A Case of Behçet's Disease with Superior Vena Cava Syndrome (상대정맥증후군을 유발한 Behçet병 1예)

  • As a chronic multisystemic inflammatory disorder, Behçet's disease may manifest vascular, cardiac, neurological and gastrointestinal abnormalities. However, involvement of large veins, such as thrombosis of the superior or inferior vena cava, is a very rare complication. Herein, a case of superior vena cava syndrome, due to the thrombotic obstructions of the subclavian and brachiocephalic vein, is reported in a 27-year old woman with chronic Behçet's disease.

  • There is increasing awareness of the devastating socio-economic cost of cardiovascular disease. As a result there have been a lot of extensive researches both on basic mechanisms involved and on the areas of epidemiology and clinical trials. Now, it is being generally accepted that dietary n-3 fatty acids abundant in marine products, may reduce the incidence of cardiovascular disease. Fish oil, with its complement of n-3 fatty acids, has been shown to have diverse and sometimes potent actions on cells of the blood and vasculature, which may explain some of its positive effects on vascular disease. However, the appropriate intake of n-3 fatty acids has not been established. This may vary with desired effects, the duration of ingestion, the type of seafood or fish oil and amount of other fatty acids, especially nf fatty acid being consumed in the diet, Although the beneficial effects have been extensively studied, little is known about potential adverse effects with excessive intake of n-3 fatty acids. Thus, it is needed to study on the possible unfavorable effects including genetic effects.

  • Prostaglandin E1(PGE1) is a potent vasodilator and is a useful drug for the treatment of occlusive peripheral vascular disease. It has been used systemically via intravenous route or regionally via intraarterial route. We tried intravenous regional administration of PGE1 for the treatment of a patient with occlusive arterial disease involving left fingers. During the 13th injection, the patient complained of severe pain at the injection site during the drug administration. Thereafter, the patient developed painful and severe swelling with blebs on his left hand. Systemic antibiotics were given together with stellate ganglion block of the affected left side. PGE1 was substituted to reserpine, which is subcutaneously injectable, for the second term treatment.

  • Cystic adventitial disease is rare, but it is one of the well-recognized causes of non-atherosclerotic arterial stenosis or obstruction. Despite one of its most common symptoms being chronic intermittent claudication, it may be misdiagnosed as arterial embolism when presented with acute ischemic symptoms. Surgical resection is recommended because of recurrence or a low success rate with aspiration or endovascular stent. We performed resection and repair with autologous vein patch for cystic adventitial disease of the popliteal artery of a 57-year-old man presenting with pain, pallor, and paresthesia, without any postoperative complications or recurrence.

  • Pharmacologic stress testing for myocardial perfusion imaging is a widely used noninvasive method for the evaluation of known or suspected coronary artery disease. The use of exercise for cardiac stress has been practiced for over 60 years and clinicians are familial with its using. However, there are inevitabe situations in which exorcise stress is inappropriate. A large number of patients with cardiac problems are unable to exercise to their full potential due to comorbidity such as osteoarthritis, vascular disease and pulmonary disease and a standard exercise stress test for myocardial perfusion imaging is suboptimal means for assessment of coronary artery disease. This problem has led to the development of the pharmacologic stress test and to a great increase in its popularity. All of the currently used pharmacologic agents have well-documented diagnostic value. This review deals the physiological actions, clinical protocols, safety, nuclear imaging applications of currently available stress agents and future development of new vasodilating agents.

Office-Based 585 nm Pulsed Dye Laser(PDL) Laryngeal Surgery (외래에서 시행되는 585 nm 펄스다이레이져 후두수술)

  • Introduction: 585 nm Pulsed dye laser (PDL) laryngeal surgery is based on the photodynamic characteristics of selective photothermolysis and photoangiolysis and recently considered to be the treatment for a variety of benign laryngeal disease. Objective: To review the indications and outcome of office-based 585nm PDL surgery and summarize new developments. Method: Retrospective study involving 402 patients was performed, The PDL surgery could be applied to various laryngeal diseases such as laryngeal papilloma, vocal fold dysplasia, laryngeal granuloma, vocal polyp, capillarectasia, scarred vocal fold and sulcus vocalis. Results : The physiologic properties of the vascular specificity of PDL provide many advantages and appear to be effective for laryngeal treatment. The PDL resulted in precise, selective coagulation of the microvasculature without damage to the surrounding tissue. Therefore PDL surgery is safe and effective for office-based treatment of benign laryngeal disease and for all patients regardless of their overall medical condition. Conclusion: PDL surgery provides potential benefits and advantage for treating common benign laryngeal disease.

Black Leg Disease in Wasabi Caused by Phoma wasabiae (Phoma wasabiae에 의한 고추냉이 먹들이병(묵입병))

  • A black leg disease in wasabi occurred, showed black spots on the leaves, changed a rhizome color to black by invading the vascular bundles of stem and root, thus lowered the quality of the rhizome. The mycelium of the pathogen was yellow at first and then turned to dark yellow on oat meal agar medium. The pycnidium was globose or subglobose, dark brown in color, and 44~120$\times$28~170 ${\mu}{\textrm}{m}$ in size and had one or two ostioles on the upper part. The pycnidiospores are single-celled, hyaline, and 4~6$\times$1.2~2.3 ${\mu}{\textrm}{m}$ in size. The causal pathogen was identified as Phoma wasabiae. The black leg disease of wasabi occurred within the range of 28 to 32% at Chonbuk province in 1994~1995. The disease was appeared from April to October and severe in June and July. The black leg caused by P. wasabiae was first described in Korea.

  • Moyamoya disease (MMD) is characterized by progressive steno-occlusive lesions of the distal or proximal branch of the internal carotid arteries, and cerebrovascular symptoms are its major complications. Extracranial vascular involvement including the coronary artery has been reported, and some case reports have described variant angina or myocardial infarction. However, no report has yet described a case of myocardial infarction after coronary artery bypass grafting (CABG). Here, we present a patient with MMD who suffered cardiac arrest caused by myocardial infarction due to a coronary spasm after offpump CABG and who was discharged successfully after treatment with a veno-arterial extracorporeal membrane oxygenator and percutaneous coronary intervention.

  • Adventitial cystic disease of the vein is a rare vascular disease. Herein, we report a case of adventitial cystic disease of the common femoral vein that was initially misdiagnosed as deep venous thrombosis in a 41-year-old male who presented with leg swelling. We focused on the imaging findings and compared them with those of deep venous thrombosis.
