Otolaryngologic Management related with Cleft Lip & Palate (구순구개열 환자의 이비인후과적 관리)

  • Team approach for the management of cleft lip & palate patients is very important. Plastic surgeon, oral-maxillofacial surgeon, orthodontist, otolaryngologist, and speech therapist should be included in the team. Main role of the ENT surgeon may be variable and is up to the team characteristics. Main topics of ENT surgeons' interesting fields are evaluation and management of hearing impairment due to SOM, voice disorder, and velopharyngeal incompetency due to submucous cleft palate & still remained VPI after curative palatoplasty. Basic review of anatomy & physiology related with otolaryngologic aspect of velopharyngeal system was done. Diseases related with hyponasality as well as hypernasality were discussed. Diagnostic and therapeutic methods were discussed. Proper management of hearing impairment and speech disorders are important.

Acoustic and Stroboscopic Characteristics in Teachers, Clergies and Telephone Operators (교사, 목사 및 교환수들의 음성발성에 대한 음향분석학적 특징)

  • Objectives : To compare the voice quality and voice problems of untrained professional voice user groups with that of normal control group without voice problem. Materials and Methods : The sustained vowel sounds of 13 male and 36 female teachers, 46 clergies and 15 telephone operators, and 40 normal male and 20 normal female persons were analyzed, using a videostroboscopy and acoustic analyzer. Together with these analyses, a questionnaire associated with risk factors for current and past voice problems was handed over to the patients. Results : The most common symptom in subjective groups was the voice fatigue. In stroboscopic examination, the professional voice user groups shelved functional voice disorder findings regardless of the Intensity of voice use. In the clergy and teacher using loud voice, vocal polyp, vocal nodule and hyperfunction of laryngeal muscle were frequently observed. In the clergy and telephone operator, jitter and shimmer were significantly increased. In the female teacher, the value of jitter, fundamental frequency variation and fundamental frequency were statiscally significant. However, the voice of male teacher showed no significant findings in the acoustic and aerodynamic studies. Conclusion : In the management of voice problems for untrained professional voice user groups, it is important to find the exact causes and patterns of voice problems, and to be individualized the management according to the causes.

  • Voice therapy is effective in many voice and speech disorders. However, patients have low accessibility to therapeutic facilities due to disease-unrelated reasons such as lack of time and pandemic of COVID-19. Contactless voice therapy could be an alternative and may helpful to all patients with voice and speech problems. We developed contactless voice and speech therapy program on the necessity of improving accessibility. Herein, we report the first case of voice therapy to 30 year-old female patient who complained voice change after total thyroidectomy using contactless voice and speech therapy service platform in Korea.

Comparison of Acoustic Parameters According to the Section of Analysis in Sustained Vowel Phonation (모음연장 음성 샘플의 분석 구간에 따른 음향학적 파라미터 비교)

  • This study aimed to investigate the acoustic differences that occur in diverse sections of sustained vowel phonation, which is often used in an objective speech analysis of voice disorder patients. The subjects included 17 voice disorder patients (vocal nodules) and 12 normal individuals without any voice disorder. The participants' sustained vowel phonation of /a/ was divided into onset, middle, and offset, and the jitter, shimmer, and NHR in each section were analyzed using the MDVP(Multi-Dimensional Voice Program). The Friedman test and post hoc analysis were used. In the vocal nodules group, the jitter, shimmer and NHR were significantly higher in the off section of sustained vowel phonation than in the middle section, and there were no significant differences between the beginning and middle sections. In contrast, in the group of normal individuals, there were no significant differences between any of the sections. The values of the acoustic parameters according to the section of analysis in the sustained vowel phonation are different and the vocal in the end section is significantly more unstable than that in the middle section. The results of this study will be useful for selecting the sections to be analyzed in sustained vowel phonation and interpreting the results of the analysis.

The Trend of the Association Between Amount of Smoking and Self-reported Voice Problem (흡연량과 본인 인지 음성장애의 관련성에 관한 경향성 분석)

  • The South Korea has the highest overall smoking rates of male among the thirty member-countries in the Organization for Economic Cooperation and Development. This study was performed to determine the association between amount of smoking and self-reported voice problem in Korean adult population using national survey data. Data were drawn from the Korea National Health and Nutritional Examination Survey 2008. Subjects were 3,600 persons (1,501 male, 2,099 female) aged 19 years and older living in the community. Adjusting for covariates (age, sex, education, income, longest occupation, alcohol drinking, pain and discomfort during the last 2 weeks), Severe smoke (>40.5-55.5 pack year), more than 20 cigarettes per day were independent risk factors for self-reported voice problem. It was found that the number of cigarettes smoked per day (CPD) the proportional increased in risk of voice disorders. These findings suggest that the investigation of CPD are required for effective screening of voice disorder.

  • The mucosal bridge of the vocal fold is an arch of mucosa, formed by some portion of the vocal fold mucosa which is detached in various extension and length. The etiology is uncertain but some is related to vocal sulcus. Because it affects the voice with variable impact, it must be differentiated from functional voice disorder. We report a case of a mucosal bridge of vocal fold with a vocal polyp treated by microlaryngeal surgery and voice therapy.

Acoustic Analysis of Voice Change According to Extent of Thyroidectomy (갑상선 수술범위에 따른 음성의 음향적 분석)

  • Voice complication without the laryngeal nerve injury can occur after thyroidectomy. The purpose of this study is to investigate voice changes according to extent of thyroidectomy with acoustic analysis. Thirty-five female patients with papillary thyroid carcinoma took voice evaluation at before and 1 month, and 3 months after thyroidectomy. Acoustic analysis parameters were speaking fundamental frequency(SFF), min $F_0$, max $F_0$, dynamic range $F_0$, jitter, shimmer, noise-to-harmonic ratio(NHR), and Cepstral prominence peak(CPP). Repeated-measured analysis of variance was applied. Time-related voice changes showed significant differences in all parameters except NHR. At 1 month after surgery, voice quality was worse and pitch was decreasing, but voice quality and pitch were improving at 3-month follow-up. Voice changes according to the extent of surgery were in SFF, max $F_0$, and dynamic range $F_0$. Time by surgery-related voice change existed only in min $F_0$. The result showed that the severity of voice complication depended on the extend of thyroidectomy which had a negative impact on $F_0$-related parameters. The deterioration of voice quality at 1 month after thyroidectomy may be affected by the loss of thyroid hormone in the blood. The descent of $F_0$-related parameters may be impacted by laryngeal fixation of surgical site adhesion.

  • Human voice reacts very sensitively to human's minute physical condition. For instance, human voice disorders affect patients profoundly especially in the case of Parkinson's disease. Acoustic tools such as MDVP, can function as an equipment that measures various voice in different objects. Many different approaches have been applied for analyzing the voice disorders for diagnosis of Parkinson's disease. According to the voice data of suspected Parkinson's patients from UCI Machine Learning Repository, it is reported to have 23 people with Parkinson's disease and 8 healthy people. Applying Mahalanobis Taguchi System (MTS) for diagnosis of Parkinson's disease, the correct diagnosis performance is compared to previous research results.

  • Functional aphonia is a disease in which normal vocal ability is suddenly lost. When voice therapy is started at an early stage, the prognosis is good. However, if the functional aphonia persists for a long time, there is a possibility that the voice disorder may become fixed, though reports of these characteristics are rare. The authors experienced a patient with functional aphonia that occurred in adolescence and lasted for 7 months and reported the result of treatment.
