Role of Multiparametric Prostate Magnetic Resonance Imaging before Confirmatory Biopsy in Assessing the Risk of Prostate Cancer Progression during Active Surveillance

  • Objective: To evaluate the impact of multiparametric magnetic resonance imaging (mpMRI) before confirmatory prostate biopsy in patients under active surveillance (AS). Materials and Methods: This retrospective study included 170 patients with Gleason grade 6 prostate cancer initially enrolled in an AS program between 2011 and 2019. Prostate mpMRI was performed using a 1.5 tesla (T) magnetic resonance imaging system with a 16-channel phased-array body coil. The protocol included T1-weighted, T2-weighted, diffusion-weighted, and dynamic contrast-enhanced imaging sequences. Uroradiology reports generated by a specialist were based on prostate imaging-reporting and data system (PI-RADS) version 2. Univariate and multivariate analyses were performed based on regression models. Results: The reclassification rate at confirmatory biopsy was higher in patients with suspicious lesions on mpMRI (PI-RADS score ≥ 3) (n = 47) than in patients with non-suspicious mpMRIs (n = 61) and who did not undergo mpMRIs (n = 62) (66%, 26.2%, and 24.2%, respectively; p < 0.001). On multivariate analysis, presence of a suspicious mpMRI finding (PI-RADS score ≥ 3) was associated (adjusted odds ratio: 4.72) with the risk of reclassification at confirmatory biopsy after adjusting for the main variables (age, prostate-specific antigen density, number of positive cores, number of previous biopsies, and clinical stage). Presence of a suspicious mpMRI finding (adjusted hazard ratio: 2.62) was also associated with the risk of progression to active treatment during the follow-up. Conclusion: Inclusion of mpMRI before the confirmatory biopsy is useful to stratify the risk of reclassification during the biopsy as well as to evaluate the risk of progression to active treatment during follow-up.

The Effect of IAA on $Colocasia$ $esculenta$'s Growth and Morphogenesis (IAA가 토란의 생장 및 형태 발생에 미치는 영향)

  • Morphological and physiological differences of $Colocasia$ $esculenta$ were investigated in the cultivation of hydorphonic and soil culture. $C.$ $esculenta$ grown in Hoa.+IAA (indole-acetic acid) showed higher growth activity representing 9%, 32%, 38% and 60% than those of the cultivation of vermiculate, Hoagland solution, soil and water, respectively. In case of $F_v/F_m$ ratio experiments, the value $F_v/F_m$ of $C.$ $esculenta$ cultivated in the water showed 0.55 after 6 weeks. $F_v/F_m$ values of $C.$ $esculenta$ cultivated in Hoagland+IAA, vermiculate and soil were between 0.84 and 0.80 indicating $F_v/F_m$ values were about 45% higher than that of $C.$ $esculenta$ cultivated in the water. Diffusion resistance was 45~35% lower in $C.$ $esculenta$ grown in Hoa.+IAA solution than that of $C.$ $esculenta$ grown in water only after 5 and 6 weeks. Therefore, the high standing levels of the growth rate, fluorescence activity and transpiration rate were Hoa.+IAA, vermiculate, Hoagland, soil and water. The distinct morphological differences of $C.$ $esculenta$ cultivated in hydorphonic and soil culture were the appearance of the seed corm and root hair. The development of seed corm was well established in soil culture but the corm in hydorphonic was slowly hydrolyzed and then disappeared. The fibrous root systems of hydorphonic were very well distinguishable compared with that in soil culture. Outstanding results of this experiment were appeared in $C.$ $esculenta$ which was cultivated in the field provided with enough mineral nutrition, organic fertilizers and compound fertilizers. The most height taros were almost 2m and the numbers of seed corm were 30~40 after 7 months.

Characterization of an Isolate of Cucumber mosaic virus Isolated from Canna generalis Bailey (칸나에서 분리한 Cucumber mosaic virus의 특성)

  • An isolate of Cucumber mosaic virus(CMV), called as Can-CMV, was originally isolated from Canna generalis showing typical streak mosaic foliar symptoms, and its properties were investigated in this study. Whereas all known isolates of CMV could induce symptoms on their systemic hosts(four kinds of Nicotiana spp and a zucchini squash), Can-CMV induced no symptoms on its systemic hosts tested. Replication and movement of the virus on upper leaves as well as inoculated leaves-were confirmed by RT-PCR suggesting that Can-CMV could only infect systemically on N. benthamiana and N. glutinosa. Size of local lesions on the Can-CMV-inoculated leaves of Chenopodium amaranticolor was much smaller than that of Fny-CMV. Whereas Fny-CMV and LS-CMV could induce distinct necrotic local lesions on Vigna unguiculata 2 to 3 days postinoculation(dpi), chlorotic spots symptom was expressed by Can-CMV 4 to 5 dpi. Virus-specific 4 kinds of dsRNAs were isolated from leaves of N. benthamiana infected with Can-CMV, and these dsRNAs corresponded to the viral genomic RNAs and subgenomic RNAs and their patterns were indistinguishable to those of Fny-CMV and LS-CMV. By restriction mapping analysis of 950 bp of RT-PCR amplified products of coat protein gene of the virus as well as by serological analysis of gel diffusion test, Can-CMV belongs to a typical member of CMV subgroup IA. These results suggest that the Can-CMV isolated from C. generalis possesses unique pathological properties to understand further insight into the various interactions between virus and host.

A Feasibility Study on Thermal Energy Resource in Deep Ocean Water (해양심층수 에너지자원 이용 타당성 분석 연구)

  • Annual power consumption of our country is positioned in the upper percentile in the world, and because the proportion of fossil power generation is high, which ranks the 10th $CO_2$ emission country. In this regard, government has established and is implementing the National Energy Basic Plan to realize to get out of fossilization in energy supply while focusing on securing the technology for renewable energy as well as its commercialization in order to reduce greenhouse gas. Resource recovery technology for deep seawater thermal energy which is one of renewable energies is newly getting attention domestically as well as in overseas for securing resources and environmental improvement as a core technology for multilateral use of marine resources for low carbon and green growth. Economic feasibility analysis was conducted for the research and development as follows on the use of ocean thermal energy conversion and seawater air conditioning. First, in the case of power generation using deep seawater and warm discharge water from ocean thermal energy conversion plant of 1MW level, it is judged that the economic feasibility is insufficient but the feasibility will be significantly improved if we consider not only power generation but also drinking water and certified emission reduction by developing the power plant to the size for commercialization. Second, the economic feasibility for the use of deep seawater as air conditioning for the power plant of 1,000RT level turned out to be very good. Especially, when we consider certified emission reduction, it will be possible to secure sufficient economic feasibility. When we use it in connection with ocean thermal energy conversion, water conversion and agricultural and fishery use, it is judged that economic ripple effect will be significant and therefore it will be necessary to conduct research and development for early commercialization, distribution and diffusion of deep seawater energy.

Occurrence Characteristics of Uranium and Radon-222 in Groundwater at ○○ Village, Yongin Area (용인 ○○마을 지하수내 우라늄 및 라돈-222의 산출특성)

  • The occurrence of natural radioactive materials such as uranium and radon-222 in groundwater was examined with hydrogeochemistry and geology at ○○ village in the Yongin area. Two rounds of 19 groundwater and 5 surface water sampling were collected for analysis. The range of pH value in groundwaters was 5.81 to 7.79 and the geochemical types of the groundwater were mostly Ca(Na)-HCO3 and Ca(Na)-NO3(Cl)-HCO3. Uranium and radon-222 concentrations in the groundwater ranged from 0.06 to 411 μg/L and from 5.56 to 903 Bq/L, respectively. Two deep groundwaters used as common potable well-water sources exceeded the maximum contaminant levels of the uranium and radon-222 proposed by the United States Environmental Protection Agency (US EPA). Three groundwater samples from residential areas contained unsuitable levels of uranium, and 12 groundwater samples were unsuitable due to radon-222 concentrations. Radioactive materials in the unsuitable groundwater are naturally occurring in a Jurassic amphibole- and biotite-bearing granitic gneiss. High uranium and radon-222 groundwater concentrations were only observed in two common wells; the others showed no relationship between bedrock geology and groundwater geochemical constituents. With such high concentrations of naturally occurring radioactive materials in groundwater, the affected areas may extend tens of meters for uranium and even farther for radon-222. Therefore, we suggest the radon-222 and the uranium did not originate from the same source. Based on the distribution of radon-222 in the study area, zones of higher radon-222 concentrations may be the result of diffusion through cracks, joint, or faults. Surface radioactivity and uranium concentrations in the groundwater show a positive relationship, and the impact areas may extend for ~200m beyond the well in the case of wells containing high concentrations of uranium. The highest uranium and thorium concentrations in rock samples were detected in thorite and monazite.

A Study on the Effect of Improving Permeability by Injecting a Soil Remediation Agent in the In-situ Remediation Method Using Plasma Blasting, Pneumatic Fracturing, and Vacuum Suction Method (플라즈마 블라스팅, 공압파쇄, 진공추출이 활용된 지중 토양정화공법의 정화제 주입에 따른 투수성 개선 연구)

  • A stratum with a complex composition and a distributed low-permeability soil layer is difficult to remediate quickly because the soil remediation does not proceed easily. For efficient purification, the permeability should be improved and the soil remediation agent (H2O2) should be injected into the contaminated section to make sufficient contact with the TPH (Total petroleum hydrocarbons). This study analyzed a method for crack formation and effective delivery of the soil remediation agent based on pneumatic fracturing, plasma blasting, and vacuum suction (the PPV method) and compared its improvement effect relative to chemical oxidation. A demonstration test confirmed the effective delivery of the soil remediation agent to a site contaminated with TPH. The injection amount and injection time were monitored to calculate the delivery characteristics and the range of influence, and electrical resistivity surveying qualitatively confirmed changes in the underground environment. Permeability tests also evaluated and compared the permeability changes for each method. The amount of soil remediation agent injected was increased by about 4.74 to 7.48 times in the experimental group (PPV method) compared with the control group (chemical oxidation); the PPV method allowed injection rates per unit time (L/min) about 5.00 to 7.54 times quicker than the control method. Electrical resistivity measurements assessed that in the PPV method, the diffusion of H2O22 and other fluids to the surface soil layer reduced the low resistivity change ratio: the horizontal change ratio between the injection well and the extraction well decreased the resistivity by about 1.12 to 2.38 times. Quantitative evaluation of hydraulic conductivity at the end of the test found that the control group had 21.1% of the original hydraulic conductivity and the experimental group retained 81.3% of the initial value, close to the initial permeability coefficient. Calculated radii of influence based on the survey results showed that the results of the PPV method were improved by 220% on average compared with those of the control group.

FLUORIDE CONCENTRATION IN URINE EXCRETED AFTER FLUORIDE ADMINISTRATION (불소투여 후 배출된 요내 불소농도 변화에 관한 연구)

  • Part of the locally applied, as well as the systemic applied, fluoride is absorbed into the body to aid in the prevention of caries. However, beyond a certain level, systemic distribution of fluoride can cause chronic fluorosis with attending systemic symptoms and dental fluorosis. Thus it is vital to determine the level of fluoride with minimal side effects which will provide optimal caries prevention. A commonly utilized method of regressively determining fluoride intade is to measure the fluoride concentration of excreted urine. Thus, the aim of this study was to determine the clearance time and concentration of fluoride in urine after administration of various doses of fluoride using HMDS-diffusion technique and fluoride ion electrode(Orion, 96-09, U.S.A.). Urine samples were collected in 7 adult subjects every morning after administration of fluoride supplements such as no fluoride(control group), 1mg fluoride(group 1), 2mg fluoride(group 2), 3mg fluoride(group 3), 4mg fluoride(group 4). The obtained results were as follows 1. Mean urinary fluoride concentration of control group was $0.707{\pm}0.362ppm$. 2. Fluoride levels followed as group 4(4.076ppm). group 3(2.400ppm), group 2(1.494ppm), group 1(1.051ppm) at day 1 after fluoride administration. There were no statistical differences between the urinary fluoride concentration of group 1, 2, 3 and control group after day 2, but there was statistical difference between group 4 and control group at day 2(p<0.05). 3. Urinary fluoride concentration increased and plateaued according to increasing fluoride dosage. The increased concentration remained significantly higher till day 2, but after day 3, there was no significant difference compared to the control.

THE BOND CHARACTERISTICS OF PORCELAIN FUSED BY TITANIUM SURFACE MODIFICATION (타이타늄의 표면개질에 따른 도재 결합 특성)

  • Statement of problem: Titanium is well known as a proper metal for the dental restorations, because it has an excellent biocompatibility, resistance to corrosion, and mechanical property. However, adhesion between titanium and dental porcelains is related to the diffusion of oxygen to the reaction layers formed on cast-titanium surfaces during porcelain firing and those oxidized layers make the adhesion difficult to be formed. Many studies using mechanical, chemical and physical methods to enhance the titanium-ceramic adhesion have been actively performed. Purpose: This study meant to comparatively analyse the adhesion characteristics depending on different titanium surface coatings after coating the casts and wrought titanium surfaces with Au and TiN. Material and method: In this study, the titanium specimens (CP-Ti, Grade 2, Kobe still Co. Japan) were categorized into cast and wrought titanium. The wrought titanium was cast by using the MgO-based investment(Selevest CB, Selec). The cast and wrought titanium were treated with Au coating($ParaOne^{(R)}$., Gold Ion Sputter, Model PS-1200) and TiN coating(ATEC system, Korea) and the ultra low fusing dental porcelain was fused and fired onto the samples. Biaxial flection test was done on the fired samples and the porcelain was separated. The adhesion characteristics of porcelain and titanium after firing and the specimen surfaces before and after the porcelain fracture test were observed with SEM. The atomic percent of Si on all sample surfaces was comparatively analysed by EDS. In addition, the constituents of specimen surface layers after the porcelain fracture and the formed compound were evaluated by X-ray diffraction diagnosis. Result: The results of this study were obtained as follows : 1. The surface characteristics of cast and wrought titanium after surface treatment(Au, TiN, $Al_2O_3$ sandblasting) were similar and each cast and wrought titanium showed similar bonding characteristics. 2. Before and after the biaxial flection test, the highest atomic weight change of Si component was found in $Al_2O_3$ sandblasted wrought titanium(28.6at.% $\rightarrow$ 8.3at.%). On the other hand, the least change was seen in Au-Pd-In alloy(24.5at.% $\rightarrow$ 9.1at.%). 3. Much amount of Si components was uniformly distributed in Au and TiN coated titanium, but less amount of Si's was unevenly dispersed on Al2O3 sandblasting surfaces. 4. In X-ray diffraction diagnosis after porcelain debonding, we could see $Au_2Ti$ compound and TiN coating layers on Au and TiN coated surfaces and $TiO_2$, typical oxide of titanium, on all titanium surfaces. 5. Debonding of porcelain on cast and wrought titanium surface after the biaxial flection is considered as a result of adhesion deterioration between coating layers and titanium surfaces. We found that there are both adhesive failure and cohesive failure at the same time. Conclusion: These results showed that the titanium-ceramic adhesion could be improved by coating cast and wrought titanium surfaces with Au and TiN when making porcelain fused to metal crowns. In order to use porcelain fused to titanium clinically, it is considered that coating technique to enhance the bonding strength between coating kKlayers and titanium surfaces should be developed first.

Functional MRI of Visual cortex in the Patients with Occipital Lobe Ischemia (후두엽의 허혈성 뇌졸중 환자에서 시각피질의 기능적 자기공명영상)

  • Purpose : To evaluate the usefulness of functional MRI (fMRI) of visual cortex in patients with ischemic infarction in the occipital lobe. Materials and Methods : Four patients with the symptoms and signs of visual cortical ischemia were included. Functional MRI was performed by 2D-FLASH technique with the parameter of 90/56msec TR/TE, $40^{\circ}$ flip angle, $240{\times}240{\;}FOV,{\;}64{\times}128$ matrix number, 8.32 seconds acquisition time, 8mm slice thickness. An axial slice including both visual cortices was selected and alternative activation and resting of the visual cortex was performed using red color photostimulator. all patients undertook visual field test, and vascular abnormality was examined by MRA (n=4) and DSA (n=2). fMRI results were compared with the results of a visual field test, conventional MRI and cerebral angiography. Results : On fMRI, decreased activity of the visual cortex was found in the occipital lobe corresponding to stenosis of the posterior cerebral artery or its branch noted on angiogram. However, 2 of 4 patients showed no abnormal findings on conventional MRI. Visual field defect was noted in 3 patients, one and of whom showed no abnormality on conventional MRI and diffusion-weighted image, but revealed decreased activity in the corresponding visual cortex on fMRI. Conclusion : fMRI may be a sensitive method for detection of the status of decreased blood flow or vascular reserve which other methods can not.

Epidemiology and Clinical Features of Invasive Pneumococcal Infections in Children (소아 침습성 폐구균 감염증의 역학적 특성과 임상 양상)

  • Purpose : To characterize the epidemiology and clinical features of invasive pneumococcal infections in Korean children. Methods : One hundred ninety four cases of invasive pneumococcal infections diagnosed at the Seoul National University Children's Hospital from October 1985 to December 2003 were analysed retrospectively. All isolates were screened for resistance to penicillin by oxacillin disc diffusion test. Serotypes were determined for 125 isolates. Results : The types of infection were bacteremia without focus 84/194(43%), meningitis 36/194(19%), pneumonia with bacteremia 36/194(19%), peritonitis 24/194(12%), other focal infections 3/194(2%). Fifty seven percent(110/194) of the episodes developed in the immunocompromised and 20%(37/194) were nosocomially acquired. The patients younger than 2 years of age was 60% in the immunocompetent patients and 25% in the immunocompromised patients. The overall case fatality rate was 7%. All the isolates by 1988 were susceptible to penicillin screened by oxacillin disk. Penicillin resistance was first detected in 1989(20%), and then increased rapidly; 89% in 1995, 69% in 1996, and 80~100% thereafter. The seven most frequently isolated serotypes were 23F, 19F, 14, 6B, 6A, 9V and 19A, which accounted for 70% of total isolates. Conclusion : S. pneumoniaeis an important cause of morbidity and mortality in children. Invasive infections caused by S. pneumoniae most often occurred in infants and young children, while they are frequent in older immunocompromised children as well. This is the largest case series on invasive pneumococcal infections in Korean children.
