A study on the relationship between the symptom of Shanghanlun(傷寒論) and the Nutritive-Gi(營氣) (『상한론(傷寒論)』 병증(病症)과 영기(營氣)의 관계에 대한 연구(硏究))

  • Objectives : The practices of Wei-qi and Nutritive-qi are generally divided into external Mai and internal Mai. However, they are closely interrelated and practiced together. While taking these aspects into consideration, this paper attempts to make interpretations in relation with Nutritive-qi the disease pathogens that appear in Shanghanlun's disease symptoms. Methods : Using the practice and function of Nutritive-qi described in Huangdineijing, the paper shall make interpretations for the patterns of Mawhangtang, patterns of Gaejitang, and the pathologies of pain, oedema, and nosebleed as described in Shanghanlun. Results & Conclusions : The pain from the patterns of Gaejitang differ from that of the patterns of Mawhangtang. First, the pain from the the patterns of Gaejitang cannot be the main symptom. Even if there is a symptom of pain, it's severity is not serious. Second, the pain from the patterns of Gaejiang takes the form of stiffness, and not general bodily pain. The reason for this stiffness is because of the emptiness of Wei-qi that leads to the congestion of Nutritive-qi which in turn causes the lack of qi and blood flow in muscula area such as abdomins. The symptom of oedema where one's body becomes swollen comes from a number of pathogens. First, the flow of meridian becomes hindered due to external dampness, a character which tends to be adhesive when added with humidty, and this results in the blockage of water qi which then causes the coagulation of nutritive blood. Second, when toxic heat is repressed and blocked within the lesser-yang channel, lesser-yang meridian stops working, which causes nutritive blood to clog at the front and back of ears since lesser-yang channel flows through that portion of body. Third, although oedema is not specifically mentioned in the sentences, but there exists the patterns of Daechungyongtang where water lumps are formed due to the accumulation and blockage of watery dampness. The patterns of Daechungyongtang is cuased when meridian is hindered from externally discharging body fluid due to a problem with meridian that blocks the fumigated internal heat which turns into bodily fluid from being discharged externally.

  • The aim of this study is to report the improvement after the Oriental medical treatment about a paraplegic patient caused by spinal cord injury. The paraplegia caused by spinal cord injury was the result of falling down. So we diagnosed it as Eohyeol(瘀血), Wei symptom(痿證), Urinary retention(癃閉) in Oriental medical system and applied herbal medicine, acupuncture, moxibustion, physical exercise to the patient for 42days. We evaluate the clinical effect of the treatment with VAS and motor/sensory function score of the body and lower extremities. After the Oriental medical treatment, we achieved the effective result on impairment in motor and sensory function of the paraplegic patient. And also we got the improvement of urinary disorder and pain. The more clinical study about paraplegic patient caused by spinal cord injury may be needed.

  • Guillain-Barre syndrome, or acute inflammatory polyneuritis, is a disorder in which the body's immune system attacks parts of the peripheral nervous system. The causes and mechanisms of this syndrome are unknown. Typically, Guillain-Barre syndrome can be diagnosed from the patient's symptoms and physical examination such as the rapid onset of weakness, paralysis and loss of reflexes. The analysis of CSF and electrical tests on nerve and muscle function can be performed to confirm the diagnosis. Most cases occur shortly after a viral infection. This is a clinical report about one patient suspected as having Guillain-Barre syndrome. The patient, a 62-year-old man had weakness in both legs after gastroduodenal disease. His weakness and general condition improved after Korean medical treatments, so this is reported as a potential treatment.

  • Objectives Seborrheic dermatitis is a chronic superficial eczematous dermatitis characterized by the redness and dry or greasy squama. This case study was designed to assess the effect of Shi Wei Ba Du-San gamibang (Jeongan-tang) for seborrheic dermatitis by retrospective methods. Methods From June 1, 2016 to January 31, 2020, 5 patients with seborrheic dermatitis were treated with Jeongan-tang for 3 months at Oriental Medicine Clinic. We analyzed information and body conditions of 5 patients. Redness, squama, and skin discomfort such as itching were assessed in 5 patients by using VAS (Visual Analogue Scale) score. We evaluated the symptom change by photographs and VAS score before and after the treatment. Results At first, the frequency of flushing and redness was decreased. Secondly, the scaly skin was change to normal state and itching sensation was lessened. After the treatment, the total VAS score was decreased. Conclusions Jeongan-tang can be effective in seborrheic dermatitis, and it also can be helpful to regenerate the skin condition as well.

  • Amyotrophic Lateral Sclerosis(ALS) is Motor Neuron Disease(MND) that reveals muscle relaxation, bulbar palsy, extremities weakness, Pneumonia, in severe case, leads to death. Objectives : Amyotrophic Lateral Sclerosis is one of the incurable disease. In Oriental medicine, Wei symptom is similar as Amyotrophic Lateral Sclerosis, so we diagnosed it as Wei symptom and treated in Oriental medical system. Methods : The patient was treated by acupunture, moxibustion, herb medication, physical treatment. The improvement of the patient was judged by Amyotrophic Lateral Sclerosis Functional Rating Scale (ALSFRS), Grasping power on right arm, circumference of thigh and calf. Result : The patient had better condition for a while but the sputum irritated breathing and the day before he discharged vital sign was not stable and could not breath well. Conclusion : It is necessary to have more examination about the incurable syndromes such as Amyotrophic Lateral Sclerosis, and keep the patient's life better and expand their lives.

Patient and Clinical Variables Account for Changes in Health-related Quality of Life and Symptom Burden as Treatment Outcomes in Colorectal Cancer: A Longitudinal Study

  • Objective: The aim of the current study was to evaluate changes in treatment outcomes in terms of health-related quality of life (HRQoL) and symptom burden at zero, one, three, and six months after an initial diagnosis of colorectal cancer. The demographic and clinical characteristics that account for outcome changes in patients were investigated using a repeated measures framework. Methods and Materials: A cohort study was performed of 134 colorectal cancer patients followed from diagnosis to 6 months post-treatment in Central Taiwan. HRQoL and symptoms were assessed at diagnosis and one, three, and six months thereafter. The Functional Assessment of Cancer Therapy-Colon (FACT-C) questionnaire, VAS pain, and the Memorial Symptom Assessment Scale (MSAS) were used for data collection. A generalized estimating equation (GEE) was applied for statistical analysis. Results: The majority of the patients were male (55%) and married (91.5%). The mean age was 60.4 years (SD = 11.71). Most were diagnosed stage III and IV colorectal cancer (54.5%). All underwent surgery; some also received chemotherapy (CT) or concurrent chemoradiation therapy (CCRT). The results of the GEE showed that overall, the HRQoL, pain, and symptoms of the patients significantly improved over the treatment period. Patients with stage IV disease who had received surgery and CCRT showed the worst HRQoL. Females, patients with comorbidity, and stage IV patients had higher pain scores over time. Female and stage IV patients had more severe physical symptoms, whereas stage II and IV patients had worse psychological symptoms over time. Conclusion: The patients' HRQoL, pain, and symptoms significantly improved over the 6-month treatment period. Certain patient and clinical variables accounted for changes in treatment outcomes regarding HRQoL and symptom burden in colorectal cancer patients.

  • Objective : Recently, Seguele of Traffic Accident patients are increasing, especially cervical injury bring only severe sequela but also mortal damage to the patient. In treatment of the disease, the east medical treatment is another effective method. Methods : The patient resisted the west theraphy improved greatly in movement and sensation by acupuncture, moxibustion, herb medication and Physical treatment. In MRI, The treatment result is proved. Acupuncture on Stomach Meridian(足陽明胃經) referring to Naijing(內經) is thought effective. Results : Tetraplegia by Traffic Accident is considered as Wei Symptom in term of the east medicine, By means of radial and physical study of the patient treated by the east medicine, we are conviced that the east medicine is the excellent method Conclusions : Referring to the past documentary data, The only Treatment on this disease is Surgical operation, but the operation isn't the successful one because seg. of spinal cord injury is mortal. The east treatment, acupuncture, moxibustion herb medication and physical treatment isn't only effective but also satisfactory. In the future, The study on this disease by the east medicine should be made thoroughly.

  • Objectives : The purpose of this study was to evaluate Oriental Medicine Therapy on the Lumbosacral plexitis patient. Methods : The patient was treated with dry needle acupuncture, bee venom pharmacupuncture, herb medicine, indirect moxibustion and physical therapy. We estimated S kinds of Symptoms by Visual Analog Scale, lower limb motor grade test, gait condition test and sensory examination to evaluate the effect of Oriental conservative treatment. Results : After treatment, patient's visual analogue scale score were generally decreased 10 to 4 and lower limb muscle power and gait condition were improved. Conclusions : Oriental Medicine Therapy might be an effective method to treat the Lumbosacral plexitis patient who was diagnosed as Wei Symptom. It is necessary to have more observation and many cases of patients with Lumbosacral plexitis.

  • Multiple Myeloma is malignant tumor that malignant proliferous plasma cell to originate from bone marrow invades bone multiply. Objectives : Therapy for Multiple Myeloma includes chemotherapy, radiation therapy and self-stem cell transplantation, but it has no effect for a majority of Multiple Myeloma patients. So we diagnosed it as Wei symptom, oliguria, or dysuria(遺尿) in Oriental medicine, and treated it using the Oriental medical system. Methods : The patient was treated using acupuncture, electroacupuncture, herbal acupuncture treatment, moxibustion, physical treatment and western medicine. We observed 12 kinds of symptoms in the patient when admitted to the hospital. Results : 1. Paraplegia, urination desire, voluntary urination, and other symptoms improved except for a period of complication. 2. Defecation desire, sensory disturbance of the body and lower extremities, self-made changes, maintenance of body posture, and other symptoms improved during admitting days. 3. Voluntary defecation, pains of the neck and lower extremities, and other symptoms had irregular changes during admitting days. Conclusions : This study demonstrates the necessity of having more clinical study about Mutiple Myeloma.

The Study on the Eastern and Western Medical Literatures for Complex Regional Pain Syndrome (복합부위통증증후군에 대한 동서의학적 고찰)

  • Objectives : The aim of this study is to suggest approach of oriental medical management and necessity by research of eastern and western medical literatures for Complex Regional Pain Syndrome(CRPS). Methods : We reviewed the clinical and experimental literatures of eastern and western concerned with CRPS which is related causation, signs and symptoms, diagnosis, etiology and management. Results : 1. CRPS is divided into type I and II by nerve injury. Two types of CRPS have been recognized: type I, corresponds to RSD and occurs without a definable nerve lesion, and type II, formerly called causalgia refers to cases where a definable nerve lesion is present. These conditions can be charaterized clinically by the sensory abnormalities, vascular abnormalities, oedema, sweating abnormalities, motor or trophic changes. 2. CRPS are well known to patients and physicians relatively, but the pathophysiology, causation and treatments are still unclear. 3. CRPS is needed to take the early diagnosis and multidisciplinary approach for significant effect. 4. CRPS can be regarded for obstruction syndrome of Ki and blood(痺證), blood stasis(瘀血), Wei symptom(痿證), numbness(痲木) in the oriental medical management of CRPS. Conclusions : Above the results, it is suggested that further studies and active approach of management of CRPS will be conducted precisely in oriental medicine.
