Clinical Characteristics of Pertussis Epidemic in Changwon (창원에서 유행한 백일해의 임상적 특징)

  • Purpose: Pertussis can be prevented with a vaccine. Despite this, there have been an increasing number of cases worldwide, and also in Korea. This study aimed to investigate the epidemiology and clinical characteristics of the recent outbreak in the Changwon area. Methods: Patients who visited Changwon Fatima Hospital from July 2015 to March 2016 with respiratory symptoms, including spasmodic cough, cough induced vomiting, inspiratory 'intake' sound (whooping), and a night-time cough for >1 week were included in this study. Respiratory specimens were collected from patients and a polymerase chain reaction (PCR) and detected anti-pertussis immunoglobulin G enzyme-linked immunosorbent assay kit test were performed. Patients with underlying diseases, or those who had received a DTaP or Tdap vaccination in recent 1 year were excluded. Results: Pertussis was diagnosed in 37 of 50 patients, two patients were positive according to the PCR, and 37 patients were positive according to serologic tests. The age distribution of the patients was 1 month to 15 years. After administering antibiotics, all patients recovered without complications. Conclusions: A pertussis outbreak occurred in Changwon in 2015 and 2016. This data can provide the basis for further study on the epidemiology of pertussis in Korea.

A Study on the Health Status of Korean Vietnam Veterans' Children - A Result of Questionnaire Survey on Vietnam Veterans of Pusan Area Who Diagnosed as Cases by Korean Veteran's Hospital Diagnostic Criteria - (베트남전 참전자 2세의 건강상태에 관한 조사 - 부산지역 고엽제 위해증 환자를 대상으로 한 설문조사 결과 -)

  • Objectives : To propose the referential data to evaluate the health impacts of Vietnam veterans' children whose father were exposed to herbicides in Vietnam War. Methods : Vietnam veterans who visited to Pusan Veteran Hospital for medical care were recruited from April to October, 1998. They were 71 and asked about their own combat history, symptoms and illness, and health status of their 182 children. The informations were collected by direct and phone interview. Exposure estimation was also performed as exposure score depending on year and unit of participation, and personal episodes related to exposure to herbicide in the war. It classified into three groups; lower(<18.0), moderate(18-53), high$(\geq53)$ exposure group. Results : The mean age and the period into the combat of the veterans were 52.8 years and 15.0 months. The mean exposure score was $18.1{\pm}9.9$, and mainly distributed in lower (46.5%) and moderate(52.1%) exposure group. Most(90.1%) of them were diagnosed as sequelae(21 cases) and suspected sequelae(43 cases) of the herbicides by Korean veteran's hospital diagnostic criteria. The major sequelae was peripheral neuropathy 13 cases, chloracne 5 cases, and the major suspected sequelae was hypertension 20 cases, diabetes mellitus 18 cases, liver disease 12 cases, central neuropathy 11 cases, etc. About birth, 42.2% and 16.9% experienced spontaneous abortion and stillbirth, respectively. The mean exposure score was higher in stillbirth experience group(p<0.05). About half of the children(90 cases, 49.5%) hold the abnormal health status: those were skin pigmentation 38 cases, rash 23 cases, congenital anomaly 15 cases, general weakness 12 cases, purpura 8 cases, visual disturbance 8 cases, etc. These health problems had no association with father's exposure level(p>0.05). Conclusions : These results were depend on their own answers, and expectation for compensation did not excluded, therefore, this study may have limitations: inaccuracy of informations due to recall bias and response bias. Nevertheless, through this study, we could image the fundamental aspect for health impacts of Vietnam veterans' children for preparing the national control program and policy. A large scale epidemiologic study with valid exposure assessment on the health impacts of Vietnam veterans' children is recommneded.

A Pilot Study on the Practical Model of Nonverbal Communication for the Effective Information Services (정보조사제공에 있어서 비언어적 커뮤니케이션의 응용모형개발을 위한 실증적 연구)

  • Traditionally, the emphasis of the information services had been placed on the information provider's knowledge of information sources or information systems. However, in view of the increasing complexity of the information environment. and changes taking place in information professions the evaluation of the information services has callded for further research in interpersonal communication aspects of the librarian user interface especially that which links to user satisfaction. This experimental study was carried out for the purpose of exploring the implications of nonverbal behaviors in measuring user satisfaction in the light of the fact that the meaning of nonverbal clues includes more than verbal communication especially in dealing with variable barriers even though they operate at low levels of awareness. In this sense, among the wide range of nonverbal communication areas in particular kinesics, artifacts and the proximics were examined to suggest alternative model which can be applied effectively in our specific set of information work environment. Two major university libraries and two research libraries in Korea were chosen and visited for the anlysis and per each refernce/information interview the librarian's nonverbal behaviors were checked on the observer's checklist and the qustionnaires completed by the user at a time. On the basis of these data firstly, the relation of user satisfaction concerned with the librarian's nonverbal communication to user satisfaction with information services was analyzed and the possible communication barriers were investigated. In addition, the effect of training the information librarian with positive nonverbal communication skills on the user's satisfaction was analyzed in a SAS computer program in the area of kinesics. On the other hand, the effect of other nonverbal behaviors such as proximics and artifacts known very important clues to improving a complex human interaction was discussed along with their experimental results. In conclusion, it was found that most of users were not sensitive to the importance of nonverbal behaviors exposed by the information librarian, but it was proved that the ultimate user satisfaction and the satisfaction of the librarian's nonverbal behaviors are correlated positively, Moreover, nonverbal behaviors displayed by the trained librarian toward the user had a considerable effect on the user's evalution of information services compared with those by the untrained. These findings imply that in order to control the librarian's nonverbal behaviors and to help the user overcome his barriers the information librarian as a specialist needs to assure and modify willilingly his communication behavior. Without making effective use of such nonverbal communication, information librarian could not expect to meet the user's information needs either. However, when these positive nonverbal behaviors are applied it would be highly desirable to be aware of cultural and contextual differences. With regard to increasing information services success the applicable nonverbal communication model for library management and education can be suggested as below.

A study on the oral-health belief and oral-health care of company employees (직장인들의 구강건강신념과 구강건강관리 행태에 관한 연구)

  • The purpose of this study was to provide information on the development of an oral-health promotion program geared toward office workers. The subjects in this study were 240 employees at the headquarters of S construction company. After a survey was conducted in July and August 2008, the answer sheets from 208 respondents were gathered, and 191 answer sheets were analyzed except 17 unanalyzable ones. The findings of the study were as follows: 1. In regard to subjective oral health status, the largest number of the participants that accounted for 37.2% found their mouth to be neither healthy nor unhealthy. The greatest number of them that represented 58.1% had never visited a dentist's office to receive preventive treatment. 2. Regarding relationship between toothbrushing education experience and toothbrushing frequency, the most common toothbrushing frequency was three times a day( 46.5%) among the company employees who had ever received that education. In the event of the office workers without that experience, the most prevalent frequency was three times(63.6%). The toothbrushing frequency was statistically significantly different according to the toothbrushing education experience(p<0.05). As to the relationship of toothbrushing education experience to tooth- brushing method, the largest number(48.2%) of those who had ever received that education brushed their upper and lower teeth along with the gums by rotating the toothbrush downward from the top and upward from the bottom respectively. In the event of the office workers without that experience, 49.4 percent brushed their teeth and gums by rotating the toothbrush up and down. Thus, whether they had ever received education about toothbrushing made a statistically significant difference to their toothbrushing method(p<0.05). 3. As a result of evaluating their oral health belief by utilizing a five-point scale, they got a mean of 2.95. In each area of oral health belief, they got the best marks in benefit(3.66), salience(3.42), barrier(2.84), susceptility(2.58) and seriousness(2.23). Given the above-mentioned finding of the study, two sorts of oral health beliefs, which are benefit and salience, should be stressed in the development of oral health education programs that cater to company employees. And sustained research efforts should be channeled into finding out the relationship between oral health and actual oral health care.

The Effectiveness of Electroglottographic Parameters in Differential Diagnosis of Laryngeal Cancer (후두암 감별진단에 있어 성문전도(Electroglottograph) 파라미터의 유용성)

  • Background and Objectives : Electroglottography(EGG) is a non-invasive method of monitoring the vocal cord vibration by measuring the variation of physiological impedance across the vocal folds through the neck skin. It reveals especially the vocal fold contact area and is widely used for basic laryngeal researches, voice analysis and synthesis. The purpose of this study is to investigate the effectiveness of EGG parameters in differential diagnosis of laryngeal cancer. Materials and Methods : The author investigated 10 laryngeal cancer and 25 benign laryngeal disease patients who visited at the Department of Otolaryngology, Pusan National University Hospital. The EGG equipment was devised in the author's Department. Among various parameters of EGG, closed quotient(CQ), speed quotient(SQ), speed index(SI), Jitter, Shimmer, Fo were determined by an analysis program made with MATLAB 6.5$^{\circledR}$(Mathwork, Inc.). In order to differentiate various laryngeal diseases from pathologic voice signals, the author has used the electroglottographic parameters using the neural network of multilayer perceptron structure. Results : SQ, SI, Jitter and Shimmer values except those of CQ and Fo showed remarkable differences between benign and malignant laryngeal disease groups. From the artificial neural network, the percentage of differentiating the laryngeal cancer was over 80% in SQ, SI, Jitter, Shimmer except for CQ and Fo. These results indicated that it is possible to discriminate the benign and malignant laryngeal diseases by EGG parameters using the artificial neural network. Conclusion : If parameters of EGG which can reveal for the pathology of laryngeal diseases are additionally developed and the current classification algorithm is improved, the discrimination of laryngeal cancer will become much more accurate.

Artificial Neural Network System in Evaluating Cervical Lymph Node Metastasis of Squamous Cell Carcinoma (편평세포암종 임파절 전이에 대한 인공 신경망 시스템의 진단능 평가)

  • Purpose: The purpose of this study was to evaluate cervical lymph node metastasis of oral squamous cell carcinoma patients by MRI film and neural network system. Materials and Methods: The oral squamous cell carcinoma patients(21 patients. 59 lymph nodes) who have visited SNU hospital and been taken by MRI. were included in this study. Neck dissection operations were done and all of the cervical lymph nodes were confirmed with biopsy. In MR images. each lymph node were evaluated by using 6 MR imaging criteria(size. roundness. heterogeneity. rim enhancement. central necrosis, grouping) respectively. Positive predictive value. negative predictive value. and accuracy of each MR imaging criteria were calculated. At neural network system. the layers of neural network system consisted of 10 input layer units. 10 hidden layer units and 1 output layer unit. 6 MR imaging criteria previously described and 4 MR imaging criteria (site I-node level II and submandibular area. site II-other node level. shape I-oval. shape II-bean) were included for input layer units. The training files were made of 39 lymph nodes(24 metastatic lymph nodes. 10 non-metastatic lymph nodes) and the testing files were made of other 20 lymph nodes(10 metastatic lymph nodes. 10 non-metastatic lymph nodes). The neural network system was trained with training files and the output level (metastatic index) of testing files were acquired. Diagnosis was decided according to 4 different standard metastatic index-68. 78. 88. 98 respectively and positive predictive values. negative predictive values and accuracy of each standard metastatic index were calculated. Results: In the diagnosis of using single MR imaging criteria. the rim enhancement criteria had highest positive predictive value (0.95) and the size criteria had highest negative predictive value (0.77). In the diagnosis of using single MR imaging criteria. the highest accurate criteria was heterogeneity (accuracy: 0.81) and the lowest one was central necrosis (accuracy: 0.59). In the diagnosis of using neural network systems. the highest accurate standard metastatic index was 78. and that time. the accuracy was 0.90. Neural network system was more accurate than any other single MR imaging criteria in evaluating cervical lymph node metastasis. Conclusion: Neural network system has been shown to be more useful than any other single MR imaging criteria. In future. Neural network system will be powerful aiding tool in evaluating cervical node metastasis.

Distribution of the lingual foramina in mandibular cortical bone in Koreans

  • Objectives: The interforminal region, between the mandibular foramen, is known as a relatively safe area that is free of anatomic structures, such as inferior alveolar nerve, submandibular fossa, and lingual side of the mandible is occasionally neglected for its low clinical importance. Even in the case of a severely constricted alveolus, perforation of the lingual cortical bone had been intended. However, anterior extension of the inferior alveolar canal, important anatomic structure, such as concavity of lingual bone, lingual foramina, and lingual canal, has recently been reported through various studies, and untypical bleeding by perforation of the lingual plate on implantation has also been reported. Therefore, in this study, we performed radiographic and statistical analysis on distribution and appearance frequencies of the lingual foramina that causes perforation of the mandibular lingual cortical bone to prevent complications, such as untypical bleeding, during surgical procedure. Materials and Methods: We measured the horizontal length from a midline of the mandible to the lingual foramina, as well as the horizontal length from the alveolar crest to the lingual foramina and from the lingual foramina to the mandibular border by multi-detector computed tomography of 187 patients, who visited Dankook University Dental Hospital for various reasons from January 1, 2008 to August 31, 2012. Results: From a total of 187 human mandibles, 110 (58.8%) mandibles had lingual foramina; 39 (20.9%) had bilateral lingual foramen; 34 (18.2%) had the only left lingual foramen; and 37 (19.8%) had the only right lingual foramen. Conclusion: When there is consistent bleeding during a surgical procedure, clinicians must consider damages on the branches of the sublingual artery, which penetrate the lingual foramina. Also, when there is a lingual foramina larger than 1 mm in diameter on a pre-implantation computed tomography, clinicians must beware of vessel damage. In order to prevent these complications and progress with a safe surgical procedure, a thorough radiographic examination before the surgery is indispensable. Further, clinicians should retract lingual flap definitely to confirm the shape of the lingual bone and existence of the lingual foramina.

A STUDY ON THE TRAUMATIC INJURIES TO PRIMARY TEETH (유치 외상에 관한 연구)

  • The purpose of this study was to research the traumatized primary teeth and investigate following factors: sex, age, cause, place and time(of the year) of injury, elapsed time, area and type of injury, dental treatment and prognosis. The analysis includes total of 1533 traumatized primary teeth from 758 children aging from 6 months to 6 years(mean age: 2.8) from 2003 to 2007. The result follows : 1. The children of age between 1-2 and 2-3 were involved in the largest number of injuries in both sexes with boy/girl ratio of 1.77:1(p<0.001). 2. Fall and collision were the main causes of traumatic dental injury, especially in younger children(p<0.05). Places of injury occurrence varied: home, outdoors, and kindergarten. Warm climate accounts for frequent outdoor injuries in May, September and October(p<0.001). 3. Most of the children visited dental clinic within 24 hours of the injury(77.6%). From March to September, dental trauma occurrences were distributed evenly, except for Winter period(p<0.001). 4. Upper central incisors were the most commonly affected teeth, and the injuries usually involved 1 tooth or 2 teeth. 5. Periodontal tissue injuries dominated and subluxation was the most common type. Lateral luxation, enamel fracture, intrusion and root fracture followed.

Full Mouth Rehabilitation in a Patient with Limited Restorable Space (수복 공간이 부족한 환자에서의 완전구강회복)

  • Loss of posterior support may cause overloading and excessive wear of remaining teeth. Moreover, the extrusion of antagonistic teeth leads to the destruction of the occlusal plane. The loss of vertical dimension of occlusion (VDO) also emerges clinically, which may bring the loss of esthetic appearance and function. These patients who suffer from the loss of posterior support, often require vigorous periodontal treatments (osteotomy, crown lengthening) and extensive oral rehabilitation. Sixty three years old female patient visited for the prosthetic treatment of the posterior edentulous area. She had no other systematic disease and parafuctional habits for prosthetic treatment. Intraoral and radiographic examinations were done. The evaluation of VDO and vertical dimension of rest position were evaluated for proper prosthetic procedures and diagnostic wax up was done. As a result of diagnosis, VDO was increased by 2 mm considering the loss of VDO and space for the prosthetic treatment. After the pretreatments, initial preparation of teeth and provisionalization were carried out. Six weeks later of provisionalizaion, final preparation and impression was performed. Using the duralay resin copings, jaw relation was registered. The master cast was mounted and definitive restoration was fabricated. After the evaluation of esthetic and function, pick up impression for clinical remounting was done. Lucia jig was made for new jaw relation and occlusal adjustment on the articulator. Definitive restoration was delivered and the patient was periodically recalled for additional occlusal adjustment. From this case, the satisfactory functional and esthetic results through full mouth rehabilitation with increase vertical dimension were achieved.

A Result of Field Survey for Ergonomic Work Risk Factors in General Hospital (의료업종의 작업위험요인에 대한 실태조사 결과)

  • The purposes of this research are to survey work-related risk factors of musculoskeletal disorders(MSDs) in various departments and tasks at general hospitals in non-manufacturing sectors, and to use basic data derived from the survey results in preventing work-related MSDs in hospital workers. Investigation started in March of 2006 and continued for 6 months in 220 general hospitals at Seoul, Incheon, Kyeongi, and Kangwon area. Investigators visited and interviewed workers in hospitals to identify the presence of tasks of musculoskeletal burden, the investigation results of ergonomic risk factors required by the Occupational Safety and Health Law, statistical analysis from questionnaire for musculoskeletal symptoms, and major departments and tasks that have such risk factors. Twenty-seven percents of hospitals finished the investigation of ergonomic risk factors, and 69% did not do the investigation while remained 4% did not have such factors in their hospitals. The rank order of major departments that had such musculoskeletal burdens was kitchen rooms of 143, managing departments/computer rooms/dispensaries of 137, physical treatment rooms of 109, nursing departments of 96, radiological and clinical laboratories of 63. Eighteen hospitals that did not hold legal duties by the section 148 of labor minister decree practiced prevention programs of MSDs according to the labor-management cooperation. Nursing departments ranked in the first place for the numbers of musculoskeletal symptoms of 438. Managing departments/computer rooms/dispensaries, kitchen rooms, and medical treatment departments held 127, 52 and 45 symptoms, respectively. The magnitude order of physical symptom areas followed shoulder of 185, backs and waist of 166, hand wrists of 120, necks of 110, and legs/feet of 106. The departments and jobs that had major work-related ergonomic risks were patient transporting, central supplying, patient nursing (moving patients into wheel-chairs, changing of patient posture and sheet alteration), manual transporting, operation, and managing/computer departments.
