Autonomy Level of Nurse according to Nursing Delivery System (간호 분담체계에 따른 간호사의 자율성 정도)

  • This study is to grope for a plan to increase nurse's autonomy by grasping autonomy degree according to nursing delivery system. The subject of this study are 265 nurses who work for 4 general hospitals in Seoul, and 73 of them work in primary nursing delivery system, 99 of them in team nursing delivery system and 93 of them in functional nursing delivery system. Data collection was done through questionaires from Sep. 1, 1997 to Sep. 30, 1997, and autonomy was measured by Professional Nursing Autonomy Scale developed by Schutzonhofer. Data analysis as inspected with $X^2$ test, ANOVA, and t-test, using SPSS program. The results are as follows : 1. When it comes to the autonomy of all the nurses, mean score was 161.99. Which is medium level, and autonomy degree according to nursing delivery system had no significant differences. 2. There was a significant difference in autonomy degree according to inservice education among the subject's work-related characteristics, and there was no significant difference in autonomy distribution in each grade according to general characteristics although older group, married group, and junior college graduates group showed rather higher trends. 3. Considering each item, questions related to direct nursing such as "Vital sign monitoring", "Nursing rounding", "Withhold contraindicated drug", showed high score in autonomy scores, and long-term and indirect nursing behaviors such as "Nursing administration", "Nursing research", "Follow-up care" and "Educational planning".

A Comparison between Home Care Nursing Interventions for Hospice and General Patients (가정 호스피스 대상자와 일반 가정간호 대상자에게 제공된 간호중재 비교)

  • The purpose of the study was to compare home care nursing intervention activities analyzed by the Nursing Intervention Classification (NIC) system for hospice and general patients. Method: For the descriptive survey study, data was collected by reviewing charts of 151 hospice patients and 421 general patients who registered in the department of home health care nursing at K Hospital. Results: According to the NIC system application, there were 2380 total nursing interventions used for the hospice patients and 8725 for the general home care patients. For both sets of patients (hospice vs. general), the most frequently used nursing intervention in level 1 was the Physiological: Complex domain (40.13 vs. 31.06 percent), followed by the Safety domain; in level 2, the Risk Management class (28.4 vs. 27.70 percent), followed by Tissue Perfusion Management; and in level 3, Vital Sign Monitoring (6.18 vs. 4.84 percent), followed by Health Screening. Conclusion: The study showed that there was a lack of specialized hospice nursing interventions such as emotional, family and spiritual support, and care for dying hospice patients.

  • 최근 생체 신호 센싱 기능의 의류가 연구 개발되어 왔으나, 생체 신호 측정 시 착용자의 동작에 의한 치명적인 잡음이 발생하는 문제가 지속적으로 보고되어 왔다. 이에 본 연구는 심전도 센싱 의류를 기반으로 생체 신호 측정의 정확성을 향상시키기 위하여 착용자의 동작에 의한 영향을 최소화할 수 있는 심전도 센싱 스마트 의류의 모형을 개발하고자 하였다. '일자형 절개 타입', '십자형 절개 타입', '엑스형 절개 타입', '곡선 엑스형 절개 타입'의 총 네 가지 타입의 생체신호 센싱 스마트 의류의 시안을 설계하고 제작하였다. 디자인 시안은 민소매 형태의 남성용 티셔츠로 신축성 있는 소재를 사용하여 인체 굴곡을 따라 의복과 전극이 밀착될 수 있도록 하였으며, 트랜스미터를 이용하여 메인 컴퓨터로 데이터가 무선 전송되게 하였다. 본 연구에서는 개발된 4 가지 의류 타임을 기반으로 인체의 정지 및 동작 상태에서의 심전도 센싱 성능을 평가하기 위해 동작에 따른 전극의 변위를 측정하고, 심전도 측정 평가를 실시하여 SNR을 분석하였다. 본 실험 곁과를 반영하여 의류 디자인 시안의 수정 및 보완 과정을 거친 후, 최종적으로 동작 잡음을 최소화하는 생체신호 센싱 스마트 의류 디자인 모형을 제시하였다.

Vital Sign and Sedative Effect of Midazolam for Conscious Sedation during Upper Gastrointestinal Endoscopy (소아에서 상부 위장관내시경 검사 시 Midazolam 투여에 따른 생명활력징후의 변화 및 진정 효과)

  • Purpose: Esophagogastroduodenoscopy (EGD) is a very useful procedure in diagnosing gastrointestinal diseases. However, there can be some difficulty in performing this procedure in children due to the associated discomfort. Recently, sedative agents such as midazolam have been introduced to alleviate this problem quite successfully. In our study, we attempted to confirm the safety and efficacy of midazolam use by monitoring the vital signs of children undergoing endoscopy. Methods: We analyzed data on 244 children who received EGD in the department of pediatrics at Severance Hospital, Yonsei University between August 2003 and July 2005. We evaluated $O_2$ saturation, heart rate and change of mental status before and during the procedure and analyzed the results using the SPSS 12.0 package (Chi-square test, cross table method, etc.). Results: There was no significant difference in vital signs measured before and during EGD between midazolam treated and non treated children. There was no significant relationship between the amount of midazolam given and $O_2$ saturation or a change of mental status. Regarding the $O_2$ saturation, it was decreased during EGD in both groups with p<0.01 though no difference was noted between the groups; the $O_2$ level was maintained above 95% in all patients. The heart rate was also increased during EGD in both groups with a p<0.01; however, no significant difference was noted between the groups. Conclusion: We confirmed the safety of low dose midazolam by evaluating the vital signs of children during endoscopies. However, the doses used were not sufficient for sedation or to obtain an amnestic effect.

  • Damages on central nervous system induced by the exposure of microwave. However, the effects of microwave on ICP are not studied yet. The ICP affected by the condition of the brain has an significant effect on vital sign. So we investigated the changes of ICP of the rabbits after exposure. Twenty four rabbits were devided into 3 groups depending on the amount of exposure to microwave. One group was composed with 8 rabbits were exposed to microwave for 10 miniutes. Other were composed to microwave for 20 miniutes, 30 miniures, respectively. Intracranial pressure on each group were measured by subdural type ICP monitoring catheter immediately, first day, 3rd day, 5th day and 7th day after exposure of microwave. Results indicates that intracranial pressure of rabbits are not affected with statistical significance by exposure of microwave.

THE USE OF BISPECTRAL ANALYSIS AND HEMODYNAMIC MONITORING IN PATIENTS UNDERGOING INTRAVENOUS SEDATION FOR MINOR ORAL SURGERY (구강악안면외과 소수술시 정맥 의식하 진정법에서의 Bispectral Index and Hemodynamics monitoring을 이용한 진정 깊이에 관한 연구)

  • Purpose: The purpose was to assess the usefulness of midazolam in patients undergoing minor oral surgery under conscious sedation. Materials and methods: Bispectral index was examined in 20 patients receiving oral minor surgery with conscious sedation supplemented with local anesthesia. All patients included were ASA I and had no contraindications to the study medications. The patients were escorted to the day surgery operation room where, before the commencement of the sedation and surgical procedures, routine monitoring was applied, including the noninvasive monitoring of arterial blood pressure, arterial oxygen saturation, and 3-lead electrocardiogram (Electrocardiogram). Bispectral index electrodes were applied on the frontotemporal region after cleansing the skin with alcohol. Bispectral index was calculated with the Electrocardiogram monitor (A-2000; Aspect Co.). Midazolam was then titrated (initially 3mg wait 2min and 2mg). Vital sign and Bispectral index checked every 5 minute until the end of the procedure. The results were then compared. Results: The Bispectral index index values throughout the sedation study period alter many level. The index was dropped at 5 minutes after administration, but raised at injection and odontomy procedure. During the operation, mean Bispectral index index was higher than conscious sedation index range($60{\sim}80$). The amnesic effect was shown 17 cases out of 20 cases(85%). Conclusion: Conscious sedation technique using midazolma is a safe and effective method of controlling behavior in oral and maxillofacial surgery.

  • With the upcoming u-healthcare era, a way of measurement for vital sign monitoring of cardiac patients is changing as well. In existing measurement of cardiac patients, various wire in ECG measuring equipment has caused much discomfort and inconvenience. In order to decrease the problem, we are developing an efficient measurement of ECG signal using Wireless sensor network. In this paper, we present a way to reduce amount of data by transmitting ECG data collected from radio electrocardiogram sensor based on Zigbee after calculating cardiac rate. And in order to control the error which can be caused by the different ECG signal intensity each individual can has, we also suggest an adaptive pulse measurement model which can measure heart rate with correcting according to different ECG intensity. To verify the suggested model, sensor application was developed and the data was acquired in TinyOS 2.0 environment and the adaptive pulse measurement model was evaluated through the data from the experiments.

  • According to increase of concerning in health and entry of aging society, sensor-based smart clothing has developed various type and applications. Sensor-based smart clothing should be designed with considering of the interaction between a human body-device-clothing, such as accuracy of signal, wearability, suitability and the configuration of the sensor and so on. In this respect, these characteristics distinguish sensor-based smart clothing process from clothing process and Sensor-based smart clothing process is expected to be needing requirements Specification for development purpose and interoperability assessment based on requirements engineering. In this study, to assess efficiency of process based on requirement engineering, the sensor-based smart clothing process was deducted in two types by analysis of empirical performance. Presented two process were empirically evaluated through qualitative and quantitative evaluation. As a result, design process II based on requiments engineering were confirmed more effective process than processI.

  • This paper introduces a customized pet care and training system based on information technology. A specific animal is recognized by nose-print recognition. Pet robot induces the animal to move to trainer device. Trainer device trains the animal by snack, play, and clicker, and it can be easily extended for specific training using add-on devices. Pet band performs vital sign monitoring, prevention of home escape, and measurement of exercise amount. By changing configuration, this system can perform various training programs such as mitigation of separation anxiety, induction to defecation place, and prohibition of tresspassing specific place. It also provides customized training by analyzing individual behavior of animal. It can exercise and play the animal when owner is out, and it can immediately notice to the owner when the animal has health trouble.

Development of KPCS(Korean Patient Classification System for Nurses) Based on Nursing Needs (간호요구 정도에 기초한 한국형 환자분류도구(KPCS)의 개발)

  • Purpose: This study was to develop a factor-type patient classification system for general nursing unit based on nursing needs (KPCS; Korean patient classification system for nurses). Method: We reviewed workload management system for nurses(WMSN) of Walter Reed Medical Center, Korean patient classification system for ICU, and nursing activities in nursing records and developed the first version of KPCS. The final version KPCS was evaluated via validity and reliability verifications based on panel discussions and data from 800 patient classifications. Content validity was performed by Delphi method and concurrent validity was verified by the correlation of two tools (r=.71). Construct validity was also tested by medical department (p<.001), patient type (p<.001), and nurse intuition (p<.001). These verifications were performed from April to October, 2008. Results: The KPCS has 75 items in classifying 50 nursing activities, and categorized into 12 different nursing area (measuring vital sign, monitoring, respiratory treatment, hygiene, diet, excretion, movement, examination, medication, treatment, special treatment, and education/emotional support). Conclusion: The findings of the study showed sound reliability and validity of KPCS based on nursing needs. Further study is mandated to refine the system and to develop index score to estimate the necessary number of nurses for adequate care.
