Usefulness of Motor-Evoked Potentials Monitoring for Neurosurgical Treatment of an Unusual Distal Anterior Choroidal Artery Aneurysm

  • A 35 years old woman presented with an acute meningeal syndrome following an intra ventricular haemorrhage without subarachnoid haemorrhage. The angiography demonstrated a 6 mm partially thrombosed saccular aneurysm at the plexal point of the right anterior choroidal artery (AChoA). It was surgically approached inside the ventricle through a trans-temporal corticotomy. The aneurysm was excised after distal exclusion of the feeding artery under motor-evoked potentials monitoring. Of the 19 cases of distal AChoA aneurysm neurosurgical treatment, this is the only one performed under electrophysiology monitoring, a simple and safe method to detect and prevent motor tract ischemia. We discuss this rare case, along with a comprehensible review of the literature of the previous surgical cases of distal AChoA aneurysms.

Depolarization and Repolarization in the Three Dimensional Cardiac Ventricular Model (3차원 심실모델에서의 탈분극과 재분극)

  • This paper describes the construction of an anisotropic three dimensional ventricular model based on the bidomain model. The cardiac activation process in the normal cardiac cell and the myocardial ischemic cell are simulated by the Huygen's principle. The depolarization process in the myocardial ischemia displays the delayed activation compared to the normal state. The repolarization process is simulated by the myocardial potential at the arbitrary ellapsed time after activation process. Using the potential data, the equivalent cardiac source at the arbitrary time can be computed. In conclusion, this simulation suggests the possibilities of the depolarization and the repolarization process in the normal and abnormal myocardiac cells.

  • 심실세동은 환자의 생명을 위협하는 가장 치명적인 부정맥으로서, 심실세동의 발견즉시 특별한 조치를 취하지 못할 경우 환자는 급사한다. 심실세동을 정지시키는 유일한 방법은 전기적 제세동이며 이는 환자의 심장 부위에 전기적 에너지를 가해 정상 심장 리듬을 되찾도록 하는 방법이다. 심실세동의 발생은 예측할 수 없고, 이를 진단하기 위한 심전도 교육을 모든 의료인에게 할 수 없으므로 심전도를 자동 분석하여 심실세동을 조기에 검출하는 자동 분석장치의 개발은 심실세동에 의한 급사를 줄이는데 매우 중요하다. 본 연구에서는 교차간섭성분의 제거 능력이 뛰어나며 쉽게 구사할 수 있는 Choi-Williams distriguteion (CWD) 방법을 이용하여 급사를 초래할 수 있는 심실세동을 자동 검출하는 알고리즘을 개발하였다. 그리고 개발된 알고리즘의 성능을 검증하기 위하여 본 논문에서는 두 그룹의 심실세동 데이터를 사용하였다. 검출 알고리즘은 심실세동의 시간 및 주파수 특성을 일곱가지 조건들에 대한 만족 여부를 이용하여 심실세동을 판정하였고, 실험의 민감도와 선택도는 각각 92.1%, 97 .2% 이다.

Neonatal arrhythmias: diagnosis, treatment, and clinical outcome

  • Arrhythmias in the neonatal period are not uncommon, and may occur in neonates with a normal heart or in those with structural heart disease. Neonatal arrhythmias are classified as either benign or nonbenign. Benign arrhythmias include sinus arrhythmia, premature atrial contraction, premature ventricular contraction, and junctional rhythm; these arrhythmias have no clinical significance and do not need therapy. Supraventricular tachycardia, ventricular tachycardia, atrioventricular conduction abnormalities, and genetic arrhythmia such as congenital long-QT syndrome are classified as nonbenign arrhythmias. Although most neonatal arrhythmias are asymptomatic and rarely life-threatening, the prognosis depends on the early recognition and proper management of the condition in some serious cases. Precise diagnosis with risk stratification of patients with nonbenign neonatal arrhythmia is needed to reduce morbidity and mortality. In this article, I review the current understanding of the common clinical presentation, etiology, natural history, and management of neonatal arrhythmias in the absence of an underlying congenital heart disease.

  • In our research, we will extract diagnostic parameters by LPC method and wavelet transform. Then, we will design artificial neural network which is based on RBF that can express input features in terms of fuzzy. Because PVC(Premature Ventricular Contraction) has possibility to cause heart attack, the detection of PVC is a very significant problem. To deal with this problem, LPC method which gives different coefficients or different morphologies and wavelet transform which has superior localization nature of time-frequency, are used to extract effective parameters or classification of normal and PVC. Because RBF network can allocate an input feature to the membership degree of each category, total system will be more flexible.

  • The two dimnesional echocardiography is widely used to evaluate regional wall motion abnormaility, because of its abilities to depict left ventricluar wall motion. A new method, color kinesis is a technology or echocardiographic assessment of left ventricular wall motion. In this paper, we proposed a algorithm or color kinesis which is based on acoustic quantification and automatically detects endocardial motion during systole on a frame-by-frame basis. The echocardiograms were obtained in the short-axis views in normal subjects. Automated edge detection and endocardial contour tracing algorithm was applied to each frames, quantitative analysis based on segmentation was performed, and pre-defined color overlays superimposed on the gray scale images. Segmental analysis of color kinesis provided automated, quantitative diagnosis of regional wall motion abnormality.

Surgical Correction of Double-outlet Right Ventricle with Atrioventricular Discordance - A case report - (심방심실착위를 동반한 양대혈관 우심실기시증의 수술치험 1례)

  • Within the group of congenital cardiac anomalies manifesting atrioventricular discordance, there exists a subset of hearts characterized by the additional presence of double outlet of the morphological right ventricle. Most of these hearts have associated pulmonary stenosis and abnormal direction of the cardiac apex, and all have a ventricular septal defect. Recently, a 10-year old boy underwent successful surgical correction of double outlet right ventricle with atrioventricular discordance. Diagnostic confirmation was made by 2-D echocardiography, cardiac catheterization and cineangiocardiography. The operation consisted of patch closure of ventricular septal defect so as to leave both great arteries draining the systemic[morphologically right] ventricle, and insertion of an extracardiac valved conduit between the morphological left ventricle and the distal end of the proximally oversewn pulmonary artery. Postoperative course was uneventful.

  • A 23-year-old male patient complained dyspnea on exertion and orthopnea since December 1977. On examination, he was tall and slender. There was grade IV/VI to-and-fro murmur on the left sternal border especially on Erb`s point. The liver was descended 2 fingers breadth below right costal margin. There were no signs of Marfan`s syndrome. Echocardiography demonstrated partial closure of aortic valve and dilated aortic root with enlargement of ascending aorta. Left heart cardiac catheterization revealed moderately elevated pulmonary wedge pressure and right ventricular pressure. The left ventricular end diastolic pressure was markedly elevated to 26 mmHg. On aortography, the aortic regurgitation was severe and it was belonged to angiographically Grade IV. The aortic valve was replaced with Carpentier-Edwards valve without excision and replacement of ascending aorta, under the impression of rheumatic valvular heart disease. After closure of aortotomy, blood pressure was transiently elevated and bleeding from the site of inserting air vent needle of ascending aorta was developed. The bleeding was not controlled by any means. On postmortem microscopic study, the histologic changes were strikingly limited to the ascending aorta from the region of the aortic valve ring.

  • Corrected transposition of great arteries associated with dextrocardia [I.D.D.] is a very rare congenital cardiac malformation in which the atria and ventricles are in discordant relation, as are the ventricles and great arteries, but the aorta is to the right of the pulmonary artery. A 13 year old male patient who complained cyanosis and dyspnea on exertion for 8 years, was diagnosed as congenitally corrected transposition of great arteries associated with dextrocardia [I.D.D.], large ventricular septal defect, patent foramen ovale and pulmonary stenosis. He was operated on Nov. 22 1978. The ventricular septal defect was closed with Teflon felt and the pulmonary hypoplasia was corrected with Rastelli operation successfully. On 34th postoperative days, complete heart block was occurred and permanent epicardial pacemaker was implanted with good result.

Ventricular Septal Defect Associated With Patent Ductus Arteriosus -A Report Of 7 Cases- (개방성 동맥관 합병 심실중격결손증 -7례 보고-)

  • Ventricular septal defect associated with patent ductus arteriosus is well known cardiac anomaly that can be treated successfully by operation. This anomaly is found at low frequency. Seven cases of the combined malformation were operated on during the period from 1959 to 1978 in the Department of Thoracic Surgery, Seoul National University Hospital. Right heart catheterization and retrograde aortogram can diagnose the disease correctly. Staged operation in one case and one-stage operation in 5 patients were done. In all cases of the open heart surgery the closure of the ductus was done by transpulmonary suture of the PDA opening. Among 6 patients of the open heart surgery 2 operative death [33.3%] and one late death [total 50%] were noted. Four type II and one type I and one Type IV VSDs were found. In one case the anomaly was accompanied with aortic insufficiency by herniation of the right coronary cusp through Type I VSD.
