Ultrasonographic Changes of Acute Renal Failure Induced by Gentamicin in Dogs (개에서 겐타마이신으로 유발된 급성 신부전의 초음파상 변화)

  • Present study was undertaken in order to document early renal ultrasonographic changes of gentamicin nephrotoxicosis and to show the value of renal ultrasonography as a contributory means of early diagnosis of acute renal failure in dogs. The experimental design was a randomized complete block design with six treatments in two blocks (gentamicin-treated & saline-treated). Acute renal failure was induced by toxic dosage of gentamicin (30 mg/kg) and saline solution sham equivalent in volume to that of the toxic dosage of gentamicin (1.5-3ml). Subjective visualization of increased renal cortex was visible as homogenous echoes that were hypoechoic relative to the surrounding tissues, whereas the renal medulla was anechoic to slightly hypoechoic. After treatment, the renal cortex was hyperechoic relative to the surrounding tissue. Increased renal cortex echogenicity was associated with significant nephrotoxicosis and was superior to serum creatinine elevation in nephrotoxicosis detection. Urine GGT was superior to other clinicopathological data utilized in the diagnosis of nephrotoxicosis. Based on the above results, increased renal cortex echogenicity seemed to be of use in detecting of acute renal failure.

Diagnosis and Clinical Staging of Head and Neck Cancer (두경부암의 진단과 임상적 병기분류)

  • Cancer of the head and neck is an uncommon disease accounting for 5 % of all cancers. In an anatomic area so readily visible and palpable for examination without special and expensive diagnositic tools, it is unfortunate that many patients still present with advanced diseases. Since the prognosis is so intimately related to stage of disease, it is very important to detect the earliest stage of cancer with a complete head and neck examination. In the evaluation of cancer at any anatomic site, the description of the extent of the lesion is important. Not only does proper staging of the tumor lead to make decision of the most appropriate treatment, it also serves as a guide for the results of treatment. Proper staging demands a careful clinical assesment of the extent of the cancer. The current staging system for head and neck cancer uses the TNM system devised by American Joint Committee for Cancer Staging and End Result Reporting. T represent the primary tumor, N, regional nodal metastases, and M, distant metastases. The detection, diagnosis, and appropriate treatment of eary cancer will result in improved survival.

  • For fault detection about the abrasion stage of rotational machineries under the dynamic loading conditions unlike the traditional diagnosis method used in the past decade, the infrared thermographic method with its distinctive advantages in non-contact, non-destructive, and visible aspects is proposed. In this paper, by applying a rotating deep-grooved ball bearing, passive thermographic experiments were conducted as an alternative way to proceeding the traditional fault monitoring on spectrum analyzer. As results, the thermographic experiment was compared with the traditional vibration spectrum analysis to evaluate the efficiency of the proposed method. Based on the results obtained as NDT, the temperature characteristics and abnormal fault detections of the ball bearing according to the abrasion stage were analyzed.

Accuracy of Preoperative Urinary Symptoms, Urinalysis, Computed Tomography and Cystoscopic Findings for the Diagnosis of Urinary Bladder Invasion in Patients with Colorectal Cancer

  • Background: To determine the accuracy of preoperative urinary symptoms, urinalysis, computed tomography (CT) and cystoscopic findings for the diagnosis of urinary bladder invasion in patients with colorectal cancer. Materials and Methods: Records of patients with colorectal cancer and a suspicion of bladder invasion, who underwent tumor resection with partial or total cystectomy between 2002 and 2013 at the Faculty of Medicine Siriraj Hospital, were reviewed. Correlations between preoperative urinary symptoms, urinalysis, cystoscopic finding, CT imaging and final pathological reports were analyzed. Results: This study included 90 eligible cases (71% male). The most common site of primary colorectal cancer was the sigmoid colon (44%), followed by the rectum (33%). Final pathological reports showed definite bladder invasion in 53 cases (59%). Significant features for predicting definite tumor invasion were gross hematuria (OR 13.6, sensitivity 39%, specificity 73%), and visible tumor during cystoscopy (OR 5.33, sensitivity 50%, specificity 84%). Predictive signs in CT imaging were gross tumor invasion (OR 7.07, sensitivity 89%, specificity 46%), abnormal enhancing mass at bladder wall (OR 4.09, sensitivity 68%, specificity 66%), irregular bladder mucosa (OR 3.53, sensitivity 70%, specificity 60% ), and loss of perivesical fat plane (OR 3.17, sensitivity 81%, specificity 43%). However, urinary analysis and other urinary tract symptoms were poor predictors of bladder involvement. Conclusions: The present study demonstrated that the most relevant preoperative predictors of definite bladder invasion in patients with colorectal cancer are gross hematuria, a visible tumor during cystoscopy, and abnormal CT findings.

  • Viroids are about 250-400 base pair of short single strand RNA fragments have been associated with economically important plant diseases. Due to the lack of protein expression capacity associated with replication, it is very difficult to diagnosis viroid diseases in serological methods. For detecting viroid at plants, molecular-based techniques such as agarose gel electrophoresis, polyacrylamide gel electrophoresis (PAGE), DNA-hybridization, blotting analysis and conventional RT-PCR are reliable. Real-time RT-PCR methods that grafted on RT-PCR methods with improved confirmation methods have been also utilized. However, they are still labor-intensive, time-consuming, and require personnel with expertise. Loop-mediated Isothermal Amplification (LAMP) method is a nucleic acid amplification method under the isothermal condition. The LAMP methodology has been reported to be simple, rapid, sensitive and field applicable in detecting a variety of pathogens. The results of LAMP method can be colorized by adding a visible material such as SYBR green I, Evagreen, Calcein, Berberine and Hydroxy naphthol blue (HNB) with simple equipment or naked eyes. The combination of LAMP method and nucleic pathogens, viroids, can be used to realize simple diagnosis platform for the genetic point-of care testing system. The aim at this review is to summary viroid-caused diseases and the simple visible approach for diagnosing viroids using Loop-mediated Isothermal Amplification (LAMP) method.

Hierarchical Non-Rigid Registration by Bodily Tissue-based Segmentation : Application to the Visible Human Cross-sectional Color Images and CT Legs Images (조직 기반 계층적 non-rigid 정합: Visible Human 컬러 단면 영상과 CT 다리 영상에 적용)

  • Non-rigid registration between different modality images with shape deformation can be used to diagnosis and study for inter-patient image registration, longitudinal intra-patient registration, and registration between a patient image and an atlas image. This paper proposes a hierarchical registration method using bodily tissue based segmentation for registration between color images and CT images of the Visible Human leg areas. The cross-sectional color images and the axial CT images are segmented into three distinctive bodily tissue regions, respectively: fat, muscle, and bone. Each region is separately registered hierarchically. Bounding boxes containing bodily tissue regions in different modalities are initially registered. Then, boundaries of the regions are globally registered within range of searching space. Local boundary segments of the regions are further registered for non-rigid registration of the sampled boundary points. Non-rigid registration parameters for the un-sampled points are interpolated linearly. Such hierarchical approach enables the method to register images efficiently. Moreover, registration of visibly distinct bodily tissue regions provides accurate and robust result in region boundaries and inside the regions.

  • Titania has become the most applicable material for photocatalytic application. Nevertheless, titania has the weak point in its wide band gap energy that is mainly activated by UV irradiation. There have been vast research challenges in order to make the wide band gap energy of titania narrow that could be activated in the presence of visible light. Various modifications of titania surface were popular because titania needs to change its surface to respond in visible light. Among the methodological approaches, N-doping to titania can be the alternative candidate because it is facile process and eco-friendly. The activated electron from valence band in N-doped $TiO_2$ migrates to conduction band in the presence of visible light irradiation, which shows photocatalytic activity as well. In this study, focused on the evaluation of nitrogen state after N-doping through brief review. Arguments are still existed in nitrogen states and their different effects on photocatalytic activity. In particular, two nitrogen states are generally reported; substitutional and interstitial states. The research articles regarding N-doped $TiO_2$ are continuously appearing because the potential application of water split in visible light is still fascinate. The future of N-doped $TiO_2$ is also presented by referrals based on various literature.

Use of laser fluorescence device 'DIAGNODent$^{(R)}$' for detecting caries (레이저 우식진단기기 'DIAGNODent$^{(R)}$'의 활용)

  • The detection of carious lesions is a key point to apply appropriate preventive measures or operative treatment of dental caries. A laser fluorescence device DIAGNOdent$^{(R)}$ (KaVo, Biberach, Germany) has also been shown to be of additional clinical value in the detection of initial caries. This report focus on the DIAGNOdent$^{(R)}$ for caries detection. DIAGNOdent$^{(R)}$ irradiate visible red light at a wavelength of 655 nm to elicit near-infrared fluorescence from caries lesion. This device is known as a reproducible method for caries detection, with good sensitivity and specificity especially for caries detection on occlusal and accessible smooth surfaces. DIAGNOdent$^{(R)}$ tended to be more sensitive method of detecting occlusal dentinal caries, however, showed more false-positive diagnoses than the visual inspection. So Clinician should not use the device as a clinician's primary diagnostic method and it is recommended that the device should be used in the decision-making process in relation to the diagnosis of caries as a second opinion in cases of doubt after visual inspection. The trend of modern dentistry would be a preventive approach rather than invasive treatment of the disease. This is possible only with early detection and respective preventive measures, DIAGNOdent$^{(R)}$ can help the changes.

Fiber optic smart monitoring of concrete beam retrofitted by composite patches

  • In order to extend the lifetime of buildings and civil infrastructure, patch type fibrous composite retrofitting materials are widely used. Retrofitted concrete columns and beams gain stiffness and strength, but lose toughness and show brittle failure. Usually, the cracks in concrete structures are visible to the naked eye and the status of the structure in the life cycle is estimated through visual inspections. After retrofitting of the structure, crack visibility is blocked by retrofitted composite materials. Therefore, structural monitoring after retrofitting is indispensable and self diagnosis method with optical fiber sensors is very useful. In this paper, we try to detect the peel out effect and find the strain difference between the main structure and retrofitting patch material when they separate from each other. In the experiment, two fiber optic Bragg grating sensors are applied to the main concrete structure and the patching material separately at the same position. The sensors show coincident behaviors at the initial loading, but different behaviors after a certain load. The test results show the possibility of optical fiber sensor monitoring of beam structures retrofitted by the composite patches.

Primary Renal Hydatid Cyst: Mis-Interpretation as a Renal Malignancy

  • Primary renal echinococcosis, a rare disease involving the kidney, accounts for 2-3% of human echinococcosis. A 64-year-old female patient from Uzbekistan presented with complaints of left flank pain. A CT scan revealed a cystic mass in the upper to midpole of the left kidney. We regarded this lesion as a renal malignancy and hand-assisted laparoscopic radical nephrectomy was performed to remove the renal mass. The mass consisted of a large unilocular cyst and multiple smaller cysts without any grossly visible renal tissue. The final pathologic diagnosis was a renal hydatid cyst. For patients from endemic areas, hydatid cyst should be included in the differential diagnosis. Here, we present a case of renal hydatid cyst in a female patient who relocated from Uzbekistan to Korea.
