Purpose: The purpose of this study is to find and suggest ways to invigorate operation of visiting nursing centers. Methods: SWOT analysis was performed based on the status of use of visiting nursing centers and opinion survey on the centers' nursing experts. Results: SO strategy was drawn to improve service satisfaction, develop standardized manuals, and improve the areas of visiting nursing services, and WO strategy was drawn to establish reliability, strengthen promotion or marketing strategy, strengthen management ability, and reinforce the governmental support of visiting nursing services. ST strategy was drawn to recover the functionality of health management, secure competitive advantage, and simplify the issuance of visiting nursing directions, and WT strategy was drawn to provide customized service, establish the cooperative system of related agencies, and adjust fees. Conclusion: For invigoration of visiting nursing centers, people must recognize the importance of the visiting nursing service and institutional standards should be established so that visiting nursing service, which is currently provided as an option according to Standard Long-Term Care Plan, can be provided on a mandatory basis.
Purpose: The purpose of this study is to analyse time taken for visiting nursing activities by visiting nurses in health centers. Method: A questionnaire was developed by a research team for the technological support of visiting nursing activities and visiting nurses. A total of 481 questionnaires were recovered by five visiting nurses from May to October 2003. Result: A visiting nurse's total length of time for visiting activities was 532.2 minutes per day and the number of households a visiting nurse visits a day was 4.5. A visiting nurse spent 133.3 minutes for actual nursing services and 119.1 minutes for travelling. Time for actual nursing activities was more than half of the total working hours. Time for travelling was 42.9 minutes on the average when using public transportation means and 25.6 minutes when using a car. Conclusion: The results of this study is expected to be used as basic data in establishing plans for the adequate supply of visiting nurses based on demands for visiting nursing services.
Purpose: This study conducted a job analysis of visiting nurses in the process of change. Methods: Participants were the visiting nurses working for the Seoul Metropolitan city. On the basis of the Public Health Intervention Wheel model, two times of the focus group interview (FGI) with seven visiting nurses and one time of the Developing a Curriculum (DACUM) with 34 visiting nurses were performed. A questionnaire survey of 380 visiting nurses was conducted to examine the frequency, importance and difficulty levels of the tasks created by using the FGI and DACUM. Results: Visiting nurses' job was derived as the theme of present versus transitional roles. The present role was categorized as 'providing individual- and group-focused services' and 'conducting organization management', while the transitional role was categorized as 'providing district-focused services' and 'responding to new health issues'. The job generated 13 duties, 28 tasks, and 73task elements. The tasks showed the levels of frequency (3.65 scores), importance (4.27 scores), and difficulty (3.81 scores). All the tasks were determined as important, exceeding the average 4.00 scores. The group- and district-focused services of the tasks were recognized as more difficult but less frequent tasks. Conclusion: The visiting nurses exert both present and transitional roles. The transitional roles identified in the present study should be recognized as an extended role of visiting nurses in accordance with the current changing healthcare needs in South Korea. Finally, the educational curriculum for visiting nurses that reflects the transitional roles from the present study is needed.
Objectives: The purpose of this study was to verify a change for family nursing phenomena and satisfaction of clients of vulnerable families in an urban community. Methods: The study subjects were 711 families, randomly selected, who had chronic diseases(arthritis diabetes, stroke, hypertension, mental disease, cancer, dementia) with basic social welfare services from the nation and had disabled persons in an urban community, South Korea. The instruments used were the family nursing phenomena in Korea by ICNP and client satisfaction. Client satisfaction consisted of client satisfaction on home visiting nurses(4 Likert scales) and home visiting services(3 Likert scales). Results: The average visiting number is 3.82. The service number of education and counseling is 3.16, patient and symptom management 3.08, assessment and diagnosis 3.08, test 2.02, medication service 1.71회, dressing 1.01, referral to social welfare institute 1.00회, referral to medical service institute 0.21. In both, pre home visiting and post home visiting, the highest rated phenomenon was the 'lack of social support system' and following that 'deficit of financial management skill and support'. 'lack of family interaction in community', and 'social isolation' 'unhealthy life style' and 'inadequate care management of sick member', in that order. The percentages of phenomena besides 'deficit of financial management skill and support' decreased. The satisfaction level of clients towards the nurses was 3.27 points on a scale of 4, and the nursing services was 2.70 points on a scale of 3. Conclusion: Home visiting nursing services should continue to provide comprehensive healthcare services and support for vulnerable families, in urban communities.
This study is conducted to promote the activation of volunteer activities on the basic of the voluntaarry participation of local by investigating the utillzation of volunteers and analyzing this realted factors in the visiting health services of health centers. Subjects in this study were the whole 245 health centers. Data were collected from April, 12, 1999 to May, 31, 1999, and data for analyses were ones of 41 respondents, which answer " they utilize volunteers in visting health services". The summary of resulth was as follows : 1. In case that the type of health centers is one of county, in case that the number of staff in health center is over 80, in case that model business in not performed, in case that the degree of local financial independence is over 50%, and in case that the location of health centers in not good, there had more of volunteers in visiting health services. 2. 51.2% of analysing health centers answered that the utilization of volunteers has been done since 1998. 56.1% answered that they utillzed volunteers "over 4 hours" per person in a week. The average number of volunteers who was engaged at those visiting health services was 43.3. 3. Most of volinteers were housewives(73.2%). As for the action duration of volunteers. 68.8% answered " under 6 months". 4. As to the tasks of volunter activities, 75.6% were "home services" and 63.4% were "movable bath services". As for the tasks they intend to utilize, 90.2% answered " home services", 73.2% answered "movable health services". 5. Asked abount the purpose in utilizing volunteers, 75.6% answered "to induce the participation og local people". 65.9% answered "to provide various kinds of services". 66.7% provided some kind of education and training for volunteers. 6. Concerming evaluation of performance by volunteers, 90.2% answered " satisfactory". With reagards to the reason for that, 52.9% answered " volunteers can provide kinds of services". and 50.0% answered " volunteers can help local people to care their health". Purpose: This research was conducted to identify methods of inducing health promotion behavior, perceived health status, social participation and empowerment in the frail elderly receiving home visiting services. Methods: The subjects were 255 frail elders aged over 65 registered in the home visiting services of five public health centers in Daegu. Data were collected from June 9 to August 10, 2015. Data were analyzed by descriptive statistics, t-test, ANOVA, Scheffe's test and Pearson's correlation coefficient using SPSS Win 18.0. Results: The mean health promotion behavior, perceived health status, social participation and empowerment scores were 2.56 (${\pm}0.33$), 7.11 (${\pm}1.98$), 2.60 (${\pm}0.69$) and 2.90 (${\pm}0.29$), respectively. There was significant difference in health promotion behavior by client classification and life satisfaction. There were significant differences in perceived health status by life satisfaction, social participation by religion and client classification and empowerment by past jobs. Health promotion behavior, perceived health status, social participation and empowerment were positively correlated. Conclusion: An integrative health care program that includes these significant variables of subjects is essential to management and prevention of deterioration of frailty in elderly.
Purpose: This study was designed to determine the effects of health coaching and mediating variables on quantitative aspect of health in low-income hypertensive people. Methods: The experimental group for the current study consisted of 21 clients who received health coaching services, and the control group consisted of 22 clients who received home-visiting nursing services. Two groups received health coaching or home-visiting nursing services once a week for 8 weeks. The evaluation variables were self-efficacy, nutrition management, health behaviors, self-rated health, and quality of life. Results: The results revealed that the level of nutrition management was significantly higher in the experimental group than the control group (F=10.33, p=.005). Conclusion: These results confirm that health coaching is a useful strategy that encourages clients to continuously maintain their own health behaviors. Thus, the findings of the current study provide useful data for establishing measures for the health management of those afflicted with chronic disease, such as hypertension. Furthermore, health coaching may be developed into useful intervention strategies for dealing with chronic diseases and improving home-visiting nursing.
Purpose: The purpose of this study is to analyze health status of women working in traditional marketplace and their needs for public healthcare services. Methods: A descriptive survey of 500 women working at three traditional marketplaces was conducted. Results: street vendors' health status were much poorer than store merchants'. Furthermore, psychosocial factors like job stress and depression were increased in street venders. Thus, the public healthcare programs required by them included exercise programs (28.2%) and health checkup (26.8%). In addition, 31.2% of the participants reported that they needed visiting nursing care services. The rate of occupational health and safety or employment insurances was as low as 10.8%. Conclusion: Effective interventions including psychosocial factors for women workers at traditional marketplaces need to be developed based on these results. Also, it is recommended that public health care services such as outreach services and visiting nursing care services for women working at traditional marketplaces be provided. Furthermore, institutional provisions such as insurances for protecting these vulnerable groups' health are needed.
Purpose: This study attempted to estimate the need for home visiting nurse at public health centers. Methods: A model was generated to estimate a community's home visiting health service needs in 16 regions and a workload analysis was adopted to estimate the number of required Nurses. Data were collected from 16 public health centers using the South Korean government's open-information systems. Subjects were divided into three groups: vulnerable social group, bottom 10% income group, and bottom 20% income group. Results: The analysis revealed that 2,158 and 6,667 nurses were needed to provide home visiting health service for the bottom 10% and 20% income groups, respectively. It was estimated that for the vulnerable social group, 10,336 nurses were needed to provide home visiting health service, implying that the need-based demand for nurses is well over 5 times the number currently employed. Conclusion: The results indicate that the number of currently employed nurses is insufficient for the health management of vulnerable social groups. The government should consider active employment policies to encourage nurses to apply for home visiting health service.
This paper provides the guidelines from which to develop a visiting health care program at the Public Health Center in Korea and involves an expanded payment compensation system of preventive services based on the new long-term health insurance system in Japan. The function and management methods to achieve the goals practiced in a community contact center for elderly support which have recently been established will guide the specific directions and strategies that the Public Health Center should pursue. That is to say, comprehensive and continuous efforts will be put forth in preventive home visiting care targeting the elderly in certain jurisdictions. At this point in time in which the visiting care nursing program has not yet started, visiting health care provided by the Public Health Center oversees chronic diseases of a vulnerable population. But after it has been developed nationwide, the visiting health care system at the Public Health Center will be distinctive and focused on health promotion and prevention.
