Assessment of vitamin $B_6$ status in Korean patients with newly diagnosed type 2 diabetes

  • The purpose of this study was to assess vitamin $B_6$ intake and status in Korean patients with newly diagnosed type 2 diabetes. Sixty-four patients with newly diagnosed type 2 diabetes and 8-11% glycated hemoglobin (A1C), along with 28 age-matched non-diabetic subjects, participated. Dietary vitamin $B_6$ intake was estimated by the 24 hour recall method and plasma pyridoxal 5'-phosphate (PLP) was measured. There was a significant difference in daily total calorie intake between the diabetic and non-diabetic groups ($1,917{\pm}376$ vs $2,093{\pm}311\;kcal$). There were no differences in intake of total vitamin $B_6$ ($2.51{\pm}0.91$ vs $2.53{\pm}0.81\;mg/d$) or vitamin $B_6$/1,000 kcal ($1.31{\pm}0.42$ vs $1.20{\pm}0.32\;mg$) between the diabetic and non-diabetic groups, and I intakes of total vitamin $B_6$ were above the Korean RDA in both groups ($180.0{\pm}57.9$ vs $179.0{\pm}65.4$). There was a higher percentage of diabetic subjects whose plasma PLP concentration was < 30 nmol/L compared to non-diabetic group. Plasma PLP levels tended to be lower in the diabetic subjects than in the non-diabetic subjects, although the difference was not statistically significant due to a large standard deviation ($80.0{\pm}61.2\;nmol/L$ vs $68.2{\pm}38.5\;nmol/L$). Nevertheless, plasma PLP levels should be monitored in pre-diabetic patients with diabetic risk factors as well as in newly diagnosed diabetic patients for long-term management of diabetes, even though this factor is not a major risk factor that contributes to the development of degenerative complications in certain patients.

  • This study investigated the effect of vitamin $B_6$ deficiency on antioxidant enzyme activities and lipid profile in rats with exercise-induced oxidative stress. Forty eight rats were fed either a vitamin $B_6$ deficient diet (B6-) or a control diet (control) for 4 weeks and then subdivided into 3 groups: pre-exercise (PreE); post-exercise (PostE); recess after exercise (recessE). Compared to those of control group, plasma catalase and hepatic cytosol superoxide dismutase (SOD, EC 1.15.1.1) activities of B6- group were lower regardless of exercise. The ratio of reduced glutathione/oxidized glutathione (GSH/GSSG) of B6 - group was lower in PreE and there was no difference between PostE and recessE. The level of malondialdehyde (MDA) of B6- was significantly higher in PreE and PostE. High-density lipoprotein-cholesterol (HDL-C) level of B6- group was lower regardless of exercise. Atherosclerotic index of $B_6$- group was higher in PreE and there was no difference between PostE and recessE. It is suggested that a reduction in antioxidative status caused by vitamin $B_6$ deficiency may be aggravated under exercise-induced oxidative stress.

High Intensity Exercise Induced a Redistribution of Pyridoxal 5-Phosphate Levels with Different Vitamin $B_6$ Status in Rats

  • The purpose of this study was to compare the changes in PLP concentrations induced by regular, moderate, and abrupt, high-intensity exercise in the plasma and tissues of vitamin B6 deficient and normal rats. Forty-eight rats were fed either a vitamin B6 deficient (-B6) diet or a normal (+B6) diet for 5 weeks and were subdivided into 4 groups:non-exercise(NE) group: regular, moderate-intensity exercise (RME) group; abrupt, high-intensity exercise (AIE) group; abrupt, high-intensity exercise and recuperation(IRE) group. The RME group was exercised on treadmill ($10^{\circ}$, 0.5-0.8km/h) for 2 hours just before sacrifice at the end of 5th week on the diet and the IER group was recuperated for three days on the diet after being exercised like the AIE group. Pyridoxal 5 -phosphate(PLP) levels were compared in the plasma, liver and skeletal muscle of the rats. Plasma PLP concentration tended to decrease in -B6 rats and tended to increase in +B6 rats with AIE. Plasma PLP concentration in both +B6 rats with AIE and no change in both -B6 and +B6 rats with RME. Muscle PLP concentration decreased in +B6 rats, showed no change in -B6 rats with AIE. Muscle PLP concentrations in both +B6 and -B6 rats did not change with RME. Plasma PLP, liver PLP and muscle PLP concentration of IER returned to those of NE in both +B6 and -B6 rats. These results suggest that changes in PLP concentration in plasma, liver and muscle occur with exercise and are affected by exercise intensity and vitamin B6 status. These changes may be due to interorgan redistribution of PLP.

Serum Vitamin B(sub)12 Levels of Maternal-Umbilical Cord Blood and Pregnancy Outcomes (임신말 모체 및 제대혈의 비타민 B(sub)12 농도와 임신결과와의 상관성)

  • Vitamin B(sub)12(cobalamin) is an essential nutrient in human and it is particularly important during pregnancy. Nevertheless very few studies have reported, concerning vitamin B(sub)12 in relation with reproduction. This study was conducted to evaluate the vitamin B(sub)12 nutrition status of Korean pregnant women and to investigate the relationship between serum vitamin B(sub)12 levels of maternal-umbilical cord blood and pregnancy outcomes. Dietary vitamin B(sub)12 intakes of the pregnants were estimated by semiquantitative frequency questionnaire. Serum vitamin B(sub)12 levels in both maternal blood and umbilical cord blood of 30 pregnant women at delivery were measured by radioimmunoassay. Mean vitamin B(sub)12 intake was 3.3$\pm$1.4$\mu\textrm{g}$/d which was 125.8% of the Korean RDA(2.6$\mu\textrm{g}$) for vitamin B(sub)12 level of umbilical cord blood was 607.8$\pm$282.9pg/ml, more than two fold of maternal vitamin B(sub)12 level 268.6$\pm$97.8pg/ml. This finding indicates that fetal uptake of vitamin B(sub)12 in the fetus may be due to an active transport mchanism across the placenta. Umbilical cord blood vitamin B(sub)12 levels were highly correlated with maternal levels($r^2$=0.548, p<0.001), showing that fetal vitamin B(sub)12 level is affected by maternal status. However there was no significant correlation between the serum vitamin B(sub)12 levels in maternal-umbilical cord blood and the pregnancy outcomes except for the birth weight. Maternal-umbilical serum vitamin B(sub)12 levels were the highest in the group of birth weight 3.0-3.5kg, and the lowest in the group of birthweight below 3.0kg. Biochemical Evaluation of Nutritional Status of Vitamins and Minerals in Patients with Alcoholic Liver Disease (생화학적 지표로 본 알코올성 간질환 환자의 비타민 및 무기질 영양상태)

  • The purpose of this study is to evaluate the nutritional status of vitamin and minerals in the patients with alcoholic liver disease and to obtain the materials for nutritional education for alcoholics. The subjects consist of 80 patients with alcoholic liver cirrhosis(ALC) and 12 patients with alcoholic fatty liver(AFL). The control group included 57 alcoholics without liver disease(A), 32 patients with viral liver cirrhosis(VLC) and 194 normal men(NL). Biochemical evaluation of nutritional status was investigated by the analysis of blood samples. The frequency of vitamin B1 deficiency in the ALC, AFL and A groups as indicated by the erythrocyte transketolase activity coefficient were 46.0%, 66.7% and 57.9% respectively. The frequency of vitamin B2 deficiency in the ALC, AFL and A groups as indicated by the erythrocyte glutathione reductase activity coefficient were 9.8%, 8.3% and 38.6% respectively. Vitamin A deficiency was not detected in the alcoholic subjects. The frequency of vi tamin E deficiency in ALC, AFL and A were 96.3%, 66.7% and 86.0% respectively. The levels of plasma lipid peroxidation products were significantly higher in the alcoholic subjects than in the normal subjects. The frequency of subjects below normal range of hemoglobin were 85.0% in ALC, 50.0% in AFL and 31.6% in A. The frequency of copper deficiency in the ALC, AFL and A groups were 48.4%, 16.7% and 17.5% respectively. The frequency of zinc deficiency in the ALC, AFL and A groups were 83.8%, 41.7% and 66.7% respectively. Overall, the vitamin and minerals status of the alcoholic subjects in this study was evaluated to be very poor on the basis of biochemical assessments. The results suggest that alcohol abuse and poor dietary intake could cause malnutrition and may be important risk factors in causing alcoholic liver disease in alcoholics. In addition, vitamin B1, vitamin B2, Cu, Fe and antioxidant supplementation may be effective in nutritional therapy for chronic alcoholics.

Dietary source of vitamin $B_{12}$ intake and vitamin $B_{12}$ status in female elderly Koreans aged 85 and older living in rural area

  • Recently, we found and analyzed vitamin $B_{12}$ in some Korean traditional plant foods which had not reported, yet. This study was to investigate vitamin $B_{12}$ intake and its dietary sources and the vitamin $B_{12}$ status in the very old elderly Koreans. We measured serum vitamin $B_{12}$ level and estimated the amounts of vitamin $B_{12}$ intake from different dietary sources in female elderly Koreans aged 85 and over who had consumed a relatively low animal traditional diet for the whole life. The average age of the subjects (n = 127) was 98.0 years (85-108 years). The assessment on energy and nutrient intake involved a one-day 24-hour recall, and serum vitamin $B_{12}$ concentration was measured by radioimmunoassay. Overall diet pattern was not different between the 85-99 yr-old group and centenarians, except centenarians were taking more dairy product. The average ratio of plant food to animal food consumption was 87.5:12.5 in weight. The average vitamin $B_{12}$ intake of our subjects was 3.2 ${\mu}g$, and 52.7% of subjects consumed under estimated average requirement, 2.0 ${\mu}g$/day. On dietary source, 67.3% of dietary vitamin $B_{12}$ was from meat, eggs and fishes and 30.6% was from plant foods, such as soybean-fermented foods, seaweeds, and kimchi. The average serum vitamin $B_{12}$ concentration was 450.5 pg/mL, and low serum vitamin $B_{12}$ (< 200 pg/mL) was found in 9.6% of subjects. Dietary vitamin $B_{12}$ intake was significantly lower in subjects with low serum vitamin $B_{12}$ (0.79 ${\mu}g$/day) than those with normal serum vitamin $B_{12}$ (3.47 ${\mu}g$/day). There were no significant difference in vitamin $B_{12}$ intake and its dietary sources and serum vitamin $B_{12}$ level between the 85-99 yr- old group and centenarians. In conclusion, several plant-origin foods including seaweed, soybean-fermented foods, and kimchi, may contribute significantly to good vitamin $B_{12}$ status in very old elderly Koreans.

Effect of in vitro B-6 Vitameric Forms on Lymphocyte Proliferation in Healthy Young Women with Oral Vitamin B-6 Supplementation

  • A vitamin B-6 (B-6) intake higher than the current Recommended Dietary Allowance (RDA) has been found to provide an improvement in immune system. Seven premenopausal women consumed their usual diet with the exception of foods relatively high in vitamin B-6 for a total of 27 d. After 7 d, all subjects received a multivitamin supplement containing 2mg B-6 and 4 subjects were given an additional 50mg of B-6 supplement for 20 d. Lymphocyte response to phytohemagglutinin (PHA) was measured before and after the supplementation. To determine the effect of different forms of B-6 on lymphocyte proliferation, cell culture media supplemented with pyridoxal (PL) and PLP, as well as B-6 free media, were tested. A 50mg B-6 supplement significantly increased vitamin B-6 status. There was no further enhancement on lymphocyte proliferation when subjects were taking an additional 50mg of vitamin B-6 supplement. In general, lymphocyte proliferation in media with either PLP or PL did not show any prominent difference. These [m-dings suggest that there may be no further benefits of a B-6 dose beyond twice that of the current RDA on lymphocyte proliferation. Further studies are necessary to examine the effect of the B-6 intake level on activities of enzymes involved in cellular B-6 metabolism in lymphocytes to provide substantial insight into the mechanisms underlying the role of B-6 in the lymphocyte proliferation.

An Effect of Children's Food Alttitude on Nutritional Status and Personality (어린이의 식생활태도가 영양 상태 및 성격에 미치는 영향에 관한 연구)

  • This survey of 527 children, aged 4 to 6 years, focused on family and dietary environment, nutritional status and personality. It was conducted from June to October, 1986. The results showed that 68.0%(breakfast)and 55.4% (evening meal ) of the subjects indicated they had meals with the family everyday. Analysis of the nutritional status children showed all nutrient intake except protein an diron was insufficient compared with RDA. Carbohydrate provided 58.3% of total calorie intake ; protein accounted for 15.7% . fat provided 26.0%. There was a positive relation between income and nutrient intake. Children with employed mothers showed lower nutrient intake than children with unemployed mothers. Food attitude was positively related to nutrient intake, nutrient intake of children with good eating behavior was higher than that of children had poor eating behavior . Activity level had a significant correlation with intake of calories, fat, vitamin A, vitamin B$_1$, and Vitamin B$_2$. Mood was correlated with intake of protein, iron, vitamin A, vitamin B$_2$ and vitamin C.

Effects of Maternal 5, 10-Methylenetetrahydrofolate Reductase (MTHFR) Genotypes, Serum Homocysteine and B Vitamin Levels on Postnatal Growth in Their Offsprings (임신부의 MTHFR 유전자형, 호모시스테인 및 비타민 B군 영양상태가 영아 성장에 미치는 영향)

  • Elevated maternal plasma homocysteine concentrations have been associated with adverse pregnancy outcomes. Serum homocysteine levels may be affected by the MTHFR genotypes and the nutritional status of B vitamins including vitamin $B_2,\;B_6$, folate and vitamin $B_{12}$. We investigated whether postnatal growth measurements were influenced by maternal MTHFR genotypes and their mid-pregnancy serum vitamin B and homocysteine levels. In 130 pregnant women of 24-28 wks of gestation, the MTHFR genotypes, serum B vitamins and homocysteine concentrations were analyzed. Physical growth status was assessed in their offsprings by measuring height, weight, and head and chest circumferences from birth up to 24 months. Serum homocysteine levels were higher in the subjects with T/T genotype than those with the C/T or C/C. Heights and head and chest circumferences of offsprings from the T/T mothers were significantly lower than those from the C/C or C/T mothers only when the serum homocysteine levels were above the median. The mean height of offsprings from the T/T mothers was significantly lower than those from the C/C and C/T mothers. The mean weight and head circumferences of offsprings born from the mothers whose mid-term pregnancy PLP levels were in the lowest quartile was significantly lower than those from mothers in the highest quartile. Heights and head circumferences of offsprings from the T/T mothers were significantly lower than those from the C/C or C/T mothers only when the serum FAD levels were in the lowest quartile. These results suggest that postnatal growth up to 24 months may be influenced by the maternal C677T MTHFR genotypes, and mid-pregnancy serum homocysteine and vitamin B status.
