, 또한 포세린에 비해 대합치의 마모가 적다. 이러한 점들을 고려하면 리튬 디실리케이트계 글라스-세라믹 재료는 전구치부의 단일치 수복에 적합한 성질을 갖고 있음을 알수 있다. 그렇지만 그의 임상 응용이 더욱 증가하기 위해서는 이들 재료에 대한 보다 많은 연구와 이해가 필요하리라고 생각된다.
지연성 운동이상증은 도파민 수용체 차단 약물의 장기간 사용으로 인해 발생하는 비자발적 신경학적 운동 장애로 불수의적인 하악의 움직임과 이갈이를 초래하여 과도한 하중이 치아 및 보철물에 위해를 가할 수 있다. 지연성 운동이상증 병력이 있는 40대 남성이 상악 전치부 상실로 인한 보철치료를 위해 내원하였다. #13, 15, 23의 지대치 형성 후 실리콘으로 인상을 채득하고 레진 블록을 밀링하여 임시 보철물을 제작하였다. 시적 과정에서 임시 보철물의 교합면 1/3을 제거하고 교합면에 자가중합 아크릴 레진을 도포하여 환자의 구강에 장착하였다. 레진이 경화되는 동안 하악의 불수의적 움직임으로 대합치에 의한 functionally generated path (FGP)가 레진 표면에서 기록되었다. 수정된 임시 보철물을 제거하여 구내 스캐너로 스캔 후 이중 스캔 방법을 사용하여 임시 보철물의 디자인을 수정하여 최종 보철물을 설계하였다. 최종 보철물은 단일구조 지르코니아로 제작하였다. 본 증례는 FGP 기술과 이중 스캔 방법 이 지연성 운동이상증 환자에서 조화로운 교합을 갖는 고정성 보철물의 제작하는 데 도움이 될 수 있음을 보여준다.
지르코니아는 다양한 공정과정을 거쳐서 제작되며, 각각의 요소는 최종 보철물의 물성에 영향을 줄 수 있다. 특히 밀링과정과 소결과정이 모두 지르코니아 보철물의 최종 완성도(integrity)에 영향을 미칠 수 있다. 밀링머신은 대부분 초정밀 5축 가공방식을 채택하고 있으며 어떤 방식을 사용하고 밀링기구를 어떻게 관리하는가에 따라서도 결과가 달라진다. 밀링블록은 절삭의 효율성과 심미재현성에 따라 선택하는데 물성의 변화를 야기할 수 있음은 주의해야 한다. 소결방식은 입자성장과 광학적 특성에 영향을 미칠 수 있는데 속도를 조절하는 최근의 방식에 대해서는 추가적인 연구가 동반되어야 정확한 평가가 이루어질 수 있다. 소결온도 뿐 아니라 온도 유지시간도 최종결과물에 영향을 줄 수 있다.
목적: 본 연구의 목적은 동일한 모형에서 제작된 지르코니아 single 코핑과 3-unit 구조물의 변연 및 내면 적합도를 평가하는 것이다. 연구 재료 및 방법: 상악 우측 제2소구치가 상실되고 상악 제1소구치와 제2대구치가 지대치로 형성된 에폭시 모형(epoxy model)을 제작한 후 레이저 스캐너를 이용하여 스캔 하였다. CAD 소프트웨어를 이용하여 지르코니아 구조물을 디자인하고 CAM을 사용하여 지르코니아 single 코핑과 3-unit 구조물을 제작 하였다. 지르코니아 single 코핑은 상악 제1소구치, 제2대구치에서 각각 10개씩 제작 되었고 지르코니아 3-unit 구조물도 10개가 제작 되었다. 적합도 측정을 위하여 제작된 실리콘 복제(silicone replica)를 4등분하고 각 면의 측정 위치(P1, P2, P3, P4, P5)에서 디지털 현미경을 사용하여 적합도를 측정하였다. 각 위치에서 측정된 불일치 값들은 변연 불일치(MG, P1), 축 불일치(AG, P2와 P3의 평균), 교합 불일치(OG, P4와 P5의 평균)의 세 범주로 분류하였다. 결과: Single 코핑에서 변연 불일치의 범위는 18.47 - 40.54 ${\mu}m$, 축 불일치의 범위는 39.73 - 73.61 ${\mu}m$, 교합 불일치의 범위는 116.90 - 134.69 ${\mu}m$이었다. 3-unit 구조물에서 변연 불일치의 범위는 45.95 - 87.44 ${\mu}m$이고, 축 불일치의 범위는 23.78 - 57.00 ${\mu}m$이고, 교합 불일치는 99.89 - 131.06 ${\mu}m$이었다. 결론: 본 연구의 결과에 따르면 single 코핑에 비해 3-unit 구조물의 변연 불일치가 증가하였지만 임상학적 수용 범위(- 120 ${\mu}m$) 내에 있었으며 3-unit 구조물의 불일치 증가를 방지하기 위해서 더욱 균질한 지르코니아 블록 사용과 소결공정들을 조절해야 할 것이다.
This work is to fabricate a precise optical fiber array using polymer composite for optical interconnection. Optical fiber array has to satisfy low optical loss requirement less than 0.4 dB according to temperature change. For this purpose, design criteria for an optical fiber array was derived. The coefficient of thermal expansion of silica particulate epoxy composites was affected by volume fraction of silica particles. And also, elastic modulus of silica particulate epoxy composites was affected by volume fraction of silica particles. To obtain the coefficients of thermal expansion below $10{\times}10E-6/^{\circ}C$ and elastic modulus more than 20 GPa , we chose the volume fraction more than 76%. Using silica particulate epoxy composites with the volume fraction 76%, 8-channel optical fiber array with dimensional tolerances below $1\;{\mu}m$ was manufactured by transfer molding technique using dies with the uniquely-designed core pin and precisely-machined zirconia ceramic V block. These optical fiber arrays showed optical loss variations within 0.4 dB under thermal cycling test and high temperature test.
연구 목적: 미세 인장결합강도 측정을 위한 시편 제작 시 발생하는 스트레스가 결합강도에 어떠한 영향을 미치는 지를 알아보고자 하였다. 연구 재료 및 방법: 원통 모양의 지르코니아 블럭을 다이아몬드 디스크를 이용해 디스크 형태로 자른 후 소결하였다. 소결된 지르코니아 디스크에 소성시 수축을 고려한 적당한 크기의 틀을 제작하여 LAVA ceram (3M ESPE, Seefeld, Germany) 도재를 축성한 후 소성하였다. 먼저 그룹 1은 디스크 모양의 시편을 충분히 물을 뿌려가며 다이아몬드 디스크를 이용하여 $1{\times}1{\times}7\;mm^3$의 막대기 모양으로 저속에서 절단하였다 (n = 15). 그룹 2는 같은 방법으로 $1.2{\times}1.2{\times}7\;mm^3$로 자른 후 두꺼운 다이아몬드 디스크를 이용해 $1{\times}1{\times}7\;mm^3$로 트리밍하였다 (n = 15). 그 후 특별히 제작된 지그에 각각의 시편을 core-veneer의 접착면이 오염되지 않도록 조심히 접착한 후 미세 인장결합강도 측정기 (Instron 8848, $Instron^{(R)}$ Co., Norwood, USA)를 이용하여 미세 인장결합강도를 측정하였다. 측정이 끝난 시편은 파절면을 stereomicroscope (MZ6, Leica Microsystems GmbH, Wetzlar, Germany) 로 30배 확대 관찰하여 파절의 종류를 구분하였다. 결과: 지르코니아 코어와 비니어링 세라믹 간의 평균 미세 인장결합강도는 절단만 시행한 그룹은 $28.8{\pm}7.0\;MPa$, 절단 후 트리밍을 한 그룹은 $11.0{\pm}33\;MPa$로 절단후 트리밍을 한 시편이 유의성 있게 결합강도가 낮아진 것으로 나타났다 (P=.00). 결론: 미세 인장결합강도 측정을 위한 시편 제작 시 가능한 한 스트레스를 가하지 않고 미세 시편을 제작하는 것이 중요하다고 사료된다.
Purpose: Fracture strength of all-ceramic 3-unit fixed partial dentures manufactured by CAD/CAM and copy-milling systems were evaluated. Methods: Zirconia cores were made by milling the pre-sintered zirconia block by CAD/CAM or copy milling method followed by subsequent sintering. By building-up the corresponding porcelains on the core, all-ceramic bridges were fabricated, and those were evaluated in comparison with PFM fixed partial denture. Results: During the flexural test of the 3-unit PFM bridge, the porcelain started to chip or break at 507.28(${\pm}62.82$)kgf and the metal framework did not break until the maximum load level of 800kgf which was set in the testing instrument of this study. However, among all-ceramic restoration test groups, Everest(EV) group showed a peeling off or breakage of the porcelain from 365.64(${\pm}64.96$)kgf and the core was broken at 491.77(${\pm}55.62$)kgf. Those values of Zirkonzahn(ZR) were 431.03(${\pm}58.47$)kgf and 602.74(${\pm}48.44$)kgf, respectively. The break strength of the porcelain of PFM(PM) group was significantly higher than that of EV (p<0.05) group and there was no significant difference when comparing to that of ZR (p>0.05). ZR group showed higher break strength than that of EV group however there was no significant difference (p>0.05). The break strength of cores were in the increasing order of EV < ZR < PM (p<0.05). Conclusion: We could find that even though the PM group fractured at much higher value than all-ceramic cores, the breakage values of the porcelain of PM group with crack formation or delamination, which will be regarded as clinical failure, was significantly higher than that of EV group and not significantly higher than that of ZR group at p-values of 0.05. The break strength of ZR group was higher than that of EV group at an insignificant level(p>0.05).
Recently, the need for esthetic results has increased the interest for all-ceramic crown prosthesis. Furthermore, the development of zirconium core via CAD/CAM system has allowed the all ceramic restorations to be applied to almost all fixed prosthesis situations. But, the increased strength has been reported to increase in proportion with the bond strength of cement, and recently, the tribochemical system which increases the bond strength through, silica coating and silanization has been introduced. The purpose of this study was to compare the $Rocatec^{TM}$ system and $CoJet^{TM}$ system with the traditional acid etching and silanization method of the irconium based ceramic. The surface character was observed via SEM(X2000), and the bond strength with the resin cement were measured. 50 In-Ceram Zirconia (Adens, Korea) discs were fabricated and embedded in resin, group 1 was treated with glass-bead blasting and cleaning, group 2 was treated with 20% HF for 10 minutes and silanized, group 3 was treated with the $Rocatec^{TM}$ system, and group 4 was treated with the $CoJet^{TM}$ system. Each group was comprised of 10 specimens. The specimens were cemented to a $3mm{\times}5mm$ resin block with Super-Bond C&B. The shear bond strength was measured with the $Instron^{(R)}$ 8871 at a crosshead speed of 0.5mm/min. The results were as follows. 1. According to SEM results, there were little difference between group 1 & group 2, but in group 3 and 4, silica coating was detected and there was increase in surface roughness. 2. The shear bond strength decreased in the order of group 3(46.28MPa), group 4(42.04MPa), group 2(31.56MPa), and group 1(27.46MPa). 3. There was significant differnce between group 1&2 and group 3&4(p<0.05). From the results above, it can be considered that the conventional method of acid etching and silane treatment cannot increase the bond strength with resin cements, and that by applying the tribochemical system of $Rocatec^{TM}$ system and $CoJet^{TM}$ system, we can achieve a stronger all ceramic restoration. Further studies on surface treatments to increase the bond strength are thought to be needed.

② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
