저장량과 환기구크기가 움저장 생강의 저장성에 미치는 영향 (The Effects of Storage Amount and Ventilator Size on the Quality of Ginger During Cellar Storage)

  • An experiment was conducted to develop the simple methods of ginger storage which decrease the weight reduction and maintain good quality of ginger during the cellar storage. The stored boxes with volume of 0.03㎥ and ventilator diameter of 3, 4, 5cm was hurried under the ground(60, 80, 100cm) in the green house. During the cellar storage at the 100cm depth the average temperature and relative humidity in the stored box were remained in 11.7~16.3$^{\circ}C$ and 73%, respect. The higher storage amount and smaller size of ventilator size increased the CO2 concentration in the stored box, and the concentration in the stored box with 50% storage quantity rate and 3cm ventilator diameter size was more than 10% for about 2 months from early Feburary. The decay rate of ginger during the cellar storage increased with higher amount of storage quantity and smaller size of ventilator. The CO2 concentration was low and remained relatively constant with the deeper location of stored box under the ground, and the decay rate was lower in the deeper stored box. Germination rate increased with the deeper location of stored box, and with the lower storage quantity and larger ventilator size. The germination rate was low in the higher decay rate box. Weight loss, total sugar and moisture contents of ginger were decreased, while crude fiber and ash were increased during the storage.

중증 외상환자의 인공호흡기 관련 폐렴 영향 요인 분석 (Analysis of Influence Factors on Ventilator-Associated Pneumonia in Severe Trauma Patients)

  • Purpose: This study was to analyze the factors affecting ventilator-associated pneumonia with severe trauma patients. Methods: This study conducted from May 1, 2018 to May 31, 2018 based on the medical records of the intensive care unit of a university hospital from May 1, 2017 to April 30, 2018 in Gangwon province. The inclusion criteria were 1) Trauma intensive care unit patient, 2) older than 19 years 3) without pneumonia at the time of admission. The collected data were analyzed using descriptive, correlation analysis, ANOVA, t-tests, $x^2$-tests and regression. Results: The severe trauma patients had a total of 2,877 days receiving ventilator, and nine VAP cases. The overall infection rate was 4.0%, and the VAP incidence rate was 3.13 per 1000 ventilator days. VAP in severe trauma patient affected ICU stay(OR=1.03), mechanical ventilator applied day(OR=1.04). Conclusion: Therefore, the development of an individualized VAP prevention bundle and nursing intervention for patients with trauma will be needed and further studies. In addition, there were no findings regarding the relationship between VAP occurrence and the severity of multiple traumatic injuries, so further studies of these factors should be performed.

방음터널의 자연환기성능 향상에 대한 연구 (The Study on the Improvement of Ventilation Performance in the Soundproof Tunnel)

  • This paper compared ventilation performance between the sound roof tunnel with flat roof and the sound roof tunnel with gable roof. The ventilation rate of the sound roof tunnel is calculated by natural ventilation rate plus ventilation by vehicle. The roof type is divided by the shape of the roof and the ventilator location on the roof. The results between calculation and CFD on the ventilation rate are almost alike. The ventilation rate on the flat roof is $558.4\;m^3/s$ with mid-ventilator and $496.8\;m^3/s$ with left-right ventilator. The ventilation rate on the gable roof is $653.2\;m^3/s$ with mid-ventilator and $611.6\;m^3/s$ with left-right ventilator. The ventilation rate of soundproof with gable roof is higher than that with flat roof. The ventilation rate and with mid-ventilator is higher than that with left-right ventilator the soundproof roof. Therefore, the ventilation performance of soundproof roof depends on the roof shape and ventilator location on the roof.

급·배기 일체형 디퓨저의 환기 및 온열환경 유지성능에 대한 연구 (A Study on The Performance of Ventilation and Maintaining Thermal Environment for a Combined Type Diffuser)

  • Installing a ventilator for an apartment house composed of over 100 dwelling units was mandated in 2006 to improve indoor air quality. In the case of mechanical ventilators, the air change rate is kept stable, however 68% of dwellers do not operate their ventilator because of an increased electrical bill and noise. In the case of natural ventilators, the former problems are settled, but there are concerns about cold draught and an increase of heating/cooling cost. Authors are developing a heat recovery type natural ventilator which is a natural ventilator equipped with total heat recovery element, and it is expected that those problems of mechanical ventilator and natural ventilator are resolved by this. The combined type diffuser of this study is an under developed fit to the heat recovery type natural ventilator above. There are no standards to evaluate the performance for this type of diffuser. Due to this issue this study investigated the performance of the ventilation and maintained a thermal environment for the combined type diffuser by comparing it with existing diffusers. The results revealed that the performances of the combined type of diffusers showed different features from the existing ones, and was estimated to be high enough in the performances above.

인공호흡기 치료를 받은 흉부외상 환자의 임상적 고찰 (Clinical experience of ventilator therapy in chest trauma)

  • A clinical evaluation was performed with a population of 49 patients of chest trauma, who were diagnosed to undergo ventilator therapy, and had gone through ventilator therapy at the Department of Thoracic and Cardiovascular Surgery, Kyungpook University Hospital. One of most common causes of chest trauma was vehicle accidents [77.5%] with the prevalent age group being their forties. The common findings were multiple rib fractures [89.8%], hemopneumothrax [81.6%], lung contusion [61.2%] and flail chest [44.9%]. Their common combined injuries were the orthopedics and neurosugical injuries [86.7%]. Complications caused by chest trauma were pneumonia, respiratory failure, atelectasis, barotrauma and empyema. Pulmonary infections were commonly associated with mechanical ventilation in the long term group and were best prevented by using bronchial hygiene therapy.The mortality rate was 5.8% of the total patients and that was 38.8% of the patients, who needed ventilator therapy. The causes of death were pneumonia, respiratory failure, acute renal failure and hypovolemic shock. Mechanical ventilation has an important place in the treatment of patients with severe chest trauma.

  • Management of mechanical ventilation is essential for patients with neuro-critical illnesses who may also have impairment of airways, lungs, respiratory muscles, and respiratory drive. However, balancing the approach to mechanical ventilation in the intensive care unit (ICU) with the need to prevent additional lung and brain injury, is challenging to intensivists. Lung protective ventilation strategies should be modified and applied to neuro-critically ill patients to maintain normocapnia and proper positive end expiratory pressure in the setting of neurological closed monitoring. Understanding the various parameters and graphic waveforms of the mechanical ventilator can provide information about the respiratory target, including appropriate tidal volume, airway pressure, and synchrony between patient and ventilator, especially in patients with neurological dysfunction due to irregularity of spontaneous respiration. Several types of asynchrony occur during mechanical ventilation, including trigger, flow, and termination asynchrony. This review aims to present the basic interpretation of mechanical ventilator waveforms and utilization of waveforms in various clinical situations in the neuro-ICU.

  • 본 연구는 인공호흡기 적용 환자들의 구강간호에 있어 클로르헥시딘의 적용에 대한 효과성을 분석하고, 이를 토대로 임상에서 실질적으로 구강간호 시행에 있어 명확한 근거를 제시 하고자 시도 되었다. 주요 검색어는 Ventilator associated pneumonia AND (Oral care OR Dental care OR Chlorhexidine*)으로 검색원으로는 국내 RISS, Koreamed, KISS와 국외 MEDLINE, Cochrane Central, CINAHL, Pub Med를 활용하여 2017년 5월까지의 문헌을 대상으로 검색하였다. 총 19편의 연구가 최종 선정되었고, Revman 5.3 프로그램으로 분석 하였으며, RoB (The Cochrane's Risk of Bias)도구를 이용해 문헌의 질을 평가하였다. 연구결과 중환자의 인공호흡기 관련 폐렴 예방에 있어 클로르헥시딘의 적용은 통계적으로 유의한 효과가 있었음이 나타났다.

User experience of MV2000-MT (SU:M2)® as a Mechanical Ventilator: A Comparative Clinical Study on Usability, Safety, and Medical Staff Satisfaction

  • In the present study, we aimed to demonstrate that MV2000-MT(SU:M2)® (MV, MEK-ICS, Paju, Korea), a domestic ventilator, is not inferior in terms of usability, safety, and medical staff satisfaction as compared to Hamilton G5 (G5, Hamilton Medical AG, Rhäzuns, Switzerland). A total of 39 patients who applied MV (group M) or G5 (group H) were included in the study sample. Usability was evaluated by the following factors: the number of alarm errors, replacement requirement of breathing circuit, replacement requirement of a right-angle connector, and ease of ventilator weaning. For safety evaluation, the number of ventilator replacements due to malfunction of the ventilator was evaluated. Items for medical staff satisfaction survey were as follows: the number of MV and G5 uses, hardware, and software assessment. In the usability evaluation, the replacement requirement of the right-angle connector was lower in Group M than in Group H (mean ± standard deviation, Group M: 7.39 ± 6.72, Group H: 14.19 ± 10.24, p = 0.021); however, the evaluations of other parts were not significantly different between the two groups. The number of ventilator replacements due to a malfunction of the ventilator did not differ between two groups. The number of MV and G5 uses was 3.0 [3.0-4.0] and 10.0 [5.0-10.0] (median [interquartile range], p < 0.001). Overall, the mean medical staff satisfaction score of Hamilton G5 was higher than that of MV2000-MT(SU:M2)®. The usability of MV is comparable to that of G5. However, medical staff satisfaction with Hamilton G5 was higher than that with MV2000-MT(SU:M2)®, and this difference could be due to the difference in the number of uses. In order to improve the penetration rate of the domestic mechanical ventilator, it is necessary to find ways to increase familiarity of medical staff with domestic mechanical ventilators.

중환자실 흡인간호 및 인공호흡기관리 표준화를 통한 인공호흡기 관련 폐렴발생 감소효과에 관한 연구 (The Effects of Standardized Suction and Ventilator Management Protocol on Ventilator Associated Pneumonia in the Intensive Care Unit)

  • Background : This study aimed at identifying the effect of the standardized protocol on lowering the incidence of the ventilator associated pneumonia(VAP). Methods : The standard protocol focusing on decreasing VAP was made and applied at 5 ICUs (Medical ICU, surgical ICU, Respiratory ICU, Neonatal ICU, Pediatric ICU) in a university affiliated tertiary hospital, from April 1, 2000 to Oct 31, 2000. The protocol involved 3 parts : hand washing, the suctioning method and ventilator circuit management. All the nursing personnel received intensive education which was consisted of lecture, video film and demonstration. 176 nurses reported the performance of handwashing pre and post intervention. And randomly selected 15 nurses were observed by charge nurse and the handwashing practice was analyzed pre and post intervention. The incidence of VAP was compared with the former year incidence. Results : The self reported frequency of hand washing increased. In the direct observation of handwashing, the frequency, time, thoroughness of hand washing during 8 hours day duty was found to be improved. The frequency was increased from 1.1 time to 4.1 times; the time was improved from 1.7 seconds to 5.7 seconds and the thoroughness of the washing practice was from 0.2 times to 3.0 times respectively (p<0.001). The incidence of VAP decreased from at a rate of 15.63 number of case per 1,000 ventilator-day (April 1~August 31, 1999) to 7.23 number of case per 1,000 ventilator-days(April 1~Oct 31, 2000)(P<0.001). Conclusion : We developed the protocols which included hand washing, the suctioning method, and ventilator circuit management. Through the implementation of the protocol, the performance of hand washing improved and the VAP incidence rate in ICU was decreased.

Family Caregivers of Korean Patients on Ventilators at Home: A Penomenological Study

  • Purpose. This study aimed to explore family caregivers' experiences with ventilator-dependent patients at home. Methods. The number of patients using mechanical ventilators at home is expected to increase and family caregivers must be able to care for them. However, few studies focus on the experiences of family caregivers. We conducted in-depth interviews with 11 family caregivers who cared for a ventilator-dependent patient at home in South Korea. Data were analyzed using phenomenological method of enquiry. Results. Statements generated from the interviews on the meaning of the family caregiver's experiences were organized into 27 themes, 5 theme clusters, and 2 categories. The theme clusters included endurance under the burden situation, role strain as a caregiver, separation from others, trying to find coping methods, and oriental ethnical customs. Family caregivers of patients using domestic mechanical ventilators need systematic education and emotional support to cope with the challenges of managing ventilator equipment and learning new ways of communicating with patients on the ventilators. Conclusion. These findings may contribute to family caregivers' knowledge and competence, thereby allowing them to better support their ventilator-dependent family members.
