The Analysis on Trend of Articles about Strabismus in Journal of Korean Medicine (국내 한의학 학술지에 게재된 사시 관련 논문들의 경향성 분석)

  • Objectives : Strabismus is a disease that can be observed and treated directly from the outside. Therefore, it is very important in Korean medicine ophthalmology in accessibility to treatment. This study is designed to investigate the trends of strabismus-related studies published in Journal of Korean medicine. Methods : 5 Internet databases were selected to search for the study subject. The following 9 keywords were used; Strabismus, Heterophoria, Binocular vision, Visual acuity, Diplopia, Paralytic, Cover test, Oculomotor nerve paralysis, Abducens nerve paralysis. Through this process, a total of 45 studies were found and analyzed into 5 categories ; publication year, publication journal, number of author, type of article, analyzation of case report. Results : There were 2 review articles, 1 original article and 42 case reports. 5 studies(11.1%) were published in 2004 year. Journal of Korean Oriental Medical Ophthalmology & Otolaryngology & Dermatology were the most common with 40%. The authors were often 2 and 3 (each 22.2%). In the case report study, the treatment period was most often 1-30 days (35.7%). Conclusions : 42 studies were on paralytic strabismus and 1 study was on paralytic and non-paralytic strabismus. These results represent that the studies are concentrated in case reports about paralytic strabismus, but this seems to be an area to be improved in future research.

A Study on the Physical Growth and Health Status in University Students (일부(一部) 대학생(大學生) 신체발육(身體發育) 및 건강상태(健康狀態)에 관(關)한 조사(調査))

  • For the purpose of ascertaining the status of physical growth and health status of the University students, a intensive survey was conducted by the authors towards a total of 2,638(male; 2,228, female; 410) who passed the written entrance examination at the Seoul National University. The items included the measurements on physical growth, various physical indixes, visual acuity, tuberculosis and dental status. The findings and results can be summarized as follows: 1. Physical Growth No significant difference was found in the physical growth between classes(literature and science) and by ages. In males, the means of body height, body weight, chest-girth and sittingheight were $168.0{\pm}5.07cm,\;55.7{\pm}5.71kg,\;85.8{\pm}4.4cm\;and\;91.5{\pm}2.21cm$, respectively. In females, these were $157.4{\pm}4.57\;cm,\;00.4{\pm}5.75kg,\;80.3{\pm}4.51cm\;and\;86.1{\pm}3.30cm$, respectively. Basing on the above measurements several physical indixes were calculated as follows ; Relative body weight 33.15 in males and 32.02 in females. Relative chest-girth 51.07 and 51.01, relative sitting-height 54.46 and 54.70 $R\ddot{o}hrer$ index 1.00 and 1.02, Kaup index 1.97 and 2.04, Vervaeck index 83.63 and 83.03, and Pelidisi index 89.94 and 92.45. 2. Visual Acuity Among the students of science classes those of 0.8 and below in vision on left and right occupied 46.4% and 45.0%, those of 0.6 and below 39.1% and 41.2%, respectively. Among the students of literature classes those of 0.8 and below occupied 46.3% and 42.9%, and those of 0.6 and below occupied 41.4% and 39.0, respectively. In males those of 0.8 and below occupied 46.8% and 44.5% and 0.6 and below occupied 41.9% and 39.7%, while in females 0.8 and below occupied 43.9%, and 42.4%, and 0.6 and below occupied 38.3% and 37.3%, respectively. In males those of 0.8 and below in corrected vision occupied 12.1% and 10.3%, while in females 12.9% and 12.2%, respectively. More students of abnormal vision were found among science classes and the proportions of corrected vision were 38.9% and 37.4% in males, and 33.9% and 33.2% in females, respectively. 3. Tuberculosis Among the total, 55 students were diagnosed as tuberculosis, of whom 50 were minimal cases, 2 were moderately advanced cases and 3 were far advanced cases. 4. Dental Status Among the total, 81.2% were of decayed teeth, 20.8% filled, 9.5% missed, 5.0% extraction needed, 9.1% sunplatinum bridges, 3.3% golden bridges and 5.5% golden inlay. Regarding tartar on teeth moderate, heavy and light grades occupied 49.5%, 29.7% and 20.3%, respectively and 0.8% had none. 5. Comprehensive evaluation According to criteria 4 grades were applied to comprehensive evaluation as follows : A...excellent in physical status B...with some physical defects but no difficulties in study C...need treatment D...need treatiment and rest Out of the total, A grade occupied 21.6%, B 76.2% and C. and D 2.2.

Long-term Results of Gamma Knife Radiosurgery for Craniopharyngioma (두개인두종의 감마나이프 치료 후 장기 추적 결과)

  • Objectives : The optimal treatment of craniopharyngioma is controversial. Despite recent advances in microsurgical management, complete surgical removal of craniopharyngioma remains very difficult. Radiation added to surgery is effective, but radiation therapy resulted in untoward side effect in young patient. Gamma knife radiosurgery offers the theoretical advantage of a reduced radiation dose to surrounding structures during the treatment of residual or recurrent craniopharyngioma compared with fractionated radiotheraphy. We described retrospective analysis of tumor size and clinical symptoms of patients after gamma knife radiosurgery in residual or recurrent craniopharyngioma were performed. Material and Methods : From September 1990 to January 2000, 18 patients of craniopharyngioma were treated by gamma knife radiosurgery. All patient had undergone surgery, but residual or recurrent tumor was found and all of them treated postoperative gamma knife radiosurgery. The mean age was 19(from 6 to 66) and male to female ratio was 10 to 8 and 8 patients were below 15 years old. In young age group(below age 15), the average volume of the tumor was $2904.8mm^3$ and mean maximal gamma knife dose was 34.9Gy. In old age group(older than 15), the average volume of the tumor was $2590.4mm^3$ and mean maximal gamma knife dose was 45.2Gy. The size of the tumor was average $2730.1mm^3$($88-12000mm^3$), mean average radiation dose was 40.7Gy and the mean prescription dose was 17.6 Gy(4-35Gy) delivered to a median prescription 50.7% isodose. Results : The follow up was from 1 year to 9 years(mean 59.1 months) after gamma knife radiosurgery. The tumor was controlled in 13(72.2%) patients. The tumor decreased in 9 patients and not changed in 4 patients. The tumor size increased in 4(22.2%) patients during follow up period. In two cases the tumor size increased because of its cystic portion was increased, but their solid portion of the tumor was not changed. In another two patients, the solid portion of the tumor was increased. So, one patient underwent reoperation and the other patient underwent operation and repeated gamma knife radiosurgery. The tumor recurred in one case(5.6%) that is a outside of irradiated site. The presenting symptoms were improved in 4 patients(improved visual acuity in 1, controlled increased intracranial presure sign in 3 patients). In one case, visual acuity decreased after gamma knife radiosurgery. The endocrine symptoms were not influenced by gamma knife radiosurgery. Conclusion : Craniopharyngioma can be treated successfully by gamma knife radiosurgery. Causes of the tumor regrowth are inadequate dose planning because of postoperatively poor margination of the tumor, close approximation of optic nerve and residual tumors outside the target lesion. Recurrence can develop 4 years after gamma knife radiosurgery. Volume is important, but the accurate targeting is more important to prevent tumor recurrence. If the tumor definition is not clear during planning gamma knife surgery, long-term image follow up is required.

Comparison of Induced Aniseikonia with Influential Factors (영향인자에 따른 유발 부등상시도의 비교)

  • Purpose: This study was performed to provide indicator of expected aniseikonia by correcting refractive error and to investigate influential factors on aniseikonia. Methods: 20 college students (14 males, 6 females, a mean age of $22.50{\pm}2.72$ years) were selected as subjects whose refractive error with spherical equivalent were within ${\pm}0.50$ D, corrected visual acuity were more than 1.0, and aniseikonia values by AWAYA were less than 1%. After correcting refractive error with spectacles in anisometropia induced by wearing contact lens on their dominant eye or non-dominant eye, practical measured values of aniseikonia were compared with theoretical expected values of it by the formula of spectacle magnification. Results: Practical measured values were higher than theoretical expected values in induced aniseikonia over the whole range of diopter of wearing contact lens. And there was higher measured value of aniseikonia in case of higher diopter of wearing contact lens to induce anisometropia and correcting refractive error with spectacles of (+) diopter after wearing contact lens of (-) diopter to induced anisometropia in dominant eye of women. Conclusions: It is considered that dominant eye plays more important role for visual function in induced aniseikonia and factors such as the induced eye of aniseikonia, the diopter of wearing contact lens, and gender have influenced on aniseikonia.

The Change of Near Point of Convergence and Fusional Reserves after Computer Gaming with Different Direction of Eye Movement (안구의 운동방향이 다른 컴퓨터 게임 후 폭주근점과 융합여력의 변화)

  • Purpose: The present study was conducted to investigate whether the directions of eye movement in playing computer games for certain period affected the change of near point of convergence (NPC) and fusional reserve (FR) or not. Methods: Total 40 subjects in 20s who have the visual acuity of 1.0 or higher without any ocular disease and accommodative dysfunction were asked to successively play computer games. After the subjects were moving eyes in horizontal and vertical directions for 40 and 90 minutes, their horizontal fusional reserves, vertical fusional vergence and near point of convergence were measured. Results: The near point of convergence showed a tendency to be receded after computer gaming in the horizontal and vertical directions, and both of horizontal and vertical fusional reserves were significantly reduced. The range of declined fusional reserves and receded near point of convergence after computer gaming for 90 minutes was smaller than those after computer gaming for 40 minutes. The change of binocular vision was affected by the horizontal eye movement rather than the vertical movement when analyzed by the direction of eye movement. Conclusions: This study revealed that the change in FR and NPC was different along with dominant direction of eye movement during visual display terminal (VDT) tasks. Therefore, the adjustment of VDT working time is required to prevent the dysfunction of binocular vision according to the dominant direction of eye movement during VDT task.

A study of refraction state of middle aged & manhood in Daegu (대구지역 중·장년층의 굴절상태 연구)

  • This study researched the visual acuity test object and Auto-refractormeter, visual of near power. The object were composed of middle aged, the old men and women who in habit Daegu. The results were as follows : 1. The subjects consisted of 537 people, 29.98% men, 70.02% women. 2. The emmetropia was 1.12% for myopia, 2.79% for hyperopia, 96.09% for astigmatism. 3. The abnormal refraction was composition for myopic compound astigmatism(16.57%), hyperopia compound astigmatism(45.62%), Mixed astigmatism(33.89%). 4. On the Myopic Spherical Equivalent(S.E) power, the range of -0.50D ${\leq}$ M.S.E < -1.00D was 21.67%, -1.00D ${\leq}$ M.S.E < -2.00D was 48.89%, -2.00D ${\leq}$ M.S.E < -6.00D was 29.44%. 5. On the Hyperopic Spherical Equivalent(S.E) power, the range of +0.50D ${\leq}$ H.S.E < +1.00D was 28.57%, +1.00D ${\leq}$ H.S.E < +2.00D was 49.30%, +2.00D ${\leq}$ H.S.E < +6.00D was 23.13%. 6. The addition power was 1.00D(8.01%), 1.50D(8.57%), 2.00D(13.78%), 2.50D(16.57%), 3.00D(16.95%), 3.50D(17.88%), 4.00D(18.25%).

The Accommodative Lag and Refractive Error in Early Adults (초년 성인의 굴절이상과 조절래그 분포의 연관성)

  • Purpose: The purpose of this study was to determine the distribution and correlation of accommodative lag with refractive error. Method: We had tested the clinical refraction and the accommodative lag in clinically normal 49 young adults (total 98 eyes) aged 18 to 25 years without abnormal binocular function. Monocular and binocular accommodative lag were tested with 0.50 D cross-cylinder lens and near vision test chart which had cross-hairs after full correction of LogMAR visual acuity over 0.05. Results: There was no statistical differences in monocular accommodative lag between right ($0.64{\pm}0.64$ D) and left eye ($0.63{\pm}0.64$)(p=0.858). The accommodative lag of male was higher than female and the range of the value was broader than female in binocular accommodative lag (p=0.015). The wider the inter-pupillary distance was, the higher the accommodative lag was (p=0.003). However, there were no differences with age (p=0.800) and dominant eye (p=0.402). The ranges of accommodative lag of low, middle, and high myopia were 0.75 ~ -0.25 D, 1.25 ~ -0.50 D, and 1.50 ~ -0.75 D, respectively, and the regression was 'y = -0.03953x+0.09205'. Conclusions: These data suggest that clinically normal young adults with high amounts of refractive error have more variable accommodative lag and increased spherical equivalent refraction.

Effect of Artificially Decreased Visual Acuity upon Eye-Hand Coordination using Lee-Ryan Eye-Hand Coordination Test (Lee-Ryan Eye-Hand Coordination Test를 이용한 인위적 시력저하가 눈-손 협응능력에 미치는 영향)

  • Purpose: The aim of this study was to explore the effect of artificially decreased eye in normal vision on eye-hand coordination (EHC) when using the Lee-Ryan Eye-Hand Coordination Test recently reported. Methods: Eleven adults with normal vision aged $29.46{\pm}5.94$ years participated for this study where a non-dominant eye artificially induced moderate refractive amblyopic vision at near by adding a plus lens conducted EHC tasks and then did the test again under normal vision following 2 weeks. To investigate the ability of EHC, 7 tasks including individually different level of difficulty in the Lee-Ryan EHC Test were selected to compare and analyze EHC in terms of two independent variables such as time taken and the number of errors. Results: In time taken, subjects with artificially decreased vision took more time than normal vision under monocular conditions (p=0.013), while those with the decreased vision completed their tasks faster than normal vision under binocular conditions (p=0.001). In the number of errors, subjects with the decreased vision made more mistakes (p<0.001) as shown in time taken, whereas there was no difference between monocular and binocular viewing conditions in the decreased vision. Conclusions: Unlike previous EHC tests including limitations for application, deficit in EHC can be screened by the Lee-Ryan EHC Test developed based on simple computer-based system. Therefore, it is considered that further studies relevant to deficits in visual function such as amblyopia will be carried out in clinics as well as research.

Radiological Apoplexy and Its Correlation with Acute Clinical Presentation, Angiogenesis and Tumor Microvascular Density in Pituitary Adenomas

  • Objective : Pituitary apoplexy is life-threatening clinical syndrome caused by the rapid enlargement of a pituitary tumor due to hemorrhage and/or infarction. The pathogenesis of pituitary apoplexy is not completely understood. We analyzed the magnetic resonance imaging (MRI) of pituitary tumors and subsequently correlated the radiological findings with the clinical presentation. Additionally, immunohistochemistry was also performed to determine whether certain biomarkers are related to radiological apoplexy. Methods : Thirty-four cases of pituitary adenoma were enrolled for retrospective analysis. In this study, the radiological apoplexy was defined as cases where hemorrhage, infarction or cysts were identified on MRI. Acute clinical presentation was defined as the presence of any of the following symptoms: severe sudden onset headache, decreased visual acuity and/or visual field deficit, and acute mental status changes. Angiogenesis was quantified by immunohistochemical expression of fetal liver kinase 1 (Flk-1), neuropilin (NRP) and vascular endothelial growth factor (VEGF) expression, while microvascular density (MVD) was assessed using Endoglin and CD31. Results : Clinically, fourteen patients presented with acute symptoms and 20 for mild or none clinical symptoms. Radiologically, fifteen patients met the criteria for radiological apoplexy. Of the fifteen patients with radiologic apoplexy, 9 patients presented acute symptoms whereas of the 19 patient without radiologic apoplexy, 5 patients presented acute symptoms. Of the five biomarkers tracked, only VEGF was found to be positively correlated with both radiological and nonradiological apoplexy. Conclusion : While pituitary apoplexy is currently defined in cases where clinical symptoms can be histologically confirmed, we contend that cases of radiologically identified pituitary hemorrhages that present with mild or no symptoms should be designated subacute or subclinical apoplexy. VEGF is believed to have a positive correlation with pituitary hemorrhage. Considering the high rate of symptomatic or asymptomatic pituitary tumor hemorrhage, additional studies are needed to detect predictors of the pituitary hemorrhage.

A USEFULNESS OF KEDI-INDIVIDUAL BASIC LEARNING SKILLS TEST AS A DIAGNOSTIC TOOL OF LEARNING DISORDERS (학습 장애아 진단 도구로 기초 학습 기능 검사의 유용성에 관한 연구)

  • The purpose of this study was to examine usefulness of KEDI-Individual Basic Learning Skills Test as a diagnostic tool of learning disorders(LD). Learning disorder group consisted of two subgroups, verbal learning disorder group(VLD, n=34) and nonverbal learning disorder group(NVLD, n=14). Comparison group consisted of Dysthymia Disorder subgroup(n=11) and Normal subgroup(n=20). Performance of intelligence test and achievement test was examined in all 4 subgroups. In KEDI-WISC, VLD subgroup revealed primary problems in vocabulary, information and verbal-auditory attention test. NVLD group revealed primary problems in almost all performance tests such as visual acuity, psycho-motor coordination speed and visual-spatial organizations ability subtest. In KEDI-Individual Basic Learning Test, VLD group revealed primary problems in phonological coding process, word recognition and mathematics. For successful classification of LD children, the importance of achievement test and intelligence test was discussed by discriminant analysis and factor analysis. The results indicate that KEDI-Individual Basic Learning Skills is of considerable usefulness in diagnosing LD, but must be used in subtests, and additional tests must be conducted for thorough exploration of LD.
