성인의 좌심방과 좌심실 크기변화에 미치는 영향 요인 분석 (Analysis of Factors Influencing Changes in Left Atrium and Left Ventricle Size in Adults)

  • This study analysed the factors that predict and influence heart disease through key indicators related to changes in left atrial and left ventricular size. Measurements recommended by the American Society of Echocardiography were used, and the influence of variables was assessed using multiple regression analysis. The results showed that left atrial volume index(LAVI) was significantly different by age, obesity, diabetes, hypertension, dyslipidaemia, and left ventricular relaxation dysfunction(p<0.05). Left ventricular mass index(LVMI) was significantly different according to age, body mass index, hypertension, diabetes, dyslipidaemia, and left ventricular relaxation dysfunction(p<0.05). Increases in LVMI and relative ventricular wall thickness(RWT) were associated with changes in LAVI(p<0.05). Age, systolic blood pressure, increased LAVI, and RWT influenced changes in LVMI, and left ventricular dysfunction was analysed as an influencing factor for both changes in LAVI and LVMI. Therefore, changes in left atrial and left ventricular size are indicators for early diagnosis and prevention of heart disease, and it is necessary to carefully observe structural changes in the heart and actively manage risk factors for the prevention and management of heart disease.

잔존성 심실중격결손증5례 보고 (Residual ventricular septal defect: report of 5 cases)

  • We had operated 5 cases of residual ventricular septal defect after primary repair of isolated ventricular septal defect from 1975 to 1981. Three were male and two were female. Their ages ranged from 7 to 25 years old. In 4 cases previous patch was detached and one case had left ventricular - right atrium communication. They had had residual symptoms after primary operation and chest x-ray showed cardiomegaly. Cardiac catheterization was performed in all cases. Residual left to right shunt was more than 1.4: 1 with pulmonary hypertension. Their postoperative course were uneventful and long term follow up revealed that they were good health until one to 4 years.

  • The authors present a case of multiple intracranial calcifications after the procedure of external ventricular drain placement in a 50-year-old man with pericallosal artery aneurysm. We believe that calcifications formed dust that had fallen into the track during the external ventricular drain procedure. The clinical features and radiological findings are presented with review of literature.

  • Because of recent advances in cardiac surgery and diagnostic techniques, left ventricular-right atrial communication has been reported with increasing frequency. Recently we experienced 2 cases of left ventricular-right atrial communication, which were corrected surgically. Preoperative diagnosis was incorrect In both cases as ventricular septal defect. The type of defect was supravalvular in case 1 and case 2. Both cases were successfully closed with the aid of extracorporeal circulation and discharged with good results.

  • After a penetrating thoracic injury early detection of intracardiac injury and early surgical repair when indicated are essential. A case presenting severe respiratory distress two weeks after a penetrating thoracic injury is reported. Transesophageal echocardiography showed massive pericardial effusion ventricular septal defect and mirtal regurgitation, The infundibular ventricular septal perforation was repaired using a Dacron patch the anterior mitral leaflet by interrupted sutures and the ruptured chordae of the posterior leaflet by a new chordae formation.

  • Radionuclide cardioangiography has been widely applied and has played major roles in non-invasive assessment of cardiac function. Three techniques, first-pass, gated first and gated equilibrium methods, have commonly been used to evaluate right ventricular ejection fraction which usually abnormal in the patients with cardiopulmonary disease. It has been known that the gated first pass method is the most accurate method among the three techniques in assessment of right ventricular ejection fraction. The radionuclide right ventricular ejection fraction values were determined in 13 normal subjects and in 15 patients with chronic obstructive pulmonary disease by the gated first pass method and compared with those of the first pass method because there ha,j been no published data of right ventricular ejection fraction by the gated first pass method in Korea. The values of right ventricular ejection fraction by the gated first pass method were compared with the detas from the pulmonary function test performed in the patients with chronic obstructive pulmomary disease. The results were as follows: 1) The values of right ventricular ejection fraction by the gated first pass method were $50.1{\pm}6.1%$ in normal subjects and $38.5{\pm}8.5$ in the patients with chronic obstructive pulmonary disease. There was statistically significant difference between the right ventricular ejection fraction of each of the two groups (p < 0.05). 2) The right ventricular ejection fraction by the gated first pass method was not linearly correlated with $FEV_1$, VC, DLCO, and FVC as well as $P_aO_2$ and $P_aCO_2$ of the patients with chronic obstructive pulmonary disease. We concluded that right ventricular ejection fraction by the gated first pass method using radionuclide cardioangiography may be useful in clinical assessment of the right ventricular function.

Central-Approach Surgical Repair of Coarctation of the Aorta with a Back-up Left Ventricular Assist Device for an Infant Presenting with Severe Left Ventricular Dysfunction

  • A two-month-old infant presented with coarctation of the aorta, severe left ventricular dysfunction, and moderate to severe mitral regurgitation. Through median sternotomy, the aortic arch was repaired under cardiopulmonary bypass and regional cerebral perfusion. The patient was postoperatively supported with a left ventricular assist device for five days. Left ventricular function gradually improved, eventually recovering with the concomitant regression of mitral regurgitation. Prompt surgical repair of coarctation of the aorta is indicated for patients with severe left ventricular dysfunction. A central approach for surgical repair with a back-up left ventricular assist device is a safe and effective treatment strategy for these patients.

Use of Cardiac Computed Tomography for Ventricular Volumetry in Late Postoperative Patients with Tetralogy of Fallot

  • Background: Cardiac computed tomography (CT) has emerged as an alternative to magnetic resonance imaging (MRI) for ventricular volumetry. However, the clinical use of cardiac CT requires external validation. Methods: Both cardiac CT and MRI were performed prior to pulmonary valve implantation (PVI) in 11 patients (median age, 19 years) who had undergone total correction of tetralogy of Fallot during infancy. The simplified contouring method (MRI) and semiautomatic 3-dimensional region-growing method (CT) were used to measure ventricular volumes. Results: All volumetric indices measured by CT and MRI generally correlated well with each other, except for the left ventricular end-systolic volume index (LV-ESVI), which showed the following correlations with the other indices: the right ventricular end-diastolic volume index (RV-EDVI) (r=0.88, p<0.001), the right ventricular end-systolic volume index (RV-ESVI) (r=0.84, p=0.001), the left ventricular end-diastolic volume index (LV-EDVI) (r=0.90, p=0.001), and the LV-ESVI (r=0.55, p=0.079). While the EDVIs measured by CT were significantly larger than those measured by MRI (median RV-EDVI: $197mL/m^2$ vs. $175mL/m^2$, p=0.008; median LV-EDVI: $94mL/m^2$ vs. $92mL/m^2$, p=0.026), no significant differences were found for the RV-ESVI or LV-ESVI. Conclusion: The EDVIs measured by cardiac CT were greater than those measured by MRI, whereas the ESVIs measured by CT and MRI were comparable. The volumetric characteristics of these 2 diagnostic modalities should be taken into account when indications for late PVI after tetralogy of Fallot repair are assessed.

심근경색후 발생한 심실중격 결손의 외과적 치료후 장기결과 (Long Term Results After Repair of Postmyocardial Infarction Ventricular Septal Defect)

  • Between January 1986 and August 1993, 11 patients underwent surgical repair of ventricular septal defect [VSD] complicated with myocardial infarction. The ages of patients were ranged from 22 years to 83 years with a mean of 64 years. There were 8 male and 3 female patients. The preoperative cineangiograms of all patients were reviewed to measure both ventricular function and to evaluate coronary artery disease. The mean time interval between occurance of VSD and operation was 13 days. The operations were performed as soon as possible if there were hemodynamic derangement. Postmyocardial infarction VSD were repaired simultaneuously with coronary artery bypass graft in 3 patients, repaired with left ventricular aneurysmectomy in 6 patients, with left ventricular thrombectomy in 1 patient and with mitral valve chordae repair in 1 patient. There was no early death [within 30 days]. There were 6 postoperative complications; one with perioperative myocardial infarction, two with recurred VSD on postoperative 1 and 6 days respectively, two with lower leg embolism associated with intraaortic balloon pump insertion, one with wound infection. Of the complicated patients, 1 patient with lower leg embolism performed left above ankle amputation. Among two patients with recurred ventricular septal defect, one patient is doing well without problem. On follow up echocardiogram, the residual VSD was occluded completely. However another patient was with recurred VSD died 3 months after the operation because of congestive heart failure. Of the long term survivors, all patients are in NEW YORK Heart Association functional Class I or II. Although number of patients were small, our results of surgical closure of postmyocardial infarction VSD were favored to the others. Moreover, seven patients with preoperative cardiogenic shock among 11 were performed early operation after diagnosis of ventricular septal rupture. All of the patients were survived and doing well during the follow up period. Therefore early diagnosis with aggressive preoperative care with intraaortic balloon pumping and early operation seems to be very important for prevention of deterioration of vital organ.

신생아및 영아기 활로씨 사징증의 완전 교정술 (Complete Repair of Tetralogy of Fallot in Neonate or Infancy)

  • From August 1982 to December 1991, 58 consecutive infants with tetralogy of Fallot underwent primary repair. Age ranged from 22 days to twelve months [n=58, 8.7$\pm$2.7 months] and body weight from 3.1 to 13 kilograms [n=58, 7.8$\pm$1.7 kilograms]. Qne infant had absence of the pulmonary valve; one had Ebstein`s anomaly and one had supramitral ring. Thirty-two patients [56%] experienced anoxic spell. Preoperative pulmonary artery indices were measured in 38 cases, ranging 126-552mm2/M2BSA[n=38, 251$\pm$79mm2/M2BSA]. All infants required a right ventricular outflow tract patch; in 41, the patch extended across the pulmonary valve annulus, in 13 of them, monocusps were constructed. All had patch closure of ventricular septal defect. Two infants had REV operation for avoiding injury to the canal branch of the right coronary artery which cross the right ventricular out flow tract. Post repair PRV/LV were measured at operating room in 40 cases, which revealed mean value of 0.49$\pm$0.12 [range: 0.25-0.74]. The hospital mortality was 10.3% [6 patients], and causes of deaths were right heart failure due to sustained right ventricular hypertension[4] and right ventricular outflow tract obstruction, intractablesuraventricular tachyarrhythmia[1], hypoxia[1] due to residual right to left shunt across the atrial septal defect in patient associated with Ebstein`s anomaly. All infants were doing well at follow-up from 1 to 101 months[20.6 months /patient, 1, 072 patient-month] Serial postoperative echocardiograms revealed no residual ventricular septal defects and estimated RVOT gradients between 0 and 40 mmHg except 3 cases [50, 50, 60 mmHg]. There were no late deaths and late ventricular arrhythmias or congestive heart failure. Redo operations were done in 2 cases because of residual right ventricular outflow tract obstruction. This experience with infants with tetralogy of Fallot suggests that, if mortality is tolerable, eletive repair of tetralogy of Fallot could be reasonably undertaken during the first year of life, and even better results could be anticipated along with improvement of methods of myocardial protection and postoperative care.
