2016년 전북대학교 치과병원 소아치과를 방문한 장애인환자에 관한 조사연구 (A SURVEY OF PATIENTS WITH DISABILITIES VISITING THE DEPARTMENT OF PEDIATRIC DENTISTRY OF CHONBUK NATIONAL UNIVERSITY DENTAL HOSPITAL IN 2016)

  • According to the Ministry of Health and Welfare, the portion of physical disability was 51.4% which was the highest among the registered disabled in December 2015, followed by visual disability(10.1%), disability of the brain lesions(10.0%), hearing disability(10.0%). The aim of this study is to investigate the types of disability in patients with disabilities visiting the Pediatric Dentistry, Chonbuk National University Dental Hospital in 2016. Of the patients who visited the pediatric dentistry, 196 patients (7.5%) had disabilities, and 19 patients had two or more disabilities. Twelve patients (6.1%) had both disabilities of brain lesion and intellectual disorder. Of the 177 patients with only one disability, 54 patients (30.5%) had physical disorders and 123 patients (69.4%) had psychiatric disorders. In psychiatric disorders, intellectual disorder was the most with 98 patients (55.3%), followed by autistic disorder with 23 patients (12.9%). In physical disorders, disability of brain lesion was the most with 37 patients (20.9%), followed by speech disability with 8 patients (4.5%). In this study, as a result of analyzing the types of disability with the patients who visited the Pediatric Dentistry of Chonbuk National University Dental Hospital, most of them had either intellectual disorder, disability of brain lesion, or autistic disorder. Dentists need to understand their general and dental characteristics to treat children and adolescents with disabilities. Since the most chief complaint of the patients was dental caries, early interventions should be made by dentist to reduce the incidence of the disease.

VDT 작업 시 의자 등받이 높이가 생체역학적 변인에 미치는 영향 (Effect of Backrest Height on Biomechanics Variables During VDT (Visual Display Terminal) Work )

  • Objective: This study identifies the difference among the heights of a chair's backrest (High, Mid, No), the biomechanical changes chair users undergo over time, and the variables that can measure musculoskeletal disorders, eventually providing information on the appropriate type of backrest. Method: Eleven healthy subjects in their 20s and 30s who had no experience with musculoskeletal disorders or surgical operations within the last 6 months participated in this study. Computer typing tasks were randomly designated and performed according to the type of chair backrest, and evaluation was performed for Flexion-Relaxation Ratio (FRR) analysis after the computer typing tasks. This study used eight infrared cameras (sampling rate: 100 Hz) and nine-channel electromyography (sampling rate: 1,000 Hz). ANOVA with repeated measures was conducted to verify the results, with the statistical significance level being α = .05. Results: Although there was no significant difference in craniovertebral angle (CVA), this study showed time and interaction effects depending on the height of the backrest (p<.05). When working without the backrest, the head-spine angle was lower compared to the chairs with backrest, based on the computer work. As for the head angle, the higher the back of the chair was, the less the head flexion and the body angle became, whereas the body flexion became less when there was a backrest. In addition, the body flexion increased over time in all types of backrests (p<.05). The muscle activity of the upper body tended to be high in the high backrest chair. On the other hand, a lower muscle activity was found with a low backrest. Conclusion: These results show that a chair is more ergonomic when the body angle is correctly set without bending and when it is supported by a low backrest. Accordingly, this study determines that the backrest affects shoulder and neck musculoskeletal disorders during typing and that medium-height backrest chairs can help prevent musculoskeletal disorders, contrary to the expectation that high-backrest chairs are preferable.

두개천골요법과 침치료로 호전된 만성 편두통 환자에 대한 증례보고 (A Case Report of Craniosacral Therapy and Acupuncture for Chronic Migraine)

  • Objectives: This case report aims to describe the effects of craniosacral therapy and acupuncture in a patient with chronic migraine. Methods: A 33-year-old man with chronic migraine was treated with 20 sessions of craniosacral therapy and acupuncture for 8 weeks. The number of migraine and headache days were monitored every month. The pain intensity of headache was measured on the visual analog scale (VAS). Korean Headache Impact Test-6 (HIT-6) and Migraine Specific Quality of Life (MSQoL) were also used. Results: The number of headache days per month reduced from 28 to 7 after 8 weeks of treatment and to 3 after 3 months of treatment. The pain intensity of headache based on VAS reduced from 7.5 to 3 after 8 weeks and further to < 1 after 3 months of treatment. Furthermore, the patient's HIT-6 and MSQoL scores improved during the treatment period, which was maintained or further improved at the 3 month follow-up. No side effects were observed during or after the treatment. Conclusion: This case indicates that craniosacral therapy and acupuncture could be effective treatments for chronic migraine. Further studies are required to validate the efficacy of craniosacral therapy for chronic migraine.

측두하악장애 임상연구에 사용된 진단 및 평가기준에 대한 문헌적 고찰 (A Literatural Investigation of Diagnosis Methods and Evaluation Outcomes for the Clinical Trials on Temporomandibular Disorders)

  • Objectives The aim of this review is to provide fundamental data for temporomandibular disorders diagnosis and evaluation criteria which can be used in clinical trial. Methods We investigated the clinical studies on temporomandibular disorders via PubMed. Also, we searched domestic articles through "OASIS", "NDSL", "KISS", "Korean Traditional Knowledge Portal (KTKP)". The articles we focused on were the recent decade from 2007 to 2016. A total of 139 studies were analyzed: 42 domestic articles and 97 overseas articles. This study focuses on the diagnosis and evaluation criteria on temporomandibular disorders. Results 1) In diagnosis criteria, Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) was used 14 times in domestic articles and 63 times in overseas articles. Clinical symptoms were used 13 times in domestic articles, 17 times in overseas articles. 2) In evaluation criteria, Visual Analog Scale (VAS) was used as a pain scale 12 times in domestic articles, 63 times in overseas articles. Pressure Pain Threshold (PPT) was used 16 times in overseas articles. Whereas, no clinical trials used PPT in domestic articles. In psychological scale, Symptom Checklist-90-Revised (SCL-90-R) was used 6 times. However, SCL-90-R was used 2 times in domestic articles. ROM(Maximum Mouth Opening (MMO), Maximum Comfortable Opening (MCO) were used 24 times in overseas. In domestic articles, MMO was counted 12 times, while MCO was counted 5 times. Conclusions This research reviewed the tendency of using diagnosis methods and evaluating outcomes of the clinical on TMD. It is expected that this investigation would develop further treatment for TMD in the Korean Medicine.

시각장애 비만대상자를 위한 동기강화 비만관리프로그램의 개발 및 효과평가 (The Development and Evaluation of a Motivation-Strengthening Obesity Management Program for Obese Subjects with Visual Impairment)

  • Purpose: The purpose of this study was to develop and apply a motivation-strengthening obesity management program for obese subjects with visual impairment and evaluate its effects. Methods: The program development and evaluation periods were from March to August 2018. The study design was an experimental study of non-equality, control repeat design. 25 subjects in the experimental group participated in the 12-week obesity management program and the 26 people in the control group did not. Results: The motivation-strengthening obesity management program included stretching exercises for 30 minutes in the morning five times a week. In addition, counseling, education, and group discussions were continued once a week for motivational strengthening. Special custom exercises for blind patients were also completed once a week simultaneously. Both of these activities were continued for 50 minutes a week for a total of 12 weeks. Measurements were recorded periodically to identify the sustained effects of the program. The obese subjects with visual impairment improved their knowledge of obesity, perceived disability, self-efficacy of exercise, and quality of life related to obesity through the motivational obesity management program. Waist circumference, systolic blood pressure, diastolic blood pressure, and fasting blood sugar were also improved in obese subjects. Conclusion: The study findings indicate that this program could be an active intervention for the control of weight gain in participants with visual impairment. Therefore, the motivation-strengthening program based on the ADDIE (Analysis, Design, Development, Implementation& Evaluation) model could be an effective strategy for better health outcomes of obese subjects with visual impairment.

전산화 인지프로그램(코트라스)이 지적장애 아동의 시지각과 일상생활에 미치는 효과 (The Effect of Computer-Based Cognitive Rehabilitation Program on Visual Perception and ADL in Children with Intellectual Disaility)

  • 본 연구는 시지각 장애와 일상생활 기능에 어려움이 있는 지적장애아동을 대상으로 아동용 전산화 인지재활 프로그램이 시지각과 일상생활 기능에 임상적인 효과가 있는지 알아보기 위함이다. 이를 증명하기 위해 공통된 특성이 있는 지적장애 아동 3명에게 아동용 전산화 인지재활 프로그램을 총 20회 실시하였다. 시지각 기능의 중재 전과 후를 비교하기 위해 K-DTVP-2를 사용하였고, 일상생활 기능의 중재 전과 후를 비교하기 위해 WeeFIM검사를 실시하였다. 그 결과 지적장애 아동에게 아동용 전산화 인지재활 프로그램을 적용하는 것이 아동의 시지각 기능 특히 비운동성 시지각 기능 향상에 긍정적인 결과를 나타내었으며, 또한 일상생활 활동이 향상되었다. 이 연구로부터 얻은 결과들은 지적장애가 있는 아동 중 특히 시지각 기능에 결여를 보이는 아동에게 아동용 전산화 인지 프로그램을 적용하면 인지능력의 하위부분인 시지각 기능에 긍정적인 효과가 있음을 기대할 수 있고, 그로 인해 일상생활 활동에 향상을 보임을 기대할 수 있다.

진전을 동반한 Fahr Disease 환자 치험례 (A Case Report of Fahr Disease with Tremor)

  • Fahr disease (FD) is a rare neurological disorder characterized by presence of abnormal and associated cell loss in certain areas of the brain, mostly in basal ganglia, thalamus, cerebellum and subcortical areas. Approximately two-thirds of the patients are symptomatic. The most common neurological manifestations include movement disorders, cognitive impairment, cerebellar signs and speech disorders. We report one case of estimated FD through brain computed tomography (bilateral calcifications of basal ganglia, thalamus, centrum semiovale, subcortical white matter of occipital lobes, cerebellum). At the first time of treatment, he complained of tremors in his upper limbs. We diagnosed the patient as deficiency of qi (氣) and movement of phlegm-heat-wind (痰熱風動) type according to symptoms and treated by herbs and acupuncture of oriental medicine. During treatments, we evaluated how well the oriental medical treatments were working using visual analogue scale (VAS) and amplitude of hands. After the oriental medical treatments about tremor, VAS dropped from 10 to 2 and amplitude of hands from 20 mm to 2 mm, but the ratio of brain calcifications was not changed. This study suggests that oriental medical treatments can be applicable to improve FD.

A Study on the VDT Workstations Usage for Office Workers

  • Objective and Background: Due to increase in Musculoskeletal Disorders (MSDs) relating to computer use, a number of ergonomics recommendations have been proposed in order to tackle this problem. However, some of these recommendations have been conflicting. Method: This study was to survey the VDT (Visual Display Terminal) use of office workers. The subjects were 452 workers at 13 places of business and the data were collected by self-administered questionnaire. Results: As a result, prevalence of self-reported MSDs of all VDT workers was 90.2% and shoulder took up 57.0% and neck 38.3% by symptom part of body. The population of computer use of more than 6h/day was 84.5%, and 33.8% also reported using the VDT 2~3h/day without the rest time. Desktop computer users were 95.6%, and a 17-inch monitor accounted for 42.0% among the desktop users. As a result of satisfaction survey on overall computer work, 21.1% of the total respondents said satisfied, desk complaint was about 24.6%, and chair complaint was 33.4%. Despite the importance of computer environment, satisfaction was from fair to uncomfortable. Conclusion and Application: In conclusion, office workers are prone to the MSDs due to their work environment. Additionally, this study found that task was a significant effect for the majority of dependent variables, and therefore, the improvement of computer workstations work environment is urgent, and the improvement of desk height adjustment, chair seat size (length, width), backrest condition, location of keyboard (mouse) and arm rest is required.

A Case Report of a Patient with Miller Fisher Syndrome Treated with Traditional Korean Medicine

  • The aim of this report is to describe improved symptom changes in eye movement disorders and dizziness of a patient with Miller Fisher syndrome after receiving combined Korean medicine treatment (CKMT). A 24-year-old male was diagnosed with MFS accompanied by eye movement disorder, diplopia, and dizziness. For 6 weeks, the patient received acupuncture, electro-acupuncture, pharmacopuncture, herbal medicine, and physical therapy treatment. Visual analogue scale were checked and other outcomes (the range of eye movement, the distance that diplopia appears, diplopia questionnaire) were measured. Following CKMT, the movement of both eyes improved in both the horizontal and vertical direction. Also for diplopia and dizziness, there was a decrease in the visual analogue scale and the distance that diplopia appeared. The present case report suggests that CKMT may have a role in treating eye movement disorders and dizziness in patient diagnosed with MFS.

장애인복지를 위한 정보통신의 발전방향 (New developmental direction of telecommunications for Disabilities Welfare)

  • 본 연구는 장애인이 일반인과 똑같이 정보사회에 적응시키기 위해 장애인복지를 위한 정보통신의 발전방향을 연구하였다. 연구의 방법으로는 델파이기법을 도입하였으며, 조사방법으로는 문헌적조사방법과 면담조사방법을 병행하여 연구분석의 틀에 의해 분석하였다. 정보통신에 대한 장애인의 문제점을 살펴보면, 정보통신 접근의 불편, 보편적 서비스 저하, 장애인의 PC 통신 이용 저조, 장애인복지의 낙후, 정보통신요금의 부담, 정보화교육의 부족, 장애인정보의 부족, 장애인복지 정책결정에 장애인 미참여 등으로 분석되었다. 장애인에 필요한 정보통신기술을 살펴보면, 지체장애인에게는 음성인식기술ㆍ화상인식기술ㆍ호흡압력감지기술, 시각장애인에게는 표시기술ㆍ음성인식기술ㆍ문자인식기술ㆍ지적변환처리기술ㆍ화상인식음성합성기술, 청각ㆍ언어장애인에게는 음성신호처리기술ㆍ음성인식기술ㆍ기적변환처리기술ㆍ문자인식기술ㆍ화상인식기술ㆍ음성합성기술이 필요하다. 장애인복지를 위한 정보통신을 발전시키기 위해서는 장애인정보통신위원회의 구성, 보편적 서비스의 제공, 정보화교육의 실시, 연구개발의 지원, 중소정보통신기업의 지원 육성, 소프트웨어산업의 육성, 장애인용 표준화 작업 추진이 요구된다.
